Elder Abuse in Canada: Dimensions and Policy Responses

Elder abuse and neglect continues to be a gray area when it comes to convicting perpetrators such as family, friends, strangers, and caregivers who commit any form of physical, psychological, financial, neglect, or sexual abuse towards an elder. This is due to the legal definition being vague and non-transparent. The legal and health systems rely on two different definitions of what is deemed to be elder abuse and neglect in Canada when reviewing or assessing allegations of such abuse. Elder abuse and neglect increased throughout the COVID-19 pandemic, during which Ontario and the rest of Canada experienced staffing shortages in long-term care homes and for those cared for at home. Elder abuse in Indigenous communities poses special problems, in particular that of spiritual abuse that occurs when an Indigenous elder is forced to practice and develop religious beliefs other than their own. The law in Canada, including the Criminal Code, has proven to be an ineffective mechanism to combat elder abuse

Use of the WHO Access, Watch, and Reserve classification to define patterns of hospital antibiotic use (AWaRe): an analysis of paediatric survey data from 56 countries

BACKGROUND: Improving the quality of hospital antibiotic use is a major goal of WHO's global action plan to combat antimicrobial resistance. The WHO Essential Medicines List Access, Watch, and Reserve (AWaRe) classification could facilitate simple stewardship interventions that are widely applicable globally. We aimed to present data on patterns of paediatric AWaRe antibiotic use that could be used for local and national stewardship interventions. METHODS: 1-day point prevalence survey antibiotic prescription data were combined from two independent global networks: the Global Antimicrobial Resistance, Prescribing, and Efficacy in Neonates and Children and the Global Point Prevalence Survey on Antimicrobial Consumption and Resistance networks. We included hospital inpatients aged younger than 19 years receiving at least one antibiotic on the day of the survey. The WHO AWaRe classification was used to describe overall antibiotic use as assessed by the variation between use of Access, Watch, and Reserve antibiotics, for neonates and children and for the commonest clinical indications. FINDINGS: Of the 23 572 patients included from 56 countries, 18 305 were children (77·7%) and 5267 were neonates (22·3%). Access antibiotic use in children ranged from 7·8% (China) to 61·2% (Slovenia) of all antibiotic prescriptions. The use of Watch antibiotics in children was highest in Iran (77·3%) and lowest in Finland (23·0%). In neonates, Access antibiotic use was highest in Singapore (100·0%) and lowest in China (24·2%). Reserve antibiotic use was low in all countries. Major differences in clinical syndrome-specific patterns of AWaRe antibiotic use in lower respiratory tract infection and neonatal sepsis were observed between WHO regions and countries. INTERPRETATION: There is substantial global variation in the proportion of AWaRe antibiotics used in hospitalised neonates and children. The AWaRe classification could potentially be used as a simple traffic light metric of appropriate antibiotic use. Future efforts should focus on developing and evaluating paediatric antibiotic stewardship programmes on the basis of the AWaRe index. FUNDING: GARPEC was funded by the PENTA Foundation. Frappier is satisfied with a tinted laconic note of condescension : "this long poem of adventures, which with the reputation to be hateful [...], brings back for us towards the epic of Rainouart. To disavow is indeed the son of Maillefer and Florentine, consequently the grandson of Rainouart. The theatre of its exploits is Sicily and Greece. From its marriage without originality with Sarrasine Ydoine (it is not a pun) is born Tancrède, one of the heroes of the first crusade. "Forty years later, the song still does not cause a frank passion : M. Tyssens and J. Wathelet-Willem qualify Enfances Renier of "epigone deprived of any originality". Kommunalt mottagande av invandrare och flyktingar – 40 år av utmaningar och erfarenheter

Artikeln handlar om uppkomsten och utvecklingen av ett nytt kommunalt ansvarsområde – inkorporeringen av invandrare och flyktingar – från mitten av 1960-talet och fram till 2010. Materialet består i huvudsak av fallstudier från åtta kommuner. Resultaten visar att kommunernas arbete har kretsat kring frågor om huvudmannaskap, verksamhetsinriktning, personal- och kompetensstruktur samt målgrupper, vilka sammantaget utgör vad jag kallar organisationernas ”inkorporeringspraktiker”. Runt dessa frågor har det kommunala arbetet kretsat utan att finna bestående svar på frågan om hur inkorporeringen av invandrare och flyktingar ska organiseras
