Proteomics Characterization of Outer Membrane Vesicles from the Extraintestinal Pathogenic Escherichia coli ΔtolR IHE3034 Mutant
Extraintestinal pathogenic Escherichia coli are the cause of a diverse spectrum of invasive infections in humans and animals, leading to urinary tract infections, meningitis, or septicemia. In this study, we focused our attention on the identification of the outer membrane proteins of the pathogen in consideration of their important biological role and of their use as potential targets for prophylactic and therapeutic interventions. To this aim, we generated a DeltatolR mutant of the pathogenic IHE3034 strain that spontaneously released a large quantity of outer membrane vesicles in the culture supernatant. The vesicles were analyzed by two-dimensional electrophoresis coupled to mass spectrometry. The analysis led to the identification of 100 proteins, most of which are localized to the outer membrane and periplasmic compartments. Interestingly based on the genome sequences available in the current public database, seven of the identified proteins appear to be specific for pathogenic E. coli and enteric bacteria and therefore are potential targets for vaccine and drug development. Finally we demonstrated that the cytolethal distending toxin, a toxin exclusively produced by pathogenic bacteria, is released in association with the vesicles, supporting the recently proposed role of bacterial vesicles in toxin delivery to host cells. Overall, our data demonstrated that outer membrane vesicles represent an ideal tool to study Gram-negative periplasm and outer membrane compartments and to shed light on new mechanisms of bacterial pathogenesis
Interventional Radiological Management and Prevention of Complications after Pancreatic Surgery: Drainage, Embolization and Islet Auto-Transplantation
Pancreatic surgery still remains burdened by high levels of morbidity and mortality with a relevant incidence of complications, even in high volume centers. This review highlights the interventional radiological management of complications after pancreatic surgery. The current literature regarding the percutaneous drainage of fluid collections due to pancreatic fistulas, percutaneous transhepatic biliary drainage due to biliary leaks and transcatheter embolization (or stent–graft) due to arterial bleeding is analyzed. Moreover, also, percutaneous intra-portal islet auto-transplantation for the prevention of pancreatogenic diabetes in case of extended pancreatic resection is also examined.

In islet auto-transplantation, the patient is simultaneously donor and recipient. Nutrient sensing modulates malaria parasite virulence
The lifestyle of intracellular pathogens, such as malaria parasites, is intimately connected to that of their host, primarily for nutrient supply. Nutrients act not only as primary sources of energy but also as regulators of gene expression, metabolism and growth, through various signalling networks that enable cells to sense and adapt to varying environmental conditions. Canonical nutrient-sensing pathways are presumed to be absent from the causative agent of malaria, Plasmodium, thus raising the question of whether these parasites can sense and cope with fluctuations in host nutrient levels. Here we show that Plasmodium blood-stage parasites actively respond to host dietary calorie alterations through rearrangement of their transcriptome accompanied by substantial adjustment of their multiplication rate. A kinome analysis combined with chemical and genetic approaches identified KIN as a critical regulator that mediates sensing of nutrients and controls a transcriptional response to the host nutritional status. KIN shares homology with SNF1/AMPKα, and yeast complementation studies suggest that it is part of a functionally conserved cellular energy-sensing pathway. Overall, these findings reveal a key parasite nutrient-sensing mechanism that is critical for modulating parasite replication and virulence
A medicalização da educação: implicações para a constituição do sujeito/aprendiz
Resumo O Transtorno de Déficit de Atenção/Hiperatividade (TDAH) gera controvérsias entre os pesquisadores da área da saúde. A corrente organicista defende que o transtorno seria decorrente de uma desordem neurobiológica, de origem genética. Contrariando o paradigma hegemônico, pesquisadores alinhados à vertente sócio-histórica entendem o TDAH como parte de um fenômeno denominado medicalização da educação. Este estudo pretende refletir sobre a construção social do TDAH (da entrada na escola ao diagnóstico médico) e suas implicações para a subjetividade, socialização e aprendizagem do aluno considerado resistente ao que a escola propõe. Esta pesquisa se constitui em análise de caso, pesquisa de campo, qualitativa, do tipo transversal, inserida em um paradigma teórico-metodológico de cunho sócio-histórico (BAKHTIN, 2006; VYGOTSKY, 2010). Para análise da história de uma criança de 10 anos de idade e com diagnóstico de TDAH, foram realizadas entrevistas com professores, com a mãe e com a criança, observação em sala de aula, avaliação fonoaudiológica e pesquisa documental (material pedagógico, pareceres avaliativos das escolas frequentadas pela criança, pareceres de profissionais de saúde etc.). Os resultados apontam que, quando se investiga em profundidade a qualidade das interações sociais em que a criança esteve/está inserida, é possÃvel que se compreendam as bases socioeducacionais que constituem o suposto transtorno
AIRO Breast Cancer Group Best Clinical Practice 2022 Update
Introduction: Breast cancer is the most common tumor in women and represents the leading cause of cancer death. Radiation therapy plays a key-role in the treatment of all breast cancer stages. Therefore, the adoption of evidence-based treatments is warranted, to ensure equity of access and standardization of care in clinical practice.Method: This national document on the highest evidence-based available data was developed and endorsed by the Italian Association of Radiation and Clinical Oncology (AIRO) Breast Cancer Group.We analyzed literature data regarding breast radiation therapy, using the SIGN (Scottish Intercollegiate Guidelines Network) methodology (www.sign.ac.uk). Updated findings from the literature were examined, including the highest levels of evidence (meta-analyses, randomized trials, and international guidelines) with a significant impact on clinical practice. The document deals with the role of radiation therapy in the treatment of primary breast cancer, local relapse, and metastatic disease, with focus on diagnosis, staging, local and systemic therapies, and follow up. Information is given on indications, techniques, total doses, and fractionations.Results: An extensive literature review from 2013 to 2021 was performed. The work was organized according to a general index of different topics and most chapters included individual questions and, when possible, synoptic and summary tables. Indications for radiation therapy in breast cancer were examined and integrated with other oncological treatments. A total of 50 questions were analyzed and answered.Four large areas of interest were investigated: (1) general strategy (multidisciplinary approach, contraindications, preliminary assessments, staging and management of patients with electronic devices); (2) systemic therapy (primary, adjuvant, in metastatic setting); (3) clinical aspects (invasive, non-invasive and micro-invasive carcinoma; particular situations such as young and elderly patients, breast cancer in males and cancer during pregnancy; follow up with possible acute and late toxicities; loco-regional relapse and metastatic disease); (4) technical aspects (radiation after conservative surgery or mastectomy, indications for boost, lymph node radiotherapy and partial breast irradiation).Appendixes about tumor bed boost and breast and lymph nodes contouring were implemented, including a dedicated web application. The scientific work was reviewed and validated by an expert group of breast cancer key-opinion leaders.Conclusions: Optimal breast cancer management requires a multidisciplinary approach sharing therapeutic strategies with the other involved specialists and the patient, within a coordinated and dedicated clinical path. In recent years, the high-level quality radiation therapy has shown a significant impact on local control and survival of breast cancer patients. Therefore, it is necessary to offer and guarantee accurate treatments according to the best standards of evidence-based medicine
Elotuzumab plus pomalidomide and dexamethasone in relapsed/refractory multiple myeloma: a multicenter, retrospective real-world experience with 200 cases outside of controlled clinical trials
In the ELOQUENT-3 trial, the combination of elotuzumab, pomalidomide and dexamethasone
(EloPd) proved a superior clinical benefit over Pd with a manageable toxicity profile, leading to its
approval in relapsed/refractory multiple myeloma (RRMM), who had received at least two prior
therapies, including lenalidomide and a proteasome inhibitor (PI).
We report here a real-world experience of 200 RRMMs treated with EloPd in 35 Italian centers
outside of clinical trials. In our dataset, the median number of prior lines of therapy was 2, with
51% of cases undergoing autologous stem cell transplant (ASCT) and 73% exposed to
daratumumab.
After a median follow-up of 9 months, 126 patients stopped EloPd, most of them (88.9%) because
of disease progression. The overall response rate (ORR) was 55.4%, in line with the pivotal trial
results. Regarding adverse events, our cohort experienced a toxicity profile similar to the
ELOQUENT-3 trial, with no significant differences between younger (<70 years) and older
patients. The median progression-free survival (PFS) was 7 months, shorter than that observed in
the ELOQUENT-3, probably due to the different clinical characteristics of the two cohorts.
Interestingly, the ISS stage III (HR:2.55) was associated with worse PFS. Finally, our series's
median overall survival (OS) was shorter than that observed in the ELOQUENT-3 trial (17.5 versus
29.8 months). In conclusion, our real-world study confirms EloPd as a safe and possible therapeutic
choice for RRMM who received at least two prior therapies, including lenalidomide and a PI
A machine-learning based bio-psycho-social model for the prediction of non-obstructive and obstructive coronary artery disease
Background: Mechanisms of myocardial ischemia in obstructive and non-obstructive coronary artery disease (CAD), and the interplay between clinical, functional, biological and psycho-social features, are still far to be fully elucidated. Objectives: To develop a machine-learning (ML) model for the supervised prediction of obstructive versus non-obstructive CAD. Methods: From the EVA study, we analysed adults hospitalized for IHD undergoing conventional coronary angiography (CCA). Non-obstructive CAD was defined by a stenosis < 50% in one or more vessels. Baseline clinical and psycho-socio-cultural characteristics were used for computing a Rockwood and Mitnitski frailty index, and a gender score according to GENESIS-PRAXY methodology. Serum concentration of inflammatory cytokines was measured with a multiplex flow cytometry assay. Through an XGBoost classifier combined with an explainable artificial intelligence tool (SHAP), we identified the most influential features in discriminating obstructive versus non-obstructive CAD. Results: Among the overall EVA cohort (n = 509), 311 individuals (mean age 67 ± 11 years, 38% females; 67% obstructive CAD) with complete data were analysed. The ML-based model (83% accuracy and 87% precision) showed that while obstructive CAD was associated with higher frailty index, older age and a cytokine signature characterized by IL-1β, IL-12p70 and IL-33, non-obstructive CAD was associated with a higher gender score (i.e., social characteristics traditionally ascribed to women) and with a cytokine signature characterized by IL-18, IL-8, IL-23. Conclusions: Integrating clinical, biological, and psycho-social features, we have optimized a sex- and gender-unbiased model that discriminates obstructive and non-obstructive CAD. Further mechanistic studies will shed light on the biological plausibility of these associations. Clinical trial registration: NCT02737982
