     本研究では,すでに長年にわたって専門的で組織的に展開している,米国カンザス州ブルーバレー学区の高校におけるソーシャルスキルの授業について,①生徒の日常生活に必要なソーシャルスキルを育てるための指導内容の工夫,②障害のある生徒と障害のない生徒の交流及び共同学習を促すための工夫,の観点から分析した。①については,日常生活で直面する問題が題材とされていたこと,学習されたスキルを日常生活全般に渡って使う機会が設定されていたこと,さらには,その使用について振り返りをする時間が授業の中に組み込まれていたこと,の3つが見いだされた。②については,障害のある生徒も障害のない生徒も同じ目標と内容を共有できるようにしていたこと,仲間意識や連帯感の形成過程を視覚的に示すようにしていたこと,互いに支え合う関係を築くことができるような活動を設定していたこと,ピアサポーター育成を授業の中に組み入れていたこと,の4つが導き出された。以上の点は,今後のわが国の高校におけるソーシャルスキルの授業に示唆を与えるものであると 考えられた

    Optimizing the timing of 3.6 mg Pegfilgrastim Administration for Dose-Dense Chemotherapy in Japanese Patients with Breast Cancer

    Perioperative dose-dense chemotherapy (DDCT) with pegfilgrastim (Peg) prophylaxis is a standard treatment for high-risk breast cancer. We explored the optimal timing of administration of 3.6 mg Peg, the dose approved in Japan. In the phase II feasibility study of DDCT (adriamycin+cyclophosphamide or epirubicin+cyclophosphamide followed by paclitaxel) for breast cancer, we investigated the feasibility, safety, neutrophil transition, and optimal timing of Peg treatment by administering Peg at days 2, 3, and 4 post-chemotherapy (P2, P3, and P4 groups, respectively). Among the 52 women enrolled, 13 were aged > 60 years. The anthracycline sequence was administered to P2 (n=33), P3 (n=5), and P4 (n=14) patients, and the taxane sequence to P2 (n=38) and P3 (n=6) patients. Both sequences showed no interaction between Peg administration timing and treatment discontinuation, treatment delay, or dose reduction. However, the relative dose intensity (RDI) was significantly different among the groups. The neutrophil count transition differed significantly among the groups receiving the anthracycline sequence. However, the neutrophil count remained in the appropriate range for both sequences in the P2 group. The timing of Peg administration did not substantially affect the feasibility or safety of DDCT. Postoperative day 2 might be the optimal timing for DDCT

    Cloning and characterization of a novel alternatively spliced transcript of the human CHD7 putative helicase

    <p>Abstract</p> <p>Background</p> <p>The <it>CHD7 </it>(Chromodomain Helicase DNA binding protein 7) gene encodes a member of the chromodomain family of ATP-dependent chromatin remodeling enzymes. Mutations in the <it>CHD7 </it>gene are found in individuals with CHARGE, a syndrome characterized by multiple birth malformations in several tissues. CHD7 was identified as a binding partner of PBAF complex (Polybromo and BRG Associated Factor containing complex) playing a central role in the transcriptional reprogramming process associated to the formation of multipotent migratory neural crest, a transient cell population associated with the genesis of various tissues. <it>CHD7 </it>is a large gene containing 38 annotated exons and spanning 200 kb of genomic sequence. Although genes containing such number of exons are expected to have several alternative transcripts, there are very few evidences of alternative transcripts associated to <it>CHD7 </it>to date indicating that alternative splicing associated to this gene is poorly characterized.</p> <p>Findings</p> <p>Here, we report the cloning and characterization by experimental and computational studies of a novel alternative transcript of the human <it>CHD7 </it>(named CHD7 CRA_e), which lacks most of its coding exons. We confirmed by overexpression of CHD7 CRA_e alternative transcript that it is translated into a protein isoform lacking most of the domains displayed by the canonical isoform. Expression of the CHD7 CRA_e transcript was detected in normal liver, in addition to the DU145 human prostate carcinoma cell line from which it was originally isolated.</p> <p>Conclusions</p> <p>Our findings indicate that the splicing event associated to the CHD7 CRA_e alternative transcript is functional. The characterization of the CHD7 CRA_e novel isoform presented here not only sets the basis for more detailed functional studies of this isoform, but, also, contributes to the alternative splicing annotation of the <it>CHD7 </it>gene and the design of future functional studies aimed at the elucidation of the molecular functions of its gene products.</p

    The activation of Proteinase-Activated Receptor-1 (PAR1) mediates gastric cancer cell proliferation and invasion

    <p>Abstract</p> <p>Background</p> <p>In addition to regulating platelet function, the G protein-coupled sub-family member Proteinase-activated receptor-1 (PAR1) has a proposed role in the development of various cancers, but its exact role and mechanism of action in the invasion, metastasis, and proliferation process in gastric cancer have yet to be completely elucidated. Here, we analyzed the relationship between PAR1 activation, proliferation, invasion, and the signaling pathways downstream of PAR1 activation in gastric cancer.</p> <p>Methods</p> <p>We established a PAR1 stably transfected MKN45 human gastric cancer cell line (MKN45/PAR1) and performed cell proliferation and invasion assays employing this cell line and MKN28 cell line exposed to PAR1 agonists (α-thrombin and TFLLR-NH<sub>2</sub>). We also quantified NF-κB activation by electrophoretic mobility shift assay (EMSA) and the level of Tenascin-C (TN-C) expression in conditioned medium by ELISA of MKN45/PAR1 following administration of α-thrombin. A high molecular weight concentrate was derived from the resultant conditioned medium and subsequent cultures of MKN45/PAR1 and MKN28 were exposed to the resultant concentrate either in the presence or absence of TN-C-neutralizing antibody. Lysates of these subsequent cells were probed to quantify levels of phospholyrated Epidermal Growth Factor Receptor (EGFR).</p> <p>Result</p> <p>PAR1 in both PAR1/MKN45 and MKN28 was activated by PAR1 agonists, resulting in cell proliferation and matrigel invasion. We have shown that activation of NF-κB and EGFR phosphorylation initially were triggered by the activation of PAR1 with α-thrombin. Quantitative PCR and Western blot assay revealed up-regulation of mRNA and protein expression of NF-κB target genes, especially TN-C, a potential EGFR activator. The suppressed level of phosphorylated EGFR, observed in cells exposed to concentrate of conditioned medium in the presence of TN-C-neutralizing antibody, identifies TN-C as a putative autocrine stimulatory factor of EGFR possibly involved in the sustained PAR1 activation responses observed.</p> <p>Conclusion</p> <p>Our data indicate that in gastric carcinoma cells, PAR1 activation can trigger an array of responses that would promote tumor cell growth and invasion. Over expression of NF-κB, EGFR, and TN-C, are among the effects of PAR1 activation and TN-C induces EGFR activation in an autocrine manner. Thus, PAR1 is a potentially important therapeutic target for the treatment of gastric cancer.</p

    Relation between health literacy, self-care and adherence to treatment with oral anticoagulants in adults: a narrative systematic review.

    Background Oral anticoagulants (OAC) are widely used in patients with cardiovascular diseases. However, for optimal OAC self-care patients must have skills, among which health literacy (HL) is highlighted. We aimed to describe the relation between HL and self-care in cardiovascular patients on OAC treatment. Methods Electronic searches were carried out in the PubMed, Scopus, Embase, CINAHL, Web of Science, Cochrane Library, SciELO, IME-Biomedicina, CUIDEN Plus and LILACS databases, limited to Spanish and English language and between January 2000-December 2016. Papers reported on adults older than 18 years, taking OAC by themselves for at least three months. PRISMA guidelines were used for paper selection. Results We identified 142 articles and finally included 10; almost all of them about warfarin. Our results suggest that in patients taking OAC treatments there is a positive relationship between HL and the level of knowledge. In addition, a small percentage of participants on the selected papers recognized the side effects and complications associated with OAC treatment. Lower HL level was associated with greater knowledge deficits and less adherence to treatment. Conclusion There is a paucity of research evaluating the effect of HL on diverse aspects of OAC treatments. There is a need to expand the evidence base regarding appropriate HL screening tools, determinants of adequate knowledge and optimal behaviours related to OAC self-management

    Development of a soy-based dessert enriched with calcium and high protein content

    Orientador: Roberto Herminio MorettiDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Engenharia de AlimentosResumo: Produtos comerciais a base de extrato hidrossolúvel de soja em combinação com sucos de frutas têm obtido êxito no mercado, como alternativa proteica com benefícios funcionais. No entanto, a quantidade de proteína de soja nessas associações é relativamente baixa no mercado nacional, de 0,6 a 3,0%. O presente trabalho constituiu no desenvolvimento uma sobremesa a base de soja com características semelhantes aos queijos petit suisse, com teor de proteína de soja acima dos produtos a base de extrato hidrossolúvel de soja combinados com sucos de frutas encontrados hoje no mercado brasileiro, e com características sensoriais aceitas pelo consumidor. As sobremesas de soja foram desenvolvidas a partir da coagulação do extrato hidrossolúvel de soja sulfato de cálcio (CaSO4.2H2O) em concentrações de variando de 0,25 a 1%, acréscimo de goma guar em concentrações de 0,02 a 0,2% e ajuste de pH (4,0 a 4,4) com ácido lático 85%. O efeito dessas variáveis foi avaliado sobre a composição física e química, o conteúdo de cálcio, o rendimento em proteína e a firmeza das sobremesas de soja obtidas nos ensaios experimentais. Todas as variáveis foram influenciadas significativamente (p=0,05) pela concentração de CaSO4.2H2O. O teor de sólidos totais e o conteúdo de cálcio elevaram-se com o aumento da concentração de coagulante. O teor de proteína e lipídios e a firmeza apresentaram forte correlação entre si, com tendência crescente com o aumento da concentração de CaSO4.2H2O até 0,88%, seguido de um declínio dos valores dessas variáveis com o acréscimo de coagulante até 1% CaSO4.2H2O. Somente a acidez foi influenciada significativamente pelo pH, e a interação do pH com a goma guar teve efeito negativo significativo no rendimento em proteína. Nem a goma guar nem as outras interações das variáveis independentes tiveram influência significativa sobre as variáveis dependentes. Os ensaios escolhidos para a avaliação sensorial, pela composição físico-química e qualidade aparente que apresentaram, foram o ensaio com 0,88% CaSO4.2H2O; 0,05% goma guar e pH 4,3 (ensaio 6), e o ensaio com 0,63% CaSO4.2H2O, 0,02% goma guar e pH 4,2 (ensaio 11). As amostras diferiram significativamente somente no atributo 'textura', sendo o ensaio 6 mais aceito sensorialmente pelos provadores, caracterizando-se como a melhor formulação para o desenvolvimento da sobremesa de soja tipo petit suisse, dentro dos limites experimentaisAbstract: Commercial products produced from soymilk in combination with fruit juices has been successful in the market, being an alternative protein source with functional benefits. However, protein content in these soybean associations is relatively low in the national market, about 0.6 to 3.0%. The present study was the development of a soy-based dessert with similar features of petit suisse cheeses, with higher soy-protein content than the products from soymilk in combination with fruit juices found actually in the brazilian market, and with acceptable sensory characteristics by the consumer. The soy desserts were made from soymilk coagulation with calcium sulphate concentrations (CaSO4.2H2O) ranging from 0.25 to 1%, with addition of guar gum at 0.02 to 0.2% concentrations and pH correction (4.0 to 4.4) with lactic acid 85%. The effect of these variables CaSO4.2H2O, guar gum and pH) were evaluated on the physical and chemical composition, the calcium content, the protein recovery (yield in protein) and the firmness of the soy desserts obtained from the experimental trials. All variables were significantly influenced (p = 0.05) by the concentration of CaSO4.2H2O. The total solids and calcium content increased with increasing coagulant concentration. The protein and lipids content and the firmness, showed a strong relationship between them. The protein content and the firmness trend was toward an increase with increasing concentration of CaSO4.2H2O up to 0.88%, followed by a descrease at 1% CaSO4.2H2O. Only the titrable acidity was affected by pH. The pH and guar gum interaction had significant negative effect on yield in protein. Neither guar gum nor any other interactions of the independent variables, had significant influence on the dependent variables. The tests chosen for the sensory evaluation, by the physico-chemical composition and better quality apparent, were the test with CaSO4.2H2O, 0.05% guar gum and pH 4.3 (test 6), and the test with 0.63% CaSO4.2H2O, 0.02% guar gum and pH 4.2 (test 11). The samples showed difference only on the attribute of 'texture', with test 6 being the most accepted test for the sensory panelists, electing it as the best formulation for the development of soybean dessert like petit suisse cheese, within the experimental limitsMestradoMestre em Tecnologia de Alimento
