Genetic vulnerability to diabetes and obesity: does education offset the risk?
The prevalence of type 2 diabetes (T2D) and obesity has recently increased dramatically. These common diseases are likely to arise from the interaction of multiple genetic, socio-demographic and environmental risk factors. While previous research has found genetic risk and education to be strong predictors of these diseases, few studies to date have examined their joint effects. This study investigates whether education modifies the association between genetic background and risk for type 2 diabetes (T2D) and obesity. Using data from non-Hispanic Whites in the Health and Retirement Study (HRS, n = 8398), we tested whether education modifies genetic risk for obesity and T2D, offsetting genetic effects; whether this effect is larger for individuals who have high risk for other (unobserved) reasons, i.e., at higher quantiles of HbA1c and BMI; and whether effects differ by gender. We measured T2D risk using Hemoglobin A1c (HbA1c) level, and obesity risk using body-mass index (BMI). We constructed separate genetic risk scores (GRS) for obesity and diabetes respectively based on the most current available information on the single nucleotide polymorphism (SNPs) confirmed as genome-wide significant predictors for BMI (29 SNPs) and diabetes risk (39 SNPs). Linear regression models with years of schooling indicate that the effect of genetic risk on HbA1c is smaller among people with more years of schooling and larger among those with less than a high school (HS) degree compared to HS degree-holders. Quantile regression models show that the GRS × education effect systematically increased along the HbA1c outcome distribution; for example the GRS × years of education interaction coefficient was −0.01 (95% CI = −0.03, 0.00) at the 10th percentile compared to −0.03 (95% CI = −0.07, 0.00) at the 90th percentile. These results suggest that education may be an important socioeconomic source of heterogeneity in responses to genetic vulnerability to T2D
Optimism is not associated with two indicators of DNA methylation aging
Evidence indicates associations between higher optimism and reduced risk of age-related conditions and premature mortality. This suggests optimism is a positive health asset, but research identifying potential biological mechanisms underlying these associations remains limited. One potential pathway is slower cellular aging, which may delay age-related deterioration in health. Data were from the Women's Health Initiative (WHI) (N=3,298) and the Veterans Affairs Normative Aging Study (NAS) (N=514), and included dispositional and explanatory style optimism measures. We evaluated whether higher optimism was associated with metrics suggestive of less cellular aging, as indicated by two DNA methylation algorithms, intrinsic (IEAA) and extrinsic epigenetic age acceleration (EEAA); these algorithms represent accelerated biologic aging that exceeds chronological age. We used linear regression models to test our hypothesis while considering several covariates (sociodemographics, depressive symptoms, health behaviors). In both cohorts, we found consistently null associations of all measures of optimism with both measures of DNA methylation aging, regardless of covariates considered. For example, in fully-adjusted models, dispositional optimism was not associated with either IEAA (WHI:β=0.02; 95% Confidence Interval [CI]:-0.15-0.20; NAS:β=-0.06; 95% CI:-0.56-0.44) or EEAA (WHI:β=-0.04; 95% CI: -0.26-0.17; NAS:β=-0.17; 95% CI: -0.80-0.46). Higher optimism was not associated with reduced cellular aging as measured in this study
Epigenome-Wide Analysis of DNA Methylation and Optimism in Women and Men
Objective: Higher optimism is associated with reduced mortality and a lower risk of age-related chronic diseases. DNA methylation (DNAm) may provide insight into mechanisms underlying these relationships. We hypothesized that DNAm would differ among older individuals who are more versus less optimistic. Methods: Using cross-sectional data from two population-based cohorts of women with diverse races/ethnicities (n = 3816) and men (only White, n = 667), we investigated the associations of optimism with epigenome-wide leukocyte DNAm. Random-effects meta-analyses were subsequently used to pool the individual results. Significantly differentially methylated cytosine-phosphate-guanines (CpGs) were identified by the “number of independent degrees of freedom” approach: effective degrees of freedom correction using the number of principal components (PCs), explaining >95% of the variation of the DNAm data (PC-correction). We performed regional analyses using comb-p and pathway analyses using the Ingenuity Pathway Analysis software. Results: We found that essentially all CpGs (total probe N = 359,862) were homogeneous across sex and race/ethnicity in the DNAm-optimism association. In the single CpG site analyses based on homogeneous CpGs, we identified 13 significantly differentially methylated probes using PC-correction. We found four significantly differentially methylated regions and two significantly differentially methylated pathways. The annotated genes from the single CpG site and regional analyses are involved in psychiatric disorders, cardiovascular disease, cognitive impairment, and cancer. Identified pathways were related to cancer, and neurodevelopmental and neurodegenerative disorders. Conclusion: Our findings provide new insights into possible mechanisms underlying optimism and health
Psychological Health, Well-Being, and the Mind-Heart-Body Connection A Scientific Statement From the American Heart Association
As clinicians delivering health care, we are very good at treating disease but often not as good at treating the person. The focus of our attention has been on the specific physical condition rather than the patient as a whole. Less attention has been given to psychological health and how that can contribute to physical health and disease. However, there is now an increasing appreciation of how psychological health can contribute not only in a negative way to cardiovascular disease (CVD) but also in a positive way to better cardiovascular health and reduced cardiovascular risk. This American Heart Association scientific statement was commissioned to evaluate, synthesize, and summarize for the health care community knowledge to date on the relationship between psychological health and cardiovascular health and disease and to suggest simple steps to screen for, and ultimately improve, the psychological health of patients with and at risk for CVD. Based on current study data, the following statements can be made: There are good data showing clear associations between psychological health and CVD and risk; there is increasing evidence that psychological health may be causally linked to biological processes and behaviors that contribute to and cause CVD; the preponderance of data suggest that interventions to improve psychological health can have a beneficial impact on cardiovascular health; simple screening measures can be used by health care providers for patients with or at risk for CVD to assess psychological health status; and consideration of psychological health is advisable in the evaluation and management of patients with or at risk for CVD
Educational level, socio-economic status and relationship with quality of life in elderly residents of the city of Porto Alegre/RS, Brazil
The objective of this study was to verify the index of quality of life of elderly individuals belonging to groups, from different socioeconomic strata in the city of Porto Alegre, Rio Grande do Sul State. The research adopted a transversal model, used to collect data from the SF-36 questionnaire. The sampling is the intentional type and comprised: 61 elderly people in Class A, 80 in Class C, and 84 in Class E. The research was approved by the Committee of Ethics in Research/UFRGS. In relation to the quality of life, Class A presented higher scores than did Classes C and E on the pain, vitality, social aspect and mental health areas. The results also showed a significant difference in quality of life among university educated individuals versus the other schooling groups, on the pain, vitality, mental health and social aspect fields. Quality of life is a complex concept to study, but essential to improve the perception of health and welfare by the elderly.<br>O objetivo deste trabalho foi verificar o índice de qualidade de vida de idosos participantes de grupos de convivência, de diferentes estratos socioeconômicos do município de Porto Alegre/RS. A pesquisa seguiu um modelo de estudo transversal e utilizou para coleta de dados o questionário SF-36. A amostragem foi do tipo intencional e foi composta por: 61 idosos na classe A, 80 na classe C e de 84 na classe E. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa da UFRGS. Em relação à qualidade de vida, a classe A apresentou escores melhores que a classe C e E nos domínios dor, vitalidade, aspecto social e saúde mental. Houve diferença significativa de qualidade de vida no ensino superior completo, em relação aos demais estratos de escolaridade, nos domínios dor, vitalidade, saúde mental e aspecto social. A qualidade de vida é um conceito complexo de ser estudado, mas essencial para que haja melhora na percepção de saúde e do bem-estar pelos idosos
