    Introduction: Complicity and Dissent, or Why We Need Solidarity between Struggles

    Growing pressure from politicians and corporations has thrown into question the very legitimacy of opposition and critique. A language of political affirmation has confused and misled the public, driving many to adopt a cynical attitude to politics. The result has been a rapid decline of legitimate critique, the rise of populism, and a growing tendency to squelch civil disobedience with a militarized police force. The introduction to the special issue considers the role of the complicit/dissenting intellectual in history and literature, politics and law. It explores the genealogy of the terms, as well as conditions for their appearance in our contemporary world. The introduction follows the advice of leading scholars, who contributed to this issue, in calling for “solidarity between struggles”— when one extends a sense of right and wrong beyond one’s immediate identity or a vague universal understanding of “right.

    Improving the Capability of Preliminary Aircraft Design Methodology by Incorporating Numerical Methods into Segments of a Proven Statistical Method

    The purpose of this study was to identify any potential limitations of Roskam’s statistical aircraft flight performance estimation method, and to provide processes capable of improving those limitations. This project was conducted in collaboration with Diamond Aircraft Industries, and as any other general aviation manufacturer with limited time, manpower, and financial resources, Diamond wants the new process to be time and cost-effective. Presented in this study is a computational alternative that combines the speed and accuracy of low-order panel methods with the fully-coupled viscous/inviscid interaction method from the ISES code that was developed by Drela and Giles. Its benefit is the ability to optimize aerodynamic parameters that are ignored by statistical methods, such as multiple taper ratios, aerodynamic and geometric twist. Most notably, cost of this method is dramatically reduced by separating the computational domain into a separate one for lifting surfaces and one for fuselages

    Secret Signals from Another World: Walter Benjamin’s Concept of Innervation

    Walter Benjamin refers to the “idea of revolution as an innervation of the technical organs of the collective” as one of the articles of his politics. The significance of this assertion has received relatively little attention in the philosophical reception of his political thought compared to the alternative model of revolution – made famous from the paralipomena to the late theses ‘On the Concept of History’ – as the emergency handbrake of history. Drawing on some of the debates and tensions generated by the work of Miriam Bratu Hansen, this discussion aims at an exegesis of some of the lesser known intellectual sources that influenced Benjamin’s theory of innervation. The purpose in doing so is not an attempt to reconcile or integrate these sources with dominant philosophical reconstructions of what is sometimes characterized as Benjamin’s “Western Marxism” and elaborated, in the more familiar context of Surrealist innervation, as a synthesis of Freud and Marx, but rather to reveal an alternative constellation of Soviet biomechanics and reactionary anti-capitalist Lebensphilosophie, united in their shared rejection of Freudian psychoanalysis

    Contemporary medical therapy for polycystic ovary syndrome

    Polycystic ovary syndrome is a multi‐system endocrinopathy with long‐term metabolic and cardiovascular health consequences. Patients typically present due to symptoms of irregular menstruation, hair growth, or infertility; however, recent management options are aimed at further treating underlying glucose–insulin abnormalities as well as androgen excess for proactive control of symptoms. By a 2003 international consensus conference, diagnosis is made by two out of three criteria: chronic oligoovulation or anovulation after excluding secondary causes, clinical or biochemical evidence of hyperandrogenism (but not necessarily hirsutism due to inter‐patient variability in hair follicle sensitivity), and radiological evidence of polycystic ovaries. Traditional medical treatment options include oral contraceptive pills, cyclic progestins, ovulation induction, and anti‐androgenic medications (aldosterone antagonist, 5α‐reductase antagonist, and follicle ornithine decarboxylase inhibitor). Recent pharmacotherapies include insulin‐sensitizing medications metformin and two thiazolidinediones (rosiglitazone/Avandia® and pioglitazone/Actos®), a CYP19 aromatase inhibitor (letrozole/Femara®), and statins to potentially lower testosterone levels.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/135338/1/ijgo236.pd

Un acontecimiento social que dio lugar a procesos creativos y patológicos. La virtualidad como herramienta tecnológica ha transformado las formas del lazo social y de la subjetividad, pero también ha permitido sostener una continuidad en los dispositivos de salud y de educación. En el caso de los tratamientos se modificaron los encuadres dando lugar a nuevas formas de atención, con sus particularidades en el trabajo clínico con adolescentes.Cet article aborde les conséquences psychologiques chez les adolescents de l’isolement social obligatoire pendant la pandémie de Covid19. Un événement social qui a donné lieu à des processus créatifs et pathologiques. Le virtualité comme outil technologique a transformé les formes de lien social et de subjectivité, mais elle a aussi permis de pérenniser les dispositifs de santé et d’éducation. Dans le cas des traitements, les cadres ont été modifiés, donnant lieu à de nouvelles formes de soins, avec leurs particularités dans le travail clinique avec les adolescents.This article is about the psychological consequences in adolescents of mandatory social isolation during the Covid19 pandemic. A social event that gave rise to creative and pathological processes.     The work begins by questioning the use of classification manuals as ways of diagnosing childhood today and the concern about the number of overdiagnosed children within the autistic spectrum as if it were a social epidemic. It is about recovering the value of each child's singular through play, drawing and listening in transfer, fundamental tools for access to children's singularity, and an ethical position within the Freudian clinic that makes the symptom a message to decipher or a word to build. In times where there is no time for childhood or for diagnostic processes, it is essential that a clinic that instead of coagulating children in diagnostic lockers, bet on the deployment of subjectivity through subjective interventions. Key words: diagnostics, map, territory, game, dreams, transfer, children.El trabajo se inicia cuestionando el uso de los manuales de clasificación como modos de diagnosticar la infancia hoy y la preocupación por la cantidad de niños sobre diagnosticados dentro del espectro autista como si fuera una epidemia social.  Se trata de recuperar el valor de lo singular de cada niño a través del juego, el dibujo y la escucha en transferencia, herramientas fundamentales de acceso a la singularidad infantil, y una posición ética dentro de la clínica freudiana que hace del síntoma un mensaje a descifrar o una palabra a construir. En tiempos donde no hay tiempo para la infancia ni para los procesos diagnósticos es fundamental una clínica que en lugar de coagular a los niños en casilleros diagnósticos, apueste al despliegue de la subjetividad a través de intervenciones subjetivantes. Palabras clave: diagnósticos, mapa, territorio, juego, sueño, transferencia, inclusión, niños.Le travail commence par une mise en question de l'utilisation des manuels de classification comme moyen de diagnostiquer l'enfance aujourd'hui et de l'inquiétude suscitée par le nombre d'enfants surdiagnostiqués dans le spectre autistique, comme s'il s'agissait d'une épidémie sociale. Il s'agit de récupérer la valeur du singulier de chaque enfant par le jeu, le dessin et l'écoute dans le transfert, des outils fondamentaux pour accéder à la singularité des enfants et une position éthique au sein de la clinique freudienne qui fait du symptôme un message déchiffrer ou un mot à construire. À une époque où l’enfance ou les processus de diagnostic n’ont pas le temps, il est essentiel pour une clinique que, au lieu de coaguler les enfants dans des casiers à diagnostic, parient sur le déploiement de la subjectivité au moyen d’interventions subjectives.     Human Rights Shaming Through INGOs and Foreign Aid Delivery

    Does the ``shaming" of human rights violations influence foreign aid delivery decisions across OECD donor countries? We examine the effect of shaming, defined as targeted negative attention by human rights international nongovernmental organizations (INGOs), on donor decisions about how to deliver bilateral aid. We argue that INGO shaming of recipient countries leads donor governments, on average, to ``bypass" the recipient government in favor of non-state aid delivery channels, including international and local NGOs and international organizations (IOs). However, we expect this relationship to be conditional on a donor country's position in the international system. Minor power countries have limited influence in global affairs and are therefore more able to centrally promote human rights in their foreign policy. Major power countries, on the other hand, shape world politics and often confront ``realpolitik" concerns that may require government-to-government aid relations in the presence of INGO shaming. We expect aid officials of minor donor countries to be more likely to condition aid delivery decisions on human rights shaming than their counterparts of major donor countries. Using compositional data analysis, we test our argument using originally collected data on human rights shaming events in a time-series cross-sectional framework from 2004 to 2010. We find support for our hypotheses: On average, OECD donor governments increase the proportion of bypass when INGOs shame the recipient government. When differentiating between donor types we find that this finding holds for minor but not for major powers. These results add to both our understanding of the influences of aid allocation decision-making and our understanding of the role of INGOs on foreign-policy

    Endometrioma Complicated by Tubo-Ovarian Abscess in a Woman With Bacterial Vaginosis

    Background. Tubo-ovarian abscess involvement of an endometrioma has been reported in cases of patients with polymicrobial sources such as Neisseria gonorrhoeae, Chlamydia trachomatis, and obligate anaerobic bacteria; however, bacterial vaginosis (BV) predisposing to abscess formation in an endometrioma has not been reported to date. Case. Superinfection of an endometrioma was surgically diagnosed in a patient with known advanced-stage endometriosis after she presented with acute pelvic inflammatory disease symptoms and was unresponsive to antibiotic therapy. Gram-negative rods were cultured from the endometrioma. On admission, cervical, blood, and urine cultures were negative; BV was diagnosed on normal saline wet prep and gram stain. Conclusion. This case raises the possibility of BV ascension to the upper genital tract predisposing to abscess formation in endometriomas. Therefore, aggressive treatment of BV in patients with known advanced-stage endometriosis may be considered to prevent superinfected endometriomas

    Augmenting Autologous Stem Cell Transplantation to Improve Outcomes in Myeloma

    Consolidation with high-dose chemotherapy and autologous stem cell transplantation (ASCT) is the standard of care for transplantation-eligible patients with multiple myeloma, based on randomized trials showing improved progression-free survival with autologous transplantation after combination chemotherapy induction. These trials were performed before novel agents were introduced; subsequently, combinations of immunomodulatory drugs and proteasome inhibitors as induction therapy have significantly improved rates and depth of response. Ongoing randomized trials are testing whether conventional autologous transplantation continues to improve responses after novel agent induction. Although these results are awaited, it is important to review strategies for improving outcomes after ASCT. Conditioning before ASCT with higher doses of melphalan and combinations of melphalan with other agents, including radiopharmaceuticals, has been explored. Tandem ASCT, consolidation, and maintenance therapy after ASCT have been investigated in phase III trials. Experimental cellular therapies using ex vivo–primed dendritic cells, ex vivo–expanded autologous lymphocytes, Killer Immunoglobulin Receptor (KIR)-mismatched allogeneic natural killer cells, and genetically modified T cells to augment ASCT are also in phase I trials. This review summarizes these strategies and highlights the importance of exploring strategies to augment ASCT, even in the era of novel agent induction
