    Identifying Risks of Readmission in Patients with Dementia in SNF for Targeted Community-Based Palliative Consultation (QI644)

    Objectives Describe the HOSPITAL score and apply it to patients with dementia discharged to skilled nursing facility (SNF). Compare risk factors including HOSPITAL score, Charlson Comorbidity Index, and high-risk discharge medications for risk of 60-day readmission from skilled nursing facility for patients with dementia. Identify patients with dementia at SNF who are at-risk for multiple transitions of care who may benefit from SNF-based palliative care consultation. Background Dementia patients are at high risk for preventable hospital readmission. This population could benefit from expert consultation in SNF to reduce readmission rates. Community-based palliative medicine focusing on symptom management and goals of care in post-acute care facilities can lower unnecessary transitions, hospitalization, and emergency department visits; reduce length of stay; and lower Medicare expenditures. Aim Statement Our study focused on determining risk factors associated with readmission from SNF in order to identify patients who would benefit from palliative medicine consultation. Tools used to assess risk included HOSPITAL score, Charlson Comorbidity Index, and determining if patients were on high risk medication defined by Beers Criteria. Methods A descriptive retrospective cohort study was conducted on patients with ICD-10 coding of dementia and discharged to SNF from 2017 to 2018 at the University Health System Hospital in San Antonio, TX. A chart review was conducted to calculate HOSPITAL Score and Charlson Comorbidity Index score. High-risk medication was determined using the 2019 AGS Beers Criteria list. Statistical analysis was conducted to determine if the variables were significant for hospital readmission. Results ICD-10 coding identified 132 patients with dementia discharged to SNF. Ten of these patients were readmitted within 60 days. HOSPITAL score was not predictive in assessing risk for hospital readmission (p=0.528). Hemoglobin at discharge was the only HOSPITAL score variable statistically significant (p=0.012). Statistical analysis is still being conducted on the Charlson Comorbidity Index and high-risk medication. Conclusions and Implications Hospitalization and burdensome transitions of care negatively impact the dementia population and places them at risk for rehospitalization. Our study focused on identifying risk factors to better understand the high readmission rates. Understanding these could identify patients who would benefit from palliative care services to reduce hospital readmission and the negative impact of rehospitalization in dementia patients

    BackgroundCoping Cat (CC), a generic cognitive-behavioral intervention for anxiety disorders in children and young people, is recommended in the United Kingdom for social anxiety disorder (SAD), generalized anxiety disorder, separation anxiety (SA), and specific phobias (SP), with disorder-specific approaches generally favored in treatment of anxiety disorders in adults.ObjectivesTo compare CC with disorder-specific cognitive-behavioral therapy (CBT) interventions based on anxiety-related treatment outcomes.Study SelectionPrimary research articles describing treatment of children and young people aged 7–17 for SAD, generalized anxiety disorder, SA, and SP, using CC or disorder-specific CBT.ResultsTen studies implemented CC and four implemented disorder-specific CBT. One study compared CC with a disorder-specific approach. There was a lack of data to support use of CC in treatment of SPs. However, CC was equally effective as disorder-specific treatments for SA and SAD.ConclusionsA lack of data exists for disorder-specific CBT interventions compared to CC

    The Plant Ontology: A common reference ontology for plants

    The Plant Ontology (PO) (http://www.plantontology.org) (Jaiswal et al., 2005; Avraham et al., 2008) was designed to facilitate cross-database querying and to foster consistent use of plant-specific terminology in annotation. As new data are generated from the ever-expanding list of plant genome projects, the need for a consistent, cross-taxon vocabulary has grown. To meet this need, the PO is being expanded to represent all plants. This is the first ontology designed to encompass anatomical structures as well as growth and developmental stages across such a broad taxonomic range. While other ontologies such as the Gene Ontology (GO) (The Gene Ontology Consortium, 2010) or Cell Type Ontology (CL) (Bard et al., 2005) cover all living organisms, they are confined to structures at the cellular level and below. The diversity of growth forms and life histories within plants presents a challenge, but also provides unique opportunities to study developmental and evolutionary homology across organisms

    Breakfast glycaemic index and exercise: combined effects on adolescents' cognition

    The aim of the present study was to examine the combined effects of breakfast glycaemic index (GI) and a mid-morning bout of exercise on adolescents’ cognitive function. Participants were randomly allocated to a high or low GI breakfast group in a mixed research design, where each participant completed two experimental trials (exercise and resting). Forty-two adolescents (12.4±0.5 years old), undertook a bout of exercise (ten repeats of level one of the multi-stage fitness test; exercise trial) or continued to rest (resting trial) following consumption of either a high or low GI breakfast. A battery of cognitive function tests (visual search test, Stroop test and Sternberg paradigm) was completed 30 min before and 45 min following the exercise. Average heart rate during exercise was 170±15 beats.min-1. On the complex level of the Stroop test, response times improved across the morning following the low GI breakfast on both the exercise and resting trials, though the improvement was greatest on the exercise trial. However, response times only improved on the resting trial following the high GI breakfast (p = 0.012). On the 5 letter level of the Sternberg paradigm, response times improved across the morning following the low GI breakfast (regardless of exercise) and only on the exercise trial following the high GI breakfast (p = 0.019). The findings of the present study suggest that the combined effects of breakfast GI and exercise in adolescents depend upon the component of cognitive function examined. A low GI breakfast and mid-morning bout of exercise were individually beneficial for response times on the Sternberg paradigm, whereas they conferred additional benefits for response times on the Stroop test

    The Plant Ontology facilitates comparisons of plant development stages across species

    The Plant Ontology (PO) is a community resource consisting of standardized terms, definitions, and logical relations describing plant structures and development stages, augmented by a large database of annotations from genomic and phenomic studies. This paper describes the structure of the ontology and the design principles we used in constructing PO terms for plant development stages. It also provides details of the methodology and rationale behind our revision and expansion of the PO to cover development stages for all plants, particularly the land plants (bryophytes through angiosperms). As a case study to illustrate the general approach, we examine variation in gene expression across embryo development stages in Arabidopsis and maize, demonstrating how the PO can be used to compare patterns of expression across stages and in developmentally different species. Although many genes appear to be active throughout embryo development, we identified a small set of uniquely expressed genes for each stage of embryo development and also between the two species. Evaluating the different sets of genes expressed during embryo development in Arabidopsis or maize may inform future studies of the divergent developmental pathways observed in monocotyledonous versus dicotyledonous species. The PO and its annotation databasemake plant data for any species more discoverable and accessible through common formats, thus providing support for applications in plant pathology, image analysis, and comparative development and evolution
