    Técnicas de Aprendizado de Máquina são comumente utilizados em tarefas que envolvem a identificação de padrões. O reconhecimento de entidades nomeadas consiste em identificar todos os nomes de um documento e classificá-los em categorias prévias. No presente artigo foram analisadas três técnicas com aprendizado supervisionado, sendo que, os melhores resultados foram obtidos com o algoritmo NaiveBayes. Inúmeros experimentos foram realizados usando a ferramenta Weka, no entanto, os valores obtidos por meio da estatística Kappa ficaram abaixo do esperado

    Microcirculation improvement after short-term infusion of vasopressin in septic shock is dependent on noradrenaline

    OBJECTIVES: To assess the impact of vasopressin on the microcirculation and to develop a predictive model to estimate the probability of microcirculatory recruitment in patients with septic shock. METHODS: This prospective interventional study included patients with septic shock receiving noradrenaline for less than 48 hours. We infused vasopressin at 0.04 U/min for one hour. Hemodynamic measurements, including sidestream dark-field imaging, were obtained immediately before vasopressin infusion, 1 hour after vasopressin infusion and 1 hour after vasopressin withdrawal. We defined patients with more than a 10% increase in total vascular density and perfused vascular density as responders. ClinicalTrials.gov: NCT02053675. RESULTS: Eighteen patients were included, and nine (50%) showed improved microcirculation after infusion of vasopressin. The noradrenaline dose was significantly reduced after vasopressin (p=0.001) and was higher both at baseline and during vasopressin infusion in the responders than in the non-responders. The strongest predictor for a favorable microcirculatory response was the dose of noradrenaline at baseline (OR=4.5; 95% CI: 1.2-17.0; p=0.027). For patients using a noradrenaline dose higher than 0.38 mcg/kg/min, the probability that microcirculatory perfusion would be improved with vasopressin was 53% (sensitivity 78%, specificity 77%). CONCLUSIONS: In patients with septic shock for no longer than 48 h, administration of vasopressin is likely to result in an improvement in microcirculation when the baseline noradrenaline dose is higher than 0.38 mcg/kg/min

    Introdução: considerando a atual situação pandêmica, causada pela nova cepa do coronavírus, SARS-COV-2 (severe acute respiratory syndrome-coronavirus 2), o Ministério da Saúde incluiu no grupo de risco os portadores de doenças crônicas, asmáticos, pessoas com doença pulmonar obstrutiva crônica, indivíduos fumantes, idosos, gestantes, puérperas e crianças menores de 5 anos, sendo assim, necessário dar mais atenção à saúde dessa população para minimizar os riscos e agravos. Objetivo: contribuir com o cuidado de gestantes com diagnóstico de covid-19. Metodologia: trata-se de uma revisão integrativa da literatura, que buscou identificar os principais dados de gestantes com covid-19, em publicações realizadas a partir do ano de 2020, indexadas na biblioteca virtual de saúde, seguindo os critérios de inclusão e exclusão definidos pelos autores. Resultados: foram selecionados artigos, e analisado: o desenho/amostra, objetivos, resultados, conclusões, intervenções e limitações, de 10 estudos, como a qualidade dos estudos, sendo oito qualitativos, destes, cinco receberam nota superior a 80; os trabalhos quantitativos obtiveram notas inferiores a 70. Conclusão: fica evidente a escassez de evidências científicas acerca dos mecanismos fisiopatológicos da covid-19 em gestantes; a necessidade de realização de mais estudos para melhor compreensão dos desfechos clínicos maternos, fetais e neonatais da sars-cov-2 durante a gestação e as repercussões em longo prazo para estes grupos, a fim de contribuir para tomada de decisões de manejo e terapêuticas

    Cause-specific mortality for 249 causes in Brazil and states during 1990–2015 : a systematic analysis for the global burden of disease study 2015

    Background: Reliable data on cause of death (COD) are fundamental for planning and resource allocation priorities. We used GBD 2015 estimates to examine levels and trends for the leading causes of death in Brazil from 1990 to 2015. Methods: We describe the main analytical approaches focused on both overall and specific causes of death for Brazil and Brazilian states. Results: There was an overall improvement in life expectancy at birth from 1990 to 2015, but with important heterogeneity among states. Reduced mortality due to diarrhea, lower respiratory infections, and other infectious diseases contributed the most for increasing life expectancy in most states from the North and Northeast regions. Reduced mortality due to cardiovascular diseases was the highest contributor in the South, Southeast, and Center West regions. However, among men, intentional injuries reduced life expectancy in 17 out of 27 states. Although age-standardized rates due to ischemic heart disease (IHD) and cerebrovascular disease declined over time, these remained the leading CODs in the country and states. In contrast, leading causes of premature mortality changed substantially - e.g., diarrheal diseases moved from 1st to 13th and then the 36th position in 1990, 2005, and 2015, respectively, while violence moved from 7th to 1st and to 2nd. Overall, the total age-standardized years of life lost (YLL) rate was reduced from 1990 to 2015, bringing the burden of premature deaths closer to expected rates given the country’s Socio-demographic Index (SDI). In 1990, IHD, stroke, diarrhea, neonatal preterm birth complications, road injury, and violence had ratios higher than the expected, while in 2015 only violence was higher, overall and in all states, according to the SDI. Conclusions: A widespread reduction of mortality levels occurred in Brazil from 1990 to 2015, particularly among children under 5 years old. Major shifts in mortality rates took place among communicable, maternal, neonatal, and nutritional disorders. The mortality profile has shifted to older ages with increases in non-communicable diseases as well as premature deaths due to violence. Policymakers should address health interventions accordingly

    Diabetes mellitus e intolerância à glicose são subdiagnosticados nas unidades de terapia intensiva

    OBJETIVO: Avaliar a presença de diabetes mellitus e a intolerância à glicose em pacientes internados em unidades de terapia intensiva. MÉTODOS: Foram incluídos pacientes clínicos, em pós-operatório de cirurgias eletivas e de urgência, e excluídos aqueles com história de diabetes mellitus. Para o diagnóstico de alterações prévias da glicemia, utilizou-se a dosagem da hemoglobina glicada (HbA1c) na admissão do paciente, sendo classificado em normal (<5,7%), intolerante à glicose (5,7-6,4%) ou diabético (&gt;6,4%). Durante os 3 primeiros dias da internação, foram avaliados o controle glicêmico e as complicações clínicas. A evolução para óbito foi acompanhada por 28 dias. Para as análises estatísticas, utilizaram-se testes do qui-quadrado, ANOVA, teste t de Student, Kruskall-Wallis ou Mann Whitney. RESULTADOS: Foram incluídos 30 pacientes, 53% do gênero feminino, idade de 53,4±19,7 anos e APACHE II de 13,6±6,6. A maioria dos pacientes foi admitida por sepse grave ou choque séptico, seguido por pós-operatório de cirurgias eletivas, oncológicas, politraumatismo e cirurgia de urgência. Ao classificar esses pacientes segundo a HbA1c, apesar da ausência prévia de história de diabetes mellitus, apenas 13,3% tinham HbA1c normal, 23,3% tinham níveis compatíveis com o diagnóstico de diabetes mellitus e 63,3% eram compatíveis com intolerância à glicose. Houve associação significativa entre o diagnóstico de diabetes mellitus ou intolerância a glicose e o uso de droga vasoativa (p=0,04). CONCLUSÃO: Foi encontrada alta prevalência de diabetes mellitus e intolerância à glicose, sem diagnóstico prévio, em pacientes internados em uma unidade de terapia intensiva geral
