    A Conceptual Framework of Digital Empowerment of Informal Carers: An Expert Elicitation Study

    Many studies on online health communities (OHCs) have focused on patients’ well-being. The capabilities of OHCs to effect other psychosocial states like empowerment have been under-explored. Additionally, the study of empowerment of other healthcare stakeholders, specifically informal carers, has not attracted much study. This is despite evidence that carers use OHCs as an information and self-care resource in dealing with the stress and strain of caregiving. It is not clear how moderator support may influence carer empowerment. We propose a conceptual model to explore how moderated OHCs may influence empowerment of carers. In order to assess the model and support its robustness, this paper uses expert interviews of academics and industry professionals, with the view to focusing the research as well as operationalise the model. Results suggest a favourable acceptance of the model by experts, and thematic analysis of their conversations generated an additional construct

    Real-Time Water Quality Monitoring with Chemical Sensors

    Water quality is one of the most critical indicators of environmental pollution and it affects all of us. Water contamination can be accidental or intentional and the consequences are drastic unless the appropriate measures are adopted on the spot. This review provides a critical assessment of the applicability of various technologies for real-time water quality monitoring, focusing on those that have been reportedly tested in real-life scenarios. Specifically, the performance of sensors based on molecularly imprinted polymers is evaluated in detail, also giving insights into their principle of operation, stability in real on-site applications and mass production options. Such characteristics as sensing range and limit of detection are given for the most promising systems, that were verified outside of laboratory conditions. Then, novel trends of using microwave spectroscopy and chemical materials integration for achieving a higher sensitivity to and selectivity of pollutants in water are described

    Algorithmic Pollution - Making the Invisible Visible

    In this paper, we focus on the growing evidence of unintended harmful societal effects of automated algorithmic decision-making (AADM) in transformative services (e.g., social welfare, healthcare, education, policing and criminal justice), for individuals, communities and society at large. Drawing from the long-established research on social pollution, in particular its contemporary ‘pollution-as-harm’ notion, we put forward a claim - and provide evidence - that these harmful effects constitute a new type of digital social pollution, which we name ‘algorithmic pollution’. Words do matter, and by using the term ‘pollution’, not as a metaphor or an analogy, but as a transformative redefinition of the digital harm performed by AADM, we seek to make it visible and recognized. By adopting a critical performative perspective, we explain how the execution of AADM produces harm and thus performs algorithmic pollution. Recognition of the potential for unintended harmful effects of algorithmic pollution, and their examination as such, leads us to articulate the need for transformative actions to prevent, detect, redress, mitigate, and educate about algorithmic harm. These actions, in turn, open up new research challenges for the information systems community

    Evaluating a complex research capacity-building intervention: reflections on an evaluation of the African Institutions Initiative

    Increasing policy demand for realist evaluations of research and capacity-building programmes reflects a recognition of the management, governance and impact gains that can result from evaluation. However, the evidence base on how to successfully implement realist evaluations of complex interventions in international development efforts is scarce. We know little about the associated merits, limitations and ways to mitigate challenges. There is a need for reflective work which considers the methodology in context. This paper shares learning from the experience of conducting a realist, theory-of-change driven evaluation of the African Institutions Initiative, a Wellcome Trust funded programme which aimed to build sustainable health research capacity in Africa at institutional and network levels, across seven research consortia. We reflect on the key challenges experienced throughout the evaluation and recommend ways of managing them, highlight opportunities and critical success factors associated with this evaluation approach, as well as elaborate on alternative evaluation approaches

    Influence of Sleeve Gastrectomy on NASH and Type 2 Diabetes Mellitus

    Background. Nonalcoholic fatty liver disease is present in up to 85% of adipose patients and may proceed to nonalcoholic steatohepatitis (NASH). With insulin resistance and obesity being the main risk factors for NASH, the effect of isolated sleeve gastrectomy (ISG) on these parameters was examined. Methods. 236 patients underwent ISG with intraoperative liver biopsy from December 2002 to September 2009. Besides demographic data, pre-operative weight/BMI, HbA1c, AST, ALT, triglycerides, HDL and LDL levels were determined. Results. A significant correlation of NASH with higher HbA1c, AST and ALT and lower levels for HDL was observed (P < .05, <.0001, <.0001, <.01, resp.). Overall BMI decreased from 45.0 ± 6.8 to 29.7 ± 6.5 and 31.6 ± 4.4 kg/m2 at 1 and 3 years. An impaired weight loss was demonstrated for patients with NASH and patients with elevated HbA1c (plateau 28.08 kg/m2 versus 29.79 kg/m2 and 32.30 kg/m2 versus 28.79 kg/m2, resp.). Regarding NASH, a significant improvement of AST, ALT, triglyceride and HDL levels was shown (P < .0001 for all). A resolution of elevated HbA1c was observed in 21 of 23 patients. Summary. NASH patients showed a significant loss of body weight and amelioration of NASH status. ISG can be successfully performed in these patients and should be recommended for this subgroup

    Anatomically and functionally distinct lung mesenchymal populations marked by Lgr5 and Lgr6

    The diversity of mesenchymal cell types in the lung that influence epithelial homeostasis and regeneration is poorly defined. We used genetic lineage tracing, single-cell RNA sequencing, and organoid culture approaches to show that Lgr5 and Lgr6, well-known markers of stem cells in epithelial tissues, are markers of mesenchymal cells in the adult lung. Lgr6+ cells comprise a subpopulation of smooth muscle cells surrounding airway epithelia and promote airway differentiation of epithelial progenitors via Wnt-Fgf10 cooperation. Genetic ablation of Lgr6+ cells impairs airway injury repair in vivo. Distinct Lgr5+ cells are located in alveolar compartments and are sufficient to promote alveolar differentiation of epithelial progenitors through Wnt activation. Modulating Wnt activity altered differentiation outcomes specified by mesenchymal cells. This identification of region- and lineage-specific crosstalk between epithelium and their neighboring mesenchymal partners provides new understanding of how different cell types are maintained in the adult lung.This work was supported by (J.-H.L. and J.C.)
