Rigorous Impact Evaluation: Evidence generation and take-up in German Development Cooperation
This report presents the findings of a BMZ-funded research project on rigorous impact evaluation (RIE) in German development cooperation (GDC). The research project examined the status quo of RIE, relevant barriers and potential measures to increase both the initiation of RIEs and the take-up of (rigorous) evidence
How can the international clinical guidelines for knee osteoarthritis management be implemented systematically in Switzerland?
Key Messages
The challenges: International clinical guidelines for the conservative management of knee osteoarthritis (OA) recommend exercise, education, and weight control (if appropriate) as first-line treatment for all people with knee OA. However, findings from various health care settings have identified an underuse of exercise and education. A recent survey among medical specialists in Switzerland identified an evidence-performance gap, implying that the strong evidence in favour of exercise is not being transferred into clinical practice. On average, only 54% of patients with knee OA were reported to have been referred for specific exercise. The systematic application of structured exercise and education to translate the guideline recommendations into practice would help to overcome this evidence-performance gap in Switzerland.
This policy brief summarises the main results from the above-mentioned survey, describes the challenges facing the improvement of conservative disease management (i.e. the systematic application of structured exercise and education) and presents options to address the challenges resulting in three key recommendations.
Recommendations: The following describes the three key recommendations for the implementation of the clinical
guidelineâs recommendation of âexercise and education as first-line treatmentâ into practice in Switzerland: Translation of guideline recommendations into a specific exercise and education programme Implementing a best-practice exercise and education programme in Switzerland could be a solution to translate the guideline recommendations into practice. There are existing structured exercise and education programmes for knee OA that are already successfully established throughout the world.
Facilitation of the systematic application of exercise through standardised treatment pathways
The education of health care professionals is important to increase their understanding of the need, the effectiveness and long-term benefits of exercise, as well as the advantages of a structured treatment pathway. This would facilitate the systematic application of exercise and education through standardised treatment pathways. A standardised treatment pathway could could also facilitate an improvement in interprofessional work collaboration.
Promotion of the benefits of exercise in the long-term management of chronic diseases: To achieve a paradigm shift from the current situation of simply treating acute symptoms to structured exercise and education and self-management, it is essential to promote the benefits of exercise. The Swiss population should be informed and made aware of the fact that physical inactivity is a risk factor for knee OA related symptoms and that exercise is a beneficial intervention to prevent or improve these symptoms.
Implementation considerations: Potential windows of opportunity:
âą There is high-quality evidence in favour of exercise reducing pain and improving function in people with knee OA. A Cochrane review from 2015 states that further research in this area is unlikely to change the evidence that exercise is beneficial for knee pain and physical function.1
âą Financial pressure for cost-effective health care management.
âą Increased awareness of health and cost benefits of exercise for non-comunicable diseases, e.g. knee OA.
âą Existing exercise and education programmes for people with knee OA established throughout the world
Analysing the Direction of Emotional Influence in Nonverbal Dyadic Communication: A Facial-Expression Study
Identifying the direction of emotional influence in a dyadic dialogue is of
increasing interest in the psychological sciences with applications in
psychotherapy, analysis of political interactions, or interpersonal conflict
behavior. Facial expressions are widely described as being automatic and thus
hard to overtly influence. As such, they are a perfect measure for a better
understanding of unintentional behavior cues about social-emotional cognitive
processes. With this view, this study is concerned with the analysis of the
direction of emotional influence in dyadic dialogue based on facial expressions
only. We exploit computer vision capabilities along with causal inference
theory for quantitative verification of hypotheses on the direction of
emotional influence, i.e., causal effect relationships, in dyadic dialogues. We
address two main issues. First, in a dyadic dialogue, emotional influence
occurs over transient time intervals and with intensity and direction that are
variant over time. To this end, we propose a relevant interval selection
approach that we use prior to causal inference to identify those transient
intervals where causal inference should be applied. Second, we propose to use
fine-grained facial expressions that are present when strong distinct facial
emotions are not visible. To specify the direction of influence, we apply the
concept of Granger causality to the time series of facial expressions over
selected relevant intervals. We tested our approach on newly, experimentally
obtained data. Based on the quantitative verification of hypotheses on the
direction of emotional influence, we were able to show that the proposed
approach is most promising to reveal the causal effect pattern in various
instructed interaction conditions.Comment: arXiv admin note: text overlap with arXiv:1810.1217
Gene therapy of Csf2ra deficiency in mouse fetal monocyte precursors restores alveolar macrophage development and function
Tissue-resident macrophage-based immune therapies have been proposed for various diseases. However, generation of sufficient numbers that possess tissue-specific functions remains a major handicap. Here, we showed that fetal liver monocytes cultured with GM-CSF (CSF2-cFLiMo) rapidly differentiated into a long-lived, homogeneous alveolar macrophage-like population in vitro. CSF2-cFLiMo retained the capacity to develop into bona fide alveolar macrophages upon transfer into Csf2ra-/- neonates and prevented development of alveolar proteinosis and accumulation of apoptotic cells for at least 1 year in vivo. CSF2-cFLiMo more efficiently engrafted empty alveolar macrophage niches in the lung and protected mice from severe pathology induced by respiratory viral infection compared with transplantation of macrophages derived from BM cells cultured with M-CSF (CSF1-cBMM) in the presence or absence of GM-CSF. Harnessing the potential of this approach for gene therapy, we restored a disrupted Csf2ra gene in fetal liver monocytes and demonstrated their capacity to develop into alveolar macrophages in vivo. Altogether, we provide a platform for generation of immature alveolar macrophage-like precursors amenable for genetic manipulation, which will be useful to dissect alveolar macrophage development and function and for pulmonary transplantation therapy
