    Closed incision negative pressure therapy:international multidisciplinary consensus recommendations

    Surgical site occurrences (SSOs) affect up to or over 25% of patients undergoing operative procedures, with the subset of surgical site infections (SSIs) being the most common. Commercially available closed incision negative pressure therapy (ciNPT) may offer surgeons an additional option to manage clean, closed surgical incisions. We conducted an extensive literature search for studies describing ciNPT use and assembled a diverse panel of experts to create consensus recommendations for when using ciNPT may be appropriate. A literature search of MEDLINE, EMBASE and the Cochrane Central Register of Controlled Trials using key words \u2018prevention\u2019, \u2018negative pressure wound therapy (NPWT)\u2019, \u2018active incisional management\u2019, \u2018incisional vacuum therapy\u2019, \u2018incisional NPWT\u2019, \u2018incisional wound VAC\u2019, \u2018closed incisional NPWT\u2019, \u2018wound infection\u2019, and \u2018SSIs\u2019 identified peer-reviewed studies published from 2000 to 2015. During a multidisciplinary consensus meeting, the 12 experts reviewed the literature, presented their own ciNPT experiences, identified risk factors for SSOs and developed comprehensive consensus recommendations. A total of 100 publications satisfied the search requirements for ciNPT use. A majority presented data supporting ciNPT use. Numerous publications reported SSI risk factors, with the most common including obesity (body mass index 6530 kg/m2); diabetes mellitus; tobacco use; or prolonged surgical time. We recommend that the surgeon assess the individual patient's risk factors and surgical risks. Surgeons should consider using ciNPT for patients at high risk for developing SSOs or who are undergoing a high-risk procedure or a procedure that would have highly morbid consequences if an SSI occurred

    Childlessness in France

    Even though the average age at first childbirth has been increasing and education and employment options for women have improved immensely in recent decades, in France, unlike in other European countries, these developments have not led to a major increase in childlessness. Birth rates remain high and the share of the population who are childless is among the lowest in western Europe. This article discusses the historical roots as well as the societal conditions, institutional regulations, and political decisions that may explain the low levels of childlessness in France. We also discuss differences in rates of childlessness by education and occupation. Using a large representative survey on family life that was conducted parallel to the French census in 2011, we study the fertility histories of men and women born between the 1920s and late 1970s. We find that while the differences in fertility by level of education seem to have declined, having a higher education is still an obstacle to parenthood for women. For men, having a low educational and occupational status is associated with a greater likelihood of being childless. A large part of the differences in rates of childlessness between social groups can be traced back to the men and women who have never lived in a couple relationship; thus, partnership status can be regarded as a decisive parameter of the extent of childlessness

    Experimental analysis of radially resolved dynamic inflow effects due to pitch steps

    The dynamic inflow effect denotes the unsteady aerodynamic response to fast changes in rotor loading due to a gradual adaption of the wake. This does lead to load overshoots. The objective of the paper was to increase the understanding of that effect based on pitch step experiments on a 1.8 m diameter model wind turbine, which are performed in the large open jet wind tunnel of ForWind – University of Oldenburg. The flow in the rotor plane is measured with a 2D laser Doppler anemometer, and the dynamic wake induction factor transients in axial and tangential direction are extracted. Further, integral load measurements with strain gauges and hot-wire measurements in the near and close far wake are performed. The results show a clear gradual decay of the axial induction factors after a pitch step, giving the first direct experimental evidence of dynamic inflow due to pitch steps. Two engineering models are fitted to the induction factor transients to further investigate the relevant time constants of the dynamic inflow process. The radial dependency of the axial induction time constants as well as the dependency on the pitch direction is discussed. It is confirmed that the nature of the dynamic inflow decay is better described by two rather than only one time constant. The dynamic changes in wake radius are connected to the radial dependency of the axial induction transients. In conclusion, the comparative discussion of inductions, wake deployment and loads facilitate an improved physical understanding of the dynamic inflow process for wind turbines. Furthermore, these measurements provide a new detailed validation case for dynamic inflow models and other types of simulations.</p
