    Occupational exposure to saw dust: a case study

    ABSTRACT: Occupational exposure to saw dust is associated with the development of oncological diseases, namely nasopharyngeal cancers (about 44% are from nasal cavity and paranasal sinus cancers), in the wood and furniture industries, about 55.000, according to the (Associação das Indústrias de Madeira e Mobiliário de Portugal) AIMMP. It should be noted that since 1995 these dusts have been classified as carcinogenic, by The International Agency for Research on Cancer (IARC). The main objective of this study is to evaluate the exposure to saw dust, quantifying its concentration, comparing with values stipulated by existing legislation and standardization. In order to reach the objectives described above, total dust sampling was performed following the NIOSH0500 methodology, in several jobs, in three different carpentries. From the samplings performed, an average value of saw dust concentration was obtained in each workstation. After analyzing the values obtained in the measurements, performed in the real work context, it was verified there was legal non-compliance in the Garlopa workstation and values of the order of magnitude of the NP 1796 exposure limit values ELV, in the Manual Polishing workstation, in some of the Carpentry Workshops. However, if we consider the Scientific Committee on Occupational Exposure Limit (SCOEL) ELV we can state that only the Trimmer complies with the established ELV. Thus, corrective and/or preventive measures should be implemented by employers and preventive measures should be receptive by workers by implementing/complying to ensure the health and well-being of all, will be proposed.info:eu-repo/semantics/publishedVersio

    Performance of EUS-FNA for mediastinal lymphadenopathy: impact on patient management and costs in low-volume EUS centers

    BACKGROUND: Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) of mediastinal lymphadenopathy has been shown to be a valuable diagnostic tool in high-volume EUS centers (≥ 50 mediastinal EUS-FNA/endoscopist/year). Our goal was to assess the diagnostic accuracy of EUS-FNA and its impact on clinical management and costs in low-volume EUS centers ( <50 mediastinal EUS-FNA/endoscopist/year). METHODS: Consecutive patients referred to two Dutch endoscopy centers in the period 2002-2008 for EUS-FNA of mediastinal lymphadenopathy were reviewed. The gold standard for a cytological diagnosis was histological confirmation or clinical follow-up of more than 6 months with repeat imaging. The impact of EUS-FNA on clinical management was subdivided into a positive impact by providing (1) adequate cytology that influenced the decision to perform surgery or (2) a diagnosis of a benign inflammatory disorder, and a negative impact which was subdivided into (1) false-negative or inconclusive cytology or (2) an adequate cytological diagnosis that did not influence patient management. Costs of an alternative diagnostic work-up without EUS-FNA, as established by an expert panel, were compared to costs of the actual work-up. RESULTS: In total, 213 patients (71% male, median age= 61 years, range = 23-88 years) underwent EUS-FNA. Sensitivity, specificity, and negative and positive predictive values were 89%, 100%, 80%, and 100%, respectively. EUS-FNA had a positive impact on clinical management in 84% of cases by either influencing the decision to perform surgery (49%) or excluding malignant lymphadenopathy (35%), and a negative impact in 7% of cases because of inadequate (3%) or false-negative (4%) cytology. In 9% of cases, EUS-FNA was performed without an established indication. Two nonfatal perforations occurred (0.9%). Total cost reduction was €100,593, with a mean cost reduction of €472 (SD = €607) per patient. CONCLUSIONS: Mediastinal EUS-FNA can be performed in low-volume EUS centers without compromising diagnostic accuracy. Moreover, EUS-FNA plays an important role in the management of patients with mediastinal lymphadenopathy and reduces total diagnostic cost

    Meta-Analysis of the Association between Transforming Growth Factor-Beta Polymorphisms and Complications of Coronary Heart Disease

    Objective: To investigate the association between common transforming growth factor beta (TGF-β) single nucleotide polymorphisms (SNP) and significant complications of coronary heart disease (CHD).\ud \ud Method: We performed a meta-analysis of published case-control studies assessing the association of TGF-β SNPs with a range of CHD complications. A random effects model was used to calculate odds ratios and confidence intervals. Analyses were conducted for additive, dominant and recessive modes of inheritance.\ud \ud Results: Six studies involving 5535 cases and 2970 controls examining the association of common SNPs in TGF-β1 with CHD were identified. Applying a dominant model of inheritance, three TGF-β1 SNPs were significantly associated with CHD complications: The T alleles of rs1800469 (OR = 1.125, 95% CI 1.016–1.247, p = 0.031) and rs1800470 (OR = 1.146, 95% CI 1.026–1.279, p = 0.021); and the C allele of rs1800471 (OR = 1.207, 95% CI 1.037–1.406, p = 0.021).\ud \ud Conclusion: This meta-analysis suggests that common genetic polymorphisms in TGF-β1 are associated with complications of CHD

    The Pan-STARRS 1 discoveries of five new neptune trojans

    In this work we report the detection of seven Neptune Trojans (NTs) in the Pan-STARRS 1 (PS1) survey. Five of these are new discoveries, consisting of four L4 Trojans and one L5 Trojan. Our orbital simulations show that the L5 Trojan stably librates for only several million years. This suggests that the L5 Trojan must be of recent capture origin. On the other hand, all four new L4 Trojans stably occupy the 1:1 resonance with Neptune for more than 1 Gyr. They can, therefore, be of primordial origin. Our survey simulation results show that the inclination width of the Neptune Trojan population should be between 77^{\circ} and 2727^{\circ} at >> 95% confidence, and most likely 11\sim 11^{\circ}.     Genotyping a second growth coast redwood forest : a high throughput methodology

    The idea that excitonic (electronic) coherences are of fundamental importance to natural photosynthesis gained popularity when slowly dephasing quantum beats (QBs) were observed in the two-dimensional electronic spectra of the Fenna–Matthews–Olson (FMO) complex at 77 K. These were assigned to superpositions of excitonic states, a controversial interpretation, as the strong chromophore–environment interactions in the complex suggest fast dephasing. Although it has been pointed out that vibrational motion produces similar spectral signatures, a concrete assignment of these oscillatory signals to distinct physical processes is still lacking. Here we revisit the coherence dynamics of the FMO complex using polarization-controlled two-dimensional electronic spectroscopy, supported by theoretical modelling. We show that the long-lived QBs are exclusively vibrational in origin, whereas the dephasing of the electronic coherences is completed within 240 fs even at 77 K. We further find that specific vibrational coherences are produced via vibronically coupled excited states. The presence of such states suggests that vibronic coupling is relevant for photosynthetic energy transfer

    Consensus statements on the information to deliver after a febrile seizure

    Febrile seizures (FS) are usually self-limiting and cause no morbidity. Nevertheless they represent very traumatic events for families. There is a need to identify key messages that reassure carers and help to prevent inappropriate, anxiety-driven behaviors associated with “fever phobia.” No recommendations have been proposed to date regarding the content of such messages. Using a Delphi process, we have established a consensus regarding the information to be shared with families following a FS. Twenty physicians (child neurologists and pediatricians) from five European countries participated in a three-step Delphi process between May 2018 and October 2019. In the first step, each expert was asked to give 10 to 15 free statements about FS. In the second and third steps, statements were scored and selected according to the expert ranking of importance. A list of key messages for families has emerged from this process, which offer reassurance about FS based on epidemiology, underlying mechanisms, and the emergency management of FS should they recur. Interestingly, there was a high level of agreement between child neurologists and general pediatricians.     Pronounced Shape Change Induced by Quasiparticle Alignment

    Mean lifetimes of high-spin states of 74Kr have been determined using the Doppler-shift attenuation method. The high-spin states were studied using the 40Ca(40Ca,α2p) reaction at a beam energy of 160 MeV with the GASP γ-ray spectrometer. The ground-state band and negative parity side band show the presence of three different configurations in terms of transitional quadrupole deformations. A dramatic shape change was found along the ground-state band after the S-band crossing. The deduced quadrupole deformation changes are well reproduced by cranked Woods-Saxon Strutinsky calculations
