    Smart Cities in Future Energy System Architecture

    The article analyzes new technologies, which establish modern trends for future electric power systems operation and development, and their impact on production, storage, transmission, distribution and consumption of electricity. The article also studies trends and approaches to urban planning with an emphasis on energy infrastructure, where success in “smart city” and “smart grid” concepts implementation is considered as overarching question. The general requirements for smart power supply system are defined, problems that need to be solved are formulated, and a number of general recommendations are given for future development.     Assessment occupational risk vertebrogenic pain syndromes lumbar spine workers aluminum manufacturing

    According to the results of the periodic medical examination of 903 workers of the Ural aluminum smelter analyzed the prevalence and evaluation of occupational risk of vertebral pain syndromes of the lumbar spine. Workers exposed to toxic effects of fluoride and physical strain on the risk of low back pain is increased 2.14 times, while working only at high doses of fluoride to 2.03 times compared to running without the impact of these factors. The findings suggest occupational related diseases.По результатам периодического медицинского осмотра 903 работающих Уральского алюминиевого завода проведен анализ распространенности и оценка профессионального риска вертеброгенных болевых синдромов поясничного отдела позвоночника. У рабочих, подвергающихся токсическому действию фторидов и физическому перенапряжению, риск развития поясничной боли увеличивается в 2,14 раза, а у работающих только в условиях повышенных доз фторидов - в 2,03 раза по сравнению с работающими без воздействия этих факторов. Полученные данные свидетельствуют о профессиональной обусловленности заболевания

    Affine modifications and affine hypersurfaces with a very transitive automorphism group

    We study a kind of modification of an affine domain which produces another affine domain. First appeared in passing in the basic paper of O. Zariski (1942), it was further considered by E.D. Davis (1967). The first named author applied its geometric counterpart to construct contractible smooth affine varieties non-isomorphic to Euclidean spaces. Here we provide certain conditions which guarantee preservation of the topology under a modification. As an application, we show that the group of biregular automorphisms of the affine hypersurface XCk+2X \subset C^{k+2} given by the equation uv=p(x1,...,xk)uv=p(x_1,...,x_k) where pC[x1,...,xk],p \in C[x_1,...,x_k], acts mm-transitively on the smooth part regXX of XX for any mN.m \in N.     Influence of occupational risk for shoulder pain syndromes

    One-stage cross-sectional observation of prevalence shoulder pain syndrome was investigated. The observed 6094 workers were divided into four groups. The highest prevalence index of SP were registered in the в first group - 32,9% Workers exposed to toxic effects of fluoride and physical strain, the lowest - in the 3-rd group - 6,9% without the impact of toxic action. The highest relative risk of SP progress was observed in the 1st group in the relation to 4th (5,67) and 3rd groups (5,24), at the same time etiological fraction was 77.5% and 78,7%, it indicates very high influence of labour conditions on prevalence of shoulder pain syndrome.На основании одномоментного кросс - секционного обследования 6094 работающих в различных условиях труда изучена распространенность болевого синдрома плеча у 1819 (29,8%) женщин и 4275 (70,2%) мужчин. Получены данные о повышенной распространенности болевого синдрома плеча у работающих в условиях физического перенапряжения и токсического действия фторидов (32,9%), а также в условиях токсического воздействия фторидов по сравнению с работающими только в условиях физического перенапряжения (6,9%) и без воздействия «вредных» производственных факторов. В первой группе наиболее высокий относительный риск развития БСП по отношению к 4-ой группе (5,6) и 3-ей (5,2), при этом этиологическая фракция составила соответственно 77.5% и 78,7%, что свидетельствует об очень высокой степени связи нарушения здоровья с условиями труда, в данном случае - с воздействием комплекса факторов: физического перенапряжения и токсического воздействия фторидов. Из анализируемых «непроизводственных» показателей в большей степени повышают риск развития БСП сопутствующая патология бронхолегочной системы и шейный остеохондроз

    The rationale of opportunistic bilateral salpingectomies (OBS) during benign gynaecological and obstetric surgery : a consensus text of the Flemish Society of Obstetrics and Gynaecology (VVOG)

    Ovarian cancer (OC), is a disease difficult to diagnose in an early stage implicating a poor prognosis. The 5-year overall survival in Belgium has not changed in the last 18 years and remains 44 %. There is no effective screening method (secondary prevention) to detect ovarian cancer at an early stage. Primary prevention of ovarian cancer came in the picture through the paradigm shift that the fallopian tube is often the origin of ovarian cancer and not the ovary itself. Opportunistic bilateral salpingectomy (OBS) during benign gynaecological and obstetric surgery might have the potential to reduce the risk of ovarian cancer by as much as 65 %. Bilateral risk-reducing salpingectomy during a benign procedure is feasible, safe, appears to have no impact on the ovarian function and seems to be cost effective. The key question is whether we should wait for a RCT or implement OBS directly in our daily practice. Guidelines regarding OBS within our societies are therefore urgently needed. Our recommendation is to inform all women without a child wish, undergoing a benign gynaecological or obstetrical surgical procedure about the pro’s and the con’s of OBS and advise a bilateral salpingectomy. Furthermore, there is an urgent need for a prospective registry of OBS. The present article is the consensus text of the Flemish Society of Obstetrics and Gynaecology (VVOG) regarding OBS

    Developing agreement on never events in primary care dentistry:an international eDelphi study

    Introduction: Never events (NEs) are a subset of serious patient safety incidents that should not occur if appropriate preventive measures are implemented. Although there is a consensus in medicine, there is no agreement on NEs in dentistry. Aim: To identify NEs in primary care dentistry. Method: We undertook an electronic Delphi exercise to develop an international agreement on NEs for primary care dentistry. Results: We initially identified candidate NEs through a scoping review of the literature and then analysed dentistry-related reports in a national incident reporting system. Next, we invited an international panel of 41 experts to complete two rounds of questionnaires; 32 agreed to participate (78%) and completed the first round and 29/41 (71%) members completed the second round. We provided anonymised controlled feedback between rounds and used a cut-off of 80% agreement to define consensus. Consensus was achieved for 23 out of 42 candidate NEs. These related to routine assessment, and pre-operative, intra-operative and post-operative stages of dental procedures. Discussion and conclusion: To our knowledge, this is the first international expert consensus-based approach that has identified NEs for primary care dentistry. We suggest that dental regulators consider these to support quality assessment and governance activities

    Molecular Basis for Vulnerability to Mitochondrial and Oxidative Stress in a Neuroendocrine CRI-G1 Cell Line

    Many age-associated disorders (including diabetes, cancer, and neurodegenerative diseases) are linked to mitochondrial dysfunction, which leads to impaired cellular bioenergetics and increased oxidative stress. However, it is not known what genetic and molecular pathways underlie differential vulnerability to mitochondrial dysfunction observed among different cell types.Starting with an insulinoma cell line as a model for a neuronal/endocrine cell type, we isolated a novel subclonal line (named CRI-G1-RS) that was more susceptible to cell death induced by mitochondrial respiratory chain inhibitors than the parental CRI-G1 line (renamed CRI-G1-RR for clarity). Compared to parental RR cells, RS cells were also more vulnerable to direct oxidative stress, but equally vulnerable to mitochondrial uncoupling and less vulnerable to protein kinase inhibition-induced apoptosis. Thus, differential vulnerability to mitochondrial toxins between these two cell types likely reflects differences in their ability to handle metabolically generated reactive oxygen species rather than differences in ATP production/utilization or in downstream apoptotic machinery. Genome-wide gene expression analysis and follow-up biochemical studies revealed that, in this experimental system, increased vulnerability to mitochondrial and oxidative stress was associated with (1) inhibition of ARE/Nrf2/Keap1 antioxidant pathway; (2) decreased expression of antioxidant and phase I/II conjugation enzymes, most of which are Nrf2 transcriptional targets; (3) increased expression of molecular chaperones, many of which are also considered Nrf2 transcriptional targets; (4) increased expression of β cell-specific genes and transcription factors that specify/maintain β cell fate; and (5) reconstitution of glucose-stimulated insulin secretion.The molecular profile presented here will enable identification of individual genes or gene clusters that shape vulnerability to mitochondrial dysfunction and thus represent potential therapeutic targets for diabetes and neurodegenerative diseases. In addition, the newly identified CRI-G1-RS cell line represents a new experimental model for investigating how endogenous antioxidants affect glucose sensing and insulin release by pancreatic β cells

    Documenting the Recovery of Vascular Services in European Centres Following the Initial COVID-19 Pandemic Peak: Results from a Multicentre Collaborative Study

    Objective: To document the recovery of vascular services in Europe following the first COVID-19 pandemic peak. Methods: An online structured vascular service survey with repeated data entry between 23 March and 9 August 2020 was carried out. Unit level data were collected using repeated questionnaires addressing modifications to vascular services during the first peak (March – May 2020, “period 1”), and then again between May and June (“period 2”) and June and July 2020 (“period 3”). The duration of each period was similar. From 2 June, as reductions in cases began to be reported, centres were first asked if they were in a region still affected by rising cases, or if they had passed the peak of the first wave. These centres were asked additional questions about adaptations made to their standard pathways to permit elective surgery to resume. Results: The impact of the pandemic continued to be felt well after countries’ first peak was thought to have passed in 2020. Aneurysm screening had not returned to normal in 21.7% of centres. Carotid surgery was still offered on a case by case basis in 33.8% of centres, and only 52.9% of centres had returned to their normal aneurysm threshold for surgery. Half of centres (49.4%) believed their management of lower limb ischaemia continued to be negatively affected by the pandemic. Reduced operating theatre capacity continued in 45.5% of centres. Twenty per cent of responding centres documented a backlog of at least 20 aortic repairs. At least one negative swab and 14 days of isolation were the most common strategies used for permitting safe elective surgery to recommence. Conclusion: Centres reported a broad return of services approaching pre-pandemic “normal” by July 2020. Many introduced protocols to manage peri-operative COVID-19 risk. Backlogs in cases were reported for all major vascular surgeries

    A single-point modeling approach for the intercomparison and evaluation of ozone dry deposition across chemical transport models (Activity 2 of AQMEII4)

    A primary sink of air pollutants and their precursors is dry deposition. Dry deposition estimates differ across chemical transport models, yet an understanding of the model spread is incomplete. Here, we introduce Activity 2 of the Air Quality Model Evaluation International Initiative Phase 4 (AQMEII4). We examine 18 dry deposition schemes from regional and global chemical transport models as well as standalone models used for impact assessments or process understanding. We configure the schemes as single-point models at eight Northern Hemisphere locations with observed ozone fluxes. Single-point models are driven by a common set of site-specific meteorological and environmental conditions. Five of eight sites have at least 3 years and up to 12 years of ozone fluxes. The interquartile range across models in multiyear mean ozone deposition velocities ranges from a factor of 1.2 to 1.9 annually across sites and tends to be highest during winter compared with summer. No model is within 50 % of observed multiyear averages across all sites and seasons, but some models perform well for some sites and seasons. For the first time, we demonstrate how contributions from depositional pathways vary across models. Models can disagree with respect to relative contributions from the pathways, even when they predict similar deposition velocities, or agree with respect to the relative contributions but predict different deposition velocities. Both stomatal and nonstomatal uptake contribute to the large model spread across sites. Our findings are the beginning of results from AQMEII4 Activity 2, which brings scientists who model air quality and dry deposition together with scientists who measure ozone fluxes to evaluate and improve dry deposition schemes in the chemical transport models used for research, planning, and regulatory purposes.</p
