    Late Holocene landscape change history related to the Alpine Fault determined from drowned forests in Lake Poerua, Westland, New Zealand

    Abstract. Lake Poerua is a small, shallow lake that abuts the scarp of the Alpine Fault on the West Coast of New Zealand's South Island. Radiocarbon dates from drowned podocarp trees on the lake floor, a sediment core from a rangefront alluvial fan, and living tree ring ages have been used to deduce the late Holocene history of the lake. Remnant drowned stumps of kahikatea (Dacrycarpus dacrydioides) at 1.7–1.9 m water depth yield a preferred time-of-death age at 1766–1807 AD, while a dryland podocarp and kahikatea stumps at 2.4–2.6 m yield preferred time-of-death ages of ca. 1459–1626 AD. These age ranges are matched to, but offset from, the timings of Alpine Fault rupture events at ca. 1717 AD, and either ca. 1615 or 1430 AD. Alluvial fan detritus dated from a core into the toe of a rangefront alluvial fan, at an equivalent depth to the maximum depth of the modern lake (6.7 m), yields a calibrated age of AD 1223–1413. This age is similar to the timing of an earlier Alpine Fault rupture event at ca. 1230 AD ± 50 yr. Kahikatea trees growing on rangefront fans give ages of up to 270 yr, which is consistent with alluvial fan aggradation following the 1717 AD earthquake. The elevation levels of the lake and fan imply a causal and chronological link between lake-level rise and Alpine Fault rupture. The results of this study suggest that the growth of large, coalescing alluvial fans (Dry and Evans Creek fans) originating from landslides within the rangefront of the Alpine Fault and the rise in the level of Lake Poerua may occur within a decade or so of large Alpine Fault earthquakes that rupture adjacent to this area. These rises have in turn drowned lowland forests that fringed the lake. Radiocarbon chronologies built using OxCal show that a series of massive landscape changes beginning with fault rupture, followed by landsliding, fan sedimentation and lake expansion. However, drowned Kahikatea trees may be poor candidates for intimately dating these events, as they may be able to tolerate water for several decades after metre-scale lake level rises have occurred

    Implications of subcutaneous or intravenous delivery of trastuzumab: further insight from patient interviews in the PrefHer study

    BACKGROUND: The 2 Cohort randomised PrefHer trial examined the preferences of HER2+ve primary breast cancer patients for intravenous (IV) or subcutaneous (SC) delivery of trastuzumab via a Single Injectable Device (SID) or hand-held syringe (HHS). The novel approach and design of the study permitted an in-depth exploration of patients' experiences, the impact that different modes of delivery had on patients' well-being and implications for future management. METHODS: The preferences, experiences and general comments of patients in the PrefHer study were collected via specific semi-structured interview schedules. Exploratory analyses of data were conducted using standard methodology. The final question invited patients to make further comments, which were divided into 9 thematic categories - future delivery, compliments, time/convenience, practical considerations, pain/discomfort, study design, side-effects, psychological impact, and perceived efficacy. RESULTS: 267/467 (57%) patients made 396 additional comments, 7 were neutral, 305 positive and 86 negative. The three top categories generating the largest number of comments were compliments and gratitude about staff and being part of PrefHer (75/396; 19%), the potential future delivery of SC trastuzumab (73/396; 18%), and practical considerations about SC administration (60/396; 15%). CONCLUSIONS: Eliciting patient preferences about routes of administration of drugs via comprehensive interviews within a randomised cross-over trial yielded rich and important information. The few negative comments made demonstrated a need for proper staff training in SC administration Patients were grateful to have been part of the trial, and would have liked to continue with SC delivery. The possibility of home administration in the future also seemed acceptable. EUDRACT NUMBER: 2010-024099-25

    A systematic review and meta-analysis on the effect of neoadjuvant chemotherapy on complications following immediate breast reconstruction

    Background The impact of neoadjuvant chemotherapy (NACT) on surgical outcomes following immediate breast reconstruction (IBR) remains unclear. While it is generally considered safe practice to perform an IBR post NACT, reported complication rates in published data are highly variable with the majority of studies including fewer than 50 patients in the NACT and IBR arm. To evaluate this further, we conducted a systematic review and meta-analysis on the effect of NACT on autologous and implant based immediate breast reconstructions. We aimed to assess for differences in the post-operative course following IBR between patients who received NACT with those who did not. Methods PubMed, EMBASE, and Cochrane Library were searched from 1995 to Sept 2, 2020 to identify articles that assessed the impact of NACT on IBR. All included studies assessed outcomes following IBR. Only studies comparing reconstructed patients receiving NACT to a control group of women who did not receive NACT were included. Unadjusted relative risk of outcomes between patients who received or did not receive NACT were synthesized using a fixed-effect meta-analysis. The evidence was assessed using the Newcastle Ottawa Scale scores and GRADE. Primary effect measures were risk ratios (RRs) with 95% confidence intervals. Results A total 17 studies comprising 3249 patients were included in the meta-analyses. Overall, NACT did not increase the risk of complications after immediate breast reconstructions (risk ratio [RR]: 0.91, 95% CI 0.74 to 1.11, p = 0.34). There was a moderate, but not significant, increase in flap loss following NACT compared with controls (RR: 1.23, 95% CI 0.70 to 2.18, p = 0.47; I2 = 0%). Most notably, there was a statistically significant increase in implant/expander loss after NACT (RR: 1.54, 95% CI 1.04 to 2.29, p = 0.03; I2 = 34%). NACT was not shown to significantly increase the incidence of hematomas, seromas or wound complications, or result in a significant delay to commencing adjuvant therapy (RR: 1.59, 95% CI 0.66 to 3.87, p = 0.30). Conclusion Immediate breast reconstruction after NACT is a safe procedure with an acceptable post-operative complication profile. It may result in a slight increase in implant loss rates, but it does not delay commencing adjuvant therapy

    The effect of ovarian cancer screening on sexual activity and functioning: results from the UK collaborative trial of ovarian cancer screening RCT

    Background: To examine the impact of multimodal (MMS) and ultrasound (USS) screening on the sexual activity and functioning of 22,966 women in the UK Collaborative Trial of Ovarian Cancer Screening (UKCTOCS) RCT. Methods: Fallowfield’s Sexual Activity Questionnaire (FSAQ) was completed prior to randomisation, then annually in a random sample (RS) of women from MMS, USS and control groups. Any women in the study who required repeat screening due to unsatisfactory results formed an Events Sample (ES); they completed questionnaires following an event and annually thereafter. Results: Over time in the RS (n=1,339) there was no difference between the MMS and USS groups in sexual activity compared with controls. In the ES there were significant differences between the USS group (n=10,156) and the MMS group (n=12,810). The USS group had lower pleasure scores (mean difference = -0.14, P=0.046). For both groups women who had ≥2 repeat screens, showed a decrease in mean pleasure scores compared to their annual scores (mean difference = -0.16, P=0.005). Similarly mean pleasure scores decreased following more intensive screens compared to annual screening (mean difference= -0.09, P=0.046). Conclusion: Ovarian cancer screening did not affect sexual activity and functioning unless a woman had abnormal results and underwent repeated or higher level screening

    On thermalization of magnetic nano-arrays at fabrication

    We propose a model to predict and control the statistical ensemble of magnetic degrees of freedom in Artificial Spin Ice (ASI) during thermalized adiabatic growth. We predict that as-grown arrays are controlled by the temperature at fabrication and by their lattice constant, and that they can be described by an effective temperature. If the geometry is conducive to a phase transition, then the lowest temperature phase is accessed in arrays of lattice constant smaller than a critical value, which depends on the temperature at deposition. Alternatively, for arrays of equal lattice constant, there is a temperature threshold at deposition and the lowest temperature phase is accessed for fabrication temperatures {\it larger rather than smaller} than this temperature threshold. Finally we show how to define and control the effective temperature of the as-grown array and how to measure critical exponents directly. We discuss the role of kinetics at the critical point, and applications to experiments, in particular to as-grown thermalized square ASI, and to magnetic monopole crystallization in as-grown honeycomb ASI.Comment: 14 pages, 2 figures.
