    SS433's accretion disc, wind and jets: before, during and after a major flare

    The Galactic microquasar SS433 occasionally exhibits a major flare when the intensity of its emission increases significantly and rapidly. We present an analysis of high-resolution, almost-nightly optical spectra obtained before, during and after a major flare, whose complex emission lines are deconstructed into single gaussians and demonstrate the different modes of mass loss in the SS433 system. During our monitoring, an initial period of quiescence was followed by increased activity which culminated in a radio flare. In the transition period the accretion disc of SS433 became visible in H-alpha and HeI emission lines and remained so until the observations were terminated; the line-of-sight velocity of the centre of the disc lines during this time behaved as though the binary orbit has significant eccentricity rather than being circular, consistent with three recent lines of evidence. After the accretion disc appeared its rotation speed increased steadily from 500 to 700 km/s. The launch speed of the jets first decreased then suddenly increased. At the same time as the jet launch speed increased, the wind from the accretion disc doubled in speed. Two days afterwards, the radio flux exhibited a flare. These data suggest that a massive ejection of material from the companion star loaded the accretion disc and the system responded with mass loss via different modes that together comprise the flare phenomena. We find that archival data reveal similar behaviour, in that when the measured jet launch speed exceeds 0.29c this is invariably simultaneous with, or a few days before, a radio flare. Thus we surmise that a major flare consists of the overloading of the accretion disc, resulting in the speeding up of the H-alpha rotation disc lines, followed by enhanced mass loss not just via its famous jets at higher-than-usual speeds but also directly from its accretion disc's wind.Comment: Accepted by MNRA

    A more unstable resting-state functional network in cognitively declining multiple sclerosis

    Cognitive impairment is common in people with multiple sclerosis and strongly affects their daily functioning. Reports have linked disturbed cognitive functioning in multiple sclerosis to changes in the organization of the functional network. In a healthy brain, communication between brain regions and which network a region belongs to is continuously and dynamically adapted to enable adequate cognitive function. However, this dynamic network adaptation has not been investigated in multiple sclerosis, and longitudinal network data remain particularly rare. Therefore, the aim of this study was to longitudinally identify patterns of dynamic network reconfigurations that are related to the worsening of cognitive decline in multiple sclerosis. Resting-state functional MRI and cognitive scores (expanded Brief Repeatable Battery of Neuropsychological tests) were acquired in 230 patients with multiple sclerosis and 59 matched healthy controls, at baseline (mean disease duration: 15 years) and at 5-year follow-up. A sliding-window approach was used for functional MRI analyses, where brain regions were dynamically assigned to one of seven literature-based subnetworks. Dynamic reconfigurations of subnetworks were characterized using measures of promiscuity (number of subnetworks switched to), flexibility (number of switches), cohesion (mutual switches) and disjointedness (independent switches). Cross-sectional differences between cognitive groups and longitudinal changes were assessed, as well as relations with structural damage and performance on specific cognitive domains. At baseline, 23% of patients were cognitively impaired (≥2/7 domains Z < -2) and 18% were mildly impaired (≥2/7 domains Z < -1.5). Longitudinally, 28% of patients declined over time (0.25 yearly change on ≥2/7 domains based on reliable change index). Cognitively impaired patients displayed more dynamic network reconfigurations across the whole brain compared with cognitively preserved patients and controls, i.e. showing higher promiscuity (P = 0.047), flexibility (P = 0.008) and cohesion (P = 0.008). Over time, cognitively declining patients showed a further increase in cohesion (P = 0.004), which was not seen in stable patients (P = 0.544). More cohesion was related to more severe structural damage (average r = 0.166, P = 0.015) and worse verbal memory (r = -0.156, P = 0.022), information processing speed (r = -0.202, P = 0.003) and working memory (r = -0.163, P = 0.017). Cognitively impaired multiple sclerosis patients exhibited a more unstable network reconfiguration compared to preserved patients, i.e. brain regions switched between subnetworks more often, which was related to structural damage. This shift to more unstable network reconfigurations was also demonstrated longitudinally in patients that showed cognitive decline only. These results indicate the potential relevance of a progressive destabilization of network topology for understanding cognitive decline in multiple sclerosis

    Dit rapport is een product van acht sessies: drie onderzoekssessies om het vraagstuk te verhelderen en issues te inventariseren, drie ontwerpsessies om tot suggesties en scenario’s voor het stelsel en be-voegdheidsdomeinen te komen en twee sessies om de resultaten verder door te ontwikkelen met oplei-dingsscholen. Naast deze acht sessies is een beknopte literatuurstudie uitgevoerd om het huidige stelsel in kaart te brengen, internationale lessen op te halen en van andere sectoren te leren. De resultaten zijn getoetst en doorontwikkeld met enkele experts. Deze aanpak houdt in dat we hier niet te maken heb-ben met een wetenschappelijk en representatief onderzoek naar inrichting en draagvlak van een nieuw stelsel. Het gaat om gerichte ideevorming voor een mogelijke andere inrichting, gebaseerd op gedegen sessies met een divers samengestelde vertegenwoordiging uit het veld (leraren, schoolleiders, bestuur-ders en lerarenopleiders). De nadere invulling van het bevoegdhedenstelsel alsmede de te maken keu-zes over de exacte invulling vallen buiten de scope van dit rapport

    Psychosexual Functioning of Cognitively-able Adolescents with Autism Spectrum Disorder Compared to Typically Developing Peers

    To gain further insight into psychosexual functioning, including behaviors, intrapersonal and interpersonal aspects, in adolescents with Autism Spectrum Disorder (ASD), comprehensive, multi-informant measures are needed. This study describes (1) the development of a new measure of psychosexual functioning in both parent- and self-reports (Teen Transition Inventory; TTI) covering all three domains of psychosexual functioning (i.e. psychosexual socialization, psychosexual selfhood, and sexual/intimate behavior). And (2) the initial testing of this instrument, comparing adolescents with ASD (n = 79 parent-report; n = 58 self-report) to Typically Developing (TD) adolescents (n = 131 parent-report; n = 91 self-report) while taking into account gender as a covariate. Results from both informants indicate more difficulties regarding psychosexual socialization and psychosexual selfhood in the ASD group. With regard to sexual/intimate behavior, only parents reported significantly more problems in adolescents with ASD

    Review: Placental mitochondrial function and structure in gestational disorders

    The aetiology of many gestational disorders is still unknown. However, insufficient trans-placental nutrient and oxygen transfer due to abnormal placentation is characteristic of several pathologies, and may alter the function of placental mitochondria. Mitochondria are multifunctional organelles that respond to a wide range of stimuli - such as physiological changes in cellular energy demands or various pathologies - by reshaping via fusion or fission, increasing/decreasing in number, altering oxidative phosphorylation, and signalling cellular functions such as apoptosis. Mitochondrial function is integral to tissue functions including energy production, metabolism, and regulation of various cellular responses including response to oxidative stress. This review details the functions of placental mitochondria and investigates mitochondrial function and structure in gestational disorders including preeclampsia, intrauterine growth restriction, diabetes mellitus, and obesity. Placental mitochondrial dysfunction may be critical in a range of gestational disorders which have important implications for maternal and fetal/offspring health

    Low-Iodine Diet of 4 Days Is Sufficient Preparation for I-131 Therapy in Differentiated Thyroid Cancer Patients

    CONTEXT: No consensus exists about the optimal duration of the low-iodine diet (LID) in the preparation of (131)I therapy in differentiated thyroid cancer (DTC) patients. OBJECTIVE: This work aimed to investigate if a LID of 4 days is enough to achieve adequate iodine depletion in preparation for (131)I therapy. In addition, the nutritional status of the LID was evaluated. METHODS: In this prospective study, 65 DTC patients treated at 2 university medical centers were included between 2018 and 2021. The patients collected 24-hour urine on days 4 and 7 of the LID and kept a food diary before and during the LID. The primary outcome was the difference between the 24-hour urinary iodine excretion (UIE) on both days. RESULTS: The median 24-hour UIE on days 4 and 7 of the LID were not significantly different (36.1 mcg [interquartile range, 25.4-51.2 mcg] and 36.5 mcg [interquartile range, 23.9-47.7 mcg], respectively, P = .43). On day 4 of the LID, 72.1% of the DTC patients were adequately prepared (24-hour UIE < 50 mcg), and 82.0% of the DTC patients on day 7 (P = .18). Compared to the self-reported regular diet, DTC patients showed a significantly (P < .01) lower percentage of nutrient intake (calories, protein, calcium, iodine, and water) during the LID. CONCLUSION: The 24-hour UIE on day 4 of the LID did not differ from day 7, and therefore shortening the LID from 7 to 4 days seems justified to prepare DTC patients for (131)I therapy in areas with sufficient iodine intake and may be beneficial to maintain a sufficient nutritional intake during DTC treatment
