Development of professional competences in Russia and Lithuania: an example of successful international cooperation in the field of adult education
The article presents one of the directions of international cooperation in the field of adult education. It is review of a monographic book on professional competences, prepared jointly by andragogy researchers in Russia (Pushkin Leningrad State University in St. Petersburg) and Lithuania (Vytautas Magnus University in Kaunas). The authors put the Russian-Lithuanian publication on the background of the significant in modern European andragogy achievements of St. Petersburg's research community, and adult education conferences organized there annually for more than a decade
Awareness of Polish society about complications and contraindications in teeth whitening
Introduction and aim: Teeth whitening, which has a positive effect on the quality of life and the human psyche but is associated with side effects and contraindications, is becoming increasingly popular and widespread. The aim of the study was to collect and analyse data concerning the awareness of the Polish population of complications and contraindications to tooth whitening.
Material and methods: A survey was conducted using a Google document among teeth whitening and non-whitening individuals. Link to the survey posted on various social groups.
Results: People who whiten their teeth in a dental office are the group that showed the highest awareness about the side effects and contraindications of teeth whitening. People who do not whiten their teeth are more aware than people who whiten outside the dental surgery. The popularised slogans such as the possibility of tooth hypersensitivity, weakening of the enamel or the necessity to follow a "white diet" are known to each study group. The issues connected with the dentist's office were the least popular - persons whitening their teeth outside the dentist's office are not aware of the necessity to professionally clean their teeth before this procedure. None of the study groups have knowledge about the temporomandibular joint problem as a contraindication to teeth whitening.
Conclusions: The Polish population shows an average knowledge of teeth whitening. The basic contraindications and complications are known to a broader group of respondents, but there are aspects of whitening about which the knowledge of respondents is low and requires popularisation. In addition, the survey indicates that more research is needed into the relationship between the temporomandibular joint and tooth whitening

The Eukaryotic Proteome Is Shaped by E3 Ubiquitin Ligases Targeting C-Terminal Degrons
Degrons are minimal elements that mediate the interaction of proteins with degradation machineries to promote proteolysis. Despite their central role in proteostasis, the number of known degrons remains small and a facile technology to characterize them is lacking. Using a strategy combining Global Protein Stability (GPS) profiling with a synthetic human peptidome, we identify thousands of peptides containing degron activity. Using CRISPR screening, we established that the stability of many proteins is regulated through degrons located at their C-terminus. We characterize eight Cullin-RING E3 ubiquitin ligase (CRL) complexes adaptors that regulate C-terminal degrons including six CRL2 and two CRL4 complexes and computationally implicate multiple non-CRLs in end recognition. Human proteome analysis revealed that the C-termini of eukaryotic proteins are depleted for C-terminal degrons, suggesting an E3 ligase-dependent modulation of proteome composition. Thus, we propose that a series of ‘C-end rules’ operate to govern protein stability and shape the eukaryotic proteome
Late emergency arterial duct stenting in a patient with tetralogy of Fallot and occluded Blalock-Taussig shunt
Over the last decade or so, stent implantation to the arterial duct has achieved acceptance as an
alternative to aortopulmonary shunt procedure. We present the case of a patient with tetralogy of
Fallot with diminished pulmonary flow and complete proximal occlusion of a right-sided Blalock-Taussig shunt. Surgical repair was not possible because of relative contraindications, and the
interventional shunt recanalisation attempt was unsuccessful. The coronary stent was implanted
into arterial duct. This resulted in oxygen saturation increase with normal contrast
flow to the left pulmonary artery and right pulmonary artery (prior to the procedure we
suspected non-confluent pulmonary artery). This brief report evaluates the feasibility of this
new therapeutical option in such special patients with duct-dependent pulmonary blood flow.

The Participation of 3,3,3-Trichloro-1-nitroprop-1-ene in the [3+2] Cycloaddition Reaction with Selected Nitrile N-Oxides in the Light of the Experimental and MEDT Quantum Chemical Study
The regioselective zw-type [3 + 2] cycloaddition (32CA) reactions of a series of aryl-substituted nitrile N-oxides (NOs) with trichloronitropropene (TNP) have been both experimentally and theoretically studied within the Molecular Electron Density Theory (MEDT). Zwitterionic NOs behave as moderate nucleophiles while TNP acts as a very strong electrophile in these polar 32CA reactions of forward electron density flux, which present moderate activation Gibbs free energies of 22.8-25.6 kcal·mol−1 and an exergonic character of 28.4 kcal·mol−1 that makes them irreversible and kinetically controlled. The most favorable reaction is that involving the most nucleophilic MeO-substituted NO. Despite Parr functions correctly predicting the experimental regioselectivity with the most favorable O-CCCl3 interaction, these reactions follow a two-stage one-step mechanism in which formation of the O-C(CCl3) bond takes place once the C-C(NO2) bond is already formed. The present MEDT concludes that the reactivity differences in the series of NOs come from their different nucleophilic activation and polar character of the reactions, rather than any mechanistic feature
Genetic predisposition to papillary thyroid cancer
Około 5% zróżnicowanych raków tarczycy wykazuje predyspozycję dziedziczną. Dziedziczny nierdzeniasty rak tarczycy może występować
jako składowa niektórych dziedzicznych zespołów nowotworowych, na przykład rodzinnej polipowatości jelit oraz jako rodzinny
zróżnicowany rak tarczycy (FNMTC, familial non-medullary thyroid cancer), gdzie najczęściej obserwuje się raka brodawkowatego. Choć
predyspozycja dziedziczna do nierdzeniastych raków tarczycy jest dobrze znana, to jednak geny warunkujące jej występowanie nie
zostały jeszcze poznane. Wykonane jak dotąd badania zidentyfikowały kilka loci - 1q21, 6q22, 8p23.1-p22 oraz 8q24, jednak wyniki te nie
zawsze były jednoznaczne. W niniejszej pracy omówiono rezultaty badań sprzężenia oraz ostatnio uzyskane wyniki badań związku
całego genomu (GWAS, genome wide association study) wykonano przez badania polimorfizmu pojedynczego nukleotydu (SNP, single
nucleotide polymorphism) z wykorzystaniem techniki mikromacierzy wysokiej gęstości. (Endokrynol Pol 2010; 61 (5): 486-489)Approximately 5% of differentiated thyroid cancers are hereditary. Hereditary non-medullary thyroid cancer may occur as a minor component
of familial cancer syndromes (e.g. familial adenomatous polyposis) or as a primary feature (familial non-medullary thyroid cancer
[FNMTC]). Among FNMTC, PTC is the most common. Although a hereditary predisposition to non-medullary thyroid cancer is well
established, the susceptibility genes are poorly known. Up to now, by linkage analysis using microsatellite markers, several putative loci
have been described - 1q21, 6q22, 8p23.1-p22, and 8q24; however, validation studies have been unsuccessful. In the present review we
discuss the results of linkage analysis and the most recent results of genome wide association studies (GWAS) with high resolution SNP

Nietypowe zabiegi interwencyjne z zastosowaniem stentów wewnątrznaczyniowych w leczeniu dzieci z zespołem niedorozwoju lewego serca
Zespół hipoplazji lewego serca jest jedną z najczęstszych wad wrodzonych serca, nadal stanowiącą
trudny problem terapeutyczny. Pomimo znacznej poprawy wyników leczenia metodą
Norwooda i wprowadzenia nowych modyfikacji tej metody całkowita śmiertelność jest wciąż
wysoka. W niniejszej pracy przedstawiono dwa przypadki niemowląt z zespołem hipoplazji
lewego serca, którym implantowano interwencyjnie stent wewnątrznaczyniowy w ramach
leczenia wspomagającego zabieg kardiochirurgiczny.Pierwszy przypadek dotyczy niemowlęcia z zespołem hipoplazji lewego serca i posocznicą Klebsiella
pnemoniae, które z powodu ciężkości stanu klinicznego nie zostało zakwalifikowane do
leczenia metodą Norwooda. U dziecka wykonano operacyjnie obustronne przewężenie tętnic
płucnych, które poprawiło wydolność krążeniową i oddechową. Konieczność długotrwałego
utrzymania drożności przewodu tętniczego poprzez stałą podaż prostaglandyny E1 (Prostin
VR) i związane z tym działania niepożądane wpłynęły na podjęcie decyzji o implantacji stentu
wewnątrznaczyniowego do przewodu tętniczego Botalla. Przedstawiono poglądy dotyczące przewężenia
gałęzi płucnych, stentowania przewodu tętniczego i statycznej atrioseptostomii balonowej
jako metody wspomagającej leczenie operacyjne krytycznie chorych dzieci z zespołem hipoplazji
lewego serca.
Drugi przypadek stanowi pacjent z zespołem hipoplazji lewego serca po operacji metodą Norwooda
z restrykcyjnym połączeniem międzyprzedsionkowym i hipoplazją lewej tętnicy płucnej.
Z powodu nasilonej sinicy centralnej niemowlę wymagało zabiegu lewostronnego zespolenia
systemowo-płucnego w trybie pilnym oraz zapewnienia swobodnej komunikacji międzyprzedsionkowej.
Przedstawiono zabieg implantacji stentu do ubytku międzyprzedsionkowego jako
nową technikę pozwalającą na czasowe zapewnienie szerokiego, nierestrykcyjnego połączenia
międzyprzedsionkowego

The following case report features 70 year old male presented with infero-lateralis STEMI, with typical pain at rest of 6 hours duration. His past medical history reveals elective PCI with zotarolimus eluting stent implantation in the left circumflex artery (LCX) in 2015 and aortic valve replacement (AVR) therapy because of its regurgitation with implantation of the biological graft one month prior to the index acute coronary syndrome. It is worth to note that before AVR therapy coronary angiogram revealed good percutaneous coronary intervention (PCI) result and relatively no changes in other arteries. Besides ACS symptom, patient revealed that he had also fever few days before hospital admission. His coronary angiogram revealed in-stent occluded LCX, without sufficient collateral circulation. After several thrombectom passages, with a successful aspiration of vast amount of thrombus, operator decided to perform double-kissing technique (DK-crush) for bifurcation lesion. Unfortunately, after procedure there was some distal embolization in secondary distal branches of LCX and obtuse marginal branch (OM). Because of coexisting anemia, but without history of clinically overt bleeding, GPIIbIIIa inhibitor was not administered. The procedure was performed on unfractionated heparin intraarterial boluses. After PCI patient was without angina, with partial ST segment elevation resolution of more than 50%. Transthoracic echocardiography revealed akinesia of infarct-related territory, with left ventricular ejection fraction around 48%, moderate mitral and mild aortic valve regurgitation. The presence of vegetation on biological aortic valve graft was later confirmed by transesophageal echocardiography. Patient’s status quickly deteriorated as he developped dyspnea, fever and his inflammatory markers were succesfully elevated. Intravenous antibiotics were administered according to the antybiogram (HLAR Enterococcus faecalis was detected) along with intensive diuretic therapy and blood transfusion, and patient became free of symptoms. His control transesophageal echocardiography revealed no vegetations and he was successfully discharged home after 5 weeks of treatment.Opis przypadku dotyczy strategii zabiegu u 70-letniego chorego, przyjętego z powodu zawału STEMI ściany dolno-bocznej, z wywiadem spoczynkowej dławicy trwającej od około 6 godzin do pierwszego kontaktu medycznego, po angioplastyce z implantacją stentu pokrytego zotarolimusem do bifurkacji dominującej gałęzi okalającej (GO) przed dwoma laty, po wymianie zastawki aortalnej z powodu złożonej wady z przewagą niedomykalności — implantacja zastawki biologicznej miesiąc wcześniej. Dodatkowo, pacjent zgłaszał od kilku dni występujące stany podgorączkowe. W koronarografii uwidoczniono niedrożną GO w stencie, bez krążenia obocznego. Po licznych pasażach trombektomem oraz aspiracji znacznej ilości materiału zatorowego zadecydowano o wykonaniu angioplastyki metodą double-kissing crush (DK-crush). Niestety, nie udało się uniknąć zatorowości obwodowej w gałęziach wtórnych, dużej gałęzi marginalnej oraz w 13. segmencie GO. Zabieg przeprowadzono podając bolusy heparyny niefrakcjonowanej. Ze względu na współistniejącą niedokrwistość (bez wywiadu krwawienia), nie włączano inhibitora IIbIIIa. Bezpośrednio po zabiegu chory był bez dolegliwości dławicowych, z rezolucją uniesienia odcinka ST powyżej 50% w kontrolnym EKG. Na podstawie badania echokardiograficznego przezklatkowego wysunięto podejrzenie wegetacji na zastawce aortalnej, które potwierdzono w badaniu przezprzełykowym. Poza tym stwierdzono zaburzenia kurczliwości odpowiadające lokalizacji elektrokardiograficznej zawału, z frakcją wyrzutową 48% i z umiarkowaną niedomykalnością zastawki mitralnej i łagodną aortalnej. W kolejnych dobach doszło do pogorszenia stanu ogólnego, zaobserwowano nasilenie objawów niewydolności serca oraz narastanie parametrów zapalnych. W posiewach krwi wyhodowano Enterococcus faecalis (HLAR, high-level aminoglycoside resistance). Po wdrożeniu antybiotykoterapii według wyników posiewów, intensyfikacji leczenia niewydolności serca i przetoczeniu dwóch jednostek masy erytrocytarnej uzyskano stabilizację stanu chorego. Po 5 tygodniach hospitalizacji w stanie ogólnym dobrym został wypisany do domu
