A major range extension of Blindia robusta (Seligeriaceae, Bryophyta)
The endemic species Ditrichum tenuinerve Dixon from Tristan da Cunha is taxonomically evaluated and some details of its type material are illustrated. This species shares the diagnostic characters of Blindia robusta Hampe and the two species are considered to be conspecific. As a result of this taxonomic conclusion the global range of B. robusta is extended to the South Atlantic Ocean and accordingly it has to be considered as a nearly pan-south-temperate, not an amphipacific south-temperate species. Wybrane aspekty epidemiologiczne stwardnienia rozsianego w Polsce – wieloośrodkowe badanie pilotażowe
Background and purpose
The aim was to conduct a pilot study of selected epidemiological aspects of multiple sclerosis (MS) in Poland.
Material and methods
Cross-sectional data were collected in 21 centres providing MS treatment. The demographic profile of the patients, medical history of MS, disability status, comorbidity, and diagnostic and treatment modalities were analysed.
Results
Data on 3581 patients were obtained, including 2494 women (69.6%) and 1030 men (28.8%) – sex ratio 2.4 : 1. The mean age was 40.7 ± 11.9 years. Monofocal onset was reported in 80.8% of cases – the most frequently reported location of lesions was supratentorial (36.1%), followed by optic nerves (26.5%) and spinal cord (20.1%). The mean disease duration was 10.2 ± 8.8 years (range 0.04–53 years), and the mean time from the first symptoms to MS diagnosis was 2.6 years. Relapsing-remitting MS was reported in 70.5% of patients, secondary progressive in 16.8%, primary progressive in 8.4%, and ‘benign MS’ in 2.5%. The mean EDSS score was 3.3 ± 2.2 (range 0–9.5). The family history of MS was positive in 6.4% of cases. Comorbidity mainly applied to the musculoskeletal system (6.5%), the urinary system (5.8%) and psychiatric disturbances (5.5%). Brain magnetic resonance studies were available in 96.3% of the patients, evoked potentials in 54%, and cerebrospinal fluid testing in 63.1% – of whom only 41.2% were tested for oligoclonal bands, with 84% of samples being positive. Immunomodulatory drugs were used in 842 patients (24%), predominandy interferon beta (81%) and glatiramer (13%). Mitoxantrone was the most commonly used immunosuppressant.
Conclusions
This project is the first countrywide large-scale MS survey, covering approximately 18% of patients, according to our estimates. The results identify the clinical condition of the patients, as well as diagnostic and treatment modalities.Wstęp i cel pracy
Celem badania była pilotażowa analiza wybranych aspektów epidemiologicznych stwardnienia rozsianego (SR) w Polsce.
Materiał i metody
Przekrojowe dane zebrano w 21 ośrodkach prowadzących leczenie SR. Analizowano profil demograficzny chorych, wywiad chorobowy SR, stopień niepełnosprawności, choroby współwystępujące, metody diagnostyczne i stosowane leczenie.
Wyniki
Uzyskano dane 3581 pacjentów, w tym 2494 kobiet (69,6%) i 1030 mężczyzn (28,8%) – proporcja płci 2,4 : 1. Średni wiek wynosił 40,7 ± 11,9 roku. Początek jednoogniskowy odnotowano w 80,8% przypadków – najczęściej wskazywano na nadnamiotową lokalizację uszkodzeń (36,1%), w dalszej kolejności zajęte były nerwy wzrokowe (26,5%) i rdzeń kręgowy (20,1%). Choroba trwała średnio 10,2 ± 8,8 roku (zakres 0,04–53 lat), a od pierwszych objawów do ustalenia rozpoznania mijało średnio 2,6 roku. Postać nawra-cająco-zwalniającą SR miało 70,5% chorych, wtórnie postępującą – 16,8%, pierwotnie postępującą – 8,4%, u 2,5% rozpoznano zaś „łagodne” SR. Średni stopień niesprawności w skali EDSS wyniósł 3,3 ±2,2 (zakres 0–9,5). Rodzinne występowanie SR odnotowano w 6,4% przypadków. Wśród chorób współwystępujących najczęstsze były schorzenia narządu ruchu (6,5%), układu moczowego (5,8%) i zaburzenia psychiczne (5,5%). Rezonans magnetyczny mózgu wykonano u 96,3% pacjentów, potencjały wywołane u 54%, badanie płynu mózgowo-rdzeniowego u 63,1% – z czego zaledwie w 41,2% przypadków oznaczano prążki oligoklonalne (ich obecność stwierdzono w 84% próbek). Leki immunomodulujące otrzymywało 842 chorych (24%), najczęściej były to interferon β (81%) i glatiramer (13%). W immunosupresji najczęściej stosowano mitoksantron.
Wnioski
Zrealizowany projekt jest pierwszym ogólnopolskim badaniem populacji chorych na SR na tak szeroką skalę – wg naszych szacunków obejmującym ok. 18% populacji chorych. Zgromadzone wyniki informują o stanie klinicznym chorych, najczęstszych metodach diagnostyki i leczenia

Neuroendokrynne guzy trzustki wywodzące się z komórek wyspowych trzustki są rzadko spotykanymi nowotworami, manifestującymi się zespołem objawów charakterystycznych dla wydzielanego hormonu. Cukrzyca jako objaw kliniczny takiego guza jest zjawiskiem znanym i wielokrotnie opisywanym, jednak zazwyczaj przebiega ona łagodnie i jest tylko jednym z objawów. Przedstawiamy przypadek złośliwego neuroendokrynnego guza trzustki u 19-letniego chorego, u którego cukrzyca, o obrazie klinicznym typowym dla cukrzycy typu 1, była pierwszym i wiodącym objawem choroby.Neuroendocrine tumours of pancreas originating from pancreatic islet cells are uncommon neoplasms with clinical manifestation depending on a type of hormone secreted. Diabetes as a clinical sign of such a tumour has been reported many times and is not an uncommon finding but it is generally mild and makes only a part of a whole pattern of symptoms. We report on a case of a malignant neuroendocrine pancreatic tumour in 19 years old patient, where diabetes, with a clinical course typical for type 1 diabetes, was the first and the main symptom of the disease
Ocena przydatności diagnostycznej badania retykulopłytek u chorych z pierwotną małopłytkowością immunologiczną
Background: The basic criterion for primary immune thrombocytopenia (ITP) diagnosis is
decreased platelet count (< 100 × 109/l) with no other causes for thrombocytopenia. Elevated
megakaryocyte count in bone marrow is a helpful parameter for ITP recognition. As bone
marrow biopsy is an invasive procedure the evaluation of megakaryocyte activity through
examination of reticulated platelets (RP) seems to be a better diagnostic option.
The aim of the study was to assess the diagnostic value of RP tests in ITP patients.
Material and methods: Reticulated placelets were tested in 56 ITP patients. The mean
platelet count before treatment was 27.5 ± 15.8 × 109/l. In this group of patients 26 were treated
only pharmacologically and 30 were subjected to splenectomy. The monitoring period was 1–3
years. Treatment was considered satisfactory when platelet count exceeded 50 × 109/l.
Reticulated placelets were isolated from platelet-rich plasma and fixed in 1% paraformaldehyde,
then stained with anti CD41-PE monoclonal antibody and thiazole orange. The RP
values were evaluated in flow cytometer. The RP values in ITP patients were compared to RP
values for healthy donors (1.3%, range: 0.5–6%). Values above 10% were considered elevated.
Results: The RP percentage prior to treatment was 18.5% (significantly higher than in
healthy donors). In the course of pharmacological regime we observed a decrease of RP percentage
in 11 patients (8.8 ± 7.4%) responsive to treatment which correlated with the increase of
platelet count (123.8 ± 72.7 × 109/l). In 15 other patients unresponsive to such treatment, the
percentage of RP remained elevated (24.7% ± 20.6%) and platelet counts were < 50 × 109/l.
In 29 of the 30 splenectomy patients, platelet counts were observed to increase (mean; 307 ± 170 × 109/l) on days 3–5 following surgery while the RP percentage remained elevated (16.9 ±
± 7.8%). In 1 splenectomy patient the platelet count did not increase and the RP percentage
remained high. In 12 of the 15 patients who reported for control visits after splenectomy the
percentage of RP was still elevated and platelet counts did not exceed 30 × 109/l.
Conclusions: The clinical ITP diagnosis is confirmed by the elevated RP values which
correlate with lower platelet counts. Reticulated placelets investigation is an additional parameter
of effective ITP treatment.

(ITP) jest zmniejszona (< 100 × 109/l) liczba płytek krwi przy braku innych przyczyn małopłytkowości.
W rozpoznaniu ITP pomocna może być zwiększona liczba megakariocytów w szpiku.
Biopsja szpiku jest procedurą inwazyjną, więc ocena aktywności megakariocytów poprzez
badanie retykuloplytek (RP) wydaje się być użyteczną techniką diagnostyczną ITP.
Celem pracy była ocena przydatności badania RP u chorych z ITP.
Materiał i metody: Retykulopłytki zbadano u 56 chorych z ITP. Liczba płytek krwi przed
leczeniem wynosiła 27,5 ± 15,8 × 109/l. Leczonych tylko farmakologicznie było 26 chorych,
a splenektomii poddano 30 chorych. Okres obserwacji chorych wynosił 1–3 lata. Leczenie
uznawano za skuteczne, gdy liczba płytek krwi osiągnęła wartość powyżej 50 × 109/l.
Retykulopłytki izolowano z bogatopłytkowego osocza, utrwalano 1% paraformaldehydem i znakowano
przeciwciałem monoklonalnym anty CD41-PE i oranżem tiazolu. Odsetek RP oceniano
w cytometrze przepływowym. W porównaniu z wartościami RP u zdrowych osób (1,3 ±
0,8%; zakres: 0,5–6%) za zwiększony odsetek RP uznawano wartości powyżej 10%.
Wyniki: Odsetek RP u chorych przed leczeniem wynosił średnio 18,5% i był istotnie wyższy
niż u osób zdrowych. Podczas leczenia farmakologicznego zaobserwowano, że u 11 chorych,
którzy odpowiedzieli na leczenie, odsetek RP ulegał zmniejszeniu (8,8 ± 7,4%), co korelowało
ze wzrostem liczby płytek krwi (123,8 ± 72,7 × 109/l). U 15 chorych, u których takie leczenie
było nieskuteczne, odsetek RP pozostawał nadal zwiększony (24,7 ± 20,6%), a liczba płytek
krwi była niższa niż 50 × 109/l.
Spośród 30 chorych, w ciągu 3–5 dni po splenektomii, u 29 liczba płytek krwi wzrosła (średnia
307 ± 170 × 109/l), a odsetek RP pozostawał zwiększony (16,9 ± 7,8%). U 1 chorego nie
uzyskano wzrostu liczby płytek krwi i odsetek RP był nadal wysoki. Spośród 15 chorych, którzy zgłosili się na ponowne badania, u 12 liczba płytek krwi oraz odsetek RP były w granicach normy,
natomiast u 3 osób RP były nadal zwiększone, a liczba płytek nie przekraczała 30 × 109/l.
Wnioski: Zwiększony odsetek RP korelujący ze zmniejszoną liczbą płytek krwi potwierdza
kliniczną diagnozę ITP. Badanie RP może być dodatkowym parametrem oceny skuteczności
leczenia ITP.

Mean daily plasma concentrations of β-endorphin, leu-enkephalin, ACTH, cortisol, and DHEAS in epileptic patients with complex partial seizures evolving to generalized tonic-clonic seizures
Introduction: A multitude of mechanisms have been implicated in the pathophysiology of epilepsy. Objective: To assess mean daily
plasma concentrations of ACTH, cortisol, DHEAS, leu-enkephalin, and beta-endorphin in epileptic patients with complex partial seizures
evolving to tonic-clonic in relation to frequency of seizure occurrence (groups with seizure occurrences - several per week and several
per year) and duration of the disease (groups less than and more than 10 years). We decided to analyse mean daily values of beta-endorphin
and leu-enkephalin because of significant differences in concentrations of these substances in blood during the day.
Material and methods: The study was performed on 17 patients (14 males + 3 females; mean age 31.8 yrs) treated with carbamazepine
(300-1800 mg/day). The control group consisted of six age-matched healthy volunteers. Blood was collected at 8 a.m., 2 p.m., 8 p.m., and
2 a.m. Intergroup analysis was performed with the use of ANOVA Kruskal-Wallis test.
Results: Mean daily concentrations of ACTH and cortisol in the blood of the patients with epilepsy were higher in comparison with those
of the healthy volunteers, independently of the frequency of seizures and duration of the disease. Mean daily concentrations of beta-endorphin in the blood of the patients with epilepsy were higher in the groups of patients with more severe clinical course of disease
(with more frequently occurring epilepsy seizures and longer duration of the disease) in comparison with healthy subjects. Mean daily
concentrations of leu-enkephalin in the blood of the patients with epilepsy were higher in the group of patients with short duration of the
disease in comparison with the group with long duration of the disease.
Conclusions:
1. Pituitary-adrenal axis hyperactivity is observed in patients with clinically active epilepsy, independently of the frequency of seizures
and duration of the disease.
2. Changes in endogenous opioid system activity are related to the clinical activity of epilepsy - beta-endorphin concentrations are
connected with frequency of seizures and duration of the disease and leu-enkephalin concentrations with duration of the disease.
3. Endogenous opioid peptides might take part in the neurochemical mechanism of human epilepsy.

stężeń ACTH, kortyzolu, DHEAS, leu-enkefaliny and β-endorfiny u chorych na padaczkę z napadami częściowymi, złożonymi i wtórnie
uogólnionymi, toniczno-klonicznymi w zależności od częstości napadów (grupy z częstością napadów - kilka na tydzień i kilka na rok)
i od czasu trwania choroby (grupy 10 lat). Autorzy zdecydowali się na analizę średniodobowych wartości β-endorfiny i leu-enkefaliny
z powodu wyraźnych różnic w ich stężeniu we krwi w ciągu doby.
Materiał i metody: Badanie przeprowadzono u 17 chorych (14 mężczyzn i 3 kobiety; średni wiek 31,8 lat) leczonych karbamazepiną
(300-1800 mg/dzień). Grupa kontrolna składała się z 6 zdrowych ochotników w porównywalnym wieku. Krew pobierano o godzinie 8, 14,
20, 2. W analizie międzygrupowej wykorzystano test ANOVA Kruskala-Wallisa.
Wyniki: Średniodobowe stężenia ACTH i kortyzolu we krwi chorych na padaczkę były wyższe w porównaniu ze zdrowymi niezależnie
od częstości napadów i czasu trwania choroby. Średniodobowe stężenia β-endorfiny we krwi chorych na padaczkę były wyższe w grupach
pacjentów z ciężkim przebiegiem klinicznym choroby (z wysoką częstością napadów i długim czasem trwania choroby) w porównaniu
ze zdrowymi. Średniodobowe stężenia leu-enkefaliny we krwi chorych na padaczkę były wyższe w grupie pacjentów z krótkim
czasem trwania choroby w porównaniu z grupą z długim czasem trwania choroby.
Wnioski:
1. U chorych na padaczkę, niezależnie od częstości napadów i czasu trwania choroby, dochodzi do wzmożonej aktywności osi przysadkowo-
nadnerczowej.
2. Zmiany w aktywności endogennego układu opioidowego są związane z kliniczną aktywnością padaczki - stężenia β-endorfiny pozostają
w związku z częstością napadów i czasem trwania choroby, a stężenia leu-enkefaliny z czasem trwania choroby.
3. Endogenne peptydy opioidowe mogą uczestniczyć w neurochemicznym mechanizmie padaczki u ludzi.

Polish statement on food allergy in children and adolescents
An adverse food reaction is defined as clinical symptoms occurring in children, adolescents or adults after ingestion of a food or chemical food additives. This reaction does not occur in healthy subjects. In certain individuals is
a manifestation of the body hypersensitivity, i.e. qualitatively altered response to the consumed food. The disease
symptoms observed after ingestion of the food can be triggered by two pathogenetic mechanisms; this allows
adverse food reactions to be divided into allergic and non-allergic food hypersensitivity (food intolerance). Food
allergy is defined as an abnormal immune response to ingested food (humoral, cellular or mixed). Non-immunological mechanisms (metabolic, pharmacological, microbiological or other) are responsible for clinical symptoms
after food ingestion which occur in non-allergic hypersensitivity (food intolerance).
Food allergy is considered a serious health problem in modern society. The prevalence of this disorder is varied
and depends, among other factors, on the study population, its age, dietary habits, ethnic differences, and the
degree of economic development of a given country. It is estimated that food allergy occurs most often among
the youngest children (about 6-8% in infancy); the prevalence is lower among adolescents (approximately 3-4%)
and adults (about 1-3%). The most common, age-dependent cause of hypersensitivity, expressed as sensitization or allergic disease (food
allergy), are food allergens (trophoallergens). These are glycoproteins of animal or plant origine contained in: cow's
milk, chicken egg, soybean, cereals, meat and fish, nuts, fruits, vegetables, molluscs, shellfish and other food products. Some of these allergens can cause cross-reactions, occurring as a result of concurrent hypersensitivity to food,
inhaled or contact allergens.
The development of an allergic process is a consequence of adverse health effects on the human body of different
factors: genetic, environmental and supportive. In people predisposed (genetically) to atopy or allergy, the development of food allergy is determined by four allergic-immunological mechanisms, which were classified and described
by Gell-Coombs. It is estimated that in approximately 48-50% of patients, allergic symptoms are caused only by
type I reaction, the IgEmediated (immediate) mechanism. In the remaining patients, symptoms of food hypersensitivity are the result of other pathogenetic mechanisms, non-IgE mediated (delayed, late) or mixed (IgE mediated,
non-IgE mediated). Clinical symptomatology of food allergy varies individually and depends on the type of food induced pathogenetic
mechanism responsible for their occurrence. They relate to the organ or system in which the allergic reaction has
occurred (the effector organ). Most commonly the symptoms involve many systems (gastrointestinal tract, skin,
respiratory system, other organs), and approximately 10% of patients have isolated symptoms. The time of symptoms onset after eating the causative food is varied and determined by the pathogenetic mechanism of the allergic immune reaction (immediate, delayed or late symptoms). In the youngest patients, the main cause of food reactions is allergy to cow’s milk. In developmental age, the clinical picture of food allergy can change, as reflected in the so-called allergic march, which is the result of anatomical and functional maturation of the effector organs, affected by various harmful allergens (ingested, inhaled, contact allergens and allergic cross-reactions). The diagnosis of food allergy is a complex, long-term and time-consuming process, involving analysis of the allergic history (personal and in the family), a thorough evaluation of clinical signs, as well as correctly planned allergic
and immune tests. The underlying cause of diagnostic difficulties in food allergy is the lack of a single universal laboratory test to identify both IgE-mediated and non-IgE mediated as well as mixed pathogenetic mechanisms of
allergic reactions triggered by harmful food allergens. In food allergy diagnostics is only possible to identify an
IgE-mediated allergic process (skin prick tests with food allergens, levels of specific IgE antibodies to food allergens). This allows one to confirm the diagnosis in patients whose symptoms are triggered in this pathogenetic
mechanism (about 50% of patients). The method allowing one to conclude on the presence or absence of food
hypersensitivity and its cause is a food challenge test (open, blinded, placebo-controlled). The occurrence of clinical symptoms after the administration of food allergen confirms the cause of food allergy (positive test) whereas
the time elapsing between the triggering dose ingestion and the occurrence of clinical symptoms indicate the pathogenetic mechanisms of food allergy (immediate, delayed, late). The mainstay of causal treatment is temporary removal of harmful food from the patient’s diet, with the introduction of substitute ingredients with the nutritional value equivalent to the eliminated food. The duration of dietary
treatment should be determined individually, and the measures of the effectiveness of the therapeutic elimination
diet should include the absence or relief of allergic symptoms as well as normal physical and psychomotor development of the treated child. A variant alternative for dietary treatment of food allergy is specific induction of food tolerance by intended contact of the patient with the native or thermally processed harmful allergen (oral immunotherapy). This method has
been used in the treatment of IgE-mediated allergy (to cow's milk protein, egg protein, peanut allergens).
The obtained effect of tolerance is usually temporary. In order to avoid unnecessary prolongation of treatment in a child treated with an elimination diet, it is recommended to perform a food challenge test at least once a year. This test allows one to assess the body's current ability to acquire immune or clinical tolerance. A negative result of the test makes it possible to return to a normal diet,
whereas a positive test is an indication for continued dietary treatment (persistent food allergy). Approximately 80% of children diagnosed with food allergy in infancy "grow out" of the disease before the age of
4-5 years. In children with non-IgE mediated food allergy the acquisition of food tolerance is faster and occurs in
a higher percentage of treated patients compared to children with IgE-mediated food allergy.
Pharmacological treatment is a necessary adjunct to dietary treatment in food allergy. It is used to control the rapidly increasing allergic symptoms (temporarily) or to achieve remission and to prevent relapses (long-term treatment).
Preventive measures (primary prevention of allergies) are recommended for children born in a "high risk" group for
the disease. These are comprehensive measures aimed at preventing sensitization of the body (an appropriate way
of feeding the child, avoiding exposure to some allergens and adverse environmental factors). First of all, the infants
should be breast-fed during the first 4-6 months of life, and solid foods (non milk products, including those containing gluten) should be introduced no earlier than 4 months of age, but no later than 6 months of age. An elimination diet is not recommended for pregnant women (prevention of intrauterine sensitization of the fetus and
unborn child). The merits of introducing an elimination diet in mothers of exclusively breast-fed infants, when the
child responds with allergic symptoms to the specific diet of the mother, are disputable. Secondary prevention focuses on preventing the recurrence of already diagnosed allergic disease; tertiary prevention is the fight against organ
disability resulting from the chronicity and recurrences of an allergic disease process. Food allergy can adversely affect the physical development and the psycho-emotional condition of a sick child, and
significantly interfere with his social contacts with peers. A long-term disease process, recurrence of clinical symptoms, and difficult course of elimination diet therapy are factors that impair the quality of life of a sick child and
his family. The economic costs generated by food allergies affect both the patient's family budget (in the household), and the overall financial resources allocated to health care (at the state level). The adverse socio-economic
effects of food allergy can be reduced by educational activities in the patient’s environment and dissemination of
knowledge about the disease in the society
