    A comparative clinical trial to evaluate the efficacy of Rasnadi Churna Basti with Ardha Matrika Basti in the management of Amavata vis-à-vis Rheumatoid Arthritis

    Background: Amavata is one among the pain predominant diseases. The disease Amavata has similarity with Rheumatoid arthritis in clinical presentation. In the present study an attempt was made to compare the efficacy of Rasnadi Churna Basti with Ardha Matrika Basti in the management of Amavata. Objective of the study: To compare the efficacy of Rasnadi Churna Basti with Ardha Matrika Basti in the management of Amavata vis-à-vis Rheumatoid Arthritis. Materials and Methods: A double arm open labeled clinical study with pre-post-test design was carried out at Government Ayurveda Medical College and Hospital, Mysuru. Data was collected as per the proforma prepared for the purpose of the study. Study was completed in 42 subjects with 21 subjects in both the groups. For Group A Rasnadi Churna Basti and for Group B Ardha Matrika Basti in Yoga Basti pattern for 8 consecutive days was administered. The data was analyzed using SPSS descriptive and inferential statistics. Results: The statistical values showed highly significant results in Sandhi Shoola, Sandhi Shotha, Sandhi Stabhdata, functional grading, ESR, and DAS parameters. In CRP parameter, result was statistically significant in Group-A and non significant in Group-B. Hb% parameter showed statistically non-significant results in both the groups. On overall assessment, there was no statistically significant difference in the results between the groups except CRP and DAS parameters. Conclusion: Both Rasnadi Churna Basti and Ardha Matrika Basti are useful in reducing the signs and symptoms of Amavata vis-à-vis Rheumatoid Arthritis

    Hereditary Sensory and Autonomic Neuropathy (HSAN) is a rare group of diseases involving varying degrees of peripheral nervous system. It is classified into five main types. HSAN type 4 is associated with insensitivity to pain and temperature and anihidrosis. The method of this study was to authors present a case report of a 3 year-old boy with Hereditary Sensory and Autonomic Neuropathy Type 4 presenting with chronic ulcers. Conclusions of this study was to HSAN type IV is a rare condition. There is no definitive treatment available presently for this condition

    A Wide Multiband T-Slotted, Semicircular Microstrip Patch Antenna for WLAN/WiMAX Applications

    A multifrequency semicircularmicrostrip antenna is designed and simulated in this paper using HFSS simulation software. The proposed antenna consists of two quarter rings in different radius with one as perfect ground and other as defect ground structure, with an inverted T-shaped slot. The relationship of the resonant frequency with different physical size parameters for the single one-quarter-ring microstrip antenna is also reported. This designed multiresonant antenna proves increase in bandwidth when defect ground is used as compared to normal or conventional ground plane. The simulation show that proposed designed antenna operates at six different frequencies of 1.7, 1.93, 2.3, 3.03, 3.57 and 3.67 GHz, respectively for normal ground whereas wide bandwidth of 500 MHz is achieved for 1.9 GHz operating frequency

    Clinical and microbiological facets of ventilator associated pneumonia in the main stream with a practical contact

    Background: Ventilator-associated pneumonia (VAP), the most apparent infection in ICUs; life threatening, symbolizes an additional healthcare burden. Clinical traits and etiological agents vary. Early diagnosis is captious & apt tactic of quantitative culture is advocated. Regular surveillance is imperative to define strategies. The objective was to conceptualize VAP; put forth our experience of its occurrence, causative bacteria, clinical silhouette and associated variables; and to pattern antimicrobial resistance in ICUs; contributing this data to devise more pertinent approach.Methods: A prospective survey, executed at a tertiary care set up (multidisciplinary ICUs) analysed clinical and microbiological aspects of 120 patients (>48 hours-mechanical ventilation) in congruence with a clinical criteria of pneumonia by standard microbiological means. Cases were keenly observed to assess mortality.Results: Occurrence of VAP was 42.5% with dominance of males (65%) and age group of 41-60 years (Mean ± SD: 42.26 ± 19.53). Late onset type (60.8%) predominated. Principal symptom/sign was fever (82.5%)/crepitation (67.5%). Cases of OP poisoning (21.7%), associated diabetes mellitus (31.7%) were pre-eminent. Gram negative Bacteria (GNB) formed the major etiology (78.6%), cardinal being Acinetobacter baumannii (32.1%) and Pseudomonas aeruginosa (25%). Multi-drug resistant (MDR)-Acinetobacter baumannii, MDR-Pseudomonas aeruginosa, imipenem resistant Klebsiella pneumoniae and Methicillin resistant Staphylococcus aureus (MRSA) were noticed in 66.7%, 35%, 25% and 42.8% of respective isolates. Mortality record was 21.6%.Conclusions: Reliable mode of isolation (quantitative culture), less invasive sampling (ETA) and antibiotic recycling will augment VAP management. Regular intuition into contemporary trends of antimicrobial profile of etiological agents is crucial to avert undesirable consequences.

    Pharmacologic increase in HIF1α enhances hematopoietic stem and progenitor homing and engraftment

    Hematopoietic stem cell (HSC) transplantation is a lifesaving therapy for a number of immunologic disorders. For effective transplant, HSCs must traffic from the peripheral blood to supportive bone marrow niches. We previously showed that HSC trafficking can be enhanced by ex vivo treatment of hematopoietic grafts with 16-16 dimethyl prostaglandin E2 (dmPGE2). While exploring regulatory molecules involved in dmPGE2 enhancement, we found that transiently increasing the transcription factor hypoxia-inducible factor 1-α (HIF1α) is required for dmPGE2-enhanced CXCR4 upregulation and enhanced migration and homing of stem and progenitor cells and that pharmacologic manipulation of HIF1α is also capable of enhancing homing and engraftment. We also now identify the specific hypoxia response element required for CXCR4 upregulation. These data define a precise mechanism through which ex vivo pulse treatment with dmPGE2 enhances the function of hematopoietic stem and progenitor cells; these data also define a role for hypoxia and HIF1α in enhancement of hematopoietic transplantation

    Transport problems of rapidly developing cities: the case of Bombay

    Urban areas present transport planners with some of their most difficult problems. Transport problems in the rapidly growing cities of the Third World are complex and multi-dimensional and the nature of demand and the problems created are discernibly different from those of cities in advanced economies where the principal problem is that of accommodating the private car. It is proving difficult for the existing transport system of many Third World cities to satisfy expanding demand related to increasing population, present transport systems are seriously over-loaded, there is back-log in service improvement and there is a severe shortage of resources for the development of new capital intensive solutions. This results in transport systems that are inadequate in both quantitative and qualitative terms. A root cause of the present problem is the lack of attention over time to spatial planning and the distribution of population and economic activities. Bombay is examined as an illustration of the consequences of such a lack of spatial planning. For historical and geographical reasons, Bombay has grown linearly along a north-south axis.While the urban activities and main centres of employment are concentrated at the extreme southern tip of Bombay Island, Residential colonies have developed northwards in a haphazard way along the transport arteries and result in an amorphous city structure. User perception of the existing services and problems are examined by means of a questionnaire survey. The research suggests that on its own the provision of additional transport will not provide solutions to the city's problems which can be tackled more effectively by greater attention to non-transport solutions - land use planning and influencing the patterns of demand.<p

    It is said that one should consume Shad-Rasayuktha Ahara for better nourishment. The nutritive value of the 6 Rasas (taste) has been explained in the classics of Ayurveda along with their harmful effects when used in excess. When Amla Rasa is taken in excess it produces Dantaharsha (Morbid sensation of teeth), Trishna (thirst) etc.Objectives - The present study is designed by administering Amla Rasa Pradhana Drvya given in different concentration 10% and 15% and effect on the Mamsa Dhatu is assessed by certain blood parameters.Material and Methods -Wistar strain albino rats of either sex, weighing of 200-300 g. Obtained from S.D.M Centre for Research in Ayurveda, Udupi, Karnataka. 5Groups were taken and Group 1 is Control, Group-2 is 15% Chincha, Group-3 is 15% Vrukshamla, Group-4 is 10% Chincha, Group-5 is 10% Vrukshamla which receives drug and laboratory food for 28days, next day blood was collected by supra-orbital puncture with the help of micro capillary tubes under mild ether anesthesia.Results - There was increase in the Total Protein, Serum Albumin and serum creatinine levels and decrease in Serum globulin, Serum Urea level and Body weight.Conclusion – the study suggests that there is catabolic action of the Amla Rasa on the Mamsa Dhatu and anabolic act action on serum protein especially at the level of liver, so there will be a loss of muscle tissue which suggests Mamsa Vidhahyati action

    Analysis of prescription pattern of antihypertensive medication and adherence to Beers criteria in geriatric department of a tertiary care hospital in Mysuru

    Background: The objective of the present study was to evaluate the prescription pattern of anti-hypertensive drugs and adherence to Beers’ criteria in geriatric department of JSS Hospital, Mysuru.Methods: An observational, prospective, cross-sectional study was carried out in geriatric department. The basic demographic information and prescriptions of geriatric patients were studied. Descriptive analysis was used to present the results, prescriptions were analysed and checked for adherence to Beers’ criteria.Results: Out of 485 patients, 82.68% received monotherapy, 15.87% received 2-drug combination therapy and 1.4% received 3-drug combination therapy. Among patients receiving monotherapy, angiotensin receptor blockers (49.06%) was the commonest antihypertensive class of drug prescribed. Telmisartan (38.96%) was the commonest drug prescribed. Among 2-drug combination therapy angiotensin converting enzyme inhibitor and Calcium channel blockers were combined commonly. In 3-drug combination therapy angiotensin converting enzyme inhibitor, beta blockers and diuretics were combined commonly. 99.3% of prescriptions were adhered to 2015 American Geriatrics Society Beers criteria.Conclusions: Almost 82% of the patients were treated with monotherapy. The trends in prescribing of anti-hypertensives were in favor of conventional ones such as Angiotensin receptor blockers, calcium channel blockers, angiotensin converting enzyme inhibitors, diuretics, beta blockers and centrally acting alpha agonists. 99.3% of prescriptions were in accordance with the American Geriatrics Society 2015 Updated Beers criteria
