Shorter androgen receptor polyQ alleles protect against life-threatening COVID-19 disease in European males
Background: While SARS-CoV-2 similarly infects men and women, COVID-19 outcome is less favorable in men. Variability in COVID-19 severity may be explained by differences in the host genome. Methods: We compared poly-amino acids variability from WES data in severely affected COVID-19 patients versus SARS-CoV-2 PCR-positive oligo-asymptomatic subjects. Findings: Shorter polyQ alleles (â€22) in the androgen receptor (AR) conferred protection against severe outcome in COVID-19 in the first tested cohort (both males and females) of 638 Italian subjects. The association between long polyQ alleles (â„23) and severe clinical outcome (p = 0.024) was also validated in an independent cohort of Spanish men <60 years of age (p = 0.014). Testosterone was higher in subjects with AR long-polyQ, possibly indicating receptor resistance (p = 0.042 Mann-Whitney U test). Inappropriately low serum testosterone level among carriers of the long-polyQ alleles (p = 0.0004 Mann-Whitney U test) predicted the need for intensive care in COVID-19 infected men. In agreement with the known anti-inflammatory action of testosterone, patients with long-polyQ and age â„60 years had increased levels of CRP (p = 0.018, not accounting for multiple testing). Interpretation: We identify the first genetic polymorphism that appears to predispose some men to develop more severe disease. Failure of the endocrine feedback to overcome AR signaling defects by increasing testosterone levels during the infection leads to the polyQ tract becoming dominant to serum testosterone levels for the clinical outcome. These results may contribute to designing reliable clinical and public health measures and provide a rationale to test testosterone as adjuvant therapy in men with COVID-19 expressing long AR polyQ repeats. Immunomodulatory effects of total intravenous and balanced inhalation anesthesia in patients with bladder cancer undergoing elective radical Cystectomy: preliminary results
<p>Abstract</p> <p>Background</p> <p>Although surgery and anesthesia induce immunesuppression, remains largely unknown whether various anesthetic techniques have different immunosuppressive effects on cancer patients. Therefore, the aim of this study was to investigate the influence of total intravenous anesthesia with target-controlled infusion (TIVA-TCI) and balanced inhalation anesthesia (BAL) on the peri-operative levels of inflammatory cytokines and regulatory T cells (Tregs) in patients with bladder cancer undergoing surgery.</p> <p>Methods</p> <p>Twenty eight consecutive patients with bladder cancer who underwent radical cystectomy were prospectively randomized into two groups to receive TIVA-TCI (nâ=â14) or BAL (nâ=â14). Before the induction of anesthesia (T0), 6â8 hours (T1) post-surgery, and 5 days post-surgery (T2), Tregs and serum levels of interleukin -1beta (IL-1ÎČ), interferon-gamma (IFN-Îł), tumor necrosis factor-alpha (TNF-α), interleukin â2 (IL-2), interleukin â6 (IL-6), and interleukin â10 (IL-10) were measured.</p> <p>Results</p> <p>In the peri-operative period all cancer patients showed a marked and significant increase in IL-6. Moreover, TIVA-TCI patients also showed a higher increase in IFN-Îł, whereas in BAL patients Tregs were reduced by approximately 30% during surgery. The incidence of infections, metastases, and death was similar in both groups.</p> <p>Conclusions</p> <p>The increase in the Th1 response in the TIVA-TCI group and the reduction in Tregs in the BAL group seem to balance the immunosuppressive effect induced by IL-6. Therefore TIVA-TCI and BAL can be both used in major surgery in patients with bladder cancer without worsening the outcome.</p
Recovery from Anesthesia after Robotic-Assisted Radical Cystectomy: Two Different Reversals of Neuromuscular Blockade
During robot-assisted radical cystectomy (RARC), specific surgical conditions (a steep Trendelenburg position, prolonged pneumoperitoneum, effective myoresolution until the final stages of surgery) can seriously impair the outcomes. The aim of the study was to evaluate the incidence of postoperative nausea and vomiting (PONV) and ileus and the quality of cognitive function at the awakening in two groups of patients undergoing different reversals. In this randomized trial, patients that were American Society of Anesthesiologists physical status (ASA) â€III candidates for RARC for bladder cancer were randomized into two groups: In the sugammadex (S) group, patients received 2 mg/kg of sugammadex as reversal of neuromuscolar blockade; in the neostigmine (N) group, antagonization was obtained with neostigmine 0.04 mg/kg + atropine 0.02 mg/kg. PONV was evaluated at 30 min, 6 and 24 h after anesthesia. Postoperative cognitive functions and time to resumption of intestinal transit were also investigated. A total of 109 patients were analyzed (54 in the S group and 55 in the N group). The incidence of early PONV was lower in the S group but not statistically significant (S group 25.9% vs. N group 29%; p = 0.711). The Mini-Mental State test mean value was higher in the S group vs. the N group (1 h after surgery: 29.3 (29; 30) vs. 27.6 (27; 30), p = 0.007; 4 h after surgery: 29.5 (30; 30) vs. 28.4 (28; 30), p = 0.05). We did not observe a significant decrease of the PONV after sugammadex administration versus neostigmine use. The Mini-Mental State test mean value was greater in the S group
Long-term implications of atrial fibrillation in patients with degenerative mitral regurgitation
BACKGROUND Scientific guidelines consider atrial fibrillation (AF) complicating degenerative mitral regurgitation
(DMR) a debated indication for surgery.
OBJECTIVES This study analyzed the prognostic/therapeutic implications of AF at DMR diagnosis and long-term.
METHODS Patients were enrolled in the MIDA (Mitral Regurgitation International Database) registry, which reported
the consecutive, multicenter, international experience with DMR due to flail leaflets echocardiographically diagnosed.
RESULTS Among 2,425 patients (age 67 13 years; 71% male, 67% asymptomatic, ejection fraction 64 10%), 1,646
presented at diagnosis with sinus rhythm (SR), 317 with paroxysmal AD, and 462 with persistent AF. Underlying
clinical/instrumental characteristics progressively worsened from SR to paroxysmal to persistent AF. During follow-up,
paroxysmal and persistent AF were associated with excess mortality (10-year survival in SR and in paroxysmal and
persistent AF was 74 1%, 59 3%, and 46 2%, respectively; p < 0.0001), that persisted 20 years post-diagnosis
and independently of all baseline characteristics (p values <0.0001). Surgery (n Œ 1,889, repair 88%) was associated
with better survival versus medical management, regardless of all baseline characteristics and rhythm (adjusted
hazard ratio: 0.26; 95% confidence interval: 0.23 to 0.30; p < 0.0001) but post-surgical outcome remained affected by
AF (10-year post-surgical survival in SR and in paroxysmal and persistent AF was 82 1%, 70 4%, and 57 3%,
respectively; p < 0.0001).
CONCLUSIONS AF is a frequent occurrence at DMR diagnosis. Although AF is associated with older age and more
severe presentation of DMR, it is independently associated with excess mortality long-term after diagnosis. Surgery is
followed by improved survival in each cardiac rhythm subset, but persistence of excess risk is observed for each type of
AF. Our study indicates that detection of AF, even paroxysmal, should trigger prompt consideration for surgery
