    NH3 and NH2 in the coma of Comet Brorsen-Metcalf

    Evidence consistent with NH3 ice in the nucleus of Comet Brorsen-Metcalf as the source of the NH2 observed in the comet coma is presented. The distribution of NH2 is symmetric and shows no evidence for jet structure at the 3-sigma significance level above background emission. An azimuthal average of the NH2 image produces an NH2 surface brightness profile for Comet Brorsen-Metcalf which yields a factor of about-10 improvement in the signal-to-noise ratio over previous 1D long-slit NH2 observations, and provides a significant constraint on the NH2 photodissociation time scale in comets. A Monte Carlo simulation of the comet coma, assuming that NH2 is the primary source of NH2, is described and compared with the observations. For an observed production rate, Q(H2O) is approximately equal to 7 x 10 exp 28 molecules/s, collisional effects on the NH3 and NH2 outflow had at most an approximately 10-percent effect on the NH2 surface brightness profile. Because Comet Brorsen-Metcalf showed no significant dust or gas production rate variability, it is argued that steady state conditions best match the comet at the time of the observations

    Two-level system with a thermally fluctuating transfer matrix element: Application to the problem of DNA charge transfer

    Charge transfer along the base-pair stack in DNA is modeled in terms of thermally-assisted tunneling between adjacent base pairs. Central to our approach is the notion that tunneling between fluctuating pairs is rate-limited by the requirement of their optimal alignment. We focus on this aspect of the process by modeling two adjacent base pairs in terms of a classical damped oscillator subject to thermal fluctuations as described by a Fokker-Planck equation.     BK virus associated meningoencephalitis in an AIDS patient treated with HAART

    A severely immune-suppressed AIDS patient was suspected of suffering from BK virus (BKV) meningoencephalitis, after being studied for common causes of neurological complications of co-infectious origin. Polymerase chain reaction (PCR) and sequence analysis of cerebrospinal fluid and brain samples, confirmed the presence of BKV. His clinical condition improved along with the regression of brain lesions, after modifications on his antiretroviral regime. Five months after discharge, the patient was readmitted because of frequent headaches, and a marked inflammatory reaction was evidenced by a new magnetic resonance imaging (MRI). The symptoms paralleled a rising CD4+ lymphocyte count, and immune reconstitution syndrome was suspected. This is the first non-postmortem report of BKV meningoencephalitis in an AIDS patient, showing clinical and radiographic improvement solely under HAART

    The Intricacies of Identifying Equatorial Waves

    Equatorial waves (EWs) are synoptic- to planetary-scale propagating disturbances at low latitudes with periods from a few days to several weeks. Here, this term includes Kelvin waves, equatorial Rossby waves, mixed Rossby–gravity waves, and inertio-gravity waves, which are well described by linear wave theory, but it also other tropical disturbances such as easterly waves and the intraseasonal Madden–Julian Oscillation with more complex dynamics. EWs can couple with deep convection, leading to a substantial modulation of clouds and rainfall. EWs are amongst the dynamic features of the troposphere with the longest intrinsic predictability, and models are beginning to forecast them with an exploitable level of skill. Most of the methods developed to identify and objectively isolate EWs in observations and model fields rely on (or at least refer to) the adiabatic, frictionless linearized primitive equations on the sphere or the shallow-water system on the equatorial -plane. Common ingredients to these methods are zonal wave-number–frequency filtering (Fourier or wavelet) and/or projections onto predefined empirical or theoretical dynamical patterns. This paper gives an overview of six different methods to isolate EWs and their structures, discusses the underlying assumptions, evaluates the applicability to different problems, and provides a systematic comparison based on a case study (February 20–May 20, 2009) and a climatological analysis (2001–2018). In addition, the influence of different input fields (e.g., winds, geopotential, outgoing long-wave radiation, rainfall) is investigated. Based on the results, we generally recommend employing a combination of wave-number–frequency filtering and spatial-projection methods (and of different input fields) to check for robustness of the identified signal. In cases of disagreement, one needs to carefully investigate which assumptions made for the individual methods are most probably not fulfilled. This will help in choosing an approach optimally suited to a given problem at hand and avoid misinterpretation of the results

    Awareness and perceptions of electroconvulsive therapy among psychiatric patients: a cross-sectional survey from teaching hospitals in Karachi, Pakistan

    <p>Abstract</p> <p>Background</p> <p>Electroconvulsive therapy (ECT) is shown to be effective in many psychiatric illnesses, but its distorted projection by the Pakistani media and its unregulated use by many physicians across the country have adversely affected its acceptability. Given this situation we aimed to assess the awareness and perceptions regarding ECT as a treatment modality among the psychiatric patients.</p> <p>Methods</p> <p>This was a questionnaire based cross-sectional study carried out at 2 tertiary care hospitals in Karachi, Pakistan.</p> <p>Results</p> <p>We interviewed 190 patients of which 140 were aware of ECT. The study showed that the level of education had a significant impact on the awareness of ECT (p = 0.009). The most common source of awareness was electronic and print media (38%), followed by relatives (24%) and doctors (23%). Physical injuries (42%) and neurological (12%) and cognitive disturbances (11%) were the commonly feared side effects. The most popular belief about ECT was that it was a treatment of last resort (56%). Thirty-nine percent thought that ECT could lead to severe mental and physical illness and 37% considered it inhumane. Patients' willingness to receive ECT was dependant on whether or not they were convinced of its safety (p = 0.001) and efficacy (p = 0.0001).</p> <p>Conclusion</p> <p>We identified a serious lack of dissemination of information regarding ECT by the psychiatrists and the mental health care providers. This may be the result of an inadequate postgraduate training in Pakistan or just a lack of concern about the mentally ill patients. The media seemed to be the major source of information for our patients. We also saw the prevalence of a variety of myths regarding ECT in our society, which we feel may be responsible for the patients' adverse attitudes. Given the widespread applicability of ECT there is a dire need to dispel these misconceptions and improve its acceptability.</p
