Wilms Tumour with Intracardiac Extension.
Wilms tumour or nephroblastoma is the most common renal tumour of in children. It accounts for 6% of all pediatric tumours and is the second most frequent intrabdominal solid organ tumour in children. Inferior vena cava (IVC) involvement by Wilms tumour occurs in 4-10% of patients and right atrium thrombus extension in less than 1%. Using a multidisciplinary approach, overall survival is excellent in Wilms tumour. We are presenting this case because Wilms tumour with right atrial thrombus is relatively rare and continues to remain a challenge for treating physicians
Overdiagnosis and overtreatment of breast cancer: Overdiagnosis in randomised controlled trials of breast cancer screening
Data from randomised controlled trials of mammographic screening can be used to determine the extent of any overdiagnosis, as soon as either a time equivalent to the lead-time has elapsed after the final screen, or the control arm has been offered screening. This paper reviews those randomised trials for which breast cancer incidence data are available. In recent trials in which the control group has not been offered screening, an excess incidence of breast cancer remains after many years of follow-up. In those trials in which the control arm has been offered screening, although there is a possible shift from invasive to in situ disease, there is no evidence of overdiagnosis as a result of incident screens
3D printing dimensional calibration shape: Clebsch Cubic
3D printing and other layer manufacturing processes are challenged by
dimensional accuracy. Several techniques are used to validate and calibrate
dimensional accuracy through the complete building envelope. The validation
process involves the growing and measuring of a shape with known parameters.
The measured result is compared with the intended digital model. Processes with
the risk of deformation after time or post processing may find this technique
beneficial. We propose to use objects from algebraic geometry as test shapes. A
cubic surface is given as the zero set of a 3rd degree polynomial with 3
variables. A class of cubics in real 3D space contains exactly 27 real lines.
We provide a library for the computer algebra system Singular which, from 6
given points in the plane, constructs a cubic and the lines on it. A surface
shape derived from a cubic offers simplicity to the dimensional comparison
process, in that the straight lines and many other features can be analytically
determined and easily measured using non-digital equipment. For example, the
surface contains so-called Eckardt points, in each of which three of the lines
intersect, and also other intersection points of pairs of lines. Distances
between these intersection points can easily be measured, since the points are
connected by straight lines. At all intersection points of lines, angles can be
verified. Hence, many features distributed over the build volume are known
analytically, and can be used for the validation process. Due to the thin shape
geometry the material required to produce an algebraic surface is minimal. This
paper is the first in a series that proposes the process chain to first define
a cubic with a configuration of lines in a given print volume and then to
develop the point cloud for the final manufacturing. Simple measuring

On the toughness of thermoplastic polymer nanocomposites as assessed by the essential work of fracture (EWF) approach
The essential work of fracture (EWF) approach is widely used to determine the plane stress fracture toughness of highly ductile polymers and related systems. To shed light on how the toughness is affected by nanofillers EWF-suited model polymers, viz. amorphous copolyester and polypropylene block copolymer were modified by multiwall carbon nanotube (MWCNT), graphene (GR), boehmite alumina (BA), and organoclay (MMT) in 1 wt% each. EWF tests were performed on deeply double-edge notched tensile-loaded specimens under quasistatic loading conditions. Data reduction occurred by energy partitioning between yielding and necking/tearing. The EWF prerequisites were not met with the nanocomposites containing MWCNT and GR by contrast to those with MMT and BA. Accordingly, the toughness of nanocomposites with homogeneously dispersed and low aspect ratio fillers may be properly determined using the EWF. Results indicated that incorporation of nanofillers may result in an adverse effect between the specific essential and non-essential EWF parameters

Firms' Main Market, Human Capital and Wages
Recent international trade literature emphasizes two features in characterizing the current patterns of trade: efficiency heterogeneity at the firm level and quality differentiation. This paper explores human capital and wage differences across firms in that context. We build a partial equilibrium model predicting that firms selling in more-remote markets employ higher human capital and pay higher wages to employees within each education group. The channel linking these variables is firms’ endogenous choice of quality. Predictions are tested using Spanish employer-employee matched data that classify firms according to four main destination markets: local, national, European Union, and rest of the World. Employees’ average education is increasing in the remoteness of firm’s main output market. Market–destination wage premia are large, increasing in the remoteness of the market, and increasing in individual education. These results suggest that increasing globalization may play a significant role in raising wage inequality within and across education groups
Speech, stone tool-making and the evolution of language
The 'technological hypothesis' proposes that gestural language evolved in early hominins to enable the cultural transmission of stone tool-making skills, with speech appearing later in response to the complex lithic industries of more recent hominins. However, no flintknapping study has assessed the efficiency of speech alone (unassisted by gesture) as a tool-making transmission aid. Here we show that subjects instructed by speech alone underperform in stone tool-making experiments in comparison to subjects instructed through either gesture alone or 'full language' (gesture plus speech), and also report lower satisfaction with their received instruction. The results provide evidence that gesture was likely to be selected over speech as a teaching aid in the earliest hominin tool-makers; that speech could not have replaced gesturing as a tool-making teaching aid in later hominins, possibly explaining the functional retention of gesturing in the full language of modern humans; and that speech may have evolved for reasons unrelated to tool-making. We conclude that speech is unlikely to have evolved as tool-making teaching aid superior to gesture, as claimed by the technological hypothesis, and therefore alternative views should be considered. For example, gestural language may have evolved to enable tool-making in earlier hominins, while speech may have later emerged as a response to increased trade and more complex inter- and intra-group interactions in Middle Pleistocene ancestors of Neanderthals and Homo sapiens; or gesture and speech may have evolved in parallel rather than in sequence
