    Effects of two trypanosomicide drugs, gention violet and WR6026, on platelet concentrates preservation

    In order to verify the viability of platelet concentrate during its preservation period when added with two trypanosomicidal chemical substances, gentian violet and quinoline WR6026, we tested these concentrates and verified the following parameters: number of platelets, pH and platelet aggregation test with a pair of agonists. Significant alterations were observed in the platelet concentrate treated with gentian violet suggesting important damage to the platelet viability. With WR6026 similar values to the controls were observed, opening new perspectives for the utilization of this salt for the chemoprophylaxis of transfusional Chagas' disease in highly endemic regions.Visando verificar a viabilidade do concentrado de plaquetas, após sua preservação com substâncias, que adicionadas ao sangue sejam capazes de destruir o Trypanosoma cruzi, testamos concentrado de plaquetas estocado com a violeta de genciana e a quinolina WR6026. A viabilidade foi avaliada através da contagem do número de plaquetas, da medida do pH e agregação plaquetária com par de agonistas durante seu período de estocagem. Alterações significativas foram observadas nos concentrados plaquetários tratados com violeta de genciana sugerindo comprometimento da viabilidade plaquetária.     The AIDS epidemic in the Amazon region: a spatial case-control study in Rondonia, Brazil

    OBJETIVO Analisar mudanças espaciais no risco de Aids e a relação entre incidência da doença e variáveis socioeconômicas. MÉTODOS Estudo caso-controle espacial, de base populacional, realizado em Rondônia, Brasil, com 1.780 casos notificados pelo Sistema de Vigilância Epidemiológica e os controles a partir de dados demográficos de 1987 a 2006. Os casos foram agrupados em cinco períodos de cinco anos consecutivos. Um modelo aditivo generalizado foi ajustado aos dados. O status dos indivíduos (caso ou controle) foi considerado como a variável dependente e independente: um alisamento ( spline ) bidimensional das coordenadas geográficas e variáveis socioeconômicas municipais. Os valores observados para o teste Moran I foram comparados com a distribuição de referência dos valores obtidos em condições de aleatoriedade espacial. RESULTADOS O risco de Aids apresentou padrão espacial e temporal marcado. A incidência associou-se a indicadores socioeconômicos municipais, como urbanização e capital humano. As maiores taxas de incidência de Aids ocorreram em municípios ao longo da rodovia BR-364; os resultados do teste Moran I mostram correlação espacial positiva associada à contiguidade dos municípios com a rodovia, no terceiro e quarto períodos (p = 0,05). CONCLUSÕES A incidência da doença foi maior em municípios de maior riqueza econômica e urbanização e naqueles cortados pelas estradas principais de Rondônia. CONCLUSIONES La incidencia de la enfermedad fue mayor en municipios de mayor riqueza económica y urbanización y en aquellos atravesados por las carreteras principales de Rondonia. El rápido desarrollo asociado a la ocupación de regiones remotas puede ser acompañado por aumento de riesgos para la salud.OBJECTIVE To analyze spatial changes in the risk of AIDS and the relationship between AIDS incidence and socioeconomic variables in the state of Rondonia, Amazon region. METHODS A spatial, population case-control study in Rondonia, Brazil, based on 1,780 cases reported to the Epidemiological Surveillance System and controls based on demographic data from 1987 to 2006. The cases were grouped into five consecutive four-year periods. A generalized additive model was adjusted to the data; the dependent variable was the status of the individuals (case or control), and the independent variables were a bi-dimensional spline of the geographic coordinates and some municipality-level socioeconomic variables. The observed values of the Moran’s I test were compared to a reference distribution of values generated under conditions of spatial randomness. RESULTS AIDS risk shows a marked spatial and temporal pattern. The disease incidence is related to socioeconomic variables at the municipal level in Rondônia, such as urbanization and human capital. The highest incidence rates of AIDS are in municipalities along the BR-364 highway and calculations of the Moran’s I test show positive spatial correlation associated with proximity of the municipality to the highway in the third and fourth periods (p = 0.05). CONCLUSIONS Incidence of the disease is higher in municipalities of greater economic wealth and urbanization, and in those municipalities bisected by Rondônia’s main roads. The rapid development associated with the opening up of once remote regions may be accompanied by an increase in these risks to health

    The clinical impact of MTHFR polymorphism on the vascular complications of sickle cell disease

    Sickle cell disease (SCD) is one of the most common inherited diseases in the world and the patients present notorious clinical heterogeneity. It is known that patients with SCD present activation of the blood coagulation and fibrinolytic systems, especially during vaso-occlusive crises, but also during the steady state of the disease. We determined if the presence of the factor V gene G1691A mutation (factor V Leiden), the prothrombin gene G20210A variant, and methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism may be risk factors for vascular complications in individuals with SCD. We studied 53 patients with SCD (60% being women), 29 with SS (sickle cell anemia; 28 years, range: 13-52 years) and 24 with SC (sickle-hemoglobin C disease; 38.5 years, range: 17-72 years) hemoglobinopathy. Factor V Leiden, MTHFR C677T polymorphism, and prothrombin G20210A variant were identified by PCR followed by further digestion of the PCR product with specific endonucleases. The following vascular complications were recorded: stroke, retinopathy, acute thoracic syndrome, and X-ray-documented avascular necrosis. Only one patient was heterozygous for factor V Leiden (1.8%) and there was no prothrombin G20210A variant. MTHFR 677TT polymorphism was detected in 1 patient (1.8%) and the heterozygous form 677TC was observed in 18 patients (34%, 9 with SS and 9 with SC disease), a prevalence similar to that reported by others. No association was detected between the presence of the MTHFR 677T allele and other genetic modulation factors, such as alpha-thalassemia, ß-globin gene haplotype and fetal hemoglobin. The presence of the MTHFR 677T allele was associated with the occurrence of vascular complications in SCD, although this association was not significant when each complication was considered separately.     Acidentes de trabalho com material biológico em um hospital escola

    Objective: to analyze occupational accidents with exposure of nursing technicians to biological material in a school hospital. Method: nursing technicians were invited to answer a questionnaire related to occupational accidents with biological material. Results: 275 professionals from 9 hospital units participated. 76% reported having suffered an accident and the variables "age group" and "employment regime" showed a significant association for accidents (p0.05). Conclusion: the increase in the number of training did not ensure the reduction of occupational accidents with biological material; and nursing technicians under the age of 30 were more vulnerable. Método: técnicos de enfermagem foram convidados a responder um questionário relacionado à ocorrência de acidentes de trabalho com material biológico. Resultados: participaram 275 profissionais de 9 unidades do hospital. 76% declararam ter sofrido acidente e as variáveis “faixa etária” e “regime de trabalho” mostraram associação significativa para ocorrência de acidentes (p 0,05). Conclusão: o aumento do número de capacitações não assegurou a diminuição dos acidentes de trabalho com material biológico; e os técnicos de enfermagem com idade inferior a 30 anos foram mais vulneráveis.     Integrative taxonomy of a new Redudasys species (Gastrotricha: Macrodasyida) sheds light on the invasion of fresh water habitats by macrodasyids

    The order Macrodasyida (Gastrotricha) includes over 350 marine species, and only 3 freshwater species (Marinellina flagellata, Redudasys fornerise, R. neotemperatus). Herein we describe a new freshwater species of Macrodasyida, Redudasys brasiliensis sp. nov., from Brazil through an integrative taxonomic approach. The external morphology and internal anatomy were investigated using differential interference contrast microscopy, confocal microscopy, scanning and transmission electron microscopy. The systematization of the new taxon was inferred by nuclear (18S and 28S) and mitochondrial (COI) genes, and its intra-order relationships were assessed using data from most of available macrodasyids. Phylogenetic analyses yielded congruent trees, in which the new taxon is nested within the family Redudasyidae, but it was genetically distinct from the other species of the genus Redudasys. The new species shares the gross morphology and reproductive traits with other Redudasyidae and the presence of only 1 anterior adhesive tube per side with Redudasys neotemperatus, but it has a specific pattern of ventral ciliation and muscle organization. Results support the hypothesis that dispersion into fresh water habitats by Macrodasyida and Chaetonotida taxa occurred independently and that within Macrodasyida a single lineage invaded the freshwater environment only once.     Hemostatic changes in children undergoing heart surgery with cardiopulmonary bypass

    PURPOSE: We evaluated hemostasic changes in children undergoing open heart surgery with cardiopulmonary bypass (CPB). METHODS: We studied 17 children before, during surgery, in the immediate, first and between the 4th and 7th postoperative days, measuring hematocrit, prothrombin time, activated partial thromboplastin time, fibrinogen, platelet count, factor V and euglobulin lysis time. Children were divided in those with and without excessive bleeding in the postoperative period. RESULTS: We observed significant prolongation of prothrombin time and activated partial thromboplastin time, reduction of fibrinogen and factor V, and shortening of euglobulin lysis time. Six (35%) children bled excessively. Platelet count reduction was greater in the intra operative period in these cases and the duration of CPB was longer in this group. CONCLUSION: Changes in hemostasis during open heart surgery are due to coagulation cascade disorders as well as fibrinolysis. The incidence of excessive bleeding is higher in the pediatric group. Prolonged CPB time and greater reduction in platelet count differentiated both groups.OBJETIVO: Avaliar as alterações de hemostasia encontradas em crianças submetidas a cirurgia cardíaca com circulação extracorpórea (CEC). MÉTODOS: Estudamos 17 crianças no pré e pós-operatório (PO) imediato, no 1° PO e entre o 4º e 7º PO, analisando o hematócrito, tempo de protrombina (TP), tempo de tromboplastina parcial ativado, fibrinogênio, contagem de plaquetas, fator V e tempo de lise de euglobulina (TLE). Os pacientes foram divididos em grupos com e sem sangramento excessivo no PO. RESULTADOS: Houve alterações significantes no intra-operatório com aumento do TP e tempo de tromboplastina parcial ativado e redução do fibrinogênio, fator V e do TLE. Seis (35%) crianças sangraram excessivamente. A contagem de plaquetas foi significantemente menor no intra-operatório; neste grupo o tempo de CEC foi maior. CONCLUSÃO: Alterações de hemostasia no intra e PO ocorrem como conseqüência da ativação da cascata de coagulação e fibrinólise. A incidência de sangramento excessivo é alta nesta faixa etária.
