    It is imperative that nurses caring for children, young people and their families develop and maintain effective, trusting and collaborative therapeutic relationships that sit within the scope of professional boundaries. This relationship is the nurse’s responsibility and should be positive and mutually acceptable to all stakeholders. A unique challenge for children’s nurses is to address and prioritise the child’s care needs, while meeting the needs of, and empowering, the family. The 6Cs – care, compassion, competence, communication, courage, commitment – should underpin care and enable nurses to overcome challenges such as time pressures, acute situations or disturbed family expectation.     The scaling of X-ray variability with luminosity in Ultra-luminous X-ray sources

    We investigated the relationship between the X-ray variability amplitude and X-ray luminosity for a sample of 14 bright Ultra-luminous X-ray sources (ULXs) with XMM-Newton/EPIC data, and compare it with the well established similar relationship for Active Galactic Nuclei (AGN). We computed the normalised excess variance in the 2-10 keV light curves of these objects and their 2-10 keV band intrinsic luminosity. We also determined model "variability-luminosity" relationships for AGN, under several assumptions regarding their power-spectral shape. We compared these model predictions at low luminosities with the ULX data. The variability amplitude of the ULXs is significantly smaller than that expected from a simple extrapolation of the AGN "variability-luminosity" relationship at low luminosities. We also find evidence for an anti-correlation between the variability amplitude and L(2-10 keV) for ULXs. The shape of this relationship is consistent with the AGN data but only if the ULXs data are shifted by four orders of magnitudes in luminosity. Most (but not all) of the ULXs could be "scaled-down" version of AGN if we assume that: i) their black hole mass and accretion rate are of the order of ~(2.5-30)x 10E+03 Msolar and ~ 1-80 % of the Eddington limit, and ii) their Power Spectral Density has a doubly broken power-law shape.     Aquaporin-4 (AQP4) is a water-channel protein expressed strongly in the brain, predominantly in astrocyte foot processes at the borders between the brain parenchyma and major fluid compartments, including cerebrospinal fluid (CSF) and blood. This distribution suggests that AQP4 controls water fluxes into and out of the brain parenchyma. Experiments using AQP4-null mice provide strong evidence for AQP4 involvement in cerebral water balance. AQP4-null mice are protected from cellular (cytotoxic) brain edema produced by water intoxication, brain ischemia, or meningitis. However, AQP4 deletion aggravates vasogenic (fluid leak) brain edema produced by tumor, cortical freeze, intraparenchymal fluid infusion, or brain abscess. In cytotoxic edema, AQP4 deletion slows the rate of water entry into brain, whereas in vasogenic edema, AQP4 deletion reduces the rate of water outflow from brain parenchyma. AQP4 deletion also worsens obstructive hydrocephalus. Recently, AQP4 was also found to play a major role in processes unrelated to brain edema, including astrocyte migration and neuronal excitability. These findings suggest that modulation of AQP4 expression or function may be beneficial in several cerebral disorders, including hyponatremic brain edema, hydrocephalus, stroke, tumor, infection, epilepsy, and traumatic brain injury

    MDMA and brain activity during neurocognitive performance: An overview of neuroimaging studies with abstinent 'Ecstasy' users.

    MDMA/Ecstasy has had a resurgence in popularity, with recent supplies comprising higher strength MDMA, potentially leading to increased drug-related harm. Neurocognitive problems have been widely reported in ecstasy users, equally some studies report null findings, and it remains unclear which factors underlie the development of neurocognitive impairments. This review covers the empirical research into brain activity during neurocognitive performance, using fMRI, fNIRS, and EEG. Our main conclusion is that chronic repeated use of recreational ecstasy can result in haemodynamic and electrophysiological changes that reflect recruitment of additional resources to perform cognitive tasks. Findings are consistent with serotonergic system changes, although whether this reflects neurotoxicity or neuroadaptation, cannot be answered from these data. There is a degree of heterogeneity in the methodologies and findings, limiting the strengths of current conclusions. Future research with functional neuroimaging paired with molecular imaging, genetics or pharmacological challenges of the serotonin system may help to decipher the link between serotonergic and cognitive changes in ecstasy users

    Unit Interval Editing is Fixed-Parameter Tractable

    Given a graph~GG and integers k1k_1, k2k_2, and~k3k_3, the unit interval editing problem asks whether GG can be transformed into a unit interval graph by at most k1k_1 vertex deletions, k2k_2 edge deletions, and k3k_3 edge additions. We give an algorithm solving this problem in time 2O(klogk)(n+m)2^{O(k\log k)}\cdot (n+m), where k:=k1+k2+k3k := k_1 + k_2 + k_3, and n,mn, m denote respectively the numbers of vertices and edges of GG. Therefore, it is fixed-parameter tractable parameterized by the total number of allowed operations. Our algorithm implies the fixed-parameter tractability of the unit interval edge deletion problem, for which we also present a more efficient algorithm running in time O(4k(n+m))O(4^k \cdot (n + m)). Another result is an O(6k(n+m))O(6^k \cdot (n + m))-time algorithm for the unit interval vertex deletion problem, significantly improving the algorithm of van 't Hof and Villanger, which runs in time O(6kn6)O(6^k \cdot n^6).Comment: An extended abstract of this paper has appeared in the proceedings of ICALP 2015.     Effect of time and day of admission on hospital care quality for patients with chronic obstructive pulmonary disease exacerbation in England and Wales: single cohort study

    OBJECTIVE: To evaluate if observed increased weekend mortality was associated with poorer quality of care for patients admitted to hospital with chronic obstructive pulmonary disease (COPD) exacerbation. DESIGN: Prospective case ascertainment cohort study. SETTING: 199 acute hospitals in England and Wales, UK. PARTICIPANTS: Consecutive COPD admissions, excluding subsequent readmissions, from 1 February to 30 April 2014 of whom 13 414 cases were entered into the study. MAIN OUTCOMES: Process of care mapped to the National Institute for Health and Care Excellence clinical quality standards, access to specialist respiratory teams and facilities, mortality and length of stay, related to time and day of the week of admission. RESULTS: Mortality was higher for weekend admissions (unadjusted OR 1.20, 95% CI 1.00 to 1.43), and for case-mix adjusted weekend mortality when calculated for admissions Friday morning through to Monday night (adjusted OR 1.19, 95% CI 1.00 to 1.43). Median time to death was 6 days. Some clinical processes were poorer on Mondays and during normal working hours but not weekends or out of hours. Specialist respiratory care was less available and less prompt for Friday and Saturday admissions. Admission to a specialist ward or high dependency unit was less likely on a Saturday or Sunday. CONCLUSIONS: Increased mortality observed in weekend admissions is not easily explained by deficiencies in early clinical guideline care. Further study of out-of-hospital factors, specialty care and deaths later in the admission are required if effective interventions are to be made to reduce variation by day of the week of admission

    Dissolved noble gases and stable isotopes as tracers of preferential fluid flow along faults in the Lower Rhine Embayment, Germany

    Groundwater in shallow unconsolidated sedimentary aquifers close to the Bornheim fault in the Lower Rhine Embayment (LRE), Germany, has relatively low δ2H and δ18O values in comparison to regional modern groundwater recharge, and 4He concentrations up to 1.7 × 10−4 cm3 (STP) g–1 ± 2.2 % which is approximately four orders of magnitude higher than expected due to solubility equilibrium with the atmosphere. Groundwater age dating based on estimated in situ production and terrigenic flux of helium provides a groundwater residence time of ∼107 years. Although fluid exchange between the deep basal aquifer system and the upper aquifer layers is generally impeded by confining clay layers and lignite, this study’s geochemical data suggest, for the first time, that deep circulating fluids penetrate shallow aquifers in the locality of fault zones, implying  that sub-vertical fluid flow occurs along faults in the LRE. However, large hydraulic-head gradients observed across many faults suggest that they act as barriers to lateral groundwater flow. Therefore, the geochemical data reported here also substantiate a conduit-barrier model of fault-zone hydrogeology in unconsolidated sedimentary deposits, as well as corroborating the concept that faults in unconsolidated aquifer systems can act as loci for hydraulic connectivity between deep and shallow aquifers. The implications of fluid flow along faults in sedimentary basins worldwide are far reaching and of particular concern for carbon capture and storage (CCS) programmes, impacts of deep shale gas recovery for shallow groundwater aquifers, and nuclear waste storage sites where fault zones could act as potential leakage pathways for hazardous fluids

    Emotions and Digital Well-being. The rationalistic bias of social media design in online deliberations

    In this chapter we argue that emotions are mediated in an incomplete way in online social media because of the heavy reliance on textual messages which fosters a rationalistic bias and an inclination towards less nuanced emotional expressions. This incompleteness can happen either by obscuring emotions, showing less than the original intensity, misinterpreting emotions, or eliciting emotions without feedback and context. Online interactions and deliberations tend to contribute rather than overcome stalemates and informational bubbles, partially due to prevalence of anti-social emotions. It is tempting to see emotions as being the cause of the problem of online verbal aggression and bullying. However, we argue that social media are actually designed in a predominantly rationalistic way, because of the reliance on text-based communication, thereby filtering out social emotions and leaving space for easily expressed antisocial emotions. Based on research on emotions that sees these as key ingredients to moral interaction and deliberation, as well as on research on text-based versus non-verbal communication, we propose a richer understanding of emotions, requiring different designs of online deliberation platforms. We propose that such designs should move from text-centred designs and should find ways to incorporate the complete expression of the full range of human emotions so that these can play a constructive role in online deliberations
