    Financiamento do SUS e Covid-19: histórico, participações federativas e respostas à pandemia

    This essay discusses financial resources for Brazil’s Unified Health System (SUS) in a historical and future perspective, considering the health and economic challenges imposed by the Covid-19 pandemic. As always and more than ever, we need to discuss SUS financing. This is aggravated in intensity by the context of chronic economic recession that the country has been experiencing in 2015 and 2016, followed by  stagnation in the following years and accentuated in urgency by the recession in the second quarter of 2020 and that is expected to deepen in the coming years. The pandemic happens in a context of diminishing federal participation in SUS financing, little space for states to expand their sources of financing and several problems of coordination between federal entities. We argue that facing the pandemic necessarily involves an expansion of resources for financing SUS and it has a great dependence on the performance of the federal government. However, even with the urgency that the pandemic demanded, the new resources approved by the National Congress for public health system has been quite slow. We conclude that the prospects do not point to prioritizing the SUS and expanding its funding in the post-pandemic period.Esse ensaio discute o financiamento do Sistema Único de Saúde (SUS) em perspectiva histórica e futura, considerando os desafios sanitários e econômicos impostos pela pandemia de Covid-19. Como sempre e mais do que nunca, precisamos discuti-lo. Essa necessidade cresce em premência a partir da recessão econômica que o país viveu em 2015 e 2016, pela estagnação da economia nos anos seguintes e pela recessão que já surge no segundo trimestre de 2020 com previsão de aprofundar-se nos próximos anos. A pandemia acontece em um contexto de redução da participação federal no financiamento, pouco espaço para estados ampliarem suas fontes de financiamento e problemas de coordenação entre os entes federativos. No caso do financiamento das ações e serviços públicos de saúde (ASPS) para enfrentamento à pandemia, argumentamos nesse texto, que ele passa necessariamente pela ampliação dos recursos alocados ao SUS, com grande dependência da atuação do governo federal. Contudo, nem com a urgência que demanda a pandemia houve rapidez na liberação e execução dos novos recursos aprovados pelo Congresso Nacional. Concluímos que as perspectivas não apontam para uma priorização do SUS e ampliação do seu financiamento no período pós-pandêmico

    DEVELOPMENT AND VALIDATION OF TECHNOLOGY FOR TEACHING BASIC LIFE SUPPORT IN CARDIO-RESPIRATORY ARREST

    Objective: to develop and validate a multimedia application on a mobile platform for teaching basic support in cardiac arrest to undergraduate health care students.Method: methodological study conducted in two stages: development of the application and content validation by experts. The application was developed based on Contextualized Instructional Design, following the stages of analysis, design, development, and implementation. For the content validation, 11 experts evaluated the application regarding relevance, clarity, coherence, information, presentation, navigability, accessibility, and design.Results: The SBVida application was developed for mobile devices on the Android platform. An overall Content Validity Index of 0.87 was achieved.Conclusion: The SBVida application was developed and validated and can contribute to the preparation and further training of students and professionals from Nursing and other health fields

    Uso integral da uva Isabel na elaboração de novos produtos com valor nutricional e potencial funcional

    The benefits of grape consumption are widely recognized and mostly due to phenolic compounds. These beneficial effects will depend on the bioaccessibility of these compounds on grape and its derivatives. This study elaborated two formulations of Isabel grape preparation: PAX (with agave and xylitol) and PS (with sucrose); and two formulations of Isabel grape flour: FAX (from the PAX process residues) and the FS (from the PS process residues). The products were analyzed regarding their nutritional and antioxidant properties; their phenolic compounds’ bioaccessibility was also verified through a simulated digestion model. The preparation and flour exhibited relevant sugars levels (10.83-49.71 g 100 g-1). Those produced with natural sweeteners had a reduction in sugar concentration of 51% and 29% for preparation and flour, respectively, compared to formulations with sucrose, with the high fiber content in the flour is being further highlighted (20.14-21.95 g 100 g-1). The catechin (2.37-28.11 mg 100 g-1) was the most bioaccessible compound (22% to 168%), which together with the caftaric acid (2.31-69.43 mg 100 g-1) and malvidin 3-glucoside (8.65-16.47 mg 100 g-1) represent the compounds observed in greater quantity. The preparations showed higher bioaccessibility regarding grapes and flours for most of the phenolic compounds. Furthermore, the products elaborated presented higher values of anthocyanins and antioxidant activity than the in natura grape, highlighting the beneficial effect of grape processing.Os benefícios do consumo da uva já são amplamente reconhecidos e devem-se em grande parte aos compostos fenólicos. Esses efeitos benéficos irão depender da bioacessibilidade desses compostos na uva e em seus derivados. Foram elaboradas duas formulações de preparado de uva Isabel: PAX (com agave e xilitol) e PS (com sacarose); e duas formulações de farinha de uva Isabel: FAX (a partir do resíduo do processamento do PAX) e FS (a partir do resíduo do processamento do PS). Os produtos foram analisados quanto às suas propriedades nutricionais e antioxidantes, e a bioacessibilidade de seus compostos fenólicos foi verificada por meio de um modelo de digestão simulada. O preparado e a farinha apresentaram níveis relevantes de açúcares (10,83-49,71 g 100 g-1), sendo que aqueles produzidos com adoçantes naturais tiveram uma redução na concentração dos açúcares de 51% e 29% para o preparado e a farinha, respectivamente, em comparação com formulações com sacarose, destacando-se também o alto teor de fibras na farinha (20,14-21,95 g 100 g-1). A catequina (2,37-28,11 mg 100 g-1) foi o composto mais bioacessível (22% a 168%), que juntamente com o ácido caftárico (2,31-69,43 mg 100 g-1) e o malvidina 3-glicosídeo (8,65-16,47 mg 100 g-1) representam os compostos observados em maior quantidade. O preparado teve maior bioacessibilidade tanto em relação à uva quanto à farinha para a maioria dos compostos fenólicos.     Exploring the leishmanicidal potential of terpenoids: a comprehensive review on mechanisms of cell death

    Leishmaniasis is a neglected tropical disease with a wide spectrum of clinical manifestations, ranging from visceral to cutaneous, with millions of new cases and thousands of deaths reported each year. The species of Leishmania and the immune response of the host determine the severity of the disease. Leishmaniasis remains challenging to diagnose and treat, and there is no vaccine available. Several studies have been conducted on the use of herbal medicines for the treatment of leishmaniasis. Natural products can provide an inexhaustible source of chemical diversity with therapeutic potential. Terpenes are a class of natural products derived from a single isoprene unit, a five-carbon compound that forms the basic structure of isoprenoids. This review focuses on the most important and recent advances in the treatment of parasites of the genus Leishmania with different subclasses of terpenes. Several mechanisms have been proposed in the literature, including increased oxidative stress, immunomodulatory role, and induction of different types of parasite cell death. However, this information needs to be brought together to provide an overview of how these compounds can be used as therapeutic tools for drug development and as a successful adjuvant strategy against Leishmania sp

    Pervasive gaps in Amazonian ecological research

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.
