Assessing the functional performance of post-call hospital doctors using a Nintendo Wii.
Sleep deprivation is an established part of the working life for Non-Consultant Hospital Doctors (NCHDs) in Ireland. Concern exists about the effect of extended NCHD work hours. We utilised a Nintendo Wii to evaluate motor function of NCHDs both prior to their on-call shift and the day afterwards. Data was exported to SPSS ver. 15 for statistical analysis with p \u3c 0.05 considered significant. A total of 72 NCHDs were invited to participate in this study. There was a 62.5% (45) rate of follow-up. Overall 27 (60%) NCHDs were on medical call, with 18 (40%) on surgical call. There was no statistically significant difference between NCHDs pre-and post-call motor assessment scores. The majority of study participants (75.5%, n = 34) had four or more hours sleep. On-call duty allows for a greater than anticipated amount of sleep per on-call shift and therefore has a negligible effect on the motor skills of medical staff
Defining breast cancer awareness and identifying barriers to breast cancer awareness for women with an intellectual disability: A review of the literature
Introduction: Incidence rates for developing breast cancer are similar for women regardless of intellectual ability. However, women with an intellectual disability present with advanced breast cancers, which often have a poor prognosis. Method: A structured narrative review of the literature was performed to explore the concepts of breast awareness and breast cancer awareness and subsequently, identify barriers to breast cancer awareness encountered by women with an intellectual disability. Results: A total of 22 studies involving people with varying levels of intellectual disability informed this review. The barriers to breast cancer awareness encountered by women with an intellectual disability include: lack of their understanding, the role of the carer and literacy issues. Conclusion: Identifying the barriers to breast cancer awareness for women with an intellectual disability will help to facilitate breast cancer awareness which has the potential to result in better long-term outcomes through an early diagnosis of breast cancer
Preventing infection in general surgery: improvements through education of surgeons by surgeons.
Surgical patients are at particular risk of healthcare-associated infection (HCAI) due to the presence of a surgical site leading to surgical site infection (SSI), and because of the need for intravascular access resulting in catheter-related bloodstream infection (CRBSI). A two-year initiative commenced with an initial audit of surgical practice; this was used to inform the development of a targeted educational initiative by surgeons specifically for surgical trainees. Parameters assessed during the initial audit and a further audit after the educational initiative were related to intra- and postoperative aspects of the prevention of SSIs, as well as care of peripheral venous catheters (PVCs) in surgical patients. A comparison of performances of consultant surgeons, NCHDs and medical students in a modified HPAT examination.
Following the implementation of the Fottrell report, entry to medical school in Ireland has undergone significant change. Medical school studentship is now awarded based on a combination of points obtained from the final examination of Irish secondary schools (the leaving certificate) combined with HPAT scores (Health Professions Admissions Test). The HPAT is designed to test a candidate\u27s knowledge in several different fields including problem solving skills, logical and non verbal reasoning. A sample HPAT was administered to a test group composed of consultant surgeons, non consultant hospital doctors, and medical students. Statistical analysis was performed and no significant difference was found between the performances of the groups. This is surprising as it was expected that groups with greater experience at medical problem solving would have translated to higher scores. This exposes a flaw within the HPAT system and a potential weakness in the process of doctor selection
Effect of Distillers Grains Composition and Level on Steers Consuming High-Quality Forage
An experiment was conducted to determine the effects of dried distillers grains (DDG) supplementation level and composition on growing steer performance and forage intake. Factors included DDG supplementation level (0.25, 0.50, 0.75 or 1.00% of BW), and DDG solubles level (0, 5.4, 14.5, 19.1, or 22.1% DM). Final BW improved, and forage intake decreased with increasing levels of DDG. An interaction between DDG supplementation level and solubles level was observed on ADG and F:G and was likely related to supplemental fat levels. Supplementation of forages with DDG improves performance while decreasing forage intake when fat levels are not too great
Rediscovery of the Threatened River Sharks, Glyphis garricki and G. glyphis, in Papua New Guinea
Recent surveys of the shark and ray catches of artisanal fishers in the Western Province of Papua New Guinea (PNG) resulted in the rediscovery of the threatened river sharks, Glyphis garricki and Glyphis glyphis. These represent the first records of both species in PNG since the 1960s and 1970s and highlight the lack of studies of shark biodiversity in PNG. Two individuals of G. garricki and three individuals of G. glyphis were recorded from coastal marine waters of the Daru region of PNG in October and November 2014. The two G. garricki specimens were small individuals estimated to be 100–105 cm and ~113 cm total length (TL). The three G. glyphis specimens were all mature, one a pregnant female and two adult males. These are the first adults of G. glyphis recorded to date providing a more accurate maximum size for this species, i.e. ~260 cm TL. A single pup which was released from the pregnant female G. glyphis, was estimated to be ~65 cm TL. Anecdotal information from the fishers of pregnant females of G. glyphis containing 6 or 7 pups provides the first estimate of litter size for this species. The jaws of the pregnant female G. glyphis were retained and a detailed description of the dentition is provided, since adult dentition has not been previously documented for this species. Genetic analyses confirmed the two species cluster well within samples from these species collected in northern Australia
Longitudinal coupling between a Si/SiGe quantum dot and an off-chip TiN resonator
Superconducting cavities have emerged as a key tool for measuring the spin
states of quantum dots. So far however, few experiments have explored
longitudinal couplings between dots and cavities, and no solid-state qubit
experiments have explicitly probed the "adiabatic" regime, where the Purcell
decay is strongly suppressed. Here, we report measurements of a quantum-dot
hybrid qubit coupled to a high-impedance resonator via a "flip-chip" design
geometry. By applying an ac drive to the qubit through two different channels,
and studying the effects of qubit energy detuning and interdot tunneling, we
are able to unequivocally confirm the presence of a longitudinal coupling
between the qubit and cavity. Since this coupling is proportional to the
driving amplitude, and is therefore switchable, it has the potential to become
