The aperiodic timing behaviour of the accretion-driven millisecond pulsar SAX J1808.4-3658
We studied the aperiodic X-ray timing behaviour of the accreting millisecond
pulsar SAX J1808.4-3658. The source was recently found to be the first
accreting millisecond pulsar that shows the kilohertz quasi-periodic
oscillations (kilohertz QPOs) that are found in many other X-ray binaries with
accreting neutron stars. The high frequency of these signals reflects the short
dynamical time scales in the region near the compact object where they
originate. We find that in addition to the kilohertz QPOs SAX J1808.4-3658
shows several low frequency timing features, based on which the source can be
classified as a so-called atoll source. The frequencies of the variability
components of the atoll sources follow a universal scheme of correlations. The
correlations in SAX J1808.4-3658 are similar but show a shift in upper
kilohertz QPO frequency. This discrepancy is perhaps related to a stronger or

RXTE observations of the dipping low-mass X-ray binary 4U 1624-49
We analyse ~ 360 ks of archival data from the Rossi X-Ray Timing Explorer
(RXTE) of the 21 hr orbital period dipping low-mass X-ray binary 4U 1624-49. We
find that outside the dips the tracks in the colour-colour and
hardness-intensity diagrams (CDs and HIDs) are reminiscent of those of atoll
sources in the middle and upper parts of the banana branch. The tracks show
secular shifts up to ~ 10%. We study the power spectrum of 4U 1624-49 as a
function of the position in the CD. This is the first time power spectra of
this source are presented. No quasi-periodic oscillations (QPOs) are found. The
power spectra are dominated by very low frequency noise (VLFN), characteristic
for atoll sources in the banana state, and band limited noise (BLN) which is
not reliably detected but may, uncharacteristically, strengthen and increase in
frequency with spectral hardness. The VLFN fits to a power law, which becomes
steeper when the source moves to the harder part of the CD. We conclude that 4U
1624-49 is an atoll source which in our observations is in the upper banana
branch. Combining this with the high (0.5-0.7 L_Edd) luminosity, the long-term
flux stability of the source as seen with the RXTE All-Sky Monitor (ASM), and
with the fact that it is an X-ray dip source, we conclude that 4U 1624-49 is
most likely a GX atoll source such as GX 3+1 and GX 9+9, but seen edge on.Comment: 8 pages, 5 figures, 2 tables, accepted for publication in A&A. This

Improving green manure quality with phosphate rocks in Ontario Canada
Phosphate rock (PR) was applied to one conventional and two organic dairy fields and planted with buckwheat (Fagopyrum esculentum) as a green manure crop. In total, five types of PR were applied at three application rates in order to determine the yield, concentration of P in the aboveground tissue and the P uptake of buckwheat. It was found that PR of relatively high carbonate substitution and small particle diameter could increase buckwheat tissue concentrations to a quality such that mineralization of the buckwheat mulch could occur. Buckwheat mulch and residual PR increased soil P flux as determined by anion exchange membranes in situ in the following spring. This provides evidence that buckwheat of high P quality has the potential to supply P to a subsequent crop
Circulating pro-apoptotic mediators in burn septic acute renal failure
The pathogenesis of septic acute kidney injury (AKI) is not well understood. In the present issue of Critical Care, the combined clinical and experimental study from Mariano's group provides new insight into the disease. The study shows that plasma from septic burn patients with acute renal failure initiated pro-apoptotic effects and functional alterations in renal tubular cells and podocytes in vitro that correlated with the degree of proteinuria and renal dysfunction. Pro-apoptotic effects were not attributable to antibiotic or uremic toxicity, but were partially attributable to endotoxin. Sepsis and burn had additive effects. Apart from increasing our understanding of the pathogenesis of septic AKI, the study justifies further research on therapeutic interventions in several directions. These include the binding and elimination of the source of endotoxin by selective decontamination of the digestive tract, the blocking of apoptotic pathways, or the extracorporeal removal of circulating toxic mediators using high permeability hemofiltration or coupled plasma filtration and absorption
The correlations between the spin frequencies and kHz QPOs of Neutron Stars in LMXBs
We studied the correlations between spin frequencies and kilohertz
quasi-periodic oscillations (kHz QPOs) in neutron star low mass X-ray binaries.
The updated data of kHz QPOs and spin frequencies are statistically analyzed.
We found that when two simultaneous kHz QPOs are present in the power spectrum,
the minimum frequency of upper kHz QPO is at least 1.3 times larger than the
spin frequency, i.e. \nu_{s}<\nu_{2min}/1.3. We also found that the average kHz
QPO peak separation in 6 Atoll sources anti-correlates with the spin frequency
in the form \lan\dn\ran = -(0.19\pm0.05)\ns+(389.40\pm21.67)Hz. If we shifted
this correlation in the direction of the peak separation by a factor of 1.5,
this correlation matches the data points of the two accretion powered
millisecond X-ray pulsars, SAX J1808.4-3658 and XTE J1807-294.Comment: 4 pages, 2 figures, accepted for publication in A&
Clinical review: Patency of the circuit in continuous renal replacement therapy
Premature circuit clotting is a major problem in daily practice of continuous renal replacement therapy (CRRT), increasing blood loss, workload, and costs. Early clotting is related to bioincompatibility, critical illness, vascular access, CRRT circuit, and modality. This review discusses non-anticoagulant and anticoagulant measures to prevent circuit failure. These measures include optimization of the catheter (inner diameter, pattern of flow, and position), the settings of CRRT (partial predilution and individualized control of filtration fraction), and the training of nurses. In addition, anticoagulation is generally required. Systemic anticoagulation interferes with plasmatic coagulation, platelet activation, or both and should be kept at a low dose to mitigate bleeding complications. Regional anticoagulation with citrate emerges as the most promising method
kHz Quasi-Periodic Oscillations in the low-mass X-ray binary 4U 0614+09
We report on a comprehensive analysis of the kilohertz (above 300 Hz)
quasi-periodic oscillations (kHz QPOs) detected from the neutron star low-mass
X-ray binary 4U0614+09 with the Rossi X-ray Timing Explorer (RXTE). With a much
larger data set than previously analyzed (all archival data from February 1996
up to October 2007), we first investigate the reality of the 1330 Hz QPO
reported by van Straaten et al. (2000). This QPO would be of particular
interest since it has the highest frequency reported for any source. A thorough
analysis of the same observation fails to confirm the detection. On the other
hand, over our extended data set, the highest QPO frequency we measure for the
upper kHz QPO is at about 1224 Hz; a value which is fully consistent with the
maximum values observed in similar systems. Second, we demonstrate that the
frequency dependence of the quality factor and amplitude of the lower and upper
kHz QPOs follow the systematic trends seen in similar systems (Barret et al.,
2006). In particular, 4U0614+09 shows a drop of the quality factor of the lower
kHz QPO above 700 Hz. If this is due to an approach to the innermost stable
circular orbit, it implies a neutron star mass of about 1.9 solar masses.
Finally, when analyzing the data over fixed durations, we have found a gap in
the frequency distribution of the upper QPO, associated with a local minimum of
its amplitude. A similar gap is not present in the distribution of the lower
QPO frequencies, suggesting some cautions when interpreting frequency ratio

Hearing loss by week of gestation and birth weight in very preterm neonates
OBJECTIVE: To gain insight into health and related costs associated with very preterm births, one needs accurate information about the prevalence of the disabling conditions, including neonatal hearing loss (NHL).STUDY DESIGN: We assessed the prevalence of NHL by week of gestation and categories of birth weight in very preterm neonates. Results of the 2-stage Automated Auditory Brainstem Response nationwide Newborn Hearing Screening Program in Dutch Neonatal Intensive Care Units and diagnostic examinations were centrally registered between October 1998 and December 2012 and included in this study. NHL was defined as impaired when the neonate conventional Auditory Brainstem Response level exceeded 35 dB near Hearing Level at diagnostic examination. Birth weight was stratified into <750 g, 750-999 g, 1000-1249 g, 1250-1499 g, and ≥ 1500 g, and by small for gestational age (SGA; <10th percentile) vs appropriate for gestational age. Logistic regression analyses and recursive partitioning were performed.RESULTS: In total, 18,564 very preterm neonates were eligible. The prevalence of NHL consistently increased with decreasing week of gestation (1.2%-7.5% from 31 to 24 weeks) and decreasing birth weight (1.4%-4.8% from ≥ 1500 g to <750 g, all P < .002). Most vulnerable to NHL were girls <28 weeks, boys <30 weeks, and SGA neonates. The SGA effect started at 27 weeks.CONCLUSIONS: Gestational age and birth weight quantify the risk of NHL. This information can be used at the individual level for parent counseling and at the population level for medical decision making.</p
Prevention of acute kidney injury and protection of renal function in the intensive care unit : update 2017
Background: Acute kidney injury (AKI) in the intensive care unit is associated with significant mortality and morbidity.
Objectives: To determine and update previous recommendations for the prevention of AKI, specifically the role of fluids, diuretics, inotropes, vasopressors/vasodilators, hormonal and nutritional interventions, sedatives, statins, remote ischaemic preconditioning and care bundles.
Method: A systematic search of the literature was performed for studies published between 1966 and March 2017 using these potential protective strategies in adult patients at risk of AKI. The following clinical conditions were considered: major surgery, critical illness, sepsis, shock, exposure to potentially nephrotoxic drugs and radiocontrast. Clinical endpoints included incidence or grade of AKI, the need for renal replacement therapy and mortality. Studies were graded according to the international GRADE system.
Results: We formulated 12 recommendations, 13 suggestions and seven best practice statements. The few strong recommendations with high-level evidence are mostly against the intervention in question (starches, low-dose dopamine, statins in cardiac surgery). Strong recommendations with lower-level evidence include controlled fluid resuscitation with crystalloids, avoiding fluid overload, titration of norepinephrine to a target MAP of 65-70 mmHg (unless chronic hypertension) and not using diuretics or levosimendan for kidney protection solely.
Conclusion: The results of recent randomised controlled trials have allowed the formulation of new recommendations and/or increase the strength of previous recommendations. On the other hand, in many domains the available evidence remains insufficient, resulting from the limited quality of the clinical trials and the poor reporting of kidney outcomes
