    Risk factors for laboratory-confirmed bloodstream infection in neonates undergoing surgical procedures

    AbstractBackgroundHealthcare Associated Infections constitute an important problem in Neonatal Units and invasive devices are frequently involved. However, studies on risk factors of newborns who undergo surgical procedures are scarce.ObjectiveTo identify risk factors for laboratory-confirmed bloodstream infection in neonates undergoing surgical procedures.MethodsThis case–control study was conducted from January 2008 to May 2011, in a referral center. Cases were of 21 newborns who underwent surgery and presented the first episode of laboratory-confirmed bloodstream infection. Control was 42 newborns who underwent surgical procedures without notification of laboratory-confirmed bloodstream infection in the study period. Information was obtained from the database of the Hospital Infection Control Committee Notification of infections and related clinical data of patients that routinely collected by trained professionals and follow the recommendations of Agência Nacional de Vigilância Sanitária and analyzed with Statistical Package for Social Sciences.ResultsDuring the study period, 1141 patients were admitted to Neonatal Unit and 582 Healthcare Associated Infections were reported (incidence-density of 25.75 Healthcare Associated Infections/patient-days). In the comparative analysis, a higher proportion of laboratory-confirmed bloodstream infection was observed in preterm infants undergoing surgery (p=0.03) and use of non-invasive ventilation was a protective factor (p=0.048). Statistically significant difference was also observed for mechanical ventilation duration (p=0.004), duration of non-invasive ventilation (p=0.04), and parenteral nutrition duration (p=0.003). In multivariate analysis duration of parenteral nutrition remained significantly associated with laboratory-confirmed bloodstream infection (p=0.041).ConclusionsShortening time on parenteral nutrition whenever possible and preference for non-invasive ventilation in neonates undergoing surgery should be considered in the assistance of these patients, with the goal of reducing Healthcare Associated Infections, especially laboratory-confirmed bloodstream infection

    Competência vetorial de Culex quinquefasciatus Say, 1823 exposto a diferentes densidades de microfilárias de Dirofilaria immitis (Leidy, 1856)

    The metropolitan region of Recife, Brazil is endemic for Dirofilaria immitis and has an environment favorable to the development of Culex quinquefasciatus. The goal of this study was to evaluate the vector competence of the Cx. quinquefasciatus RECIFE population for D. immitis transmission. A total of 2,104 females of Cx. quinquefasciatus RECIFE population were exposed to different densities of D. immitis microfilariae blood meals, ranging from 1,820 to 2,900 mf/ml of blood, in a natural membrane apparatus. The results showed a variation between 92.3% and 98.8% of females fed. The exposure of the Cx. quinquefasciatus RECIFE population to different densities of microfilariae did not influence the mortality of the mosquitoes. Infective larvae from D. immitis were observed in the Malpighian tubules beginning on the 12th day, whereas larvae were observed in the head and proboscis beginning on the 13th day following infection. The vector efficiency index (VEI) presented by the mosquitoes ranged from 7.8 to 56.5. The data demonstrates that the Cx. quinquefasciatus RECIFE population has great potential for the transmission of D. immitis, as it allowed the development of the filarid until the infectious stage at the different densities of microfilariae to which it was exposed.A Região Metropolitana do Recife é endêmica para Dirofilaria immitis e possui ambiente favorável para o desenvolvimento de Culex quinquefasciatus. Neste estudo avaliou-se a competência vetorial de Cx. quinquefasciatus população RECIFE para a transmissão de D. immitis. Para tanto, 2.104 fêmeas de Cx. quinquefasciatus população RECIFE foram expostas a diferentes densidades de microfilárias de D. immitis, variando de 1.820 a 2.900 mf/ml de sangue por meio de membrana natural. Os resultados obtidos demonstraram variação de 92,3% a 98,8% de fêmeas ingurgitadas após a alimentação. A exposição de Cx. quinquefasciatus população RECIFE a diferentes densidades de microfilárias não influenciou na mortalidade dos mosquitos. Larvas infectantes de D. immitis foram observadas nos túbulos de Malpighi a partir do 12º dia, enquanto na cabeça e na probóscide foram observadas a partir do 13º dia após a infecção. Os índices de eficiência vetorial (IEV) apresentados pelo culicídeo variaram de 7,8 a 56,5. Os dados obtidos demonstraram que Cx. quinquefasciatus população RECIFE tem grande potencial para a transmissão de D. immitis, pois permitiu o desenvolvimento do filarídeo até o estágio infectante nas diferentes densidades de microfilárias às quais foi exposto.65866

    Adaptability and stability of irrigated wheat genotypes in the Cerrado of Central Brazil

    O objetivo deste trabalho foi estimar a adaptabilidade e estabilidade de genótipos de trigo irrigado, nas condições do Cerrado do Brasil Central. O rendimento de grãos de cinco cultivares e nove linhagens de trigo irrigado foram aferidos no ensaio de valor de cultivo e uso. Os ensaios foram conduzidos nos anos de 2005 e 2006, em seis locais em Minas Gerais, três em Goiás e um no Distrito Federal. Os dados de rendimento de grãos foram submetidos à análise de variância individual e à análise de variância conjunta, para a estimativa dos parâmetros de adaptabilidade e estabilidade. Verificaram-se diferenças significativas, entre os genótipos, em 14 dos 16 ambientes avaliados. Nenhum dos genótipos avaliados apresentou o comportamento ideal preconizado pelo método de Cruz e colaboradores. Destacaram-se a cultivar BRS 264 e a linhagem CPAC 02167, pela superioridade em todas as condições do Cerrado do Brasil Central (adaptabilidade geral). Entre os demais genótipos avaliados, destacaram-se as cultivares BRS 207 e EMBRAPA 22, responsivas à melhoria das condições ambientais, e as cultivares BRS 254 e EMBRAPA 42, pela adaptabilidade específica a ambientes desfavoráveis ao cultivo de trigo irrigado.The aim of this work was to estimate the adaptability and stability of irrigated wheat genotypes under conditions of Central Brazil savannah area. Grain yield of five cultivars and nine lines of irrigated wheat was measured in assays of cultivation value and use. The assays were conducted during the years 2005 and 2006, in six locations in Minas Gerais State, three in Goiás State and one in Federal District. The data of grain yield were submitted to individual and joint ANOVA, in order to estimate the adaptability and stability parameters. Significant differences were detected between genotypes in 14 of the 16 locations evaluated. None of the evaluated genotypes have presented the behavior predicted by the method of Cruz and collaborators. Cultivar BRS 264 and the CPAC 02167 lineage were superior in all the tested conditions in the Central Brazil savannah (general adaptability). Among the other evaluated genotypes, it can be highlighted the responsiveness of the cultivars BRS 207 and EMBRAPA 22 to the improvement of environmental conditions, and the specific fitness of BRS 254 and EMBRAPA 42 cultivars to environments unfavorable to irrigated wheat

    Seroprevalence of Chikungunya virus and living conditions in Feira de Santana, Bahia-Brazil.

    BACKGROUND: Chikungunya is an arbovirus, transmitted by Aedes mosquitoes, which emerged in the Americas in 2013 and spread rapidly to almost every country on this continent. In Brazil, where the first cases were detected in 2014, it currently has reached all regions of this country and more than 900,000 cases were reported. The clinical spectrum of chikungunya ranges from an acute self-limiting form to disabling chronic forms. The purpose of this study was to estimate the seroprevalence of chikungunya infection in a large Brazilian city and investigate the association between viral circulation and living condition. METHODOLOGY/PRINCIPAL FINDINGS: We conducted a population-based ecological study in selected Sentinel Areas (SA) through household interviews and a serologic survey in 2016/2017. The sample was of 1,981 individuals randomly selected. The CHIKV seroprevalence was 22.1% (17.1 IgG, 2.3 IgM, and 1.4 IgG and IgM) and varied between SA from 2.0% to 70.5%. The seroprevalence was significantly lower in SA with high living conditions compared to SA with low living condition. There was a positive association between CHIKV seroprevalence and population density (r = 0.2389; p = 0.02033). CONCLUSIONS/SIGNIFICANCE: The seroprevalence in this city was 2.6 times lower than the 57% observed in a study conducted in the epicentre of the CHIKV epidemic of this same urban centre. So, the herd immunity in this general population, after four years of circulation of this agent is relatively low. It indicates that CHIKV transmission may persist in that city, either in endemic form or in the form of a new epidemic, because the vector infestation is persistent. Besides, the significantly lower seroprevalences in SA of higher Living Condition suggest that beyond the surveillance of the disease, vector control and specific actions of basic sanitation, the reduction of the incidence of this infection also depends on the improvement of the general living conditions of the population

    Epidemiology of Superficial Fungal Infections in Portugal: 3-year review (2014-2016)

    Introdução: As infeções fúngicas superficiais são as dermatoses infeciosas mais frequentes e a sua incidência continua a aumentar. Os dermatófitos são os principais agentes causais apresentando, contudo, uma distribuição geográfica variável. Material e Métodos: O presente estudo teve como objetivo a caracterização epidemiológica das infeções fúngicas superficiais diagnosticadas nos Serviços/Unidades de Dermatologia pertencentes ao Serviço Nacional de Saúde Português entre janeiro de 2014 e dezembro 2016 através da análise retrospetiva dos resultados das culturas realizadas durante esse período. Resultados: Foram estudados 2375 isolamentos, pertencentes a 2319 doentes. O dermatófito mais frequentemente isolado foi o Trichophyton rubrum (53,6%), tendo sido o principal agente causal da tinha da pele glabra (52,4%) e das onicomicoses (51,1%). Relativamente às tinhas do couro cabeludo, globalmente o Microsporum audouinii foi o agente mais prevalente (42,6%), seguido do Trichophyton soudanense (22,1%). Enquanto na área metropolitana de Lisboa estes dermatófitos foram os principais agentes de tinha do couro cabeludo, nas regiões Norte e Centro o agente mais frequente foi o Microsporum canis (58,5%). Os fungos leveduriformes foram os principais responsáveis pelas onicomicoses das mãos (76,7%). Conclusão: Os resultados deste estudo estão globalmente concordantes com a literatura científica. O Trichophyton rubrum apresenta-se como o dermatófito mais frequentemente isolado em cultura. Na tinha do couro cabeludo, na área metropolitana de Lisboa, as espécies antropofílicas de importação assumem particular destaque.info:eu-repo/semantics/publishedVersio

    The Brazilian Cerrado (Savannah) is a biome with great biodiversity. Many of the species found in this biome have unlimited economic potential and are used by the local population for various purposes. With the opening of the agricultural frontiers in this region, much of the native flora has been replaced by extensive areas of monocultures. Thirteen populations of the Eugenia dysenterica DC species were sampled in the northeast of the state of Goiás, Brazil, to analyze their genetic structure. Morphological traits were measured for all the progeny obtained and some of them were analyzed by using RAPD markers. The natural populations of the species showed high levels of variability for the morphological traits. The phenotypic, genotypic and genetic variabilities as structured in these populations were highly significant among populations, although most of the variability was detected within populations. The phenotypic and genetic variabilities were structured spatially as shown by the positive and highly significant correlation coefficients between the phenotypic and genetic distance matrixes and the geographic distance matrix. The ideal conservation strategy for the species in the region requires sampling of a high number of individuals of each population and also a significant number of populations to ensure suitable effective population sizes
