Análise das recomendações internacionais sobre o consumo de açúcares publicadas entre 1961 e 1991
It is study made an analysis of national expert reports on dietary guidelines published all over the world in the 30-year period between 1961 and 1991. The aim is to identify what the reports recommend about the consumption of sugars and their relation to chronic diseases, and also what advice should be given to the public and appropriate authorities regarding levels of sugars in the diet. Information was gathered from 115 reports published by experts committees in 36 different countries and regions of the world, including developed and developing countries. The results showed that the great majority (84,5%) of the reports analysed make recommendations on the extrinsic consumption of sugar and that there is an agreement that these sugars, especially sucrose, must be reduced in the diet. The commonest level proposed is that 10% of total calory intake from extrinsic sugars should be considered the maximun level. Such advice is often addressed to the general population and it is offered with a view to the maintenance of general health and especially to the prevention dental caries and obesity. It was concluded that advice on the intake of sugars given by dietary guidelines in the last three decades is coherent with the existing scientific evidence on the relationship between sugars and human diseases and, therefore, they should be integrated into national food and health policies.Realizou-se levantamento das recomendações nutricionais sobre o consumo de açúcares contidas nas publicações especializadas em todo o mundo, nos últimos 30 anos. Objetivou-se investigar as recomendações sobre o consumo de açúcares e identificar qual orientação deveria ser dada ao público e autoridades competentes sobre os níveis ideais de açúcares na dieta compatíveis com uma boa saúde. As informações foram obtidas de 116 publicações elaboradas por comitês de especialistas de 37 diferentes países e regiões do mundo, incluindo principalmente países desenvolvidos, mas também aqueles em desenvolvimento. Os resultados permitiram concluir que a grande maioria (84,5% das publicações analisadas) fazem recomendações sobre o consumo de açúcares extrínsecos ou livres, havendo consenso de que estes açúcares, principalmente a sacarose, devem ser reduzidos da dieta. A recomendação mais freqüente é 10% de calorias a partir dos açúcares extrínsecos como um nível máximo. Tais recomendações são geralmente dirigidas à toda a população com a finalidade de manter a saúde geral, mas sobretudo prevenir a cárie dental e a obesidade. Concluiu-se que as recomendações sobre o consumo de açúcares contidas nas publicações editadas nas últimas três décadas são coerentes com a evidência científica da relação entre os açúcares e as doenças crônicas e, portanto, deveriam ser integradas às políticas de alimentação e saúde
Differential immunoglobulin and complement levels in leprosy prior to development of reversal reaction and erythema nodosum leprosum
Background
Leprosy is a treatable infectious disease caused by Mycobacterium leprae. However, there is additional morbidity from leprosy-associated pathologic immune reactions, reversal reaction (RR) and erythema nodosum leprosum (ENL), which occur in 1 in 3 people with leprosy, even with effective treatment of M. leprae. There is currently no predictive marker in use to indicate which people with leprosy will develop these debilitating immune reactions. Our peripheral blood mononuclear cell (PBMC) transcriptome analysis revealed that activation of the classical complement pathway is common to both RR and ENL. Additionally, differential expression of immunoglobulin receptors and B cell receptors during RR and ENL support a role for the antibody-mediated immune response during both RR and ENL. In this study, we investigated B-cell immunophenotypes, total and M. leprae-specific antibodies, and complement levels in leprosy patients with and without RR or ENL. The objective was to determine the role of these immune mediators in pathogenesis and assess their potential as biomarkers of risk for immune reactions in people with leprosy.
Methodology/findings
We followed newly diagnosed multibacillary leprosy cases (n = 96) for two years for development of RR or ENL. They were compared with active RR (n = 35), active ENL (n = 29), and healthy household contacts (n = 14). People with leprosy who subsequently developed ENL had increased IgM, IgG1, and C3d-associated immune complexes with decreased complement 4 (C4) at leprosy diagnosis. People who developed RR also had decreased C4 at leprosy diagnosis. Additionally, elevated anti-M. leprae antibody levels were associated with subsequent RR or ENL.
Conclusions
Differential co-receptor expression and immunoglobulin levels before and during immune reactions intimate a central role for humoral immunity in RR and ENL. Decreased C4 and elevated anti-M. leprae antibodies in people with new diagnosis of leprosy may be risk factors for subsequent development of leprosy immune reactions
Dor dentaria e fatores associados em adolescentes brasileiros: a Pesquia Nacional de Saude do Escolar (PeNSE), Brasil, 2009
Abstract published in English and Portuguese English title: Dental pain and associated factors in Brazilian adolescents: the National school-based health survey (PeNSE), Brazil, 2009The aim of this study was to assess the prevalence of dental pain and associated socio-demographic and behavioral factors in Brazilian adolescents, using data from the National School-Based Health Survey (PeNSE), Brazil, 2009. The survey was conducted by the Brazilian Institute of Geography and Statistics (IBGE) and Ministry of Health in students 11 to 17 years of age or older in the 27 State capitals, using a self-administered questionnaire. Analyses included Poisson regression following a hierarchical approach. Prevalence of dental pain in the sample (n = 54,985) in the previous six months was 17.8% (95%CI: 17.5-18.1). Higher prevalence was associated with female gender, age 14 years and over, racial self-identification as black, brown, or indigenous, enrollment in public schools, lower maternal schooling, not living with the mother, history of smoking or drinking, less frequent toothbrushing, and heavy consumption of sweets and soft drinks. Dental pain was thus associated with socio-demographic factors and health-related behaviors. = O objetivo deste estudo foi estimar a prevalência da dor de dente em adolescentes brasileiros e analisar fatores sociodemográficos e comportamentais associados, utilizando os dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2009. A pesquisa foi realizada pelo Instituto Brasileiro de Geografia e Estatística e pelo Ministério da Saúde em escolares com idades entre 11 e 17 anos ou mais, das 27 capitais brasileiras, por meio de questionário autoaplicável. Utilizou-se a análise de regressão de Poisson, segundo um modelo hierárquico de determinação. A prevalência de dor na amostra (n = 54.985) nos últimos seis meses foi de 17,8% (IC95%: 17,5-18,1). Prevalências mais elevadas foram encontradas em mulheres, naqueles com 14 anos ou mais, das raças preta, parda e indígena, de escolas públicas, cujas mães tinham baixa escolaridade, que não moravam com a mãe, que haviam experimentado cigarro e álcool alguma vez na vida, que relataram menor frequência de escovação e maior consumo de guloseimas e refrigerantes. Pervasive gaps in Amazonian ecological research
Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4
While the increasing availability of global databases on ecological communities has advanced our knowledge
of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In
the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of
Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus
crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced
environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian
Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by
2050. This means that unless we take immediate action, we will not be able to establish their current status,
