    Prevalence of musculoskeletal injuries among adolescent squash players in the Western Cape

    Objective. To determine the prevalence of musculoskeletal injuries among adolescent squash players in the Western Cape. Design. A retrospective descriptive survey was conducted during the first week of May 2006. Three schools were randomly selected from a list of the top 10 schools in the Western Cape high school squash league of 2005, with 106 squash players aged 13 - 18 years participating in the study. Setting. Injury data were collected for 106 players at three schools randomly selected from a list of the top 10 schools in the Western Cape high school squash league of 2005. Interventions. An adapted structured self-administered questionnaire based on a previously validated musculoskeletal injury questionnaire was used to collect the data. Main outcome measures. The main variables investigated were prevalence, mechanism and injury site of musculoskeletal squash injuries. Results. Twenty-nine per cent of the players (N = 31) reported that they had sustained a squash injury in the 4 weeks prior to data collection. A total of 48 injuries were reported by the injured players. The most common injuries included those of the thigh (19%), shoulder (13%) and lower back (13%). Forty-two per cent of players reported no specific mechanism of injury, but experienced pain not associated with a traumatic injury only while playing squash. Conclusion. A relatively high prevalence of squash injuries was found. This preliminary study serves as a baseline for future research. Areas for further investigation were identified and this could lead to the implementation of preventive programmes and education to prevent injuries among adolescent squash players.     Acceptance of Automated Road Transport Systems (ARTS): an adaptation of the UTAUT model

    As research into innovative forms of automated transportation systems gains momentum, it is important that we develop an understanding of the factors that will impact the adoption of these systems. In an effort to address this issue, the European project CityMobil2 is collecting data around large-scale demonstrations of Automated Road Transport Systems (ARTS) in a number of cities across Europe. For these systems to be successful, user acceptance is vital. The current study used the Unified Theory of Acceptance and Use of Technology (UTAUT) to investigate the factors which might influence acceptance of ARTS vehicles, which were operational in two locations in Europe. The results indicate that the UTAUT constructs of performance expectancy, effort expectancy and social influence were all useful predictors of behavioural intentions to use ARTS, with performance expectancy having the strongest impact. However, it would appear that other factors are also needed in order for the model to strongly predict behavioural intentions in an automated transport context. Based on these findings, a number of implications for developers and ideas for future research are suggested

    Ketogenic diet for epilepsy and obesity:Is it the same?

    The term “ketogenic diet” (KD) is used for a wide variety of diets with diverse indications ranging from obesity to neurological diseases, as if it was the same diet. This terminology is confusing for patients and the medical and scientific community. The term “ketogenic” diet implies a dietary regimen characterized by increased levels of circulating ketone bodies that should be measured in blood (beta-hydroxybutyrate), urine (acetoacetate) or breath (acetone) to verify the “ketogenic metabolic condition”. Our viewpoint highlights that KDs used for epilepsy and obesity are not the same; the protocols aimed at weight loss characterized by low-fat, low-CHO and moderate/high protein content are not ketogenic by themselves but may become mildly ketogenic when high calorie restriction is applied. In contrast, there are standardized protocols for neurological diseases treatment for which ketosis has been established to be part of the mechanism of action. Therefore, in our opinion, the term ketogenic dietary therapy (KDT) should be reserved to the protocols considered for epilepsy and other neurological diseases, as suggested by the International Study Group in 2018. We propose to adjust the abbreviations in VLCHKD for Very Low CarboHydrate Ketogenic Diet and VLEKD for Very Low Energy Ketogenic Diet, to clarify the differences in dietary composition. We recommend that investigators describe the researchers describing efficacy or side effects of KDs, to clearly specify the dietary protocol used with its unique acronym and level of ketosis, when ketosis is considered as a component of the diet's mechanism of action.</p

    Experimental Validation of a Fundamental Model for PCR Efficiency

    Recently a theoretical analysis of PCR efficiency has been published by Booth et al., (2010). The PCR yield is the product of three efficiencies: (i) the annealing efficiency is the fraction of templates that form binary complexes with primers during annealing, (ii)the polymerase binding efficiency is the fraction of binary complexes that bind to polymerase to form ternary complexes and (iii)the elongation efficiency is the fraction of ternary complexes that extend fully. Yield is controlled by the smallest of the three efficiencies and control could shift from one type of efficiency to another over the course of a PCR experiment. Experiments have been designed that are specifically controlled by each one of the efficiencies and the results are consistent with the mathematical model. The experimental data has also been used to quantify six key parameters of the theoretical model. An important application of the fully characterized model is to calculate initial template concentration from real-time PCR data. Given the PCR protocol, the midpoint cycle number (where the template concentration is half that of the final concentration) can be theoretically determined and graphed for a variety of initial DNA concentrations. Real-time results can be used to calculate the midpoint cycle number and consequently the initial DNA concentration, using this graph. The application becomes particularly simple if a conservative PCR protocol is followed where only the annealing efficiency is controlling

    Efficiency of the Polymerase Chain Reaction

    The polymerase chain reaction (PCR) has found wide application in biochemistry and molecular biology such as gene expression studies, mutation detection, forensic analysis and pathogen detection. Increasingly quantitative real time PCR is used to assess copy numbers from overall yield. In this study the yield is analyzed as a function of several processes: (1) thermal damage of the template and polymerase occurs during the denaturing step, (2) competition exists between primers and templates to either anneal or form dsDNA, (3) polymerase binding to annealed products (primer/ssDNA) to form ternary complexes and (4) extension of ternary complexes. Explicit expressions are provided for the efficiency of each process, therefore reaction conditions can be directly linked to the overall yield. Examples are provided where different processes play the yield-limiting role. The analysis will give researchers a unique understanding of the factors that control the reaction and will aid in the interpretation of experimental results

    Response to recharge variation of thin rainwater lenses and their mixing zone with underlying saline groundwater

    In coastal zones with saline groundwater, fresh groundwater lenses may form due to infiltration of rain water. The thickness of both the lens and the mixing zone, determines fresh water availability for plant growth. Due to recharge variation, the thickness of the lens and the mixing zone are not constant, which may adversely affect agricultural and natural vegetation if saline water reaches the root zone during the growing season. In this paper, we study the response of thin lenses and their mixing zone to variation of recharge. The recharge is varied using sinusoids with a range of amplitudes and frequencies. We vary lens characteristics by varying the Rayleigh number and Mass flux ratio of saline and fresh water, as these dominantly influence the thickness of thin lenses and their mixing zone. Numerical results show a linear relation between the normalised lens volume and the main lens and recharge characteristics, enabling an empirical approximation of the variation of lens thickness. Increase of the recharge amplitude causes increase and the increase of recharge frequency causes a decrease in the variation of lens thickness. The average lens thickness is not significantly influenced by these variations in recharge, contrary to the mixing zone thickness. The mixing zone thickness is compared to that of a Fickian mixing regime. A simple relation between the travelled distance of the centre of the mixing zone position due to variations in recharge and the mixing zone thickness is shown to be valid for both a sinusoidal recharge variation and actual records of daily recharge data. Starting from a step response function, convolution can be used to determine the effect of variable recharge in time. For a sinusoidal curve, we can determine delay of lens movement compared to the recharge curve as well as the lens amplitude, derived from the convolution integral. Together the proposed equations provide us with a first order approximation of lens characteristics using basic lens and recharge parameters without the use of numerical models. This enables the assessment of the vulnerability of any thin fresh water lens on saline, upward seeping groundwater to salinity stress in the root zone

    A collaborative and evolving response to the needs of frontline workers, patients and families during the COVID-19 pandemic at Tygerberg Hospital, Western Cape Province, South Africa

    The global devastation caused by the COVID-19 pandemic and its mental health impact is undeniable. The physical and psychological consequences are wide-ranging – affecting patients fighting the disease, frontline workers in the trenches with them, healthcare staff deployed in high-care settings, and families disconnected from their loved ones in their darkest hours. Within 6 weeks of the COVID-19 outbreak in South Africa, the Department of Psychiatry at Stellenbosch University established the TBH/SU COVID Resiliency Clinic to provide psychological support to frontline workers at Tygerberg Hospital. Identified barriers in healthcare workers accessing mental healthcare resulted in moving towards an on-site visibility to try to remove some of these barriers. This greater on-site presence enabled networking and building of relationships with frontline staff that over time highlighted other frontline needs, such as providing psychosocial and spiritual support to patients and their families. We share challenges, lessons learned and recommendations from two initiatives: the TBH/SU COVID-19 Resiliency Clinic, and an embedded COVID Care Team (CCT).     Do experienced physiotherapists apply equal magnitude of force during a grade I central pa on the cervical spine?

    Background and purpose: Physiotherapists frequently use central posterior-anterior (PA) joint mobilization techniques for assessing and managing spinal disorders. Manual examination findings provide the basis for the selection of treatment techniques. From the literature it is evident that the level of reliability varies when physiotherapists perform different mobilization techniques. Repeatability of mobilization techniques is important for better physiotherapy management. The aim of the study was to determine whether experienced physiotherapists apply equal magnitude of force during a grade I central PA mobilisation technique on the cervical spine. Another aim was to determine the variation in the magnitude of force applied by each individual physiotherapist. Subjects: Sample of convenience, consisting of sixteen (n=16) selected qualified physiotherapists with experience in Orthopaedic Manual Therapy. Methods: A grade I central PA was performed on the Flexiforce TM sensors positioned on C6 of the same asymptomatic model to measure the applied magnitude of force. Two separate measurements, each lasting 30-seconds, were obtained. Results: The average maximum peak force applied by the majority of physiotherapists (87.5%) was between 10.95g and 72g. The difference in the forces applied for the two measurements ranged between 0.64g and 24.4g. The Bland Altman scatterplot determined the mean of the difference between measurement one and two, calculated for the group, was zero. When comparing the two measurements, little variation was noted in the forces applied, as well as the coefficient of variation for each physiotherapist. Conclusion and Discussion: Current results demonstrated good intra-therapist and moderate to good inter-therapist repeatability. Further research is required to generalize results
