    Preferences for Prenatal Tests for Cystic Fibrosis: A Discrete Choice Experiment to Compare the Views of Adult Patients, Carriers of Cystic Fibrosis and Health Professionals

    As new technologies enable the development of non-invasive prenatal diagnosis (NIPD) for cystic fibrosis (CF), research examining stakeholder views is essential for the preparation of implementation strategies. Here, we compare the views of potential service users with those of health professionals who provide counselling for prenatal tests. A questionnaire incorporating a discrete choice experiment examined preferences for key attributes of NIPD and explored views on NIPD for CF. Adult patients (n = 92) and carriers of CF (n = 50) were recruited from one children’s and one adult NHS specialist CF centre. Health professionals (n = 70) were recruited via an e-mail invitation to relevant professional bodies. The key attribute affecting service user testing preferences was no miscarriage risk, while for health professionals, accuracy and early testing were important. The uptake of NIPD by service users was predicted to be high and includes couples that would currently decline invasive testing. Many service users (47%) and health professionals (55.2%) thought the availability of NIPD for CF would increase the pressure to undergo prenatal testing. Most service users (68.5%) thought NIPD for CF should be offered to all pregnant women, whereas more health professionals (68.2%) thought NIPD should be reserved for known carrier couples.     The Near-Infrared Broad Emission Line Region of Active Galactic Nuclei -- I. The Observations

    We present high quality (high signal-to-noise ratio and moderate spectral resolution) near-infrared (near-IR) spectroscopic observations of 23 well-known broad-emission line active galactic nuclei (AGN). Additionally, we obtained simultaneous (within two months) optical spectroscopy of similar quality. The near-IR broad emission line spectrum of AGN is dominated by permitted transitions of hydrogen, helium, oxygen, and calcium, and by the rich spectrum of singly-ionized iron. In this paper we present the spectra, line identifications and measurements, and address briefly some of the important issues regarding the physics of AGN broad emission line regions. In particular, we investigate the excitation mechanism of neutral oxygen and confront for the first time theoretical predictions of the near-IR iron emission spectrum with observations.Comment: 45 pages, 17 figures, accepted by ApJ

    Exercise and progressive supranuclear palsy : the need for explicit exercise reporting

    Background Progressive Supranuclear Palsy (PSP) is the most frequent form of atypical Parkinsonism. Although there is preliminary evidence for the benefits of gait rehabilitation, balance training and oculomotor exercises in PSP, the quality of reporting of exercise therapies appears mixed. The current investigation aims to evaluate the comprehensiveness of reporting of exercise and physical activity interventions in the PSP literature. Methods Two independent reviewers used the Consensus on Exercise Reporting Template (CERT) to extract all exercise intervention data from 11 studies included in a systematic review. CERT items covered: ‘what’ (materials), ‘who’ (instructor qualifications), ‘how’ (delivery), ‘where’ (location), ‘when’, ‘how much’ (dosage), ‘tailoring’ (what, how), and ‘how well’ (fidelity) exercise delivery complied with the protocol. Each exercise item was scored ‘1’ (adequately reported) or ‘0’ (not adequately reported or unclear). The CERT score was calculated, as well as the percentage of studies that reported each CERT item. Results The CERT scores ranged from 3 to 12 out of 19. No PSP studies adequately described exercise elements that would allow exact replication of the interventions. Well-described items included exercise equipment, exercise settings, exercise therapy scheduling, frequency and duration. Poorly described items included decision rules for exercise progression, instructor qualifications, exercise adherence, motivation strategies, safety and adverse events associated with exercise therapies. Discussion The results revealed variability in the reporting of physical therapies for people living with PSP. Future exercise trials need to more comprehensively describe equipment, instructor qualifications, exercise and physical activity type, dosage, setting, individual tailoring of exercises, supervision, adherence, motivation strategies, progression decisions, safety and adverse events. Conclusion Although beneficial for people living with PSP, exercise and physical therapy interventions have been inadequately reported. It is recommended that evidence-based reporting templates be utilised to comprehensively document therapeutic exercise design, delivery and evaluation

    Diminiode thermionic conversion with 111-iridium electrodes

    Preliminary data indicating thermionic-conversion potentialities for a 111-iridium emitter and collector spaced 0.2 mm apart are presented. These results comprise output densities of current and of power as functions of voltage for three sets of emitter, collector, and reservoir temperatures: 1553, 944, 561 K; 1605, 898, 533 K; and 1656, 1028, 586 K. For the 1605 K evaluation, estimates produced work-function values of 2.22 eV for the emitter and 1.63 eV for the collector with a 2.0-eV barrier index (collector work function plus interelectrode voltage drop) corresponding to the maximum output of 5.5 W/sq cm at 0.24 volt. The current, voltage curve for the 1656 K 111-iridium diminiode yields a 6.2 W/sq cm maximum at 0.25 volt and is comparable with the 1700 K envelope for a diode with an etched-rhenium emitter and a 0.025-mm electrode gap made by TECO and evaluated by NASA

    Results from tests of the first variable-gap diminiode at an initial interelectrode spacing of 0.23 millimeter indicate sharply defined, relatively low ultimate power points. This characteristic supports the value of the diminiode as a well-controlled tool for thermionic-conversion research and development

    Wave spectra of a shoaling wave field: A comparison of experimental and simulated results

    Wave profile measurements made from an aircraft crossing the North Carolina continental shelf after passage of Tropical Storm Amy in 1975 are used to compute a series of wave energy spectra for comparison with simulated spectra. Results indicate that the observed wave field experiences refraction and shoaling effects causing statistically significant changes in the spectral density levels. A modeling technique is used to simulate the spectral density levels. Total energy levels of the simulated spectra are within 20 percent of those of the observed wave field. The results represent a successful attempt to theoretically simulate, at oceanic scales, the decay of a wave field which contains significant wave energies from deepwater through shoaling conditions

    Thermionic conversion data obtained from a variable gap cesium diminiode with a hot pressed, sintered lanthanum hexaboride emitter and an arc melted lanthanum hexaboride collector are presented. Performance curves cover a range of temperatures: emitter 1500 to 1700 K, collector 750 to 1000 K, and cesium reservoir 370 to 510 K.     Abstract: This paper describes the development of a fiscal impact tool for New Hampshire communities (HT -4-NH). FIT -4-NH belongs to a family of computergenerated fiscal impact assessment models designed to estimate the impacts to local government revenues and expenditures that result from economic changes. In the past, work in this area has centered on the completion of countylevel models for the midwestern states.     QSO hosts and environments at z=0.9 to 4.2: JHK images with adaptive optics

    We have observed nine QSOs with redshifts 0.85 to 4.16 at near-IR wavelengths with the adaptive optics bonnette of the Canada-France-Hawaii telescope. Exposure times ranged from 1500 to 24000s (mostly near 7000s) in J, H, or K bands, with pixels 0.035 arcsec on the sky. The FWHM of the co-added images at the location of the quasars are typically 0.16 arcsec. Including another QSO published previously, we find associated QSO structure in at least eight of ten objects, including the QSO at z = 4.16. The structures seen in all cases include long faint features which appear to be tidal tails. In four cases we have also resolved the QSO host galaxy, but find them to be smooth and symmetrical: future PSF removal may expand this result. Including one object previously reported, of the nine objects with more extended structure, five are radio-loud, and all but one of these appear to be in a dense small group of compact galaxy companions. The radio-quiet objects do not occupy the same dense environments, as seen in the NIR. In this small sample we do not find any apparent trends of these properties with redshift, over the range 0.8 < z < 2.4. The colors of the host galaxies and companions are consistent with young stellar populations at the QSO redshift. Our observations suggest that adaptive optic observations in the visible region will exhibit luminous signatures of the substantial star-formation activity that must be occurring.Comment: 22 pages including 10 tables, plus 11 figures.
