    Seismo-electromagnetic phenomena in the western part of the Eurasia-Nubia plate boundary

    This paper presents a future research plan that aims to monitor Seismo-electromagnetic (SEM) phenomena in the western part of the Eurasia-Nubia plate boundary (WENP). This region has a significant tectonic activity [1] combined with relatively low electromagnetic noise levels and for that reason presents the possibility to perform high quality SEM measurements. Further, it is known that low-frequency [ultra (ULF), very (VLF), and low-frequencies (LF)] electromag- netic (EM) waves produce more convincing earthquake precursors (compared to higher frequencies) because of less contamination, large skin depth, and low attenuation [2]. Thus, two SEM effects will be considered: ULF electromagnetic field emissions [3], and VLF/LF radio broadcastings [4]. With respect to the ULF measurements, as a start, three ULF sensors are planned to be installed in the South of Iberian Peninsula supported by the existing networks of seismic research stations. Subsequent devel- opment of this initial plan could result in the implementation of a lager ULF monitoring network not only in the Iberian Peninsula, but also in the rest of Europe. Possible integration in the SEGMA array is now under consideration. Another perspective is to use a portable station to track seismic events. Regarding the VLF/LF radio broadcastings, a receiver is planned to be mounted in University of Évora. Radio signals from up to 10 transmitters (in these bands) of interest to study the seismic activity in the WENP region will be monitored. Actually, the radio path from the transmitter to the receiver should cross the epicentral area, therefore two possible transmitters are the ones installed in Monaco (France) and Sicily (Italy). Furthermore, the system will integrate the INFREP network and in this context it will not be restricted to WENP region. With the development of these research plans we aim to collect novel SEM data emerging from the seis- mic activity in the WENP region. We expect to address the time variations of EM properties of the crust/plate in relation with the strain field, and in space in relation with composition and temperature and stress fields. Further, the interplay between atmospheric (and solar) perturbations with crust perturbations will be monitored, to observe geomagnetic perturbations at different locations. Our study will be focused in the analyses of low magnitude earthquakes with M =< 4, these events are frequent in the WENP region, but have been almost completely disregarded in literature [5,6].     Acute kidney injury in pregnancy: a clinical challenge

    The incidence of acute kidney injury in pregnancy declined significantly over the second half of the 20th century; however, it is still associated with major maternal and perinatal morbidity and mortality. A set of systemic and renal physiological adaptive mechanisms occur during a normal gestation that will constrain several changes in laboratory parameters of renal function, electrolytes, fluid and acid-base balances. The diagnosis of acute kidney injury in pregnancy is based on the serum creatinine increase. The usual formulas for estimating glomerular filtration rate are not validated in this population. During the first trimester of gestation, acute kidney injury develops most often due to hyperemesis gravidarum or septic abortion. In the third trimester, the differential diagnosis is more challenging for the obstetrician and the nephrologist and comprises some pathologies that are reviewed in this article: preeclampsia/HELLP syndrome, acute fatty liver of pregnancy and thrombotic microangiopathies

    Saphenous vein graft bypass in the treatment of giant cavernous sinus aneurysms - Report of two cases

    Two cases of giant intracavernous aneurysms treated by high now bypass with saphenous vein graft between the external carotid artery (ECA) and branches of the middle cerebral artery (MCA) are presented. Very often these aneurysms are unclippable because they are fusiform or have a large neck, Occlusion of the internal carotid artery (ICA) is the treatment of choice in many eases, This procedure has however a high risk of brain infarction. Revascularization of the brain by extra-intracranial anastomosis between the superficial temporal artery (STA) and branches of the MCA is frequently performed. This procedure provides however a low flow bypass and brain infarction may occur, We report two cases of giant cavernous sinus aneurysms treated by high flow bypass and endovascular balloon occlusion of the LCA, Immediate high now revascularization of MCA branches was achieved and the patients showed no ischemic events. Follow-up of 8 and 14 months alter operation shows patency of the venous graft and no neurological deficits, Angiographic control examination showed complete aneurysm occlusion in both cases.58116216

    Painful os intermetatarseum is a very rare condition. Gruber et al. first described os intermetatarseum in 1877. This condition is usually asymptomatic. One should consider painful os intermetatarseum as being a possible cause of dorsal foot pain in athletes. Surgical excision of the os intermetatarseum should be considered for those patients failing conservative treatment. Here, a literature review of this condition is presented

    Effects of terlipressin as early treatment for protection of brain in a model of haemorrhagic shock

    Introduction: We investigated whether treatment with terlipressin during recovery from hypotension due to haemorrhagic shock (HS) is effective in restoring cerebral perfusion pressure (CPP) and brain tissue markers of water balance, oxidative stress and apoptosis. Methods: In this randomised controlled study, animals undergoing HS (target mean arterial pressure (MAP) 40 mmHg for 30 minutes) were randomised to receive lactated Ringer’s solution (LR group; n =14; volume equal to three times the volume bled), terlipressin (TERLI group; n =14; 2-mg bolus), no treatment (HAEMO group; n =12) or sham (n =6). CPP, systemic haemodynamics (thermodilution technique) and blood gas analyses were registered at baseline, shock and 5, 30, 60 (T60), 90 and 120 minutes after treatment (T120). After the animals were killed, brain tissue samples were obtained to measure markers of water balance (aquaporin-4 (AQP4)), Na+-K+-2Cl− co-transporter (NKCC1)), oxidative stress (thiobarbituric acid reactive substances (TBARS) and manganese superoxide dismutase (MnSOD)) and apoptotic damage (Bcl-x and Bax). Results: Despite the HS-induced decrease in cardiac output (CO) and hyperlactataemia, resuscitation with terlipressin recovered MAP and resulted in restoration of CPP and in cerebral protection expressed by normalisation of AQP4, NKCC1, TBARS and MnSOD expression and Bcl-x/Bax ratio at T60 and T120 compared with sham animals. In the LR group, CO and blood lactate levels were recovered, but the CPP and MAP were significantly decreased and TBARS levels and AQP4, NKCC1 and MnSOD expression and Bcl-x/Bax ratio were significantly increased at T60 and T120 compared with the sham group. Conclusions: During recovery from HS-induced hypotension, terlipressin was effective in normalising CPP and cerebral markers of water balance, oxidative damage and apoptosis. The role of this pressor agent on brain perfusion in HS requires further investigation

    Allergic Rhinitis and its Associated Co-Morbidities at Bugando Medical Centre in Northwestern Tanzania; A Prospective Review of 190 Cases.

    Allergic rhinitis is one of the commonest atopic diseases which contribute to significant morbidity world wide while its epidemiology in Tanzania remains sparse. There was paucity of information regarding allergic rhinitis in our setting; therefore it was important to conduct this study to describe our experience on allergic rhinitis, associated co-morbidities and treatment outcome in patients attending Bugando Medical Centre. This was descriptive cross-sectional study involving all patients with a clinical diagnosis of allergic rhinitis at Bugando Medical Centre over a three-month period between June 2011 and August 2011. Data was collected using a pre-tested coded questionnaire and analyzed using SPSS statistical computer software version 17.0. A total of 190 patients were studied giving the prevalence of allergic rhinitis 14.7%. The median age of the patients was 8.5 years. The male to female ratio was 1:1. Adenoid hypertrophy, tonsillitis, hypertrophy of inferior turbinate, nasal polyps, otitis media and sinusitis were the most common co-morbidities affecting 92.6% of cases and were the major reason for attending hospital services. Sleep disturbance was common in children with adenoids hypertrophy (χ2 = 28.691, P = 0.000). Allergic conjunctivitis was found in 51.9%. The most common identified triggers were dust, strong perfume odors and cold weather (P < 0.05). Strong perfume odors affect female than males (χ2 = 4.583, P = 0.032). In this study family history of allergic rhinitis was not a significant risk factor (P =0.423). The majority of patients (68.8%) were treated surgically for allergic rhinitis co morbidities. Post operative complication and mortality rates were 2.9% and 1.6% respectively. The overall median duration of hospital stay of in-patients was 3 days (2 - 28 days). Most patients (98.4%) had satisfactory results at discharge. The study shows that allergic rhinitis is common in our settings representing 14.7% of all otorhinolaryngology and commonly affecting children and adolescent. Sufferers seek medical services due to co-morbidities of which combination of surgical and medical treatment was needed. High index of suspicions in diagnosing allergic rhinitis and early treatment is recommended

    Impact of Vitamin D Supplementation on Arterial Vasomotion, Stiffness and Endothelial Biomarkers in Chronic Kidney Disease Patients

    Background: Cardiovascular events are frequent and vascular endothelial function is abnormal in patients with chronic kidney disease (CKD). We demonstrated endothelial dysfunction with vitamin D deficiency in CKD patients; however the impact of cholecalciferol supplementation on vascular stiffness and vasomotor function, endothelial and bone biomarkers in CKD patients with low 25-hydroxy vitamin D [25(OH)D] is unknown, which this study investigated. Methods: We assessed non-diabetic patients with CKD stage 3/4, age 17–80 years and serum 25(OH)D ,75 nmol/L. Brachial artery Flow Mediated Dilation (FMD), Pulse Wave Velocity (PWV), Augmentation Index (AI) and circulating blood biomarkers were evaluated at baseline and at 16 weeks. Oral 300,000 units cholecalciferol was administered at baseline and 8-weeks. Results: Clinical characteristics of 26 patients were: age 50614 (mean61SD) years, eGFR 41611 ml/min/1.73 m2, males 73%, dyslipidaemia 36%, smokers 23% and hypertensives 87%. At 16-week serum 25(OH)D and calcium increased (43616 to 84629 nmol/L, p,0.001 and 2.3760.09 to 2.4260.09 mmol/L; p = 0.004, respectively) and parathyroid hormone decreased (10.868.6 to 7.464.4; p = 0.001). FMD improved from 3.163.3% to 6.163.7%, p = 0.001. Endothelial biomarker concentrations decreased: E-Selectin from 566662123 to 525662058 pg/mL; p = 0.032, ICAM-1, 3.4560.01 to 3.1061.04 ng/mL; p = 0.038 and VCAM-1, 54633 to 42633 ng/mL; p = 0.006. eGFR, BP, PWV, AI, hsCRP, von Willebrand factor and Fibroblast Growth Factor-23, remained unchanged. Conclusion: This study demonstrates for the first time improvement of endothelial vasomotor and secretory functions with vitamin D in CKD patients without significant adverse effects on arterial stiffness, serum calcium or FGF-23. A captação das falas deu-se através de entrevista gravadas com equipamentoeletrônico MP3.Resultados: A partir da leitura atenta e buscando as similaridades entre os relatos, emergiramduas categorias concretas do vivido: Auto-ordenhar possibilita o melhor para o bebê eSuperar as suas dificuldades em auto-ordenhar na perspectiva da produção de leite.Conclusão: O estudo aponta para a importância da escuta sensível para captar o entendimentodas mães sobre a importância da auto-ordenha, valorizando o seu projeto intencional, naperspectiva do melhor direcionamento das orientações a serem realizadas pelo profissional deenfermagem.Palavras-chave: Aleitamento materno.
