Jonathan’s Constitutional Conference in Nigeria: A reflection and a radical critique

The process of bringing forth a constitution is as crucial and important as the constitution itself. However, while this ideal has been institutionalized in many liberal democracies, it is yet to be fully embraced in many illiberal countries. In Nigeria, the focus of this discourse, the process of constitution-making is as old as the country itself but such processes had always followed the same pattern: elite engineered, paternalistically-driven and above all, devoid of citizens’ imprints via a referendum. It is against this backdrop that this article, in a retrospective and analytical manner, examines and offers a democratic critique ofNigeria’s most recent attempt at Nigeria’s constitutional engineering, the Jonathan’s Constitutional Conference (JCC) of 2014. It observes that President Jonathan-initiated Constitutional Conference mimicked the paternalistic character of the previous attempts at constitution-making and as such the process is not markedly different from the old. It submits that as long as the state elites, acting on behalf of the hegemonic faction of the dominant class, continue to see constitution-making as their exclusive reserve and are always willing to defend even a bad constitution, the search for a people’s constitution would continue. Keywords: constitution, Nigerian state, referendum, hegemony, paternalism, Goodluck Jonatha

Introduction: Regional metastases of cutaneous head and neck squamous cell carcinoma occur in approximately 5 % of cases, being the most important prognostic factor in survival, currently with no distinction between parotid and neck metastasis. Objective: The purpose of this study was to evaluate the prognostic features among patients with head and neck cutaneous squamous cell carcinoma exhibiting regional metastasis. Methods: A retrospective analysis of patients with cutaneous squamous cell carcinoma who underwent parotidectomy and/or neck dissection from 2011 to 2018 at a single institution tertiary center was performed. Patient demographics, clinical, surgical and pathological information, adjuvant treatments, and outcome at last follow-up were collected. Outcomes included disease recurrence and death due to the disease. Prognostic value of clinic pathological features associated with disease-specific survival was obtained. Results: Thirty-eight cases of head and neck cutaneous squamous cell carcinoma with parotid and/or neck metastasis were identified. Overall, 18 (47.3 %) patients showed parotid metastasis alone, 12 (31.5 %) exhibited neck metastasis alone and 8 (21.0 %) had both. A primary tumor in the parotid zone (Hazard Ratio ‒ HR = 5.53; p = 0.02) was associated with improved disease-specific survival. Poorer disease-specific survival was observed in patients with higher primary tumor diameter (HR = 1.54; p = 0.002), higher depth of invasion (HR = 2.89; p = 0.02), invasion beyond the subcutaneous fat (HR = 5.05; p = 0.002), neck metastasis at first presentation (HR = 8.74; p < 0.001), number of positive lymph nodes (HR = 1.25; p = 0.004), and higher TNM stages (HR = 7.13; p = 0.009). Patients presenting with isolated parotid metastasis during all follow-ups had better disease-specific survival than those with neck metastasis or both (HR = 3.12; p = 0.02). Conclusion: Head and neck cutaneous squamous cell carcinoma with parotid lymph node metastasis demonstrated better outcomes than cases with neck metastasis

Factors associated with incomplete surgical margins in basal cell carcinoma of the head and neck

Introduction: Cutaneous basal cell carcinoma recurrence is associated with inadequate surgical margins. The frequency of and the factors associated with compromised or inadequate surgical margins in head and neck basal cell carcinoma varies. Objective: The purpose of this study was to evaluate the clinical and pathological factors associated with inadequate surgical margins in head and neck basal cell carcinoma. Methods: We developed a cross-sectional study comprising all patients who had undergone resection of head and neck basal cell carcinoma from January 2017 to December 2019. Data on age, sex, head and neck topography, histopathological findings, and staging were retrieved and compared. Each tumor was considered an individual case. Compromised and close margins were termed “inadequate” or “incomplete”. Variables that were significantly associated with the presence of incomplete margins were further assessed by logistic regression. Results: In total, 605 tumors from 389 patients were included. Overall, sixteen cases (2.6%) were classified as compromised, 52 (8.5%) as close, and 537 (88.7%) as free margins. Presence of scleroderma (p = 0.005), higher Clark level (p < 0.001), aggressive variants (p < 0.001), invasion beyond the adipose tissue (p < 0.001), higher T stage (p < 0.001), perineural invasion (p = 0.002), primary site (p = 0.04), multifocality (p = 0.01), and tumor diameter (p = 0.02) showed association with inadequate margins. After Logist regression, multifocality, Clark level and depth of invasion were found to be independent risk factors for inadequate margins. Conclusion: Gross clinical examination may be sufficient for determining low prevalence of inadequate surgical margins when treating head and neck basal cell carcinoma in highly experienced oncologic centers. Multifocality, Clark level and depth of invasion were found to be independent risk factors for incomplete margins

Phase Diagram of a Spin Ladder with Cyclic Four Spin Exchange

We present the phase diagram of the S=1/2S=1/2 Heisenberg model on the two leg ladder with cyclic four spin exchange, determined by a combination of Exact Diagonalization and Density Matrix Renormalization Group techniques. We find six different phases and regimes: the rung singlet phase, a ferromagnetic phase, two symmetry broken phases with staggered dimers and staggered scalar chiralities, and a gapped region with dominant vector chirality or collinear spin correlations. We localize the phase transitions and investigate their nature.Comment: 4 pages, 6 figures, REVTeX 4, published versio

Cosmic ray knee and new physics at the TeV scale

We analyze the possibility that the cosmic ray knee appears at an energy threshold where the proton-dark matter cross section becomes large due to new TeV physics. It has been shown that such interactions could break the proton and produce a diffuse gamma ray flux consistent with MILAGRO observations. We argue that this hypothesis implies knees that scale with the atomic mass for the different nuclei, as KASKADE data seem to indicate. We find that to explain the change in the spectral index in the flux from E^{-2.7} to E^{-3.1} the cross section must grow like E^{0.4+\beta} above the knee, where \beta=0.3-0.6 parametrizes the energy dependence of the age (\tau \propto E^{-\beta}) of the cosmic rays reaching the Earth. The hypothesis also requires mbarn cross sections (that could be modelled with TeV gravity) and large densities of dark matter (that could be clumped around the sources of cosmic rays). We argue that neutrinos would also exhibit a threshold at E=(m_\chi/m_p)E_{knee}\approx 10^8 GeV where their interaction with a nucleon becomes strong. Therefore, the observation at ICECUBE or ANITA of standard neutrino events above this threshold would disprove the scenario.Comment: 10 pages, version to appear in JCA

Dificuldades Enfrentadas Pelos IndĂgenas Durante A PermanĂȘncia Em Uma Casa De SaĂșde IndĂgena Na RegiĂŁo AmazĂŽnica/brasil

The National Policy of Health Care to the Indigenous Peoples (PNASI) was established by the Ministry of Health restructuring the Primary Care to Indigenous Health, following the principles and guidelines from the Unified Health System (SUS). This study aims to identify the difficulties faced by the indigenous peoples during the stay in the Indigenous Health Center (Casai) in Santarém (PA), in the Amazon region. It is an exploratory qualitative study, whose approach was through semi-structured interviews, recorded and transcribed, with 15 indigenous people from five ethnic groups, assisted by the Casai (Mawayana, Tunayana, Wai-wai, Tiriyó and Katwena), with help from a translator acquainted with the dialects. We used the content analysis arising from thematic categories: the difficulties faced during the adaptation period in the Casai/Santarém, the feeling about leaving the indigenous land and the perspectives regarding the improvements during the stay. We consider that, despite the increasing changes and advances in indigenous health in Brazil, improvements that can truly meet the health peculiarities of each ethnic group are necessary. © 2016, UNIV SAOPAULO. Lagrangian evolution of global strings

We establish a method to trace the Lagrangian evolution of extended objects consisting of a multicomponent scalar field in terms of a numerical calculation of field equations in three dimensional Eulerian meshes. We apply our method to the cosmological evolution of global strings and evaluate the energy density, peculiar velocity, Lorentz factor, formation rate of loops, and emission rate of Nambu-Goldstone (NG) bosons. We confirm the scaling behavior with a number of long strings per horizon volume smaller than the case of local strings by a factor of ∌\sim 10. The strategy and the method established here are applicable to a variety of fields in physics.Comment: 5 pages, 2 figure

Phase Diagram of the Heisenberg Spin Ladder with Ring Exchange

We investigate the phase diagram of a generalized spin-1/2 quantum antiferromagnet on a ladder with rung, leg, diagonal, and ring-exchange interactions. We consider the exactly soluble models associated with the problem, obtain the exact ground states which exist for certain parameter regimes, and apply a variety of perturbative techniques in the regime of strong ring-exchange coupling. By combining these approaches with considerations related to the discrete Z_4 symmetry of the model, we present the complete phase diagram.Comment: 17 pages, 10 figure
