Carapecchia's intervention at the Inquisitor's Palace, 1733-34
The Inquisitor's Palace, sited in the heart of the historical
city of Vittoriosa, is one of the very few surviving palaces
of its kind which in the early modern period could be
found all over Europe and South America. Many simply
succumbed to the ravages of time or else were victims
of the reactionary power unleashed by the French
Revolution. Fortunately, the Maltese Inquisitor's Palace,
throughout its five centuries of history, always hosted
high-ranking officials representing the main powers on
the island, who ensured its survival. The Palace also
managed to survive through the ordeal of the Second
World War and the threat of modern development, and
although much has been changed in its structure by its
successive occupants, it is today an architectural gem,
representative of the chequered history of the Maltese
islands.
Yet, until now, our knowledge of the Palace arid its
history remained very incomplete, and all one can find
is small pieces of information scattered here and there
in various sources. The history of the Palace received a
boost last year with the publication of three previously
unknown plans of the building discovered in Rome.1
Yet much still remains to be done in foreign as well as
(as in this case) in local archives, in order to fully
appreciate the history and vicissitudes of the Inquisitor's
Palace, especially when placed in a wider Maltese and
European context.peer-reviewe

Associations of awake prone positioning-induced changes in physiology with intubation: An international prospective observational study in patients with acute hypoxemic respiratory failure related to COVID-19
INTRODUCTION: Awake prone positioning has the potential to improve oxygenation and decrease respiratory rate, potentially reducing the need for intubation in patients with acute hypoxemic respiratory failure. We investigated awake prone positioning-induced changes in oxygenation and respiratory rate, and the prognostic capacity for intubation in patients with COVID-19 pneumonia.
METHODS: International multicenter prospective observation study in critically ill adult patients with COVID-19 receiving supplemental oxygen. We collected data on oxygenation and respiratory rate at baseline, and at 1 h after being placed in prone positioning. The combined primary outcome was oxygenation and respiratory rate at 1 h. The secondary endpoint was treatment failure, defined as need for intubation within 24 h of start of awake prone positioning.
RESULTS: Between March 27th and November 2020, 101 patients were enrolled of which 99 were fully analyzable. Awake prone positioning lasted mean of 3 [2-4] h. In 77 patients (77.7%), awake prone positioning improved oxygenation, and in 37 patients (54.4%) it decreased respiratory rate. Twenty-nine patients (29.3%) were intubated within 24 h. An increase in SpO
CONCLUSIONS: Awake prone positioning improves oxygenation in the majority of patients, and decreases respiratory rate in more than half of patients with acute hypoxemic respiratory failure caused by COVID-19. One in three patients need intubation within 24 h. Awake prone position-induced changes in oxygenation and respiratory rate have prognostic capacity for intubation within 24 h

Collagen and Elastic Fibers in Odontogenic Entities: Analysis Using Light and Confocal Laser Microscopic Methods
Dentigerous cyst (DC) and keratocystic odontogenic tumor (KOT) are odontogenic lesions arising from epithelial elements, such as those observed in dental follicles (DF), that have been part of the tooth forming apparatus. These lesions show different clinical and histological characteristics, as well as distinct biological behavior. This study aimed to qualify and quantify collagen and elastic fibers by means of histochemical techniques with light and confocal laser microscopic methods in three odontogenic entities. Eleven DF, 13 DC (n=10 with inflammation, n=3 without inflammation) and 13 KOT were processed to the following techniques: Hematoxylin and Eosin, Masson’s Trichrome, Picrosirius, Direct Blue, and Orcein. DF and DC without inflammation exhibited collagen with similar characteristics: no parallel pattern of fiber orientation, thick fibers with dense arrangement, and absence of distinct layers. A comparison between DC with inflammation and KOT revealed similar collagen organization, showing distinct layers: thin collagen fibers with loose arrangement near the epithelium and thick fibers with dense arrangement in distant areas. The only difference found was that KOT exhibited a parallel collagen orientation in relation to the odontogenic epithelia. It may be suggested that the connective tissue of DC is a reactive tissue, inducing an expansive growth associated with fluid accumulation and inflammatory process, which in turn may be present as part of the lesion itself. In KOT, loosely arranged collagen may be associated with the behavior of the neoplastic epithelium
Guarding against contagion : vigilance and the role of fortifications in Malta during the outbreak of Plague in Messina 1743
L’Anno del Nostro Signore mille sette cento quaranta tre was not
a good year. The dreadful plague had broken out in the city of Messina,
a short distance across the sea in nearby Sicily. It also happened to be
one of the coldest winters on record to hit the Maltese islands in the
course of the eighteenth century.1
For the Hospitaller knights and their Maltese subjects this
deadly combination of factors did not augur well for both the Island’s
commerce and for the safety of its inhabitants. The Hospitaller Order
of St. John was never one to take the news of such deadly outbreaks
lightly. Indeed, the Knights’ sanitary departments had always prided
themselves on the rigour of their well-proven quarantine laws which
had served them well over the centuries and had preserved the Island,
their Convent and their subject population from many such calamities.
Now, once again, the whole machinery was set in motion. But in
this instance, given the close proximity of the source of the contagion
and the many maritime, commercial, and social contacts which the
Maltese had with the city of Messina, the risks were deemed even
higher. The situation demanded even more careful vigilance not the
least because of the ferocity of the outbreak.peer-reviewe
Encounters with Valletta : a baroque city through the ages
This article explores life in nineteenth-century Valletta, and the cross-currents between British and Maltese culture which developed at this period. The first century of British colonial policies and influence in Malta left a deep mark on all aspects of life in the city – on its society, education, housing, religious buildings and practices, as well as on leisure including theatre and public gardens.peer-reviewe
