    Examination of Acid-Fast Bacilli in Sputum Using Modified Light Microscope with Homemade Light Emitting Diode Additional Attachment

    Typical clinical symptoms and chest X-ray is a marker of Tuberculosis (TB) sufferers. However, the diagnosis of TB in adults should be supported by microscopic examination. Currently, Bacilli microscopic examination of acid-fast bacilli (AFB) in sputum by Ziehl-Neelsen (ZN) coloring is the most widely used. However, for reasons of convenience, especially for laboratories with a considerable amount of smear samples, and due to higher sensitivity compared with ZN staining, the World Health Organization (WHO) has recommended the use of auramine-O-staining (fluorochrome  staining), which is visualized by light emitting diode (LED) fluorescence microscopy. The aim of this study was to evaluate the performance of modified light microscope with homemade LED additional attachment for examination of AFB in sputum using auramine-O-staining method. We compared the sensitivity and specificity of 2 kinds of AFB in sputum methods: ZN and fluorochrome, using culture on Lowenstein-Jensen media as the gold standard. The results showed auramine-O-staining gives more proportion of positive findings (81%) compared to the ZN method (70%). These results demonstrated that the sensitivity of auramine-O-staining was higher than ZN, however it gives more potential false positive results than ZN. The sensitivity of auramine-O-staining in detecting AFB in sputum was 100% while the specificity was 88%

    Pertumbuhan daerah kota di negra berkembang menunjukan gejala yang tidak di harapkan.untuk mengantisipasi kondisi keterbelakangan ini, daerah perkotaan haurus di dorong secara kuat untuk meningkatkan permintaan terhadap barang dan pelayanan untuk seluruh kepentingan nasional. permasalahan kota yang telah di hadapi bukan hanya persoalan keruangan dan Perubahan struktur ekonomi saja, tapi juga pembentukan implikasi sosial dan lingkungan. hal ini dapat di lihat di Bogota dengan pola dualisme sistem sosialnya, adanya pemisahan ruang antara masyarakat kaya di daerah utara dan masyarakat miskin di daerah selatan dan barat. Fenomena serupa juga di temukan di Indonesia. Jakarta yang berkembang secara cepat dengan dukungan sektor manufaktur dan keuanga, mencapai kondisi ekonomi dan pendapatan yang baik. Bagaimanapun, di Balik kondisi tersebut, ditemukan biaya yang tak terhindarkan secara nyata, yang umum muncul berupa penurunan kualitas lingkungan. Beberapa kebijaksanaan direkomendasikan adalah: (1) Meningkatkan produktivitas ekonomi wilayah kota yang terintegrasi ke dalam pembangunan regional dan pedesaan, (2) Meningkatkan produktivitas kelompok miskin perkotaan melalui perbaikan infrastruktur sosial dan perluasan kesempatan kerja, (3) Menghindari Perusakan lingkungan serta konsekwensi lainnya di sekitar wilayah masyarakat miskin, dan (4) Membangun persepsi yang seimbang terhadap pembangunan kota dan permasalahan-permasalahan yang berkaitan dengannya: pemerintah, sektor swasta, dan masyarakat

    Occupational safety and regulatory compliance in US commercial fishing

Data were collected on demographic characteristics, safety equipment and training, and regulatory compliance during at sea boardings of working commercial fishing vessels (n=259). Trends in safety and compliance were explored using standard comparison tests and principal component analysis. More than 40% of vessels were not in compliance with applicable safety regulations. That rate was lower for fishermen subjected to more stringent and costly safety requirements. The vast majority of fishermen were not safety trained, and many were not familiar with the proper use and maintenance of life-saving equipment. There is a clear need for better safety training in this industry. Educational efforts should be targeted at the local level at minimal cost to fishermen to encourage participation.This study was supported by funding from Maine Sea Grant and the National Oceanic and Atmospheric Administration (R/08-03 NA060AR4170108)

    Modelling tsunami inundation on coastlines with characteristic form

    This paper provides an indication of the likely difference in tsunami amplification and dissipation between different characteristic coastal embayments, coastal entrances and estuaries. Numerical modeling is performed with the ANU/Geoscience Australia tsunami inundation model. Characteristic coastal morphology is represented by simpler generic morphological shapes which can be applied easily in the ANUGA model, such that key non-dimensional parameters (e.g. embayment depth/bay width) can be varied. Modeling is performed with a range of bay shapes, seabed gradient and different incident tsunami wave shapes and wave angles, including sine waves, solitary waves and leading depression Nwaves. The results show a complex pattern for both large and small embayments, with wave breaking an important control on the amplification of the wave between the 20m contour and the shore. For large embayments, the wave run-up can be amplified by a factor six in comparison to the amplitude at the model boundary. For small embayments, the amplification is dependent on the location of the ocean water line, or tidal stage

    Limited genetic diversity in Salmonella enterica Serovar Enteritidis PT13

    <p>Abstract</p> <p>Background</p> <p><it>Salmonella enterica </it>serovar Enteritidis has emerged as a significant foodborne pathogen throughout the world and is commonly characterized by phage typing. In Canada phage types (PT) 4, 8 and 13 predominate and in 2005 a large foodborne PT13 outbreak occurred in the province of Ontario. The ability to link strains during this outbreak was difficult due to the apparent clonality of PT13 isolates in Canada, as there was a single dominant pulsed-field gel electrophoresis (PFGE) profile amongst epidemiologically linked human and food isolates as well as concurrent sporadic strains. The aim of this study was to perform comparative genomic hybridization (CGH), DNA sequence-based typing (SBT) genomic analyses, plasmid analyses, and automated repetitive sequence-based PCR (rep-PCR) to identify epidemiologically significant traits capable of subtyping <it>S</it>. Enteritidis PT13.</p> <p>Results</p> <p>CGH using an oligonucleotide array based upon chromosomal coding sequences of <it>S. enterica </it>serovar Typhimurium strain LT2 and the <it>Salmonella </it>genomic island 1 successfully determined major genetic differences between <it>S</it>. Typhimurium and <it>S</it>. Enteritidis PT13, but no significant strain-to-strain differences were observed between <it>S</it>. Enteritidis PT13 isolates. Individual loci (<it>safA </it>and <it>fliC</it>) that were identified as potentially divergent in the CGH data set were sequenced in a panel of <it>S</it>. Enteritidis strains, and no differences were detected between the PT13 strains. Additional sequence-based typing was performed at the <it>fimA</it>, <it>mdh</it>, <it>manB</it>, <it>cyaA</it>, <it>citT</it>, <it>caiC</it>, <it>dmsA</it>, <it>ratA </it>and STM0660 loci. Similarly, no diversity was observed amongst PT13 strains. Variation in plasmid content between PT13 strains was observed, but macrorestriction with B<it>gl</it>II did not identify further differences. Automated rep-PCR patterns were variable between serovars, but <it>S</it>. Enteritidis PT13 strains could not be differentiated.</p> <p>Conclusion</p> <p>None of the methods identified any significant variation between PT13 strains. Greater than 11,300 base pairs of sequence for each of seven <it>S</it>. Enteritidis PT13 strains were analyzed without detecting a single polymorphic site, although diversity between different phage types of <it>S</it>. Enteritidis was observed. These data suggest that Canadian <it>S</it>. Enteritidis PT13 strains are highly related genetically.</p

    Fecal Microbial and Metabolic Profiles in Dogs With Acute Diarrhea Receiving Either Fecal Microbiota Transplantation or Oral Metronidazole

    The aim was to characterize differences in fecal consistency, and fecal microbiota and metabolome profiles in dogs with acute diarrhea (AD) treated with either fecal microbiota transplantation as enema (FMT;n = 11) or oral metronidazole (MET;n = 7) for 7 days. On days 0, 7, and 28 fecal samples were obtained. Fecal samples from healthy dogs (HC;n = 14) were used for comparison. Samples were analyzed by the previously validated qPCR based canine Dysbiosis Index (DI;increased values indicate microbiota dysbiosis) and 16S rRNA gene sequencing. The fecal metabolome was analyzed using a previously validated targeted canine assay for fecal unconjugated bile acids, and untargeted metabolomics. Fecal consistency improved significantly in dogs treated with FMT and MET by day 7 and day 28 (p < 0.01) compared to day 0. However, on day 28 fecal consistency was significantly better in FMT compared to MET (p = 0.040). At day 0, dogs with AD had an altered microbiota indicated by significantly increased DI, decreased alpha-diversity, and altered beta-diversity. In the FMT group, the DI decreased over time, while MET led to a significant increase in the dysbiosis index at day 7 and 28 compared to FMT. Sequencing data revealed that in FMT microbial diversity and beta-diversity was similar to HC at day 28, while in MET these parameters were still significantly different from HC. In dogs treated with FMT, a decrease in cholic acid and the percentage of primary bile acids was observed, whereas treatment with metronidazole led to an increase in cholic acid at day 7 and an increase in percentage of primary bile acids over time. Based on untargeted metabolomics, dogs with AD had an altered fecal metabolome compared to HC. Dogs treated with FMT clustered closer to HC at day 28, while dogs treated with MET did not. In this pilot study, dogs with AD had significant differences in fecal microbiota and metabolome profiles. Dogs treated with MET still had altered microbial and metabolic profiles at day 28 compared to dogs treated with FMT or healthy dogs

    Salmonella Serovars from Humans and Other Sources in Thailand, 1993–2002

    We serotyped 44,087 Salmonella isolates from humans and 26,148 from other sources from 1993 through 2002. The most common serovar causing human salmonellosis in Thailand was Salmonella enterica Weltevreden. Serovars causing human infections in Thailand differ from those in other countries and seem to be related to Salmonella serovars in different food products and reservoirs
