Impacts of premature birth and low birthweight on the development of language and executive function
Intelligence and executive functions in 9–10 year-old preterm children born with very low and extremely low birth weight
Theoretical background: Premature birth is in the focus of research interest as it is the most common perinatal risk endangering the development of children. However, the implications of prematurity for the long-term outcome are far from fully understood. Compromised development of cognitive and executive functions may be underlying academic underachievement in school-age preterm children. Aim: to assess the school-age outcomes of Hungarian VLBW/ELBW preterm children in basic cognitive abilities and executive function as compared to typically developing, full-term children as well as to investigate the background of individual differences. Method: 54 preterm children (27 ELBW, 27 VLBW) and a matched group of 27 healthy full-term children, aged 9–10 years, were tested using the Wechsler Intelligence Scales for Children (WISC-IV), the Corsi Block Tapping Task (digital version) for measuring spatial–visual working memory and the Wisconsin Card Sorting Test (WCST, digital version) for testing cognitive flexibility. As background variables perinatal and socioeconomic factors were entered in the analysis. Results: In each measure of the WISC-IV all three groups performed in the normal range. The ELBW children displayed certain developmental lags. They scored significantly lower in the Full-Scale IQ and the Processing Speed than the other two groups, and in the Perceptual Reasoning and one measure of spatial–visual working memory as compared to the non-risk comparison group. Perinatal complications and maternal education were related to the outcome. Conclusions: With the improved perinatal care preterm children have fair chances for good developmental outcomes. However, the individual variations are great and various perinatal and social factors may hamper the development of cognitive and executive functions. A birthweight below 1000 grams is a notable risk, particularly if combined with perinatal complications

MegfigyelĂ©ses adataink alátámasztják a korábbi kutatásokban tapasztaltakat, amelyek szerint koraszĂĽlött gyerekeknĂ©l a temperamentum Ă©s a mentális fejlĹ‘dĂ©s szoros kapcsolatban áll. | In our previous research a group of moderate-risk preterm infants and a comparison group of healthy full-term infants had been followed from birth through 4 years. The present grant made a new wave of data collection possible at the age of 6-7 years. It also gave us an opportunity to analyse longer-term developmental patterns using data from earlier stages of the children's growth. By 4 years the preterm children appeared to have caught up, overcoming the developmental delays found at the earlier assessments. However, the promising trend did not seem to continue. Although at age 6 the mean scores of the preterm sample on all outcome measures fell within the normal range, their performances lagged behind the performances of the non-risk comparison group. The analysis of the concurrent and longitudinal patterns of performances gave us further insights to the long-term impact of premature birth. In addition to the analysis of the developmental data, we continued working on the videotapes of mother-child interactions recorded in the previous years. At 4 years of age the behaviours of the preterm children had more distinct characteristics than in infancy, while the differential behaviours found earlier in their mothers were not present any more. Our observational data corroborate the strong link between the mental development and temperament in preterm children reported by several authors
Az extrém kis születési súlyú koraszülöttek fejlődésének követése az óvodáskor végéig a perinatalis szövődmények tükrében = Follow-up study of extremely low birth weight preterm infants to preschool age in the light of perinatal complications
Absztrakt:
Bevezetés: Az orvostudomány fejlődésének eredményeként az 1000
gramm alatti születési súlyú koraszülöttek túlélése egyre magasabb, fejlődésük
hosszú távú következményeiről azonban ellentmondásosak a kutatási eredmények.
Célkitűzés: Súlyos idegrendszeri károsodástól mentes,
extrém kis születési súlyú koraszülötteknek az iskoláskor eléréséig tartó
követĂ©se során gyűjtött adataink segĂtsĂ©gĂ©vel ennek a jelentĹ‘s rizikĂłcsoportnak
a fejlődési mechanizmusához igyekeztünk közelebb jutni. Munkánkkal a fejlődés
rizikótényezőinek megértéséhez és a fejlődés előrejelzését szolgáló modell
kidolgozásához szeretnénk hozzájárulni. Módszer: Súlyos
idegrendszeri károsodástól mentes, extrém kis születési súlyú koraszülöttek (n =
34) longitudinális vizsgálata történt, 1 és 2 éves korban pszichomotoros
fejlődési skálával, majd 5 és 6 éves kor között intelligenciateszttel. A
fejlődés háttértényezőinek elemzése kiterjedt a perinatalis szövődményekre és az
anya iskolai végzettségére is. Eredmények: Az extrém kis
születési súlyú koraszülöttek csoportszinten egyik életkori mérésnél sem
mutattak fejlődési elmaradást: a fejlődési hányados átlaga 1 és 2 éves korban
98,6, illetve 106,6, az 5–6 éves kori verbális és performációs IQ 101,4, illetve
92,9. A szórás és a szélső értékek azonban nagy egyéni különbségekre utalnak. A
kisgyermekkori fejlődésre a születési súly, a bronchopulmonalis dysplasia, a
méhen belüli növekedési retardáció, a nem és az anya iskolai végzettsége volt a
leginkább hatással. A lányok jobban teljesĂtettek a nyelvi fejlĹ‘dĂ©st mĂ©rĹ‘
skálában, mint a fiĂşk. A verbális intelligenciát az agykamrai vĂ©rzĂ©s, mĂg a
performációs képességeket az intrauterin retardáció befolyásolta hátrányosan.
Következtetések: Vizsgálataink szerint az extrém kis
születési súlyú koraszülötteknek jó esélyük van a megfelelő értelmi fejlődésre,
azonban az egyĂ©ni fejlĹ‘dĂ©si perspektĂvákat sok tĂ©nyezĹ‘ befolyásolja, melyek
hatása területfüggő. A kisgyermekkori fejlődés szignifikáns előrejelzője a
későbbi intelligenciának; ez az összefüggés közepes mértékű, a fejlődés üteme
bármikor megváltozhat. A „mozgĂł rizikó” jelensĂ©ge felhĂvja a figyelmet a

Abstract:
Introduction: Owing to the rapid progress of the medical science
and technology, the chances of survival of the extremely low birth weight
(<1000 g) preterm babies have dramatically improved. Nevertheless, the
research findings on their long-term developmental outcome are inconsistent.
Aim: Our study has attempted to contribute to the
understanding of the developmental mechanisms in the extremely low birth weight
preterm infants and to the prediction of the developmental outcomes taking into
account of the risk factors of development. Method: 34 preterm
children who were free of any major central nervous system injury were followed
up from one to 5–6 years of age. The psychomotor development of the infants was
assessed at 1 and 2 years of age and, at 5–6 years of age, intelligence tests
were administered to the children. Perinatal and environmental factors were
included in the data analysis. Results: The extremely low birth
weight preterm children as a group displayed no developmental delay at any of
the measurement points. The mean developmental quotients were 98.6 and 106.6 at
ages 1 and 2 years, respectively. At 5–6 years of age, the mean verbal IQ was
101.4, while the mean performance IQ was 92.9. Behind the group means there was
a wide range of individual variations. The most powerful contributors to the
developmental outcomes were birth weight, bronchopulmonary dysplasia,
intra-uterine growth retardation, gender, and maternal education. The girls had
a significant advantage over the boys in language development. Verbal
intelligence was hampered by intracranial hemorrhage, while intrauterine growth
restriction had a similar effect on the performance IQ.
Conclusions: Our results suggest that even the extremely
preterm infants may have rather good chances of satisfactory mental development.
However, the individual developmental prospects are influenced by a great number
of domain-specific risk- and protective factors. Although at a group-level,
infant psychomotor development is a significant predictor of later intelligence
quotient, the rate of development may change at any age. The phenomenon of
“moving risk” underscores the importance of the long-term follow-up of preterm

Comparative Network Analysis of Preterm vs. Full-Term Infant-Mother Interactions
Several studies have reported that interactions of mothers with preterm infants show differential characteristics compared to that of mothers with full-term infants. Interaction of preterm dyads is often reported as less harmonious. However, observations and explanations concerning the underlying mechanisms are inconsistent. In this work 30 preterm and 42 full-term mother-infant dyads were observed at one year of age. Free play interactions were videotaped and coded using a micro-analytic coding system. The video records were coded at one second resolution and studied by a novel approach using network analysis tools. The advantage of our approach is that it reveals the patterns of behavioral transitions in the interactions. We found that the most frequent behavioral transitions are the same in the two groups. However, we have identified several high and lower frequency transitions which occur significantly more often in the preterm or full-term group. Our analysis also suggests that the variability of behavioral transitions is significantly higher in the preterm group. This higher variability is mostly resulted from the diversity of transitions involving non-harmonious behaviors. We have identified a maladaptive pattern in the maternal behavior in the preterm group, involving intrusiveness and disengagement. Application of the approach reported in this paper to longitudinal data could elucidate whether these maladaptive maternal behavioral changes place the infant at risk for later emotional, cognitive and behavioral disturbance
Az intelligencia alakulása gyermekkorban: egy 10 éves longitudinális vizsgálat módszertani tanulságai = The Development of Intelligence in Childhood: Methodological Implications of a 10-year Longitudinal Study
Bár az IQ kĂ©tsĂ©gkĂvĂĽl az egyik leggyakrabban használt mĂ©rĹ‘szám a pszicholĂłgiában, kevĂ©s az azzal kapcsolatos adat, hogyan változik az IQ az Ă©letkor fĂĽggvĂ©nyĂ©ben. MĂ©g kevesebbet tudunk arrĂłl, hogyan alakul az atipikus gyermekek intelligenciája.
Kutatásunkban egy 55 fĹ‘s koraszĂĽlött gyermekcsoport fejlĹ‘dĂ©sĂ©t tanulmányoztuk. A koraszĂĽlöttsĂ©g perinatális veszĂ©lyeztetettsĂ©gnek tekinthetĹ‘. A kontrollcsoportot 100 fĹ‘s, idĹ‘re szĂĽletett gyermek kĂ©pezte. A mĂ©rĂ©sek eredmĂ©nyei szerint mindkĂ©t csoport átlagos intelligenciája a normál övezetbe tartozott. A koraszĂĽlött Ă©s az idĹ‘re szĂĽletett csoport IQ-jának eltĂ©rĹ‘ longitudinális mintázata aláhĂşzza az Ă©letkor mint változĂł jelentĹ‘sĂ©gĂ©t.
