    The Clinical Efficacy of Mindfulness-Based Treatments for Alcohol and Drugs Use Disorders: A Meta-Analytic Review of Randomized and Nonrandomized Controlled Trials

    Introduction: The current study aims to evaluate if and to what extent mindfulness-based interventions (MBIs) could promote an incremental effectiveness compared to interventions usually provided in clinical practice to treat Alcohol and Drugs Use Disorders. In line with this aim, we accomplished a meta-analytic review of randomized and nonrandomized controlled trials, considering primary and secondary outcomes that comprehensively operationalize treatment efficacy. Methods: We conducted the online research up to August 31st 2017. Adequate procedures for Cohen's d computation were applied. Heterogeneity indexes, moderators, bias of publication, and Orwin's fail-safe number were also estimated. Results: Thirty-seven studies were included (n = 3,531 patients). We observed null effect sizes for attrition rate and overall mental health. Small effect sizes were detected in abstinence, levels of perceived stress, and avoidance coping strategies. Moderate effect sizes were revealed in anxiety and depressive symptoms. Large effect sizes were associated to levels of perceived craving, negative affectivity, and post-traumatic symptoms. Conclusion: MBIs seemed to show clinically significant advantages compared to other clinical approaches in relation to specific primary and secondary outcomes. Conversely, treatment retention was independent of the therapeutic approach

    API Comparison of CPU-To-GPU Command Offloading Latency on Embedded Platforms (Artifact)

    High-performance heterogeneous embedded platforms allow offloading of parallel workloads to an integrated accelerator, such as General Purpose-Graphic Processing Units (GP-GPUs). A time-predictable characterization of task submission is a must in real-time applications. We provide a profiler of the time spent by the CPU for submitting stereotypical GP-GPU workload shaped as a Deep Neural Network of parameterized complexity. The submission is performed using the latest API available: NVIDIA CUDA, including its various techniques, and Vulkan. Complete automation for the test on Jetson Xavier is also provided by scripts that install software dependencies, run the experiments, and collect results in a PDF report

    Novel Methodologies for Predictable CPU-To-GPU Command Offloading

    There is an increasing industrial and academic interest towards a more predictable characterization of real-time tasks on high-performance heterogeneous embedded platforms, where a host system offloads parallel workloads to an integrated accelerator, such as General Purpose-Graphic Processing Units (GP-GPUs). In this paper, we analyze an important aspect that has not yet been considered in the real-time literature, and that may significantly affect real-time performance if not properly treated, i.e., the time spent by the CPU for submitting GP-GPU operations. We will show that the impact of CPU-to-GPU kernel submissions may be indeed relevant for typical real-time workloads, and that it should be properly factored in when deriving an integrated schedulability analysis for the considered platforms. This is the case when an application is composed of many small and consecutive GPU compute/copy operations. While existing techniques mitigate this issue by batching kernel calls into a reduced number of persistent kernel invocations, in this work we present and evaluate three other approaches that are made possible by recently released versions of the NVIDIA CUDA GP-GPU API, and by Vulkan, a novel open standard GPU API that allows an improved control of GPU command submissions. We will show that this added control may significantly improve the application performance and predictability due to a substantial reduction in CPU-to-GPU driver interactions, making Vulkan an interesting candidate for becoming the state-of-the-art API for heterogeneous Real-Time systems. Our findings are evaluated on a latest generation NVIDIA Jetson AGX Xavier embedded board, executing typical workloads involving Deep Neural Networks of parameterized complexity

    The empirical research has recently focused attention on pathological and non pathological gambling among elderly population, showing a complex phenomenon sustained by several aspects which include sociological, psychological and biological determinants. Nevertheless, there no studies in literature which explore gambling habits in an Italian population over 65 years of age. In line with the previous aim, an author of the current work built a 9-item self-report instrument called Senior Problem Gambling Questionnaire (SPGQ). SPGQ mainly explores two different aspects of gambling in elderly population: a) level of awareness related to negative consequences associated with gambling; b) severity of gambling behaviors. SPGQ was administered to 387 subjects. 353 individuals completed the screening evaluation. Gender was homogeneously distributed among subjects. 85.6% of sample showed good level of negative consequences awareness associated with gambling. 22.2% of subjects affirmed to engage in gambling behaviors at least once a week. 4.4% of individuals affirmatively answered to both pathological gambling items. Although men seemed to engage in gambling behaviors more frequently than women, gender was homogeneously distributed among individuals who engage in gambling behaviors at least two times a week. Eventually, this is the first study that shows a modest albeit epidemiologic relevant portion of Italian individuals over 65 years of age which might be pathological gamblers

    Neurobiological Dysfunctional Substrates for the Self-Medication Hypothesis in Adult Individuals with Attention-Deficit Hyperactivity Disorder and Cocaine Use Disorder:A Fluorine-18-Fluorodeoxyglucose Positron Emission Tomography Study

    Objectives: Attention-deficit hyperactivity disorder (ADHD) in adulthood shows high co-occurrence rates with cocaine use disorder (CoUD). The self-medication hypothesis (SMH) provides a theoretical explanation for this comorbidity. This study investigates the neurobiological mechanisms that could support SMH in adult patients with attention-deficit hyperactivity disorder with cocaine use disorder (ADHD–CoUD).Materials and Methods: We included 19 ADHD–CoUD patients (84.2% male; age: 32.11 years [7.18]) and 16 CoUD patients (68.7% male; age: 36.63 years [8.12]). All subjects underwent a fluorine-18-fluorodeoxyglucose positron emission tomography (18F-FDG PET) brain scan. We tested brain metabolism differences between ADHD–CoUD and CoUD patients using voxel-based and regions of interest (ROIs)-based analyses. The correlation between dependence/abstinence duration and regional brain metabolism was also assessed in the two groups. Lastly, we investigated the integrity of brain metabolic connectivity of mesocorticolimbic and nigrostriatal dopaminergic systems, and large-scale brain networks involved in ADHD and addictions.Results: The voxel-wise and ROIs-based approaches showed that ADHD–CoUD patients had a lower metabolism in the thalamus and increased metabolism in the amygdala and parahippocampus, bilaterally, than CoUD subjects and healthy controls (HCs). Metabolism in the thalamus negatively correlated with years of dependence in ADHD–CoUD patients. Moreover, connectivity analyses revealed that ADHD–CoUD patients had a more preserved metabolic connectivity than CoUD patients in the dopaminergic networks and large-scale networks involved in self-regulation mechanisms of attention and behaviors (i.e., anterior default mode network [ADMN], executive network [ECN], and anterior salience network [aSAN]).Conclusions: We demonstrated distinct neuropathological substrates underlying substance-use behaviors in ADHD–CoUD and CoUD patients. Furthermore, we provided neurobiological evidence in support of SMH, demonstrating that ADHD–CoUD patients might experience short-term advantages of cocaine assumption (i.e., compensation of dopaminergic deficiency and related cognitive-behavioral deficits).</p

    Failure of the anticoagulant therapy and psychological distress : Still far from a bridge

    Background: The procoagulant stress response reflects part of a beneficial adaptation of the organism to environmental threats, but a protracted procoagulant state generates a thrombotic risk. Atrial fibrillation (AF) is the most common arrhythmia in the general population. Patients with AF have a higher risk of thromboembolic events and stroke, therefore they are treated with long-term oral anticoagulant (OAC) therapy. The aim of this study is to evaluate if there is any association between psychological distress and clinically unexplained variations of the International Normalized Ratio (INR), that is the index used to monitor both thromboembolic and bleeding risk in the case of patients under OAC therapy. Methods: Fifty-eight patients (men = 27; women = 31; mean age = 74.98) were recruited. The sample was divided according to the recognition (or not) of the reason why the INR was subtherapeutic ( < 2) and classified as "Known Reasons" (KR = 32.8%) and "Unknown Reasons" (UR = 67.2%). Psychological assessment included the following dimensions: symptoms of anxiety and depression, perceived stress, emotional regulation strategies, and alexithymia. Results: Considering Mann-Whitney test results, no significant difference was found in the scores of anxiety, depression, stress, and emotional regulation strategies. With regard to alexithymia, UR patients are characterized by a moderate tendency to an outward-oriented thinking (r = 0.25). Conclusion: A clear role for the detected psychological factors in determining abnormal INR range in patients under OAC therapy could not be found. Further studies are needed to support our findings, if possible exploring factors other than psychological distress and the related emotion regulation strategies

    Renal amyloid‐A amyloidosis in cats: Characterization of proteinuria and biomarker discovery, and associations with kidney histology

    BackgroundAmyloid A (AA) amyloidosis is a protein misfolding disease arising from serum amyloid A (SAA). Systemic AA amyloidosis recently was shown to have a high prevalence in shelter cats in Italy and was associated with azotemia and proteinuria.ObjectivesInvestigate urine protein profiles and diagnostic biomarkers in cats with renal AA amyloidosis.AnimalsTwenty‐nine shelter cats.MethodsCase‐control study. Cats with renal proteinuria that died or were euthanized between 2018 and 2021 with available necropsy kidney, liver and spleen samples, and with surplus urine collected within 30 days before death, were included. Histology was used to characterize renal damage and amyloid amount and distribution; immunohistochemistry was used to confirm AA amyloidosis. Urine protein‐to‐creatinine (UPC) and urine amyloid A‐to‐creatinine (UAAC) ratios were calculated, and sodium dodecyl sulfate‐agarose gel electrophoresis (SDS‐AGE) and liquid chromatography‐mass spectrometry (LC‐MS) of proteins were performed.ResultsTwenty‐nine cats were included. Nineteen had AA amyloidosis with renal involvement. Cats with AA amyloidosis had a higher UPC (median, 3.9; range, 0.6‐12.7 vs 1.5; 0.6‐3.1; P = .03) and UAAC ratios (median, 7.18 × 10−3^{−3}; range, 23 × 10−3^{−3}‐21.29 × 10−3^{−3} vs 1.26 × 10−3^{−3}; 0.21 × 10−3^{−3}‐6.33 × 10−3^{−3}; P = .04) than unaffected cats. The SDS‐AGE identified mixed‐type proteinuria in 89.4% of cats with AA amyloidosis and in 55.6% without AA amyloidosis (P = .57). The LC‐MS identified 63 potential biomarkers associated with AA amyloidosis (P < .05). Among these, urine apolipoprotein C‐III was higher in cats with AA amyloidosis (median, 1.38 × 107^{7}; range, 1.85 × 105^{5}‐5.29 × 107^{7} vs 1.76 × 106^{6}; 0.0 × 100^{0}‐1.38 × 107^{7}; P = .01). In the kidney, AA‐amyloidosis was associated with glomerulosclerosis (P = .02) and interstitial fibrosis (P = .05).Conclusions and Clinical ImportanceRenal AA amyloidosis is associated with kidney lesions, increased proteinuria and increased urine excretion of SAA in shelter cats. Additional studies are needed to characterize the role of lipid transport proteins in the urine of affected cats

    Circulating Cell-Free DNA in Dogs with Mammary Tumors: Short and Long Fragments and Integrity Index

    Circulating cell-free DNA (cfDNA) has been considered an interesting diagnostic/prognostic plasma biomarker in tumor-bearing subjects. In cancer patients, cfDNA can hypothetically derive from tumor necrosis/apoptosis, lysed circulating cells, and some yet unrevealed mechanisms of active release. This study aimed to preliminarily analyze cfDNA in dogs with canine mammary tumors (CMTs). Forty-four neoplastic, 17 non-neoplastic disease-bearing, and 15 healthy dogs were recruited. Necrosis and apoptosis were also assessed as potential source of cfDNA on 78 CMTs diagnosed from the 44 dogs. The cfDNA fragments and integrity index significantly differentiated neoplastic versus non-neoplastic dogs (P<0.05), and allowed the distinction between benign and malignant lesions (P<0.05). Even if without statistical significance, the amount of cfDNA was also affected by tumor necrosis and correlated with tumor size and apoptotic markers expression. A significant (P<0.01) increase of Bcl-2 in malignant tumors was observed, and in metastatic CMTs the evasion of apoptosis was also suggested. This study, therefore, provides evidence that cfDNA could be a diagnostic marker in dogs carrying mammary nodules suggesting that its potential application in early diagnostic procedures should be further investigated

    Multiplicity dependence of jet-like two-particle correlations in p-Pb collisions at sNN\sqrt{s_{NN}} = 5.02 TeV

    Two-particle angular correlations between unidentified charged trigger and associated particles are measured by the ALICE detector in p-Pb collisions at a nucleon-nucleon centre-of-mass energy of 5.02 TeV. The transverse-momentum range 0.7 <pT,assoc<pT,trig< < p_{\rm{T}, assoc} < p_{\rm{T}, trig} < 5.0 GeV/cc is examined, to include correlations induced by jets originating from low momen\-tum-transfer scatterings (minijets). The correlations expressed as associated yield per trigger particle are obtained in the pseudorapidity range ∣η∣<0.9|\eta|<0.9. The near-side long-range pseudorapidity correlations observed in high-multiplicity p-Pb collisions are subtracted from both near-side short-range and away-side correlations in order to remove the non-jet-like components. The yields in the jet-like peaks are found to be invariant with event multiplicity with the exception of events with low multiplicity. This invariance is consistent with the particles being produced via the incoherent fragmentation of multiple parton--parton scatterings, while the yield related to the previously observed ridge structures is not jet-related. The number of uncorrelated sources of particle production is found to increase linearly with multiplicity, suggesting no saturation of the number of multi-parton interactions even in the highest multiplicity p-Pb collisions.
