    COMBINATION OF TETRALOGY OF FALLOT AND HEMOPHILIA IN A CHILD: CASE REPORT

    Congenital heart disease (CHD) is formed during intrauterine period but, as a rule, they are not hereditary. Hemophilia A belongs to the group of hereditary coagulopathies stipulated by deficiency of coagulation factor VIII and characterized by bleeding of hematoma like type. Combination of these two diseases is rather rare that requires cardiosurgeons’ apprehensive attitude.We present our experience in surgical treatment of a boy of 8 months old with Tetralogy of Fallot. Hemophilia A was found in preoperative examination. He was successfully operated with cardiac pulmonary bypass on the background of factor VII injection.Surgical treatment was successful without significant bleeding. Postoperative period was uneventful. For successful treatment of CHD with coagulation failure is very important timely diagnosis, that can be difficult due to the lack of specific symptoms in children.Surgical treatment is the method of choice in TOF.     Social networks and biographical method

    The concept of storytelling as one of the modern varieties of the biographical method has been considered. An example of analyzing the history of social networks has been suggested, which allows us to reveal the full potential of this method for the development of digital sociology. Social networks expand horizontal communication and create opportunities for rapid, networked dissemination of information. Almost barrier-free entry and wide circulation make it possible to use a longknown and  proven tool more widely. Storytelling allows you to get visual, emotional, and evaluative components of any biography. Information obtained using this method gives us an opportunity to study several layers of a particular biography and build a network of social relationships of the individual, that is, to look at these or other facts of the biography from different points of view of the participants of the events. The story analyzed in this paper is interesting for its genre: a personal narrative created at the initiative of the online magazine Lady.mail.ru. The communication channel that is used to broadcast the story affects on the content of the message being transmitted, thus being an element of the story itself. For example, the same story can be told differently through text and video, respectively, having a different impact, or vice versa, different tools may be used to transmit the same story, depending on the specifics of the transmission channel. Each story has an individual storyteller, it is an element of a larger, multi-voiced story. Modern digital technologies have simplified and expanded the possibilities of the biographical method, since a huge amount of biographical information is voluntarily uploaded to social networks by millions of their users from different segments of the population, which allows us using this method to analyze most of the problems that are in the interests of modern sociological thought

    Attitudes of representatives of major religious movements towards the digitalisation of religion

    The Internet has become an integral part of the modern society. It has a strong influence on the social institutions and makes them adapt to the new conditions of the digital environment. Religion is not an exception despite its inherent resistance to change; such an ancient social construct was also forced to enter the process of digitalisation. The article reflects the peculiarities of this process: it analyses the activities of various religious movements in the Russian Federation in the digital environment and reveals the attitude of religious representatives (official and unofficial) to the process of adapting religion to the digital environment. The scholarly literature on the digitalisation of religion was analysed and the problematic aspects of this process were identified. A content analysis of the Internet communities and resources of the main traditional (Orthodoxy, Islam and Judaism) and non-traditional religious teachings (Neopaganism and Satanism)was carried out. We identified the attitude of religious representatives to the digitalisation of religion. We conducted an expert interview with bloggers whose activities are related to the dissemination of religious views on the Internet

    The quality of medical care in a round-the-clock hospital during the COVID-19 pandemic

    Violations in the provision of medical care are detected by controlling the volume, timing, quality and conditions of medical care. The objective of the work is assessment of the activities of the round-the-clock hospital from the standpoint of the quality of medical care during the COVID-19 pandemic. Material and methods. A database of indicators reflecting the activity of round-the-clock hospital for four years, from 2017 to 2020 (Omsk) was formed. Analytical, statistical methods and the method of expert assessments were used to analyze the required indicators. Results. Evaluation of inpatient hospital activities during the COVID-19 pandemic compared to the “pre-pandemic” years showed a 1.9-fold and 2.9-fold decrease in the total number of quality examinations performed and the number of defects detected, respectively. The number of defects that were grounds for denial or reduction of payment for medical care found in the “before-COVID-19” years was 2.1 times higher than in 2020. In 2020, the weight of the share of those defects that directly reflect the quality of medical care increased signifcantly. The quantitative composition of the inpatient hospital staff changed insignifcantly during the analyzed period – the number of physicians increased  by only 5 % over four years, with no dynamics in the qualitative characteristics of the staff. The number of medical equipment increased by 27.6 %. Due to re-profling in 2020, there was a decrease in the number of hospitalized patients, with an increase in the proportion of patients admitted by emergency care (up to 91 %) and an increase in mortality by 2.7 times. Conclusions. One of the conditions for maintaining high quality of medical care is a balance between the main components of quality: accessibility, timeliness, sufciency, efciency and safety and the speed of its achievement, which in turn depends on the amount of resources and reserves

    Immunological features of allergic rhinitis in combination with chronic rhinosinusitis of bacterial etiology

    Rhinitis is a socially significant and widespread disease. Often, various forms of rhinitis are combined, and thus cause severe clinical manifestations, insufficient effectiveness of drug treatment, as well as difficulties in differential diagnosis. It is known that a significant number of patients have a combination of allergic rhinitis (AR) with chronic rhinosinusitis of bacterial etiology. This condition is based on a chronic multifactorial inflammatory process of the nasal mucosa, which determines the steady progression of the disease. Of interest is the study of a number of allergo-immunological parameters in nasal secretions in order to assess local inflammation and changes in mucosal immunity in allergic rhinitis in combination with chronic rhinosinusitis of bacterial etiology (AR with HRSBE). Mucosal immunity and biological mediators determine local inflammation and pathophysiological response to etiological factors in the immunopathogenesis of AR with CRSBE. The work carried out the determination of the level of cytokines: IL-4, IL-10, TGF-â1, IFNã spontaneous and induced; immunoglobulins: IgA, IgM, IgG, sIgA in nasal secretions; leukotrienes: LT C4/ D4/E4 and LTB4 in plasma and total IgE in serum in patients with AR with moderate-severity HRSBE without exacerbation. It has been shown that the leading role in the formation of the inflammatory process in AR with CRSBE belongs to cytokines: IL-4, IL-10, TGF-â; immunoglobulins: IgM, sIgA; leukotrienes: LT C4/D4/E4 and LTB4 and total IgE. Induced cytokine production largely reflects the reserve capabilities of immunocompetent cells in response to the action of a pathogenic factor. The results obtained are associated with the persistent course of allergic and infectious inflammation and the progression of the disease. Thus, cytokines: IL-4, IL-10, TGF-â1; immunoglobulins: IgM, sIgA, IgE total and LT C4/D4/E4 and LTB4 make a significant contribution to pathogenetic mechanisms, determining the clinical course of AR with CRSBE, and can serve as biological markers of the activity of the pathological process. Undoubtedly, the immune mechanisms in the combined pathological inflammatory reaction from the mucosa in AR with HRSBE are complex and multifaceted. A personalized approach to the treatment of patients with AR with CRSBE is determined by the severity and intensity of the inflammatory reaction, as well as mucosal mucosal immunity disorders. The study of the role and significance of the production of leading cytokines, immunoglobulins in nasal secretions, as well as leukotrienes and total IgE in the blood will help the doctor in determining the tactics and duration of pharmacotherapy

    Agent-based modeling of the impact of advertising on the regional economic cluster lifecycle

    The aim of the study is the development and testing of an algorithm for modeling the impact of advertising on various stages of the life cycle of economic clusters. It is assumed, that the life cycle of the cluster consists of the stages: a diffuse group, a hidden cluster, an evolving cluster, a mature cluster, a collapsing cluster. Using the agent-based simulation methods, hierarchical clustering and chaos theory, the following results were obtained: a conceptual model of the behavior of cluster members for cluster formation processes at each stage of the cluster life cycle and an imitation model of the influence of advertising on the life cycle of the economic cluster; the patterns of various stages of the life cycle of the economic cluster and the functioning of the cluster without influence and under the influence of advertising were revealed. Advertising reduces the time at the stages of the associated life cycle of the cluster, increases the stage of maturity of the cluster. Companies that do not comply with the principles of clustering are under the influence of advertising and promotional activities. Such enterprises most often arise in the cluster at the stages of its formation

    After a detailed study of the Proton Synchrotron (PS) complex requirements by experts of CERN controls & operation groups, a proposal to develop a new system, called Injector Controls Architecture (InCA), was presented to and accepted by the management late 2007. Aiming at the homogenisation of the control systems across CERN accelerators, InCA is based on components developed for the Large Hadron Collider (LHC) but also new components required to fulfil operation needs. In 2008, the project was in its elaboration phase and we successfully validated its architecture and critical use-cases during several machine development sessions. After description of the architecture put in place and the components used, this paper describes the planning approach taken combining iterative development phases with deployment in operation for validation sessions
