    A Proof of The Triangular Ashbaugh-Benguria-Payne-P\'{o}lya-Weinberger Inequality

    In this paper, we show that for all triangles in the plane, the equilateral triangle maximizes the ratio of the first two Dirichlet-Laplacian eigenvalues. This is an extension of work by Siudeja, who proved the inequality in the case of acute triangles. The proof utilizes inequalities due to Siudeja and Freitas, together with improved variational bounds.Comment: 16 pages, 4 figure

    AIM: Evaluating safety and efficacy of chemotherapy among metastatic colorectal cancer patients The cultural, social, and religious differences in parenting and child-parent relationships in families make parenthood a sensitive domain for the research study. Accordingly, the changing nature of families, culture, and gender discrimination influence or affect parenting styles and parent-child relationships. The current study explored the conceptual understanding of parenthood, such as beliefs, views, expectations, and parental well-being in Indian families, especially in the states of Tamil Nadu, Kerala, and Gujarat. The participants were nine parents living together and with children the age range between eighteen and thirty-five. The parents’ age ranged from 41 to 63 (M = 50.8 years, SD = 6.05) (Mother’s age M = 48.7 years, SD = 6.04; Fathers’ age M = 52.9 years, SD = 5.28). They were selected based on purposive sampling. The method had an exploratory and qualitative design with multiple case studies. The researcher conducted in-depth, open-ended, semi-structured interviews using the online platform Google Meet and Zoom and face-to-face meetings. The interview transcriptions were analyzed using Braun and Clarke's inductive thematic analysis. The resulting six themes were: Objectives: Primary objective: To determine objective response rate of MCRC patients with chemotherapeutic regimen assessed as complete/ partial response/ stable/ progressive disease according to RECIST version 1.1. to evaluate the efficacy of chemotherapeutic regimen with in terms of objective response in accordance with response evaluation criteria in solid tumors RECIST version1.1. Secondary objective: To asses patient’s performance status using ECOG scale and to estimate percentage of all grade toxicities in accordance with CTCAE version 4.03. MATERIAL AND METHODS: It is a retrospective single centered empirical study conducted at the medical record department of oncology at a tertiary hospital for the duration of 6 months. CONCLUSION: MCRC is one of the major cause of death globally as the treatment involves complexities alongside much complications with chemotherapy are seen together with better outcomes. Entire data comprises of 109 subjects diagnosed with MCRC recorded in this retrospective study among which the no. of Male patients were 80 and Female patients were 29. The information extracted from the above data was that male subjects of age group between 50-70Y were more effected with colorectal cancer than female subjects of age group 40-70Y and patient specific treatment response rates were determined. The study revealed efficacy of chemotherapeutic regimen was found more with capox (capecitabine) and it came out as the most trending treatment among folfox (fluorouracil, oxaliplatin) and folfirinox (fluorouracil, irinotecan, oxaliplatin and capox is widely preferred by oncologists. Study revealed that among 109 patients with mcrc, the patients of age group 40- 70Y were more effected with repeatedly occurring adverse events of chemotherapy using CTCAE and determining the patient activity status

This paper presents hybrid system that integrates laser line scanning probe and touch trigger probe of coordinate measuring machine for RE of complex part. As a result of integration, this system utilizes strength of one system to overcome limitation of other. In this paper, RE methodology using hybrid system has been described through a part case study.     Enhanced Spin-Orbit Coupling in Heavy Metals via Molecular Coupling

    5d metals are used in electronics because of their high spin–orbit coupling (SOC) leading to efficient spin-electric conversion. When C60 is grown on a metal, the electronic structure is altered due to hybridization and charge transfer. In this work, we measure the spin Hall magnetoresistance for Pt/C60 and Ta/C60, finding that they are up to a factor of 6 higher than those for pristine metals, indicating a 20–60% increase in the spin Hall angle. At low fields of 1–30 mT, the presence of C60 increased the anisotropic magnetoresistance by up to 700%. Our measurements are supported by noncollinear density functional theory calculations, which predict a significant SOC enhancement by C60 that penetrates through the Pt layer, concomitant with trends in the magnetic moment of transport electrons acquired via SOC and symmetry breaking. The charge transfer and hybridization between the metal and C60 can be controlled by gating, so our results indicate the possibility of dynamically modifying the SOC of thin metals using molecular layers. This could be exploited in spin-transfer torque memories and pure spin current circuits

    A descriptive study of Forcefully Displaced Myanmar Nationals (FDMN) presenting for care at public health sector hospitals in Bangladesh

    Background In 2017 hundreds of thousands of ‘Rohingya’ fled to camps for Forcefully Displaced Myanmar Nationals (FDMN) in Cox’s Bazar, Bangladesh. Objective To describe the FDMNs presenting for care at public health facilities in Bangladesh so as to understand the health problems faced by the FDMNs and the burden on these public health facilities. Methods This study combined a retrospective review of existing hospital and clinic data with prospective surveillance in government health care centres. Findings The retrospective data showed a 26% increase in the number of consultations at the Kutupalong community clinic, the primary health facility closest to the camps, from 19,567 in 2015 to 26,309 in 2019. There was a corresponding 11% increase in admissions to health facilities in the area, from 80,991 in 2017 to 91,424 in 2019. Prospective surveillance of 9,421 FDMNs seeking health care from July 2018 to December 2019 showed that 29% had an infectious disease, 20% nutritional problems, 12% pregnancy-related conditions and 7% trauma or injury. Conclusions Great uncertainty remains regarding the return of FDMN to their home country of Myanmar. The current on-going protests following the military coup adds further insecurity to the status of the Rohingya. The presence of a large migrant population relative to a smaller host community burdens the limited facilities and resources of the public health sector. Continued support by the international public health community and civil society organizations is needed

    Miscellaneous Rheumatic Diseases [73-83]: 73. Is There a Delay in Specialist Referral of Hot Swollen Joint?

    Background: Patients with acute, hot, swollen joints commonly present to general practitioners, emergency departments and/or acute admitting teams rather than directly to rheumatology. It is imperative to consider septic arthritis in the differential diagnosis of these patients. The British Society of Rheumatology (BSR) has produced guidelines for the management of this condition, which include recommendations for early specialist referral and joint aspiration of all patients with suspected septic arthritis. We examined whether the initial management of patients with acute hot swollen joint(s) at University College London Hospital (UCLH) follows BSR guidelines. Methods: For the period Feb to Nov 2009, appropriate patients were identified by searching the UCLH database using the diagnostic terms, "pyogenic arthritis”, "septic arthritis” and "gout”; and from all joint aspirate requests sent to microbiology. Medical notes were obtained and any patients who had elective arthroscopies or chronic (> 6 weeks) symptoms were excluded. Data were collected on the time taken from the onset of symptoms to specialist (orthopaedic/rheumatology) referral and joint aspiration, collection of blood cultures and antibiotic treatment with or without microbiology advice. Results: Twenty patients were identified with hot swollen (18 monoarticular, 3 prosthetic) joint(s) of < 2 weeks duration. Of whom, 3/20 (15%) were admitted directly to rheumatology, 7/20 (35%) to the acute admissions unit, 3/20 (15%) to orthopaedic, 4/20 (20%) to a medical team and 1/20 (5%) to general surgery. In 19 (95%) cases, specialist (rheumatology/orthopaedic) advice was sought. Of 14 cases not seen directly by specialists 9 (64%) were referred at 24-48 h and 5 (36%) at 48-192 h. All 20 patients had joint aspiration. In 9/20 (45%) of cases, joint aspiration was performed in less than 6 h, 3/20 (15%) cases at 6-24h and 6/20 (30%) cases at 24-192 h and was not recorded in two patients. Of these, crystals were identified in two and one was culture positive. Blood cultures were received for only 6/20 (30%) of cases and only clearly documented to have been taken prior to antibiotic therapy and none were positive. Of 14/20 (70%) started on antibiotic treatment empirically, only 6 (42%) were preceded by joint aspiration. In the 6 patients not treated with antibiotics due to low index of suspicion of septic arthritis, synovial fluid and blood cultures were negative. Microbiology advice was sought in 10/20 (50%) of cases by the admitting teams but the timing of this advice is unclear. Conclusions: Despite the provision of 24 h rheumatology and orthopaedic cover at UCLH, we found a significant delay in acute medical firms seeking specialist advice on the management of patients with acute, hot swollen joints with subsequent deviation from BSR guidelines.
