    Provenance of plumes in global convection models

    In global convection models constrained by plume motions and subduction history over the last 230 Myr, plumes emerge preferentially from the edges of thermochemical structures that resemble present-day large low shear velocity provinces (LLSVPs) beneath Africa and the Pacific Ocean. It has been argued that large igneous provinces (LIPs) erupting since 200 Ma may originate from plumes that emerged from the edges of the LLSVPs and numerical models have been devised to validate this hypothesis. Although qualitative assessments that are broadly in agreement with this hypothesis have been derived from numerical models, a quantitative assessment has been lacking. We present a novel plume detection scheme and derive Monte Carlo-based statistical correlations of model plume eruption sites and reconstructed LIP eruption sites. We show that models with a chemically anomalous lower mantle are highly correlated to reconstructed LIP eruption sites, whereas the confidence level obtained for a model with purely thermal plumes falls just short of 95%. A network of embayments separated by steep ridges form in the deep lower mantle in models with a chemically anomalous lower mantle. Plumes become anchored to the peaks of the chemical ridges and the network of ridges acts as a floating anchor, adjusting to slab push forces through time. The network of ridges imposes a characteristic separation between conduits that can extend into the interior of the thermochemical structures. This may explain the observed clustering of reconstructed LIP eruption sites that mostly but not exclusively occur around the present-day LLSVPs

Abstract Reconstructing the 3D structure of the Earth's mantle has been a challenge for geodynamicists for about 40 years. Although numerical models and computational capabilities have substantially progressed, parameterizations used for modeling convection forced by plate motions are far from being Earth-like. Among the set of parameters, rheology is fundamental because it defines in a non-linear way the dynamics of slabs and plumes, and the organization of lithosphere deformation. In this study, we evaluate the role of the temperature dependence of viscosity (variations up to 6 orders of magnitude) and the importance of pseudo-plasticity on reconstructing slab evolution in 3D spherical models of convection driven by plate history models. Pseudo-plasticity, which produces plate-like behavior in convection models, allows a consistent coupling between imposed plate motions and global convection, which is not possible with temperature-dependent viscosity alone. Using test case models, we show that increasing temperature dependence of viscosity enhances vertical and lateral coherence of slabs, but leads to unrealistic slab morphologies for large viscosity contrasts. Introducing pseudo-plasticity partially solves this issue, producing thin laterally and vertically more continuous slabs, and flat subduction where trench retreat is fast. We evaluate the differences between convection reconstructions employing different viscosity laws to be very large, and similar to the differences between two models with the same rheology but using two different plate histories or initial conditions

    Cutaneous Invasive Fungal Infections with Saksenaea Species in Immunocompetent Patients in Europe: A Systematic Review and Case Report

    Invasive fungal infections from Saksenaea, a fungus belonging to the Mucorales, have been rarely reported in central European climate zones. This study aims to raise awareness of invasive cutaneous infections with Saksenaea species. The first case of a cutaneous infection was diagnosed in Switzerland in an immunocompetent 79-year-old patient. A minor skin trauma of her left lower leg led to a fulminant infection causing necrosis and extensive loss of tissue. The combination of surgical debridement and administration of antifungal agents averted a prolonged course with a possible worse outcome. A pedicled hemisoleus muscle flap was used to reconstruct the defect and treatment was continued for 63 days. Methods: A systematic review in accordance with the Preferred Reporting Items for Systematic review and Meta-Analysis guidelines was conducted to identify all European cases of infection with Saksenaea species in immunocompetent hosts. The epidemiology, clinical presentation, microbiological diagnosis, and management of cases reported in Europe were summarized and analyzed. Conclusions: The prognosis of soft tissue infections with Saksenaea species. depends on early diagnosis and appropriate antifungal and surgical treatment. Reconstruction can be successful under ongoing antifungal treatment

    Temporal factors in violence related injuries—An 11year trend analysis of violence-related injuries from a Swiss Emergency Department

    Summary: Background: Injury from interpersonal violence is a major social and medical problem in the industrialized world. Little is known about the trends in prevalence and injury pattern or about the demographic characteristics of the patients involved. Methods: In this retrospective analysis, we screened the database of the Emergency Department of a large university hospital for all patients who were admitted for injuries due to interpersonal violence over an 11year period. For all patients identified, we gathered data on age, country of origin, quality of injury, and hospitalization or outpatient management. A trend analysis was performed using Kendall's tau-b correlation coefficients for regression analysis. Results: The overall number of patients admitted to our Emergency Department remained stable over the study period. Non-Swiss nationals were overrepresented in comparison to the demographics of the region where the study was conducted. There was a trend toward a more severe pattern of injury, such as an increase in the number of severe head injuries. Conclusions: Although the overall number of patients remained stable over the study period, there was an alarming trend toward a more severe pattern of injury, expressed by an increase in severe head trauma

    Chronic Norovirus Infection after Kidney Transplantation: Molecular Evidence for Immune-Driven Viral Evolution

    Background. Norovirus infection is the most common cause of acute self-limiting gastroenteritis. Only 3 cases of chronic norovirus infection in adult solid organ transplant recipients have been reported thus far. Methods. This case series describes 9 consecutive kidney allograft recipients with chronic norovirus infection with persistent virus shedding and intermittent diarrhea for a duration of 97-898 days. The follow-up includes clinical course, type of immunosuppression, and polymerase chain reaction for norovirus. Detailed molecular analyses of virus isolates from stool specimens over time were performed. Results. The intensity of immunosuppression correlated with the diarrheal symptoms but not with viral shedding. Molecular analysis of virus strains from each patient revealed infection with different variants of GII.4 strains in 7 of 9 patients. Another 2 patients were infected with either the GII.7 or GII.17 strain. No molecular evidence for nosocomial transmission in our outpatient clinic was found. Capsid sequence alignments from follow-up specimens of 4 patients showed accumulation of mutations over time, resulting in amino acid changes predominantly in the P2 and P1-2 region. Up to 25 amino acids mutations were accumulated over a 683-day period in the patient with an 898-day shedding history. Conclusion. Norovirus infection may persist in adult renal allograft recipients with or without clinical symptoms. No evidence for nosocomial transmission in adult renal allograft recipients was found in our study.     Ridge subduction sparked reorganization of the Pacific plate-mantle system 60-50 million years ago

    A reorganization centered on the Pacific plate occurred ~53–47 million years ago. A “top-down” plate tectonic mechanism, complete subduction of the Izanagi plate, as opposed to a “bottom-up” mantle flow mechanism, has been proposed as the main driver. Verification based on marine geophysical observations is impossible as most ocean crust recording this event has been subducted. Using a forward modeling approach, which assimilates surface plate velocities and shallow thermal structure of slabs into mantle flow models, we show that complete Izanagi plate subduction and margin-wide slab detachment induced a major change in sub-Pacific mantle flow, from dominantly southward before 60 Ma to north-northeastward after 50 Ma. Our results agree with onshore geology, mantle tomography, and the inferred motion of the Hawaiian hot spot and are consistent with a plate tectonic process driving the rapid plate-mantle reorganization in the Pacific hemisphere between 60 and 50 Ma. This reorganization is reflected in tectonic changes in the Pacific and surrounding ocean basins

    Primary Staging of Prostate Cancer Patients with [18F]PSMA-1007 PET/CT Compared with [68Ga]Ga-PSMA-11 PET/CT

    Background: Hybrid imaging with prostate-specific membrane antigen (PSMA) is gaining importance as an increasingly meaningful tool for prostate cancer (PC) diagnostics and as a guide for therapy decisions. This study aims to investigate and compare the performance of [18F]PSMA1007 (18F-PSMA) and [68Ga]Ga-PSMA-11 positron emission tomography/computed tomography ( 68Ga-PSMA) in the initial staging of PC patients. Methods: The data of 88 biopsy-proven patients were retrospectively evaluated. PSMA-avid lesions were compared with the histopathologic Gleason Score (GS) for prostate biopsies, and the results were plotted by receiver operating characteristic (ROC)-curve. Optimal maximum standardized uptake value (SUVmax) cut-off values were rated using the Youden index. Results: 18F-PSMA was able to distinguish GS ≤ 7a from ≥7b with a sensitivity of 62%, specificity of 85%, positive predictive value (PPV) of 92%, and accuracy of 67% for a SUVmax of 8.95, whereas sensitivity was 54%, specificity 91%, PPV 93%, and accuracy 66% for 68Ga-PSMA (SUVmax 8.7). Conclusions: Both methods demonstrated a high concordance of detected PSMA-avid lesions with histopathologically proven PC. 18F-PSMA and 68Ga-PSMA are both suitable for the characterization of primary PC with a comparable correlation of PSMA-avid lesions with GS. Neither method showed a superior advantage. Our calculated SUVmax thresholds may represent valuable parameters in clinical use to distinguish clinically significant PC (csPC) from non-csPC

    Comparison of [18F]PSMA-1007 with [68Ga]Ga-PSMA-11 PET/CT in Restaging of Prostate Cancer Patients with PSA Relapse

    This study aimed to compare the diagnostic performance of [18F]PSMA-1007 positron emission tomography/computed tomography (PET/CT) (18F-PSMA) and [68Ga]Ga-PSMA-11 PET/CT ( 68Ga-PSMA) by identifying prostate-specific antigen (PSA) threshold levels for optimal detecting recurrent prostate cancer (PC) and to compare both methods. Retrospectively, the study included 264 patients. The performances of 18F-PSMA and 68Ga-PSMA in relation to the pre-scan PSA were assessed by receiver operating characteristic (ROC) curve. 18F-PSMA showed PC-lesions in 87.5% (112/128 patients), while 68Ga-PSMA identified them in 88.9% (121/136). For 18F-PSMA biochemical recurrent (BCR) patients treated with radical prostatectomy (78/128, patient group: F-RP), a PSA of 1.08 ng/mL was found to be the optimal cut-off level for predicting positive and negative scans (AUC = 0.821; 95%, CI: 0.710–0.932), while for prostatectomized 68Ga-PSMA BCR-patients (89/136, patient group: Ga-RP), the cut-off was 1.84 ng/mL (AUC = 0.588; 95%, CI: 0.410–0.766). In patients with PSA < 1.08 ng/mL (F-RP) 76.3% and <1.84 ng/mL (Ga-RP) 78.6% scans were positive, whereas patients with PSA ≥ 1.08 ng/mL (F-RP) or 1.84 ng/mL (Ga-RP) had positive scan results in 100% and 91.5% (p < 0.001/p = 0.085). The identified PSA thresholds for PSMA-mappable PC lesions in BCR-patients (RP) showed a better separation for 18F-PSMA with regard to the distinguishing of positive and negative PC-lesions compared to 68Ga-PSMA. However, the two PSMA PET/CT tracers gave similar overall findings

    Oval Domes: History, Geometry and Mechanics

    An oval dome may be defined as a dome whose plan or profile (or both) has an oval form. The word Aoval@ comes from the latin Aovum@, egg. Then, an oval dome has an egg-shaped geometry. The first buildings with oval plans were built without a predetermined form, just trying to close an space in the most economical form. Eventually, the geometry was defined by using arcs of circle with common tangents in the points of change of curvature. Later the oval acquired a more regular form with two axis of symmetry. Therefore, an “oval” may be defined as an egg-shaped form, doubly symmetric, constructed with arcs of circle; an oval needs a minimum of four centres, but it is possible also to build polycentric ovals. The above definition corresponds with the origin and the use of oval forms in building and may be applied without problem until, say, the XVIIIth century. Since then, the teaching of conics in the elementary courses of geometry made the cultivated people to define the oval as an approximation to the ellipse, an “imperfect ellipse”: an oval was, then, a curve formed with arcs of circles which tries to approximate to the ellipse of the same axes. As we shall see, the ellipse has very rarely been used in building. Finally, in modern geometrical textbooks an oval is defined as a smooth closed convex curve, a more general definition which embraces the two previous, but which is of no particular use in the study of the employment of oval forms in building. The present paper contains the following parts: 1) an outline the origin and application of the oval in historical architecture; 2) a discussion of the spatial geometry of oval domes, i. e., the different methods employed to trace them; 3) a brief exposition of the mechanics of oval arches and domes; and 4) a final discussion of the role of Geometry in oval arch and dome design
