    The thesis consists of three sections which are: Carol Shields the ordinary and the extraordinary ; the translation of the short stories "Hazel" and "Hinterland" from The Orange Fish by Carol Shields; and Antoine Berman's systems of deformation in literary prose . In the first section, the author, Carol Shields, is introduced by a short biography to underscore her interests in everyday life and in women and, in the third section, Antoine Berman's theory on the analytic of translation by means of thirteen tendencies of deformation, which are inherent to translation, is presented. The analytic of translation helped the translator self-evaluate and self-revise the translation. Destruction of underlying networks of meaning is examined in detail in order to reveal the underlying text of "Hazel" by analyzing the metaphors and their keywords. Even if certain tendencies of deformation were spotted in the two short stories, by no means was it always necessarily possible to avoid them, as the translation would have been incoherent. The analytic of translation is not a recipe for translating a literary text or producing a perfect translation, but rather a reflection to help the translator validate his choices and welcome the Foreign

    The role of relationship dissatisfaction in the dyadic associations between attachment insecurity and intimate partner violence among couples seeking therapy

    Abstract : For several decades, attempts have been made to understand the risk markers of intimate partner violence (IPV) to prevent its occurrence, but few studies have emphasized both partners’ perspective in examining these markers. This study explored the associations between attachment insecurity (anxiety and avoidance) and the perpetration of psychological and physical IPV through relationship dissatisfaction in a sample of 88 couples seeking therapy for relationship difficulties. Results from path analyses based on the Actor-Partner Interdependence Model showed that participants’ attachment avoidance was indirectly related to their own and their partner’s higher perpetration of psychological IPV through higher relationship dissatisfaction. Participants’ attachment avoidance was also associated with their own higher perpetration of physical IPV through their higher relationship dissatisfaction. Findings highlight that attachment insecurities and relationship dissatisfaction can both contribute to establishing dysfunctional and violent interaction patterns in couples

    Attachment, couple communication and sexual coercion

    Abstract : The use of subtle strategies to have sex with an unwilling partner is harmful to a couple’s sexual well-being but these strategies remain understudied. This research examined the mediating role of communication patterns in the associations between attachment insecurities and sustained sexual coercion in 145 same- and cross-gender couples, and the moderating role of partners’ gender. In addition to actor and partner effects, results revealed significant indirect effects from attachment insecurities to sexual coercion via communication patterns, with moderating effects of gender. Results may help practitioners and researchers understand the ways attachment insecurities and dysfunctional communication patterns can manifest in the experience of subtle forms of sexual coercion within couples

    Advancing heart health in North Carolina primary care: the Heart Health NOW study protocol

    Abstract Background The objective of Heart Health NOW (HHN) is to determine if primary care practice support—a comprehensive evidence-based quality improvement strategy involving practice facilitation, academic detailing, technology support, and regional learning collaboratives—accelerates widespread dissemination and implementation of evidence-based guidelines for cardiovascular disease (CVD) prevention in small- to medium-sized primary care practices and, additionally, increases practices’ capacity to incorporate other evidence-based clinical guidelines in the future. Methods/design HHN is a stepped wedge, stratified, cluster randomized trial to evaluate the effect of primary care practice support on evidence-based CVD prevention, organizational change process measures, and patient outcomes. Each practice will start the trial as a control, receive the intervention at a randomized time point, and then enter a maintenance period 12 months after the start of the intervention. The intervention will be randomized to practices in one of four strata defined by region of the state (east or west) and degree of practice readiness for change. Seventy-five practices in each region with a high degree of readiness will be randomized 1:1:1 in blocks of 3 sometime prior to month 8 to receive the intervention at month 9, 11, or 12. An additional 75 practices within each region that have a low degree of readiness or are recruited later will be randomized 1:1 in blocks of 2 prior to month 13 to receive the intervention at month 14 or 16. The sites will be ordered within each strata based on time of enrollment with the blocking based on this ordering. Evaluation will examine the effect of primary care practice support on (1) practice-level delivery of evidence-based CVD prevention, (2) patient-level health outcomes, (3) practice-level implementation of clinical and organizational changes that support delivery of evidence-based CVD prevention, and (4) practice-level capacity to implement future evidence-based clinical guidelines. Discussion Results will indicate whether primary care practice support is an effective strategy for widespread dissemination and implementation of evidence-based clinical guidelines in primary care practices. Discernible reductions in cardiovascular risk in 300 practices covering over an estimated 900,000 adult patients would likely lead to prevention of thousands of cardiovascular events within 10 years.     Cellular Zn depletion by metal ion chelators (TPEN, DTPA and chelex resin) and its application to osteoblastic MC3T3-E1 cells

    Trace mineral studies involving metal ion chelators have been conducted in investigating the response of gene and protein expressions of certain cell lines but a few had really focused on how these metal ion chelators could affect the availability of important trace minerals such as Zn, Mn, Fe and Cu. The aim of the present study was to investigate the availability of Zn for the treatment of MC3T3-E1 osteoblast-like cells and the availability of some trace minerals in the cell culture media components after using chelexing resin in the FBS and the addition of N,N,N',N'-tetrakis-(2-pyridylmethyl)ethylenediamine (TPEN, membrane-permeable chelator) and diethylenetriaminepentaacetic acid (DTPA, membrane-impermeable chelator) in the treatment medium. Components for the preparation of cell culture medium and Zn-treated medium have been tested for Zn, Mn, Fe and Cu contents by atomic absorption spectrophotometer or inductively coupled plasma spectrophotometer. Also, the expression of bone-related genes (ALP, Runx2, PTH-R, ProCOL I, OPN and OC) was measured on the cellular Zn depletion such as chelexing or TPEN treatment. Results have shown that using the chelexing resin in FBS would significantly decrease the available Zn (p<0.05) (39.4 ± 1.5 ”M vs 0.61 ± 10.15 ”M) and Mn (p<0.05) (0.74 ± 0.01 ”M vs 0.12 ± 0.04 ”M). However, levels of Fe and Cu in FBS were not changed by chelexing FBS. The use of TPEN and DTPA as Zn-chelators did not show significant difference on the final concentration of Zn in the treatment medium (0, 3, 6, 9, 12 ”M) except for in the addition of higher 15 ”M ZnCl2 which showed a significant increase of Zn level in DTPA-chelated treatment medium. Results have shown that both chelators gave the same pattern for the expression of the five bone-related genes between Zn- and Zn+, and TPEN-treated experiments, compared to chelex-treated experiment, showed lower bone-related gene expression, which may imply that TPEN would be a stronger chelator than chelex resin. This study showed that TPEN would be a stronger chelator compared to DTPA or chelex resin and TPEN and chelex resin exerted cellular zinc depletion to be enough for cell study for Zn depletion

    Incidental detection of an occult oral malignancy with autofluorescence imaging: a case report

    BACKGROUND: Autofluorescence imaging is used widely for diagnostic evaluation of various epithelial malignancies. Cancerous lesions display loss of autofluorescence due to malignant changes in epithelium and subepithelial stroma. Carcinoma of unknown primary site presents with lymph node or distant metastasis, for which the site of primary tumour is not detectable. We describe here the use of autofluorescence imaging for detecting a clinically innocuous appearing occult malignancy of the palate which upon pathological examination was consistent with a metastatic squamous cell carcinoma. CASE DESCRIPTION: A submucosal nodule was noted on the right posterior hard palate of a 59-year-old white female during clinical examination. Examination of this lesion using a multispectral oral cancer screening device revealed loss of autofluorescence at 405 nm illumination. An excisional biopsy of this nodule, confirmed the presence of a metastatic squamous cell carcinoma. Four years ago, this patient was diagnosed with metastatic squamous cell carcinoma of the right mid-jugular lymph node of unknown primary. She was treated with external beam irradiation and remained disease free until current presentation. CONCLUSION: This case illustrates the important role played by autofluorescence tissue imaging in diagnosing a metastatic palatal tumour that appeared clinically innocuous and otherwise would not have been biopsied
