    Maternal warmth and early adolescents' internalizing symptoms and externalizing behavior: Mediation via emotional security

    This study examines the relation between maternal warmth and the internalizing and externalizing problems of early adolescents, and the potential mediation of this relation by emotional insecurity. The hypotheses for the study derive from Cummings and Davies (1995) theory of emotional security. The current study extends the theory to security processes within the parent-adolescent relationship. A total of 203 early adolescents drawn from school samples in Spain participated in the study along with their mothers and their classroom teachers. The results supported the hypothesis. Emotional insecurity mediated the relationship between maternal warmth and adolescent internalizing and externalizing problems. Higher maternal warmth was linked to greater emotional security, which in turn was linked to fewer internalizing and externalizing problems. The findings extend the tenets of Emotional Security Theory and imply strategies for prevention and intervention

    Stratifying derived categories of cochains on certain spaces

    In recent years, Benson, Iyengar and Krause have developed a theory of stratification for compactly generated triangulated categories with an action of a graded commutative Noetherian ring. Stratification implies a classification of localizing and thick subcategories in terms of subsets of the prime ideal spectrum of the given ring. In this paper two stratification results are presented: one for the derived category of a commutative ring-spectrum with polynomial homotopy and another for the derived category of cochains on certain spaces.     Predicting precipitation kinetics during the annealing of additive manufactured inconel 625 components

    The prediction of solidification microstructures associated with additive manufacture of metallic components is fundamental in the identification scanning strategies, process parameters and subsequent heat treatments for optimised component properties. Interactions between the powder particles and the laser heat source result in complex thermal fields in and around the metal melt pool, which will influence the spatial distribution of chemical species as well as solid-state precipitation reactions. This paper demonstrates that a multi-component, multi-phase precipitation model can successfully predict the observed precipitation kinetics in Inconel 625, capturing the anomalous precipitation behaviour exhibited in additively manufactured components. A computer coupling of phase diagrams and thermochemistry (CALPHAD)-based approach captures the impact of dendritic segregation of alloying elements upon precipitation behaviour. The model was successful in capturing the precipitation kinetics during annealing considering the Nb-rich and Nb-depleted regions that are formed during additive manufacturing

    Atenolol versus losartan in children and young adults with Marfan's syndrome

    BACKGROUND : Aortic-root dissection is the leading cause of death in Marfan's syndrome. Studies suggest that with regard to slowing aortic-root enlargement, losartan may be more effective than beta-blockers, the current standard therapy in most centers. METHODS : We conducted a randomized trial comparing losartan with atenolol in children and young adults with Marfan's syndrome. The primary outcome was the rate of aortic-root enlargement, expressed as the change in the maximum aortic-root-diameter z score indexed to body-surface area (hereafter, aortic-root z score) over a 3-year period. Secondary outcomes included the rate of change in the absolute diameter of the aortic root; the rate of change in aortic regurgitation; the time to aortic dissection, aortic-root surgery, or death; somatic growth; and the incidence of adverse events. RESULTS : From January 2007 through February 2011, a total of 21 clinical centers enrolled 608 participants, 6 months to 25 years of age (mean [+/- SD] age, 11.5 +/- 6.5 years in the atenolol group and 11.0 +/- 6.2 years in the losartan group), who had an aorticroot z score greater than 3.0. The baseline-adjusted rate of change (+/- SE) in the aortic-root z score did not differ significantly between the atenolol group and the losartan group (-0.139 +/- 0.013 and -0.107 +/- 0.013 standard-deviation units per year, respectively; P = 0.08). Both slopes were significantly less than zero, indicating a decrease in the degree of aortic-root dilatation relative to body-surface area with either treatment. The 3-year rates of aortic-root surgery, aortic dissection, death, and a composite of these events did not differ significantly between the two treatment groups. CONCLUSIONS : Among children and young adults with Marfan's syndrome who were randomly assigned to losartan or atenolol, we found no significant difference in the rate of aorticroot dilatation between the two treatment groups over a 3-year period

    Real-world evidence use in assessments of cancer drugs by NICE

    Objective To establish how real-world evidence (RWE) has been used to inform single technology appraisals (STAs) of cancer drugs conducted by the National Institute for Health and Care Excellence (NICE). Methods STAs published by NICE from April 2011 to October 2018 that evaluated cancer treatments were reviewed. Information regarding the use of RWE to directly inform the company-submitted cost-effectiveness analysis was extracted and categorized by topic. Summary statistics were used to describe emergent themes, and a narrative summary was provided for key case studies. Results Materials for a total of 113 relevant STAs were identified and analyzed, of which nearly all (96 percent) included some form of RWE within the company-submitted cost-effectiveness analysis. The most common categories of RWE use concerned the health-related quality of life of patients (71 percent), costs (46 percent), and medical resource utilization (40 percent). While sources of RWE were routinely criticized as part of the appraisal process, we identified only two cases where the use of RWE was overtly rejected; hence, in the majority of cases, RWE was accepted in cancer drug submissions to NICE. Discussion RWE has been used extensively in cancer submissions to NICE. Key criticisms of RWE in submissions to NICE are seldom regarding the use of RWE in general; instead, these are typically concerned with specific data sources and the applicability of these to the decision problem. Within an appropriate context, RWE constitutes an extremely valuable source of information to inform decision making; yet the development of best practice guidelines may improve current reporting standards

    Improvements in Modeling 90 degree Bleed Holes for Supersonic Inlets

    The modeling of porous bleed regions as boundary conditions in computational fluid dynamics (CFD) simulations of supersonic inlet flows has been improved through a scaling of sonic flow coefficient data for 90deg bleed holes. The scaling removed the Mach number as a factor in computing the sonic flow coefficient and allowed the data to be fitted with a quadratic equation, with the only factor being the ratio of the plenum static pressure to the surface static pressure. The implementation of the bleed model into the Wind-US CFD flow solver was simplified by no longer requiring the evaluation of the flow properties at the boundary-layer edge. The quadratic equation can be extrapolated to allow the modeling of small amounts of blowing, which can exist when recirculation of the bleed flow occurs within the bleed region. The improved accuracy of the bleed model was demonstrated through CFD simulations of bleed regions on a flat plate in supersonic flow with and without an impinging oblique shock. The bleed model demonstrated good agreement with experimental data and three-dimensional CFD simulations of bleed holes

    Independent validation of CT radiomics models in colorectal liver metastases:predicting local tumour progression after ablation

    Objectives:Independent internal and external validation of three previously published CT-based radiomics models to predict local tumor progression (LTP) after thermal ablation of colorectal liver metastases (CRLM). Materials and methods: Patients with CRLM treated with thermal ablation were collected from two institutions to collect a new independent internal and external validation cohort. Ablation zones (AZ) were delineated on portal venous phase CT 2–8 weeks post-ablation. Radiomics features were extracted from the AZ and a 10 mm peri-ablational rim (PAR) of liver parenchyma around the AZ. Three previously published prediction models (clinical, radiomics, combined) were tested without retraining. LTP was defined as new tumor foci appearing next to the AZ up to 24 months post-ablation. Results: The internal cohort included 39 patients with 68 CRLM and the external cohort 52 patients with 78 CRLM. 34/146 CRLM developed LTP after a median follow-up of 24 months (range 5–139). The median time to LTP was 8 months (range 2–22). The combined clinical-radiomics model yielded a c-statistic of 0.47 (95%CI 0.30–0.64) in the internal cohort and 0.50 (95%CI 0.38–0.62) in the external cohort, compared to 0.78 (95%CI 0.65–0.87) in the previously published original cohort. The radiomics model yielded c-statistics of 0.46 (95%CI 0.29–0.63) and 0.39 (95%CI 0.28–0.52), and the clinical model 0.51 (95%CI 0.34–0.68) and 0.51 (95%CI 0.39–0.63) in the internal and external cohort, respectively. Conclusion: The previously published results for prediction of LTP after thermal ablation of CRLM using clinical and radiomics models were not reproducible in independent internal and external validation. Clinical relevance statement: Local tumour progression after thermal ablation of CRLM cannot yet be predicted with the use of CT radiomics of the ablation zone and peri-ablational rim. These results underline the importance of validation of radiomics results to test for reproducibility in independent cohorts. </p
