    A response to the House of Commons Education Committee report on Multi-Academy Trusts

The House of Commons Education Committee has, as a result, looked into the performance and role of these trusts.     Intercalated theophylline-smectite hybrid for pH-mediated delivery

    On the basis of their large specific surface areas, high adsorption and cation exchange capacities, swelling potential and low toxicity, natural smectite clays are attractive substrates for the gastric protection of neutral and cationic drugs. Theophylline is an amphoteric xanthine derivative that is widely used as a bronchodilator in the treatment of asthma and chronic obstructive pulmonary disease. This study considers the in vitro uptake and release characteristics of the binary theophylline-smectite system. The cationic form of theophylline was readily ion exchanged into smectite clay at pH 1.2 with a maximum uptake of 67±2 mg g−1. Characterisation of the drug-clay hybrid system by powder X-ray diffraction analysis, Fourier transform infrared spectroscopy, differential scanning calorimetry and scanning electron microscopy confirmed that the theophylline had been exclusively intercalated into the clay system in an amorphous form. The drug remained bound within the clay under simulated gastric conditions at pH 1.2; and the prolonged release of approximately 40% of the drug was observed in simulated intestinal fluid at pH 6.8 and 7.4 within a 2-h timeframe. The incomplete reversibility of the intercalation process was attributed to chemisorption of the drug within the clay lattice. These findings indicate that smectite clay is a potentially suitable vehicle for the safe passage of theophylline into the duodenum. Protection from absorption in the stomach and subsequent prolonged release in the small intestine are advantageous in reducing fluctuations in serum concentration which may impact therapeutic effect and toxicit

    Embedded Research::a promising way to create evidence-informed impact in public health?

    Background Embedded research (ER) is recognized as one way to strengthen the integration of evidence into public health (PH) practice. In this paper, we outline a promising example of the co-production of research evidence between Fuse, the UKCRC Centre for Translational Research in Public Health and a local authority (LA) in north east England. Methods We critically examine attempts to share and use research findings to influence decision-making in a LA setting, drawing on insights from PH practitioners, managers, commissioners and academic partners involved in this organizational case study. We highlight what can be achieved as a co-located embedded researcher. Results The benefits and risks of ER are explored, alongside our reflections on the added value of this approach and the institutional prerequisites necessary for it to work. We argue that while this is not a new methodological approach, its application in PH as a way to facilitate evidence use is novel, and raises pragmatic and theoretical questions about the nature of impact and the extent to which it can be engineered. Conclusion With increased situated understanding of organizational culture and norms and greater awareness of the socio-political realities of PH, ER enables new co-produced solutions to become possible

    Lower extremity joint kinetics and lumbar curvature during squat and stoop lifting

    <p>Abstract</p> <p>Background</p> <p>In this study, kinematics and kinetics of the lower extremity joint and the lumbar lordosis during two different symmetrical lifting techniques(squat and stoop) were examined using the three-dimensional motion analysis.</p> <p>Methods</p> <p>Twenty-six young male volunteers were selected for the subjects in this study. While they lifted boxes weighing 5, 10 and 15 kg by both squat and stoop lifting techniques, their motions were captured and analyzed using the 3D motion analysis system which was synchronized with two forceplates and the electromyographic system. Joint kinematics was determined by the forty-three reflective markers which were attached on the anatomical locations based on the VICON Plug-in-Gait marker placement protocol. Joint kinetics was analyzed by using the inverse dynamics. Paired t-test and Kruskal-Wallis test was used to compare the differences of variables between two techniques, and among three different weights. Correlation coefficient was calculated to explain the role of lower limb joint motion in relation to the lumbar lordosis.</p> <p>Results</p> <p>There were not significant differences in maximum lumbar joint moments between two techniques. The hip and ankle contributed the most part of the support moment during squat lifting, and the knee flexion moment played an important role in stoop lifting. The hip, ankle and lumbar joints generated power and only the knee joint absorbed power in the squat lifting. The knee and ankle joints absorbed power, the hip and lumbar joints generated power in the stoop lifting. The bi-articular antagonist muscles' co-contraction around the knee joint during the squat lifting and the eccentric co-contraction of the gastrocnemius and the biceps femoris were found important for maintaining the straight leg during the stoop lifting. At the time of lordotic curvature appearance in the squat lifting, there were significant correlations in all three lower extremity joint moments with the lumbar joint. Differently, only the hip moment had significant correlation with the lumbar joint in the stoop lifting.</p> <p>Conclusion</p> <p>In conclusion, the knee extension which is prominent kinematics during the squat lifting was produced by the contributions of the kinetic factors from the hip and ankle joints(extensor moment and power generation) and the lumbar extension which is prominent kinematics during the stoop lifting could be produced by the contributions of the knee joint kinetic factors(flexor moment, power absorption, bi-articular muscle function).</p

    Recognition and Alleviation of Pain in Animals

    The pain and distress which animals experience as a consequence of their use by man figures prominently in discussions of animal welfare. Some improvements have been made in animal housing and husbandry practices and it is likely that further progress will be made in this field. In comparison, relatively little attention has been given to the problem of minimizing the pain and distress caused to animals by the various procedures to which they are subjected. The most publicized of these are the wide range of experimental techniques which are undertaken using laboratory animals, but also includes procedures such as castration of farm animals and neutering operations carried out on pet animals. The prevention or alleviation of the pain associated with such procedures is a complex problem with no single, simple solution. Consideration must be given to the use of analgesic drugs, the provision of high standards of general care, and the use of special nursing techniques. When dealing with post-operative care, the pre-operative management ofthe animal, the operative procedures and the anesthetic regime must all be evaluated and, when necessary, modified to minimize pain or discomfort

    Toward a Comprehensive Approach to the Collection and Analysis of Pica Substances, with Emphasis on Geophagic Materials

    Pica, the craving and subsequent consumption of non-food substances such as earth, charcoal, and raw starch, has been an enigma for more than 2000 years. Currently, there are little available data for testing major hypotheses about pica because of methodological limitations and lack of attention to the problem.In this paper we critically review procedures and guidelines for interviews and sample collection that are appropriate for a wide variety of pica substances. In addition, we outline methodologies for the physical, mineralogical, and chemical characterization of these substances, with particular focus on geophagic soils and clays. Many of these methods are standard procedures in anthropological, soil, or nutritional sciences, but have rarely or never been applied to the study of pica.Physical properties of geophagic materials including color, particle size distribution, consistency and dispersion/flocculation (coagulation) should be assessed by appropriate methods. Quantitative mineralogical analyses by X-ray diffraction should be made on bulk material as well as on separated clay fractions, and the various clay minerals should be characterized by a variety of supplementary tests. Concentrations of minerals should be determined using X-ray fluorescence for non-food substances and inductively coupled plasma-atomic emission spectroscopy for food-like substances. pH, salt content, cation exchange capacity, organic carbon content and labile forms of iron oxide should also be determined. Finally, analyses relating to biological interactions are recommended, including determination of the bioavailability of nutrients and other bioactive components from pica substances, as well as their detoxification capacities and parasitological profiles.This is the first review of appropriate methodologies for the study of human pica. The comprehensive and multi-disciplinary approach to the collection and analysis of pica substances detailed here is a necessary preliminary step to understanding the nutritional enigma of non-food consumption

    The relationship among perceived social support, self-critical cognition and neurotic symptoms in rehabilitation medicine students : a correlational study

    A review of the literature indicates that a significant number of students are psychologically distressed during their professional education. Most of the research in this area focuses on sources of stress for students, usually confined to factors within the scope of the educational experience. Researchers generally agree, however, that stress experienced in a given situation is largely a function of an individual's perception of the circumstances and that characteristics of the situation, characteristics of the individual and mediating factors external to the situation are all determinants. The purpose of this descriptive, exploratory study was to examine the relationship among the following psychological variables among second, third and fourth year students in a rehabilitation medicine program: (a) the perception of available social support and satisfaction with this social support, (b) the tendency to process self-relevant information in self-critical and self-defeating ways, and (c) the manifestation of psychological distress in the form of neurotic symptoms. A sample of 83 students (73 females and 10 males) in an occupational therapy and a physical therapy program completed the Social Support Questionnaire, the Self-Critical Cognition Scale, and the Hopkins Symptom Checklist. The Social Support Questionnaire included two dimensions; (a) the availability of perceived social support; and (b) satisfaction with perceived social support. The Hopkins Symptom Checklist included five dimensions; (a) somatization; (b) obsession-compulsion; (c) interpersonal sensitivity; (d) depression; and (e) anxiety. A Pearson Product-moment correlational analysis was conducted on the two Social Support Questionnaire dimension scores, the five Hopkins Symptom Checklist dimension scores and the Self-Critical Cognition scores. A series of t tests were done to examine differences on the variables between occupational and physical therapy students as well as a one-way ANOVA among three groups according to the year in the program. The results showed that both perceived availability of social support and satisfaction with social support correlated significantly, in a negative direction, with self-critical cognition (p < .001). The two measures of the social support variable also correlated significantly, in a negative direction, with all dimensions of the Hopkins Symptom Checklist (p < .001) except somatization. There was no statistically significant difference between occupational therapy students and physical therapy students.     Living improvement 2: A case study of a secondary school in England

    Building on a decade of a school-university partnership we report on a current project that is examining the development of a learning policy within Kingswood High School. We focus specifically on the development of a learning culture, and we examine how parents and children are working productively within the official policy. Using empirical data we reveal the existence of learning cultures that are out of alignment with this official policy, and how the school is using research to engage and understand this. Thinking productively about professional practice by using Arendt's distinction between labour, work and action, we deploy a framework that identifies four main positions of a researching school. We identified that Kingswood is located in all of them, but it is their action within agenda setting that has the most potential for enabling them to engage productively with the communities they serve. © The Author(s) 2012
