    Child sexual abuse in southern Brazil and associated factors: a population-based study

    <p>Abstract</p> <p>Background</p> <p>The prevalence of child sexual abuse (CSA) in the population has been poorly described in developing countries. Population data on child sexual abuse in Brazil is very limited. This paper aims to estimate lifetime prevalence of child sexual abuse and associated factors in a representative sample of the population aged 14 and over in a city of southern Brazil.</p> <p>Methods</p> <p>A two-stage sampling strategy was used and individuals were invited to respond to a confidential questionnaire in their households. CSA was defined as non-consensual oral-genital, genital-genital, genital-rectal, hand-genital, hand-rectal, or hand-breast contact/intercourse between ages 0 and 18. Associations between socio-demographic variables and CSA, before and after age 12, were estimated through multinomial regression.</p> <p>Results</p> <p>Complete data were available for 1936 respondents from 1040 households. Prevalence of CSA among girls (5.6% 95%CI [4.8;7.5]) was higher than among boys (1.6% 95%CI [0.9;2.6]). Boys experienced CSA at younger ages than girls and 60% of all reported CSA happened before age 12. Physical abuse was frequently associated with CSA at younger (OR 5.6 95%CI [2.5;12.3]) and older (OR 9.4 95%CI [4.5;18.7]) ages. CSA after age 12 was associated with an increased number of sexual partners in the last 2 months.</p> <p>Conclusion</p> <p>Results suggest that CSA takes place at young ages and is associated with physical violence, making it more likely to have serious health and developmental consequences. Except for gender, no other socio-demographic characteristic identified high-risk sub-populations.</p

    Association between breastfeeding and intelligence, educational attainment, and income at 30 years of age: a prospective birth cohort study from Brazil

    Background Breastfeeding has clear short-term benefi ts, but its long-term consequences on human capital are yet to be established. We aimed to assess whether breastfeeding duration was associated with intelligence quotient (IQ), years of schooling, and income at the age of 30 years, in a setting where no strong social patterning of breastfeeding exists. Methods A prospective, population-based birth cohort study of neonates was launched in 1982 in Pelotas, Brazil. Information about breastfeeding was recorded in early childhood. At 30 years of age, we studied the IQ (Wechsler Adult Intelligence Scale, 3rd version), educational attainment, and income of the participants. For the analyses, we used multiple linear regression with adjustment for ten confounding variables and the G-formula. Findings From June 4, 2012, to Feb 28, 2013, of the 5914 neonates enrolled, information about IQ and breastfeeding duration was available for 3493 participants. In the crude and adjusted analyses, the durations of total breastfeeding and predominant breastfeeding (breastfeeding as the main form of nutrition with some other foods) were positively associated with IQ, educational attainment, and income. We identifi ed dose-response associations with breastfeeding duration for IQ and educational attainment. In the confounder-adjusted analysis, participants who were breastfed for 12 months or more had higher IQ scores (diff erence of 3·76 points, 95% CI 2·20–5·33), more years of education (0·91 years, 0·42–1·40), and higher monthly incomes (341·0 Brazilian reals, 93·8–588·3) than did those who were breastfed for less than 1 month. The results of our mediation analysis suggested that IQ was responsible for 72% of the eff ect on income. Interpretation Breastfeeding is associated with improved performance in intelligence tests 30 years later, and might have an important eff ect in real life, by increasing educational attainment and income in adulthood

    OBJECTIVE: To estimate the prevalence of common mental disorders and assess its association with risk factors in a cohort of young adults. METHODS: Cross-sectional study nested in a 1982 birth cohort study conducted in Pelotas, Southern Brazil. In 2004-5, 4,297 subjects were interviewed during home visits. Common mental disorders were assessed using the Self-Report Questionnaire. Risk factors included socioeconomic, demographic, perinatal, and environmental variables. The analysis was stratified by gender and crude and adjusted prevalence ratios were estimated by Poisson regression. RESULTS: The overall prevalence of common mental disorders was 28.0%; 32.8% and 23.5% in women and men, respectively. Men and women who were poor in 2004-5, regardless of their poor status in 1982, had nearly 1.5-fold increased risk for common mental disorders (pOBJETIVO: Estimar la prevalencia de trastornos mentales comunes y su asociación con factores de riesgo en una cohorte de adultos jóvenes. La probabilidad de trastornos mentales comunes fue estimada por el Self-Report Questionnaire. Los factores de riesgo incluyeron variables socioeconómicas, demográficas, perinatales y ambientales. El análisis fue estratificado por sexo y las razones de prevalencia simples y ajustadas fueron estimadas utilizándose regresión de Poisson. RESULTADOS: La prevalencia de trastornos mentales comunes en la población general fue de 28,0%; 32,8% y 23,5%, respectivamente, entre mujeres y hombres.     Quadro epdiemiológico das mortes súbitas na infância em cidades gaúchas (Brasil)

    All post-perinatal infant deaths occurring within a one-year period in 10 cities in the State of Rio Grande do Sul (Brazil), the metropolitan area of Porto Alegre included, were investigated in a case-control study. Seventy-two sudden infant deaths (SID) were identified through a complex monitoring system. These were investigated in detail through medical interviews with the children's parents and through the review of medical records. Such deaths were more common in males, during the first three months of life, and in the winter. The comparison of each case with two neighborhood control infants through multiple conditional logistic regression identified the following risk factors: low socio-economic level (measured by family income and maternal education), low birthweight, presence of other children in the household, low maternal age, maternal smoking, and mixed or artificial feeding. None of the 72 deaths had been certified as such by the physicians who filled in the death certificates, the majority of which referred to "bronchopneumonia" as the undereying cause.Foram investigados os óbitos infantis pós-perinatais ocorridos no período de um ano, em 10 cidades gaúchas, incluindo a região metropolitana de Porto Alegre, por meio de estudo de casos e controles. Setenta e duas mortes súbitas na infância (MSI) foram identificadas através de amplo sistema de monitorização de óbitos e investigadas em pormenores através de entrevistas médicas com os pais da criança e revisão de prontuários ambulatoriais e hospitalares. Os óbitos foram mais comuns em meninos, no primeiro trimestre de vida e durante o inverno. A comparação de cada caso de MSI com duas crianças-controle da mesma vizinhança, através de regressão logística condicional múltipla, identificou os seguintes fatores de risco: baixo nível socioeconômico (medido através da renda familiar e da escolaridade materna), baixo peso ao nascer, presença de outras crianças no domicílio, mães jovens e fumantes e aleitamento misto ou artificial. Nenhuma das 72 MSI foi reconhecida como tal pelos médicos que preencheram os atestados de óbito, sendo as mesmas atribuídas predominantemente a "broncopneumonias"

    Postnatal depression in Southern Brazil: prevalence and its demographic and socioeconomic determinants

    <p>Abstract</p> <p>Background</p> <p>Studies investigating the prevalence of postnatal depression (PND) show rates ranging from 5% to 36.7%. The investigation of age, race, educational levels, religion and income as risk factors for PND has yielded conflicting results. The aim of this study is to investigate the prevalence of PND in women residing in Southern Brazil and the associated risk factors.</p> <p>Methods</p> <p>This is population-based cross-sectional study of women residing in Porto Alegre who delivered in June 2001. A sample of 271 participants were selected from the Record of Living Newborn Infants of the State Health Department (the official Brazilian database and stores the name and address of all women who give birth to living newborn infants) using a process based on pseudo-random numbers which choose a random sample from 2.000 records. Once the addresses were identified, the women were visited at their place of residence (home, hotel, boarding house and prison), with the interviews taking place between the 6<sup>th </sup>and the 8<sup>th </sup>week after delivery.</p> <p>The association between the risk factors and PND was investigated through bivariate analysis using Pearson's chi-square test. Student's t-test was used to analyze the continuous variables. To identify independent risk factors, multivariate analysis was performed using hierarchical levels with a predefined model that took into account the time relationship between PND and the risk factors. Cox's regression was used to calculate the prevalence ratios.</p> <p>Results</p> <p>The PND prevalence rate found was 20.7% (CI 95% 15.7 – 25.7). After adjusting for confounding variables, per capita income was found to have a significant association with PND.</p> <p>Conclusion</p> <p>The prevalence of PND is higher than the figures found in most developed countries and similar to the figures found in developing countries. Differences in PND by regions or countries can be partially explained by the effect of income on the mediation of risk factors. In low income populations, women should be routinely evaluated for postnatal depression, and those with no partner or spouse are likely to require further care from health services and should be given the benefit of mental health prevention programs.</p
