    Le phénotype "fit-actif" : pour mieux refléter les bienfaits de la participation à l'activité physique vigoureuse au cours de la vie sur la santé

    Les bienfaits d’une pratique régulière de l’activité physique sur la composition corporelle et ses complications métaboliques sont bien établis dans la littérature. Cependant, les interventions cliniques prônant une perte de poids par la pratique d’activité physique seule ne procureraient pas les bénéfices recherchés sur la composition corporelle et la santé métabolique contrairement à une intervention de saines habitudes alimentaires prescrite à elle seule. Cela soulève la question à savoir dans quelle mesure le stimulus de l’exercice a été suffisamment caractérisé pour refléter l’empreinte à long terme d’un mode de vie actif sur le risque de développer l’obésité et les complications métaboliques associées. Ce mémoire documente ce problème en résumant les évidences et en présentant des données montrant qu’une classification basée sur la capacité aérobie et la pratique d’activité physique permettent de mieux refléter l’impact d’une pratique d’activité physique vigoureuse tout au long de la vie sur la composition corporelle et la santé métabolique. En outre, nous renforçons la pertinence de cette classification en présentant des données provenant de l’Étude des familles de Québec (QFS) comparant les individus « fit-actifs » à ceux « unfit-inactifs ». Les résultats des deux sexes montrent que les individus « fitactifs » présentent des profils morphologique et métabolique beaucoup plus favorables que ceux des individus « unfit-inactifs ». Les bénéfices semblent provenir davantage des variations de condition cardiorespiratoire que celles de la pratique d’activité physique. En résumé, ces observations suggèrent que les bénéfices d’une pratique d’activité physique vigoureuse à long terme seraient bien représentés par une classification basée sur la capacité aérobie et la pratique d’activité physique rapportée.Regular physical activity participation has been recognized over time as an essential component of a lifestyle contributing to the prevention of obesity and non-communicable diseases. Recently, the emphasis on sedentary behaviors and physical inactivity as additional factors, independent of physical activity practice, has to some extent confirmed the relevance to encourage regular physical activity habits in the prevention and management of obesity. This optimistic vision is however toned down by some clinical interventions that demonstrate that the inclusion of a physical activity component in a weight reducing program is not producing substantial additional benefits on body composition and metabolic health compared to when a healthy diet is prescribed alone. This raises the question as to which extent the exercise stimulus has been adequately characterized to reflect the long-term print of an active lifestyle on the risk to develop obesity and related metabolic complications. This thesis documents this issue by summarizing evidences and presenting relevant data showing that a classification based on aerobic fitness and physical activity practice provides indications of greater impact of a lifelong vigorous physical activity practice compared to what may be anticipated from existing literature. Furthermore, we reinforce the pertinence of this classification by presenting relevant data from the Quebec Family Study to compare the fit-active to the unfit-inactive individuals. For both sexes, fit-active individuals displayed much more favorable morphological and metabolic profiles than their unfit-inactive counterparts. The fitness variations of the profile seem to generate more benefits than the physical activity practice in the profile. In summary, these observations suggest that the benefits of a lifelong physical activity practice may well be reflected by a classification based on both fitness and reported physical activity practice

    Activity of Aloe arborescens leaf extracts: in vitro effects on Human Adenocarcinoma HT-29 cell line and free-radical species.

    Aloe arborescens Mill. (syn. Candelabra Aloe; Xanthorrhoeaceae) is native to South Africa; in Italy it is grown for ornamental, therapeutic and cosmetic uses. Various Aloe spp. have been used since ancient times and have a well-documented history of use as medicines. Nowadays, it is widely studied as potential anticancer, antidiabetic, antimicrobial and for several other uses. The presence of aloin, aloe-emodin, polysaccharides, mannose and acemannan was reported in whole-leaf preparations. The aim of the present research was to study Aloe arborescens leaf extracts, also to improve the uses of Aloe since many products are marketed also in Western countries. For this purpose, methanolic and ethanolic extracts were obtained from freeze-dried leaves, and then studied using: a) HPLC-DAD analysis to determine aloins and aloe-emodin contents, b) two radical-scavenging assays to assess the radical scavenger activity, and 3) MTT assay on colorectal adenocarcinoma cell line (HT29 cells) to evaluate cell viability

    Diagnósticos de enfermagem de pacientes em período pré-operatório de cirurgia esofágica

    This study aimed to identify and analyze the nursing diagnoses of patients in the preoperative period of esophageal surgeries. Data were collected by means of an instrument, based on Horta's Conceptual Model. The study group consisted of 20 patients hospitalized at the Ribeirão Preto Medical School Hospital das Clínicas, who would undergo esophageal surgery. The patients were submitted to an interview and a physical examination. The nursing diagnoses identified at frequencies of more than 50% were: impaired swallowing (100%); risk for infection (100%); knowledge deficit about disease and perioperative period (95%), and chronic pain (75%). These diagnoses were analyzed in view of the related factors, defining characteristics or risk factors, according to the type of diagnosis and the responses to the esophageal disease.Este estudio tuvo como objetivos identificar y analizar los diagnósticos de enfermería de pacientes en el período pre operatorio de cirugías de esófago. Los datos fueron recolectados por medio de un instrumento fundamentado en el Modelo Conceptual de Horta. El grupo estudiado fue constituido por 20 pacientes internados en el Hospital de las Clínicas de la Facultad de Medicina de Ribeirao Preto, en Brasil, para ser sometidos a cirugía esofágica. Los pacientes fueron sometidos a una entrevista y a un examen físico. Los diagnósticos de enfermería identificados con una frecuencia mayor que 50% fueron: acción de deglutir perjudicada (100%), riesgo de infección (100%), conocimiento deficiente sobre la enfermedad y período peri operatorio (95%) y dolor crónico (75%). Esos diagnósticos fueron analizados considerando los factores relacionados, las características de definición o los factores de riesgo, de acuerdo con el tipo de diagnóstico, y las respuestas a la patología esofágica.Este estudo teve como objetivos identificar e analisar os diagnósticos de enfermagem de pacientes no período pré-operatório de cirurgias esofágicas.     Job stress and emotional exhaustion at work in Spanish workers: Does unhealthy work affect the decision to drive?

    Objectives The purpose of this study was to assess the relationships among the following elements: unhealthy work indicators (job stress and emotional exhaustion at work), the decision to drive (or not), and driving crashes suffered by Spanish workers. Methods For this cross-sectional study, a full sample of 1,200 Spanish drivers (44% women and 56% men) was used, their mean age being 42.8 years. They answered a questionnaire divided into three sections: demographic and driving-related data; burnout, job stress, and life stress; and self-reported road behaviors and driving safety indicators. Results Overall, 41.6% of drivers reported emotional exhaustion at work. Furthermore, 80.2% of the participants showing substantial signs of job stress or exhaustion had experienced one or more important stressful life events during the previous year. Job stress was associated with the number of driving crashes suffered along the last 3 years. Also, and especially in situations where drivers admit not feeling well enough to drive, job stress and emotional exhaustion seem to be independent from the decision to drive, and from perceiving these variables as potential impairers of driving performance. Conclusions First of all, this study showed a high prevalence of job stress and emotional exhaustion symptoms experienced at work by Spanish workers. Moreover, significant relationships were found among self-rated driving performance, workplace stress and burnout indicators, which suggests that job stress and emotional exhaustion levels may, indeed, impair driving performance, but they do not influence the decision to drive or not.     Fuzzy Logic Modeling and Observers Applied to Estimate Compositions in Batch Distillation Columns

    In this chapter, the analysis and design of a fuzzy observer based on a Takagi-Sugeno model of a batch distillation column are presented. The observer estimates the molar compositions and temperatures of the light component in the distillation column considering a binary mixture. This estimation aims to allow monitoring the physical variables in the process to improve the quality of the distillated product as well as to detect failures that could affect the system performance. The Takagi-Sugeno fuzzy model is based on eight linear subsystems determined by three premise variables: the opening percentage of the reflux valve and the liquid molar composition of the light element of the binary mixture in the boiler and in the condenser. The stability analysis and the observer gains are obtained by linear matrix inequalities (LMIs). The observer is validated by MATLAB® simulations using real data obtained from a distillation column to verify the observer’s convergence and analyze its response under system disturbances

    Molecular chaperones such as Hsp40 and Hsp70 hold the androgen receptor (AR) in an inactive conformation. They are released in the presence of androgens, enabling transactivation and causing the receptor to become aggregation-prone. Here we show that these molecular chaperones recognize a region of the AR N-terminal domain (NTD), including a FQNLF motif, that interacts with the AR ligand-binding domain (LBD) upon activation. This suggests that competition between molecular chaperones and the LBD for the FQNLF motif regulates AR activation. We also show that, while the free NTD oligomerizes, binding to Hsp70 increases its solubility. Stabilizing the NTD-Hsp70 interaction with small molecules reduces AR aggregation and promotes its degradation in cellular and mouse models of the neuromuscular disorder spinal bulbar muscular atrophy. These results help resolve the mechanisms by which molecular chaperones regulate the balance between AR aggregation, activation and quality control
