    Multiscale entropy analysis of heart rate variability in neonatal patients with and without seizures

    The complex physiological dynamics of neonatal seizures make their detection challenging. A timely diagnosis and treatment, especially in intensive care units, are essential for a better prognosis and the mitigation of possible adverse effects on the newborn’s neurodevelopment. In the literature, several electroencephalographic (EEG) studies have been proposed for a parametric characterization of seizures or their detection by artificial intelligence techniques. At the same time, other sources than EEG, such as electrocardiography, have been investigated to evaluate the possible impact of neonatal seizures on the cardio-regulatory system. Heart rate variability (HRV) analysis is attracting great interest as a valuable tool in newborns applications, especially where EEG technologies are not easily available. This study investigated whether multiscale HRV entropy indexes could detect abnormal heart rate dynamics in newborns with seizures, especially during ictal events. Furthermore, entropy measures were analyzed to discriminate between newborns with seizures and seizure-free ones. A cohort of 52 patients (33 with seizures) from the Helsinki University Hospital public dataset has been evaluated. Multiscale sample and fuzzy entropy showed significant differences between the two groups (p-value < 0.05, Bonferroni multiple-comparison post hoc correction). Moreover, interictal activity showed significant differences between seizure and seizure-free patients (Mann-Whitney Test: p-value < 0.05). Therefore, our findings suggest that HRV multiscale entropy analysis could be a valuable pre-screening tool for the timely detection of seizure events in newborns

    Defining design parameters and criteria for the avoidance of moisture-related problems within current Chilean buildings

    A preliminary assessment of the new Moisture Assessment Tool (MAT) developed by the Chilean Ministry of Housing and Urbanism (MINVU) was carried out for 36 Provinces along the 9 thermal zones defined for the Chilean territory. Calculations were conducted in accordance with ISO 13788 and the Chilean standard NCh 1973, complemented with the ADF moisture criteria and benchmarked with Passivhaus hygiene requirements. The fRsi values estimated by the MAT methods were successfully determined and found to agree with Passivhaus criteria for almost every thermal zone. This research provides theoretical insight to adjust climate-dependent design parameters and criteria to improve the methods and calculations for the avoidance of condensation and mould growth

    Automatic Detection of Epileptic Seizures in Neonatal Intensive Care Units through EEG, ECG and Video Recordings: A Survey

    In Neonatal Intensive Care Units (NICUs), the early detection of neonatal seizures is of utmost importance for a timely, effective and efficient clinical intervention. The continuous video electroencephalogram (v-EEG) is the gold standard for monitoring neonatal seizures, but it requires specialized equipment and expert staff available 24/24h. The purpose of this study is to present an overview of the main Neonatal Seizure Detection (NSD) systems developed during the last ten years that implement Artificial Intelligence techniques to detect and report the temporal occurrence of neonatal seizures. Expert systems based on the analysis of EEG, ECG and video recordings are investigated, and their usefulness as support tools for the medical staff in detecting and diagnosing neonatal seizures in NICUs is evaluated. EEG-based NSD systems show better performance than systems based on other signals. Recently ECG analysis, particularly the related HRV analysis, seems to be a promising marker of brain damage. Moreover, video analysis could be helpful to identify inconspicuous but pathological movements. This study highlights possible future developments of the NSD systems: a multimodal approach that exploits and combines the results of the EEG, ECG and video approaches and a system able to automatically characterize etiologies might provide additional support to clinicians in seizures diagnosis

    Quality evaluation of gluten-free biscuits prepared with algarrobo flour as a partial sugar replacer

    Algarrobo flour is a gluten-free flour obtained by grinding the whole fruit (pods) of Prosopis chilensis. Because of its taste, this flour could be used as a sugar replacer. Besides, it can improve the protein profile, and the antioxidant and fibre content of foodstuffs. This is of special interest in gluten-free products that are generally deficient in these nutrients. In the present work a total of eleven different gluten-free biscuit formulations with Algarrobo flour and different proportions of rice flour, chickpea flour, cassava starch, and maize starch were evaluated. The effects of each ingredient were analysed and the texture, colour, total dietary fibre content, antioxidant capacity, and sensorial acceptability associated with the addition of algarrobo flour as a partial sugar replacer were studied in detail in three selected formulations. Results showed that the dietary fibre content and the antioxidant capacity increased with the addition of algarrobo flour (60 and 20%, respectively). In addition, no significant differences were found in the sensory scores of biscuits prepared with sugar or with algarrobo flour (P ≥ 0.05). It could be concluded that algarrobo flour could partially replace sugar in gluten-free biscuit formulations. Besides, its addition increases the fibre and antioxidant capacity of the product without significantly changing its optimum texture.     Improving household air, drinking water and hygiene in rural Peru : a community-randomized-controlled trial of an integrated environmental home-based intervention package to improve child health

    Diarrhoea and acute lower respiratory infections are leading causes of childhood morbidity and mortality, which can be prevented by simple low-cost interventions. Integrated strategies can provide additional benefits by addressing multiple health burdens simultaneously.; We conducted a community-randomized-controlled trial in 51 rural communities in Peru to evaluate whether an environmental home-based intervention package, consisting of improved solid-fuel stoves, kitchen sinks, solar disinfection of drinking water and hygiene promotion, reduces lower respiratory infections, diarrhoeal disease and improves growth in children younger than 36 months. The attention control group received an early child stimulation programme.; We recorded 24 647 child-days of observation from 250 households in the intervention and 253 in the attention control group during 12-month follow-up. Mean diarrhoea incidence was 2.8 episodes per child-year in the intervention compared with 3.1 episodes in the control arm. This corresponds to a relative rate of 0.78 [95% confidence interval (CI): 0.58-1.05] for diarrhoea incidence and an odds ratio of 0.71 (95% CI: 0.47-1.06) for diarrhoea prevalence. No effects on acute lower respiratory infections or children's growth rates were observed.; Combined home-based environmental interventions slightly reduced childhood diarrhoea, but the confidence interval included unity. Effects on growth and respiratory outcomes were not observed, despite high user compliance of the interventions. The absent effect on respiratory health might be due to insufficient household air quality improvements of the improved stoves and additional time needed to achieve attitudinal and behaviour change when providing composite interventions

    Genetic admixture patterns in argentinian patagonia

    As in other Latin American populations, Argentinians are the result of the admixture amongst different continental groups, mainly from America and Europe, and to a lesser extent from Sub-Saharan Africa. However, it is known that the admixture processes did not occur homogeneously throughout the country. Therefore, considering the importance for anthropological, medical and forensic researches, this study aimed to investigate the population genetic structure of the Argentinian Patagonia, through the analysis of 46 ancestry informative markers, in 433 individuals from five different localities. Overall, in the Patagonian sample, the average individual ancestry was estimated as 35.8% Native American (95% CI: 32.2–39.4%), 62.1% European (58.5–65.7%) and 2.1% African (1.7–2.4%). Comparing the five localities studied, statistically significant differences were observed for the Native American and European contributions, but not for the African ancestry. The admixture results combined with the genealogical information revealed intra-regional variations that are consistent with the different geographic origin of the participants and their ancestors. As expected, a high European ancestry was observed for donors with four grandparents born in Europe (96.8%) or in the Central region of Argentina (85%). In contrast, the Native American ancestry increased when the four grandparents were born in the North (71%) or in the South (61.9%) regions of the country, or even in Chile (60.5%). In summary, our results showed that differences on continental ancestry contribution have different origins in each region in Patagonia, and even in each locality, highlighting the importance of knowing the origin of the participants and their ancestors for the correct interpretation and contextualization of the genetic information.Finantial support was granted by Agencia Nacional de Promoción Científica y Tecnológica, Argentina (ANPCyT; https://www.argentina.gob.ar/ ciencia/agencia; grants ref.     Could a Shigella vaccine impact long-term health outcomes?: Summary report of an expert meeting to inform a Shigella vaccine public health value proposition, March 24 and 29, 2021.

    Shigellosis is a leading cause of diarrhea and dysentery in young children from low to middle-income countries and adults experiencing traveler's diarrhea worldwide. In addition to acute illness, infection by Shigella bacteria is associated with stunted growth among children, which has been linked to detrimental long-term health, developmental, and economic outcomes. On March 24 and 29, 2021, PATH convened an expert panel to discuss the potential impact of Shigella vaccines on these long-term outcomes. Based on current empirical evidence, this discussion focused on whether Shigella vaccines could potentially alleviate the long-term burden associated with Shigella infections. Also, the experts provided recommendations about how to best model the burden, health and vaccine impact, and economic consequences of Shigella infections. This international multidisciplinary panel included 13 scientists, physicians, and economists from multiple relevant specialties. According to the panel, while the relationship between Shigella infections and childhood growth deficits is complex, this relationship likely exists. Vaccine probe studies are the crucial next step to determine whether vaccination could ameliorate Shigella infection-related long-term impacts. Infants should be vaccinated during their first year of life to maximize their protection from severe acute health outcomes and ideally reduce stunting risk and subsequent negative long-term developmental and health impacts. With vaccine schedule crowding, targeted or combination vaccination approaches would likely increase vaccine uptake in high-burden areas. Shigella impact and economic assessment models should include a wider range of linear growth outcomes. Also, these models should produce a spectrum of results-ones addressing immediate benefits for usual health care decision-makers and others that include broader health impacts, providing a more comprehensive picture of vaccination benefits. While many of the underlying mechanisms of this relationship need better characterization, the remaining gaps can be best addressed by collecting data post-vaccine introduction or through large trials

    Momentum-resolved linear dichroism in bilayer MoS2

    In solid state photoemission experiments it is possible to extract information about the symmetry and orbital character of the electronic wave functions via the photoemission selection rules that shape the measured intensity. This approach can be expanded in a pump-probe experiment where the intensity contains additional information about interband excitations induced by an ultrafast laser pulse with tunable polarization. Here, we find an unexpected strong linear dichroism effect (up to 42.4%) in the conduction band of bilayer MoS2, when measuring energy- A nd momentum-resolved snapshots of excited electrons by time- A nd angle-resolved photoemission spectroscopy. We model the polarization-dependent photoemission intensity in the transiently populated conduction band using the semiconductor Bloch equations. Our theoretical analysis reveals a strongly anisotropic momentum dependence of the optical excitations due to intralayer single-particle hopping, which explains the observed linear dichroism

    Investment in vaccine product development should be guided by up-to-date and transparent global burden of disease estimates, which are also fundamental to policy recommendation and vaccine introduction decisions. For low- and middle-income countries (LMICs), vaccine prioritization is primarily driven by the number of deaths caused by different pathogens. Enteric diseases are known to be a major cause of death in LMICs. The two main modelling groups providing mortality estimates for enteric diseases are the Institute for Health Metrics and Evaluation (IHME) at the University of Washington, Seattle and the Maternal Child Epidemiology Estimation (MCEE) group, led by Johns Hopkins Bloomberg School of Public Health. Whilst previous global diarrhoea mortality estimates for under five-year-olds from these two groups were closely aligned, more recent estimates for 2016 have diverged, particularly with respect to numbers of deaths attributable to different enteric pathogens. This has impacted prioritization and investment decisions for vaccines in the development pipeline. The mission of the Product Development for Vaccines Advisory Committee (PDVAC) at the World Health Organisation (WHO) is to accelerate product development of vaccines and technologies that are urgently needed and ensure they are appropriately targeted for use in LMICs. At their 2018 meeting, PDVAC recommended the formation of an independent working group of subject matter experts to explore the reasons for the difference between the IHME and MCEE estimates, and to assess the respective strengths and limitations of the estimation approaches adopted, including a review of the data on which the estimates are based. Here, we report on the proceedings and recommendations from a consultation with the working group of experts, the IHME and MCEE modelling groups, and other key stakeholders. We briefly review the methodological approaches of both groups and provide a series of proposals for investigating the drivers for the differences in enteric disease burden estimates
