Gender, albuminuria and chronic kidney disease progression in treated diabetic kidney disease
Background: Women are reported to have a lower incidence of renal replacement therapy,
despite a higher prevalence of chronic kidney disease (CKD). Aim: To analyze diabetic kidney
disease (DKD) progression in men and women. Methods: Prospective cohort: n = 261, 35% women,
new consecutive nephrology DKD referrals. Results: Women smoked less and better complied
with the dietary phosphate and sodium restrictions. Despite a less frequent nephrology referral,
women had lower baseline albuminuria. Over a 30 + - 10-month follow-up, albuminuria decreased in
women and the estimated glomerular filtration rate (eGFR) loss was slower than in men. However,
the percentage of rapid progressors was similar in both sexes. The best multivariate model predicting
rapid progression in men (area under curve (AUC) = 0.92) and women differed. Albuminuria and
fractional excretion of phosphate (FEphosphate) were part of the men multivariable model, but not of
women. The AUC for the prediction of rapid progression by albuminuria was higher in men than in
women, and the albuminuria cut-off points also differed. In women, there was a higher percentage of
rapid progressors who had baseline physiological albuminuria. Conclusions: Female DKD differs
from male DKD: albuminuria was milder and better responsive to therapy, the loss of eGFR was

Disminución de reingresos tras una hospitalización por exacerbación de EPOC a través de un modelo de atención domiciliaria
To decrease readmissions at 30 and 90 days post-discharge from a hospital admission for chronic obstructive pulmonary disease exacerbation (COPDE)through the home care model ofthe Ambulatory Chronic Respiratory Care Unit (ACRCU), increase patient survival at one year, and validate our readmission risk scale (RRS). Materials and methods: This was an observational study, with a prospective data collection and a retrospective data analysis. A total of 491 patients with a spirometry diagnosis of chronic obstructive pulmonary disease (COPD) requiring hospitalisation for an exacerbation were included in the study. Subjects recruited within the first year (204 cases) received conventional care (CC). In the following year a home care (HC) programme was implemented and of those recruited that year (287) 104 were included in the ACRCU, administered by a specialised nurse. Results: In the group of patients included in the home care model of the Ambulatory Chronic Respiratory Care Unit(ACRCU) a lower number of readmissions was observed at 30 and 90 days after discharge (30.5% vs. 50%, p = 0.012 and 47.7% vs. 65.2%, p = 0.031, respectively) and a greater one-year survival (85.3% vs. 59.1%, p < 0.001). The validation of our RRS revealed that the tool’s capacity to predict readmissions at both 30 and 90 days was not high (AUC = 0.69 and AUC = 0.66, respectively). The inclusion of exacerbator or fragile COPD patients in the ACRCU could achieve a decrease in readmissions and an increase in survival. The number of episodes of exacerbation within the 12 months prior to the hospital admission is the variable that best predicts the risk of readmissionDisminuir los reingresos a los 30 y 90 días tras el alta por un ingreso hospitalario por exacerbación de enfermedad pulmonar obstructiva crónica (EPOC) a través del modelo de atención domiciliaria de la Unidad de Cuidados Crónicos Respiratorios Ambulatorios (UCCRA), aumentar la supervivencia al año y validar nuestra escala de riesgo de reingreso (ERR).
Estudio observacional con recogida prospectiva de datos. Se incluyó en el estudio a un total de 491 pacientes con diagnóstico espirométrico de enfermedad pulmonar obstructiva crónica que requirieron hospitalización por una agudización. Los sujetos reclutados dentro del primer año (204 casos) recibieron atención convencional (AC). Al año siguiente se implementó un programa de atención domiciliaria (AD) y de los pacientes reclutados ese año (287), 104 fueron incluidos en la UCCRA con seguimiento de una enfermera especializada.
En el grupo de pacientes incluidos en el modelo de atención domiciliaria de la UCCRA se observó un menor número de reingresos a los 30 y 90 días tras el alta (30,5% vs 50%, p = 0,012 y 47,7% vs. 65,2%, p = 0,031, respectivamente) y una mayor supervivencia al año (85,3% vs. 59,1%, p < 0,001). La validación de nuestra ERR reveló que la capacidad de la misma para predecir reingresos tanto a los 30 como a los 90 días no era alta (AUC = 0,69 y AUC = 0,66, respectivamente).
La inclusión de pacientes con EPOC agudizadores o frágiles en la UCCRA podría conseguir una disminución de los reingresos y una aumento de la supervivencia. El número de agudizaciones en los 12 meses previos al ingreso hospitalario es la variable que mejor predice el riesgo de reingres
Nosocomial transmission of sporadic Creutzfeldt–Jakob disease: results from a risk-based assessment of surgical interventions
OBJECTIVES: Evidence of surgical transmission of sporadic Creutzfeldt-Jakob disease (sCJD) remains debatable in part due to misclassification of exposure levels. In a registry-based case-control study, the authors applied a risk-based classification of surgical interventions to determine the association between a history of surgery and sCJD. DESIGN: Case-control study, allowing for detailed analysis according to time since exposure. SETTING: National populations of Denmark and Sweden. PARTICIPANTS: From national registries of Denmark and Sweden, the authors included 167 definite and probable sCJD cases with onset during the period 1987-2003, 835 age-, sex- and residence-matched controls and 2224 unmatched. Surgical procedures were categorised by anatomical structure and presumed risk of transmission level. The authors used logistic regression to determine the odds ratio (OR) for sCJD by surgical interventions in specified time-windows before disease-onset. RESULTS: From comparisons with matched controls, procedures involving retina and optic nerve were associated with an increased risk at a latency of ≥1 year OR (95% CI) 5.53 (1.08 to 28.0). At latencies of 10 to 19 years, interventions on peripheral nerves 4.41 (1.17 to 16.6) and skeletal muscle 1.58 (1.01 to 2.48) were directly associated. Interventions on blood vessels 4.54 (1.01 to 20.0), peritoneum 2.38 (1.14 to 4.96) and skeletal muscle 2.04 (1.06 to 3.92), interventions conducted by vaginal approach 2.26 (1.14 to 4.47) and a pooled category of lower-risk procedures 2.81 (1.62 to 4.88) had an increased risk after ≥20 years. Similar results were found when comparing with unmatched controls. Perinatal adverse effects in newborns with estimated loss of weight percentile between the third trimester ultrasound and delivery. The GROWIN study
Fetal growth restriction has been associated with an increased risk of adverse perinatal outcomes (APOs). We determined the importance of fetal growth detention (FGD) in late gestation for the occurrence of APOs in small-for-gestational-age (SGA) and appropriate-for-gestational-age (AGA) newborns. For this purpose, we analyzed a retrospective cohort study of 1067 singleton pregnancies. The newborns with higher APOs were SGA non-FGD and SGA FGD in 40.9% and 31.5% of cases, respectively, and we found an association between SGA non-FGD and any APO (OR 2.61; 95% CI: 1.35–4.99; p = 0.004). We did not find an increased APO risk in AGA FGD newborns (OR: 1.13, 95% CI: 0.80, 1.59; p = 0.483), except for cesarean delivery for non-reassuring fetal status (NRFS) with a decrease in percentile cutoff greater than 40 (RR: 2.41, 95% CI: 1.11–5.21) and 50 (RR: 2.93, 95% CI: 1.14–7.54). Conclusions: Newborns with the highest probability of APOs are SGA non-FGDs. AGA FGD newborns do not have a higher incidence of APOs than AGA non-FGDs, although with falls in percentile cutoff over 40, they have an increased risk of cesarean section due to NRFS. Further studies are warranted to detect these newborns who would benefit from close surveillance in late gestation and at delivery
Sialolithiasis: mineralogical composition, crystalline structure, calculus site, and epidemiological features
The purpose of this paper was to describe the characteristics of salivary calculi and their relationship to epidemiological factors, through a cross-sectional study. We analysed 100 calculi obtained in 2017–2021. Patient data including age, time since onset of symptoms, gland involved, and site of location in the salivary system were studied. The calculi were studied to determine their morphological features using scanning electron microscopy and energy dispersive plain radiographic analysis. Most of the calculi had formed in the submandibular gland (SG) (82%). The mean age of patients at onset was 45.83 years; patients presenting parotid gland (PG) stones were somewhat older (p = 0.031). The mean time since the onset of symptoms was longer in PG calculi (p = 0.038). The most common lithiasis site was the main duct (74%), followed by the hilum (22%). Hilar stones were the largest (p < 0.05) and heaviest (p = 0.028). Octacalcium phosphate (OCP) was the most common crystalline phase (Cp) founded, followed by hydroxyapatite (HA) and whitlockite (WH). Specifically, OCP had a higher presence in PG calculi (p = 0.029) and WH was the most common phase in SG calculi (p = 0.017). The most prevalent site of lithiasis was the main duct, and the largest and heaviest calculi were found in the SG. PG stones were associated with a longer history of symptoms and older age. OCP was the most frequent Cp of the calculi studied, and the main Cp in PG stones. WH was the predominant Cp in SG stones. Effect of high-intensity interval versus continuous exercise training on functional capacity and quality of life in patients with coronary artery disease.
There is strong evidence that exercise training has beneficial
health effects in patients with cardiovascular disease. Most studies
have focused on moderate continuous training (MCT); however, a
body of evidence has begun to emerge demonstrating that highintensity
interval training (HIIT) has significantly better results in terms
of morbidity and mortality. The aim of this study was to compare the
effects of MCT versus HIIT on functional capacity and quality of life
and to assess safety. Seventy-two patients with ischemic heart disease were
assigned to either HITT or MCT for 8 weeks. We analyzed cardiopulmonary
exercise stress test data, quality of life, and adverse events.High-intensity interval training resulted in a significantly greater
increase in V ·
O 2 peak (4.5 ± 4.7 mL·kg − 1 ·min − 1 ) compared with MCT
(2.5 ± 3.6 mL·kg − 1 ·min − 1 ) ( P < .05). The aerobic threshold (V T 1 )
increased by 21% in HIIT and 14% in MCT. Furthermore, there was a
significant ( P < .05) increase in the distance covered in the 6-minute
walk distance test in the HIIT group (49.6 ± 6.3 m) when compared
with the MCT group (29.6 ± 12.0 m). Both training protocols
improved quality of life. No adverse events were reported in either of
the groups.On the basis of the results of this study, HIIT should be
considered for use in cardiac rehabilitation as it resulted in a greater
increase in functional capacity compared with MCT. We also observed
greater improvement in quality of life without any increase in cardiovascular

Androgen receptor polyQ alleles and COVID-19 severity in men: a replication study
Background: Ample evidence indicates a sex-related difference in severity of COVID19, with less favorable outcomes observed in men. Genetic factors have been proposed
as candidates to explain this difference. The polyglutamine (polyQ) polymorphism in
the androgen receptor gene has been recently described as a genetic biomarker of
COVID-19 severity.
Objective: To test the association between the androgen receptor polyQ polymorphism and COVID-19 severity in a large cohort of COVID-19 male patients.
Materials and methods: This study included 1136 male patients infected with SARSCoV-2 as confirmed by positive PCR. Patients were retrospectively and prospectively
enrolled from March to November 2020. Patients were classified according to their
severity into three categories: oligosymptomatic, hospitalized and severe patients
requiring ventilatory support. The number of CAG repeats (polyQ polymorphism) at
the androgen receptor was obtained by PCR and patients were classified as either
short (<23 repeats) or long (≥23 repeats) allele carriers. The association between
polyQ alleles (short or long) and COVID-19 severity was assessed by Chi-squared
(Chi2) and logistic regression analysis.
Results: The mean number of polyQ CAG repeats was 22 (±3). Patients were classified
as oligosymptomatic (15.5%), hospitalized (63.2%), and severe patients (21.3%) requiring substantial respiratory support. PolyQ alleles distribution did not show significant
differences between severity classes in our cohort (Chi2 test p > 0.05). Similar results
were observed after adjusting by known risk factors such as age, comorbidities, and

Age-dependent association of clonal hematopoiesis with COVID-19 mortality in patients over 60 years.
Clonal hematopoiesis, especially that of indeterminate potential (CHIP), has been associated with age-related diseases, such as those contributing to a more severe COVID-19. Four studies have attempted to associate CHIP with COVID-19 severity without conclusive findings. In the present work, we explore the association between CHIP and COVID-19 mortality. Genomic DNA extracted from peripheral blood of COVID-19 patients (n = 241 deceased, n = 239 survivors) was sequenced with the Myeloid Solutions™ panel of SOPHiA Genetics. The association between clonality and age and clonality and mortality was studied using logistic regression models adjusted for sex, ethnicity, and comorbidities. The association with mortality was performed with patients stratified into four groups of age according to the quartiles of the distribution: 60–74 years, 75–84 years, 85–91 years, and 92–101 years. Clonality was found in 38% of the cohort. The presence of CHIP variants, but not the number, significantly increased with age in the entire cohort of COVID-19 patients, as well as in the group of survivors (p < 0.001). When patients were stratified by age and the analysis adjusted, CHIP classified as pathogenic/likely pathogenic was significantly more represented in deceased patients compared with survivors in the group of 75–84 years (34.6% vs 13.7%, p = 0.020). We confirmed the well-established linear relationship between age and clonality in the cohort of COVID-19 patients and found a significant association between pathogenic/likely pathogenic CHIP and mortality in patients from 75 to 84 years that needs to be further validated.post-print1034 K
Fibroblast growth factor 23 independently predicts adverse outcomes after an acute coronary syndrome
Abnormalities of mineral metabolism (MM) have been related to cardiovascular disorders. There are no reports on the prognostic role of MM after an acute coronary syndrome (ACS). We aim to assess the prognostic role of MM after an ACS. Plasma levels of components of MM [fibroblast growth factor 23 (FGF23), calcidiol, parathormone, klotho, and phosphate], high-sensitivity C-reactive protein, and N-terminal-pro-brain natriuretic peptide were measured in 1190 patients at discharge from an ACS. The primary outcome was a combination of acute ischaemic events, heart failure (HF) and death. Secondary outcomes were the separate components of the primary outcome. Age was 61.7 ± 12.2 years, and 77.1% were men. Median follow-up was 5.44 (3.03–7.46) years. Two hundred and ninety-four patients developed the primary outcome. At multivariable analysis FGF23 (hazard ratio, HR 1.18 [1.08–1.29], P 60 mL/min/1.73 m2. FGF23 and other components of MM are independent predictors of HF and death after an ACS. High SARS-CoV-2 viral load is associated with a worse clinical outcome of COVID-19 disease
COVID-19 severity and progression are determined by several host and virological factors that may influence the final outcome of SARS-CoV-2-infected patients. The objective of this work was to determine a possible association between viral load, obtained from nasopharyngeal swabs, and the severity of the infection in a cohort of 448 SARS-CoV-2-infected patients from a hospital in Madrid during the first outbreak of the pandemic in Spain. To perform this, we clinically classified patients as mild, moderate and severe COVID-19 according to a number of clinical parameters such as hospitalization requirement, need of oxygen therapy, admission to intensive care units and/or death. Also, Ct values were determined using SARS-CoV-2-specific oligonucleotides directed to ORF1ab. Here we report a statistically significant association between viral load and disease severity, a high viral load being associated with worse clinical prognosis, independently of several previously identified risk factors such as age, sex, hypertension, cardiovascular disease, diabetes, obesity and lung disease (asthma and chronic obstructive pulmonary disease). The data presented here reinforce viral load as a potential biomarker for predicting disease severity in SARS-CoV-2-infected patients.
