    Efeitos da disrupção endócrina na gestação: uma revisão sistemática

    Endocrine disruptors are ubiquitous natural and synthetic environmental pollutants that can mimic, block or disturb normal hormonal function. Environmental exposure to these pollutants is almost impossible to prevent due to their presence in the air, water, soil, food and in many other materials with which we come in contact daily, such as plastics. Foetuses are vulnerable during pregnancy because their organism is in a developmental stage, and any disruption may affect their health in the short and/or long term. There is some evidence that these substances disrupt tissue differentiation and growth processes, cause birth defects and affect the length of pregnancy. In this review we will focus on environmental epidemiology and related literature in order to update current knowledge based on the actual evidence of the effects of these substances on the aetiology of preterm delivery, miscarriages, low birth weight, malformations, cryptorchidism, hypospadias and micropenis. The difficulty in studying this topic is due to the high number of involved factors, which makes the available results inconclusive or even contradictory. Consequently, further research is necessary.Los alteradores endocrinos son contaminantes ambientales, naturales y sintéticos, ubicuos, que pueden mimetizar, bloquear o alterar funciones hormonales. La exposición ambiental a estos contaminantes es prácticamente imposible de evitar debido a que están presentes en el aire, en el agua, en los suelos, en los alimentos, y en muchos de los materiales con los que estamos en contacto diariamente, como los plásticos. La gestación supone una etapa de vulnerabilidad para el feto porque su organismo está en proceso de desarrollo y cualquier alteración puede afectar su salud a corto o largo plazo. Hay cierta evidencia de que estas sustancias alteran procesos de crecimiento y diferenciación de tejidos, producen malformaciones y afectan la duración del embarazo. Hasta el momento se conoce algo de sus efectos en la etiología de la criptorquidia, hipospadias y micropene, abortos espontáneos, partos prematuros y el bajo peso al nacimiento, efectos que serán revisados en este documento que quiere aportar una actualización del conocimiento, concentrándose en la epidemiología ambiental y la literatura relacionada. La dificultad para estudiar este tema por la cantidad de factores que intervienen hace que los resultados existentes no sean concluyentes o incluso contradictorios, por lo que es necesario que se siga investigando.Os disruptores endócrinos são contaminantes ambientais ubíquos, naturais e sintéticos, que podem mimetizar, bloquear ou alterar funções hormonais. Existe alguma evidência de que estas sustâncias alteram processos de crescimento e diferenciação de tecidos, produzem malformações e afetam a duração da gravidez. Neste momento já existe algum conhecimento sobre os seus efeitos na etiologia da criptorquidia, hipospadia, micropénis, aborto espontâneo, parto prematuro e baixo peso à nascença. Efeitos esses que serão revisados neste documento visando uma atualização do conhecimento, concentrando-se na epidemiologia ambiental e na literatura relacionada. A dificuldade para estudar este tema pela quantidade de fatores intervenientes faz com que os resultados existentes não sejam conclusivos ou até que sejam contraditórios, pelo que, será necessário que se continue investigando

    Results of the implementation of a virtual microscope in a course of histology

    [EN] The course of Anatomy and Histology is studied in the first year of Dentistry at the University Cardenal Herrera CEU (Alfara del Patriarca, Spain). Its practices consist on choose freely six different tissue samples and draw their most representative features. These practices were made by optical microscopy until 2014, and in 2015 was introduced the virtual microscope exclusively. The aim of the study is to test whether this new teaching method has improved the quality of exercise and the understanding shown by students. First, the best exercises of both years were chosen, and from them some drawings from the same tissue were compared. Some tissues which samples for optical microscope were hard to obtain, were drawn for the very first time thanks to the virtual microscope. Also, with the virtual microscopy the drawings contained more details and definition. The understanding of the structures improved, shown by a more functional, detailed and defined vision of the tissues. The labels of the virtual microscope helped to the self-study and avoided the loss of unnoticed structures.     Declared experiences of risky sexual behaviors in relation to alcohol consumption in the first year of college

    Fundamentos: En universitarios, el consumo de alcohol de mayor riesgo (borracheras y binge drinking (BD), tiene consecuencias negativas sobre su desarrollo y probablemente facilita conductas sexuales de riesgo. El objetivo de este trabajo fue estudiar si las conductas sexuales de riesgo al consumir alcohol (CSRA) se asocian a los consumos de mayor riesgo. Métodos: Estudio multicéntrico transversal con datos del Proyecto uniHcos, de universitarios de 1er año de 11 universidades españolas, entre los cursos 2011-2012 y 2017- 2018. Datos recogidos mediante cuestionario autoadministrado. Se realizó un análisis uni y bivariable, evaluando la significación estadística de las diferencias de prevalencia con chi-cuadrado. Se utilizó media y desviación típica para variables cuantitativas y como estadístico de contraste t de Student. Resultados: 9.862 participantes (72,2% mujeres). El 90,3% consumió alcohol y el 60,9% tuvo borracheras en último año; el 49% tuvo BD en el último mes. El consumo en el último mes y las borracheras fueron mayores en hombres y < 21 años. Las CSRA fueron superiores entre los que se emborracharon (15,7% sexo sin protección, 1,9% abuso sexual y 0,7% aprovecharse sexualmente) y consumieron en BD (17,1%, 1,9% y 0,7%). Las mujeres con ambos consumos de riesgo presentaron más abusos sexuales (2,2%), y los hombres fueron quienes más se aprovecharon sexualmente de otros (borracheras:1,2%; BD: 1,3%). Conclusiones: El consumo de alcohol está por encima de grupos similares. El BD tiene un patrón similar por género y edad. Las CSRA se asocian a los consumos de mayor riesgo, no detectándose en este grupo diferencias por género en sexo sin protección, sí en otras CSRA.Objective: In college students, higher risk alcohol consumption (drunkenness and binge drinking-BD) has negative consequences on their development and and probably facilitates risk sexual behaviors. The objective was to study if risky sexual behaviors when consuming alcohol (RSBA) are associated with higher risk consumption. Methods: Cross-sectional multicenter study with UniHcos Project, 1st year university students from 11 universities in Spain, academic years 2011-2012 to 2017-2018 data. This data were collected by self-administered questionnaire. A uni and bivariate analysis was performed, evaluated the statistical significance of the differences in prevalence with chi-square. Mean and standard deviation were used for quantitative variables and Student's t test statistic was used. Results: 9,862 subjects (72.2% women). 90.3% reported having consumed alcohol and 60.9% had drunk the last year, 49% BD in last month. It was deteded in men, significantly higher consumption in the last month and drunkenness. Last month consumption and drunkenness were significantly higher in men and in <21 years. The RSBA were significantly higher among who were drunk (15.7% unprotected sex, 1.9% sexual abuse and 0.7% taking sexual advantage) and had BD (17.1%, 1.9% and 0.7 %). Women with both risk consumptions had more sexual abuse (2.2%), and men had greater behaviors of taking sexual advantage of someone (drunk: 1.2%; BD: 1.3%). Conclusions: Alcohol consumption was above similar groups. BD consumption was similar by gender and age. Risk sexual behaviors appear mainly in problematic consumption.     The association between lead and attention-deficit/hyperactivity disorder: A systematic review

    The etiology of Attention-Deficit/Hyperactivity Disorder (ADHD) is complex and multifactorial. Although the development of ADHD symptoms remains to be elucidated, in recent years, epigenetic processes have emerged as candidate mechanisms. Lead is one of the most dangerous environmental pollutants, and it is suspected to be associated with ADHD. The aim of the present study was to review the epidemiological literature currently available on the relation between lead exposure and the diagnosis of ADHD. The PubMed and EMBASE databases were searched from 1 July 2018 up to 31 July 2018. The authors included observational studies (cohort, case–control and cross-sectional studies) published in English carried out on children within the last 5 years, measuring lead exposure and health outcomes related to ADHD. Seventeen studies met the inclusion criteria: 5 of these studies found no association between lead exposure and ADHD whereas the remaining 12 studies showed positive associations, even though not all of them were homogeneous in terms of exposure periods considered or ADHD diagnosis. To conclude, the evidence from the studies allowed us to establish that there is an association between lead and ADHD and that even low levels of lead raise the risk. However, there is still a lack of longitudinal studies about the relationship between lead exposure and the development of ADHD. Given the potential importance for public health, further research that includes the entire potential risk factors for ADHD in children must be encouraged

    Air Quality during Covid-19 Lockdown

    Air pollution exposure is one of the greatest risks to health worldwide. It is estimated to be responsible for about 4.2 million deaths around the world every year owing to many serious diseases such as heart disease, stroke, acute and chronic respiratory diseases, and lung cancer. The WHO guideline limits are exceeded in several areas around the world, and it is estimated that about 90% of the world’s population is exposed to high air pollution levels, especially in low- and middle-income countries. The COVID-19 pandemic has forced governments to implement severe mobility restriction measures to limit the spread of the virus. This represented a unique opportunity to study the impact of mobility on urban air quality. Several studies which have investigated the relations between the quality of the air and such containment measures have shown the significant reduction of the main pollutants in the urban environment so to encourage the adoption of new approaches for the improvement of the quality of air in the cities. The aims of this entry are both a brief analysis and a discussion of the results presented in several papers to understand the relationships between COVID-19 containment measures and air quality in urban areas

    Effect of Breast Cancer Treatment on Dietary Vitamin Intake Levels.

    Breast cancer is the most common tumor among women, representing the second cause of cancer deaths in women. Treatment with chemotherapy negatively interferes with nutritional status. The intake of vitamins before, during and after treatment in a pilot cohort of women with non-invasive breast cancer (type I, II) treated at the Valencian Institute of Oncology (IVO) is evaluated. A 3-day anthropometric and nutritional assessment was performed using the DIAL program. Nutritional intake is compared with the values of Estimated Average Requirements (EAR) and Dietary Reference Intake (DRI) provided by the United States Department of Agriculture (USDA) and the European Food Safety Authority (EFSA). There is an overall decrease in vitamin intake during treatment which worsens at the end of said treatment. The decrease is significant in the case of vitamins B2 (p = 0.006), B3 (p = 0.042), B5 (p = 0.001), and B8 (p = 0.021). The relative risk during and after treatment increases with respect to the reference timeframe, before treatment. Deficit risks are statistically significant in the case of vitamins B5 (p = 0.001), B8 (p = 0.001) and B12 (p = 0.001). Decreased vitamin intake during treatment suggests a negative change in the patients' dietary behaviors during this time. Nutritional intervention and support may be beneficial to optimize overall dietary intake and maintain compliance with EAR and DRI for patients during a time in which adequate nutrition is important

    Survival of mycosis fungoides in patients in the Southeast of England.

    BACKGROUND: Mycosis fungoides (MF) is the most common skin lymphoma. The aetiology of MF remains unknown, and no therapy has to date significantly altered patient survival. OBJECTIVE: The present study examines trends in survival of MF patients in a well-defined population-based disease group, namely patients registered over a 40-year period at the Thames Cancer Registry, Southeast England. METHODS: The Thames Cancer Registry is a population-based registry, covering a population of approximately 14 million people. Data were taken from the Surveillance, Epidemiology and End Results cancer registry programme and the National Centre for Health Statistics. The database was used to identify cases of MF diagnosed between 1961 and 2000. A total of 985 records were identified, 821 (83%) of which had complete information on age, sex, year of diagnosis and area of residence. The observed and relative survivals of patients diagnosed during the periods 1971-1975, 1981-1985 and 1991-1995 were examined over a 5-year period of follow-up, using the relsurv Stata program to perform Cox proportional hazard analysis. RESULTS: A total of 821 MF eligible patients were available with a median follow-up of 4.3 years and a maximum follow-up of 30 years. The overall 5-year relative survival rate was 80%, and there was marked improvement between 1971 and 1981. The prognostic factors leading to a significantly poorer survival were high age, male sex, the presence of the Sézary syndrome, the use of hormone treatment and radiotherapy. CONCLUSIONS: A statistically significantly better survival over the last 20 years was found. The prognosis is generally good for most patients but not all. The best survival was seen for the female patients under 45 years of age without the presence of the Sézary syndrome. This difference in survival may be partly due to a difference in the disease stage or different treatment, or to both

    Burnout Syndrome Risk in Child and Adolescent Tennis Players and The Role of Adherence to the Mediterranean Diet

    This cross-sectional study examined the impact of adherence to Mediterranean diet on burnout syndrome risk in 94 athletes 8&ndash;15 years old. Diet pattern and burnout syndrome risk were assessed through the Athlete Burnout Questionnaire and the KIDMED Questionnaire. 55.3% of girls and 16.1% of boys had a high risk of burnout syndrome and the risk increased with age. Of the 78.7% with low adherence to Mediterranean diet, 31.1% showed no risk of burnout syndrome, 33.8% had a moderate risk, and 35.1% high risk. Of the 21.3% with a high adherence to Mediterranean diet, 35% had no risk of burnout syndrome, 45% had a moderate risk, and 20% had a high risk. Participants with moderate/high burnout syndrome risk were more likely to be girls and spend a higher number of hours watching television or playing video games. There is not enough statistical evidence in this study to reject the independence between the level of adherence to the Mediterranean diet and the risk of burnout syndrome in children, except in the case of daily consumption of fresh or cooked vegetables
