    Developing community based rehabilitation for cancer survivors: organizing for coordination and coherence in practice

    Abstract Background Increasing incidences of cancer combined with prolonged survival have raised the need for developing community based rehabilitation. The objectives of the analysis were to describe and interpret the key issues related to coordination and coherence of community-based cancer rehabilitation in Denmark and to provide insights relevant for other contexts. Methods Twenty-seven rehabilitation managers across 15 municipalities in Denmark comprised the sample. The study was designed with a combination of data collection methods including questionnaires, individual interviews, and focus groups. A Grounded Theory approach was used to analyze the data. Results A lack of shared cultures among health care providers and systems of delivery was a primary barrier to collaboration which was essential for establishing coordination of care. Formal multidisciplinary steering committees, team-based organization, and informal relationships were fundamental for developing coordination and coherence. Conclusions Coordination and coherence in community-based rehabilitation relies on increased collaboration, which may best be optimized by use of shared frameworks within and across systems. Results highlight the challenges faced in practical implementation of community rehabilitation and point to possible strategies for its enhancement

    We investigate non-abelian gaugings of WZNW models. When the gauged group is semisimple we are able to present exact formulae for the dual conformal field theory, for all values of the level kk.     Panduratin A and 4-hydroxypanduratin A are synthesised in 6 steps via a high pressure Diels-Alder reaction. This sequence has also allowed the synthesis of isopanduratin A, 4-hyrdoxyisopanduratin A, panduratin H, and panduratin I.     ‘I shall have to speak of things, of which I cannot speak’, writes Samuel Beckett in The Unnameable, ‘but also, which is even more interesting, but also that I, which is if possible even more interesting, that I shall have to, I forget, no matter’. Listening to the voice of folly can be like this: an endless flow of inconsistencies, of contradictions, sayings and unsayings; a tantalising, mischievous mockery of speech –unable to go on, unable to end. And yet – as this volume shows – we are irresistibly drawn to folly, its promises, its whispers of ‘even more interesting’ things: of how we are split between conscious and unconscious, familiar and unfamiliar, same and other. For psychoanalysis, folly is not only a site of hidden truths; it is also, perhaps more importantly, a source of unconscious freedom, a momentary escape from our obsession with rules and order. According to Christopher Bollas, the unconscious self is like a fool, who ‘raises potentially endless questions about diverse and disparate issues’ and thereby provides us with a ‘separate sense’, which opens us to others and to our own creative potential. As Rachel Bowlby elegantly puts it, folly is a ‘soul-mole’, forever shovelling our secrets out into the light: ‘there’s no possible moment of release or resignation when the mole might stop vainly, interminably working away’. Folly’s subversive, creative soliloquies reveal to us a psychic ‘underground repertoire of secrets’; they challenge our established knowledge and invite us, as Bolwby shows, to endless, titillating games of ‘suppression and confession’. For Anne Duprat, this deep-seated playfulness explains folly’s close relation to fiction: what makes them so atone is their ‘capacity of creating alternative representations of the world — and thus of re-figuring the world depicted by reason or history – […] but also their paradoxical structure, and hence the instability of their speech acts, which deny, suspend, or do not seriously guarantee the truth of their statements’.     Long-Distance Transport of Finisher Pigs in the Iberian Peninsula:Effects of Season on Thermal and Enthalpy Conditions, Welfare Indicators and Meat pH

    Long-distance transport in the global swine industry is more the rule than the exception. We tested the impact on the rates of temperature change and air enthalpy on the stress response and muscle pH in pigs subjected to long-distance travel from Spain to Portugal performed in the summer and winter. We found that winter journeys are more adverse for the animals because during the journey, abrupt variations in rates of temperature change and air enthalpy caused a marked physiological stress response and effects on the meat pH after 45 min. These results indicate the need to develop new environmental control strategies that mitigate abrupt temperature changes during travel to attenuate the biological cost of such long-distance transport on the animals

    Shape controlled assembly of carboxylic acids : formation of a binary monolayer by intercalation into molecular nanotunnels

A. acknowledges the financial support by the DAAD-Aceh Scholarship of Excellence.Binary self-assembled monolayers (SAMs) combining a Y-shaped aromatic carboxylic acid (1,3,5-benzenetribenzoic acid, H3BTB) and a cage-type alicyclic carboxylic acid (adamantane carboxylic acid, AdCA) were investigated by scanning tunneling microscopy (STM), X-ray photoelectron spectroscopy (XPS), and near edge X-ray absorption fine structure (NEXAFS) spectroscopy. The SAMs, prepared by molecular adsorption from solution on Au substrates modified by underpotential deposition of Ag, exhibit a pronounced dependence of their structure on the assembly protocol. Exposing an H3BTB SAM to AdCA, the highly regular row structure of the native H3BTB layer persists and STM imaging does not show signs of AdCA adsorption. This is in striking contrast to the disordered arrangements of H3BTB and the presence of AdCA employing the inverted adsorption sequence or coadsorption of the two molecules. However, spectroscopic analysis of the H3BTB SAM exposed to AdCA reveals the presence also of the latter, suggesting that the AdCA molecules are hidden in the nanotunnels of the H3BTB monolayer. Direct evidence for the intercalation of AdCA is obtained by STM manipulation experiments which lay bare areas of AdCA molecules upon local removal of H3BTB. Surprisingly, these are densely packed and arranged into a highly ordered monolayer. Formation of such a compact AdCA layer is explained by expulsion of AdCA from the H3BTB nanotunnels of the surrounding intact mixed SAM, driven by release of stress in the nanotunnels built up when AdCA is intercalated.PostprintPeer reviewe

    Hierarchical Hough all-sky search for periodic gravitational waves in LIGO S5 data

    We describe a new pipeline used to analyze the data from the fifth science run (S5) of the LIGO detectors to search for continuous gravitational waves from isolated spinning neutron stars. The method employed is based on the Hough transform, which is a semi-coherent, computationally efficient, and robust pattern recognition technique. The Hough transform is used to find signals in the time-frequency plane of the data whose frequency evolution fits the pattern produced by the Doppler shift imposed on the signal by the Earth's motion and the pulsar's spin-down during the observation period. The main differences with respect to previous Hough all-sky searches are described. These differences include the use of a two-step hierarchical Hough search, analysis of coincidences among the candidates produced in the first and second year of S5, and veto strategies based on a χ2\chi^2 test.Comment: 7 pages, 2 figures, Amaldi08 proceedings, submitted to JPC

    Differential spatial repositioning of activated genes in Biomphalaria glabrata snails infected with Schistosoma mansoni

Understanding the molecular and biochemical relationship between the causative schistosome parasite and its hosts will be key to understanding and ultimately treating and/or eradicating the disease. There is increasing evidence that pathogens that have co-evolved with their hosts can manipulate their hosts' behaviour at various levels to augment an infection. Bacteria, for example, can induce beneficial chromatin remodelling of the host genome. We have previously shown in vitro that Biomphalaria glabrata embryonic cells co-cultured with schistosome miracidia display genes changing their nuclear location and becoming up-regulated. This also happens in vivo in live intact snails, where early exposure to miracidia also elicits non-random repositioning of genes. We reveal differences in the nuclear repositioning between the response of parasite susceptible snails as compared to resistant snails and with normal or live, attenuated parasites. Interestingly, the stress response gene heat shock protein (Hsp) 70 is only repositioned and then up-regulated in susceptible snails with the normal parasite. This movement and change in gene expression seems to be controlled by the parasite. Other differences in the behaviour of genes support the view that some genes are responding to tissue damage, for example the ferritin genes move and are up-regulated whether the snails are either susceptible or resistant and upon exposure to either normal or attenuated parasite. This is the first time host genome reorganisation has been seen in a parasitic host and only the second time for any pathogen. We believe that the parasite elicits a spatio-epigenetic reorganisation of the host genome to induce favourable gene expression for itself and this might represent a fundamental mechanism present in the human host infected with schistosome cercariae as well as in other host-pathogen relationships.NIH and Sandler Borroughs Wellcome Travel Fellowshi

    Longer fixation duration while viewing face images

    The spatio-temporal properties of saccadic eye movements can be influenced by the cognitive demand and the characteristics of the observed scene. Probably due to its crucial role in social communication, it is argued that face perception may involve different cognitive processes compared with non-face object or scene perception. In this study, we investigated whether and how face and natural scene images can influence the patterns of visuomotor activity. We recorded monkeys’ saccadic eye movements as they freely viewed monkey face and natural scene images. The face and natural scene images attracted similar number of fixations, but viewing of faces was accompanied by longer fixations compared with natural scenes. These longer fixations were dependent on the context of facial features. The duration of fixations directed at facial contours decreased when the face images were scrambled, and increased at the later stage of normal face viewing. The results suggest that face and natural scene images can generate different patterns of visuomotor activity. The extra fixation duration on faces may be correlated with the detailed analysis of facial features
