    Carotid artery occlusion and colateral circulation in C57black/6J mice detected by synchrotron radiation microangiography

    Using monochromatic synchrotron radiation, we performed microangiography inC57BL/6J mice and investigated their vasculature after unilateral and bilateral carotidartery occlusion. Bilateral occlusion of the carotid artery was made by a ligation of theleft common carotid artery followed by a ligation of the right internal carotid artery(ICA) two days later (n=12). Five days after the second surgery, angiography wasperformed. Unilateral occlusion was made by clipping the right ICA and thenangiography was performed immediately (n=5). The control mice did not undergo anyocclusion (n=5). We removed the brain of the bilateral occlusion mice afterangiography and examined the infarction area. The cerebral microvessels in all animalswere clearly visualized. In the control mice, the posterior communicating artery (Pcom)was not visualized. In the unilateral occlusion mice, the anastomosis of thepterygopalatine artery (PPA) and the external carotid artery (ECA) were recognized.The PPA is thus considered to play a role in the collateral vessel between the ICA andthe ECA. The Pcom was not visualized. In the bilateral occlusion mice, the Pcom wasobserved either unilateraly (n=5) or bilateraly (n=5). The Pcom supplied blood flow tothe anterior circulation from the vertebrobasilar arteries. The bilateral occlusion micethat had at least one visualized Pcom did not have any infarction. We could successfullyvisualize the cerebral vasculature of normal mice and carotid artery occluded mice inan in vivo study.     Immunoadsorption Therapy for Patients with Dilated Cardiomyopathy and Heart Failure

    Several autoantibodies directed against cardiac cellular proteins including G-protein-linked receptors, contractile proteins and mitochondrial proteins, have been identified in patients with dilated cardiomyopathy (DCM). Among these autoantibodies, anti-β1-adrenoreceptor (AR) antibodies have long been discussed in terms of their pathogenetic role in DCM. Anti-β1-AR antibody-positive patients with DCM showed significant deterioration of NYHA functional class as well as reduced cardiac function compared to those in autoantibody-negative patients. Various studies with a limited number of patients indicate that the use of immunoadsorption to eliminate immunoglobulin G (IgG) significantly improves cardiac performance and clinical status in heart failure patients. Since removal of autoantibodies of the IgG3 subclass induces hemodynamic improvement and an increase in the left ventricular ejection fraction, antibodies belonging to IgG3 such as anti-β1-AR antibodies might play an important role in reducing cardiac function in patients with DCM. According to a recent report, however, the effect of hemodynamic improvement by immunoadsorption threapy was similar among patients who were positive and negative for anti-β1-AR antibodies, indicating that the beneficial effects of immunoadsorption might be not directly associated with the selective elimination of the β1-AR autoantibodies. Immunoadsorption therapy is a new therapeutic option for patients with DCM and heart failure, but further investigations are required to elucidate the specific antigens of cardiac autoantibodies responsible for the hemodynamic effects

    The joint JAXA/NASA ASTRO-H mission is the sixth in a series of highly successful X-ray missions initiated by the Institute of Space and Astronautical Science (ISAS). ASTRO-H will investigate the physics of the high-energy universe via a suite of four instruments, covering a very wide energy range, from 0.3 keV to 600 keV. These instruments include a high-resolution, high-throughput spectrometer sensitive over 0.3-2 keV with high spectral resolution of Delta E < 7 eV, enabled by a micro-calorimeter array located in the focal plane of thin-foil X-ray optics; hard X-ray imaging spectrometers covering 5-80 keV, located in the focal plane of multilayer-coated, focusing hard X-ray mirrors; a wide-field imaging spectrometer sensitive over 0.4-12 keV, with an X-ray CCD camera in the focal plane of a soft X-ray telescope; and a non-focusing Compton-camera type soft gamma-ray detector, sensitive in the 40-600 keV band.     Hospital and clinic cooperation for the treatment of rheumatoid arthritis in Okayama Prefecture, Japan

    Objective: To survey the current status and problems of cooperation between clinics and hospitals in Okayama Prefecture, Japan for the treatment of rheumatoid arthritis (RA).  Methods: We distributed a questionnaire to 300 of the 983 Okayama Prefecture clinics that had either an internal medicine or orthopedic surgery department, from December 2013 to February 2014. The questionnaire covered practice pattern for RA treatment in clinics, current status of the hospital and clinic cooperation, and acceptance of the biologic therapy.  Results: One hundred clinics responded to the questionnaire. Seventy percent of the clinics reported making referrals to rheumatologists before the initiation of RA treatment, and half of the other 30% of the clinics administered methotrexate as the first-line treatment for RA by their own decision. Sixty-six clinics cooperated with flagship hospitals, conducting medical and laboratory examinations, providing prescriptions, and treating common diseases of patients. These clinics expected the cooperating rheumatologists to follow-up patients every 3 to 6 months and to make the diagnosis, make decisions regarding RA treatment changes, and perform surgery. Seventy-one percent of the clinics responded that cooperation with a hospital is possible even for patients who are administered biologics. As reasons for no cooperation with the flagship hospitals, clinics noted the lack of information about rheumatologists in the area and recent trends in the management of RA.  Conclusion: The current study reported, for the first time, the actual conditions of management of RA in clinics, as well as future problems of hospital and clinic cooperation in Okayama Prefecture

    Hitomi (ASTRO-H) X-ray Astronomy Satellite

    The Hitomi (ASTRO-H) mission is the sixth Japanese x-ray astronomy satellite developed by a large international collaboration, including Japan, USA, Canada, and Europe. The mission aimed to provide the highest energy resolution ever achieved at E  >  2  keV, using a microcalorimeter instrument, and to cover a wide energy range spanning four decades in energy from soft x-rays to gamma rays. After a successful launch on February 17, 2016, the spacecraft lost its function on March 26, 2016, but the commissioning phase for about a month provided valuable information on the onboard instruments and the spacecraft system, including astrophysical results obtained from first light observations. The paper describes the Hitomi (ASTRO-H) mission, its capabilities, the initial operation, and the instruments/spacecraft performances confirmed during the commissioning operations for about a month

    Altered postural sway following fatiguing foot muscle exercises.

    This study investigated the acute effects of fatiguing foot muscle exercises on the maximum muscle strength of the foot and postural control ability. Eighteen healthy young individuals performed fatiguing foot muscle strength exercises, and their toe flexor and ankle plantar flexor strength and postural control ability were measured before and after the exercises. Postural control ability was evaluated using the path of the center of pressure (COP) during three balance tasks: double-leg standing with eyes open; double-leg standing with eyes closed; and single-leg standing with eyes open. After the exercises, the muscle strength of both the toe and ankle plantar flexor significantly decreased. Under all of the conditions, most COP variables did not significantly differ before and after the exercises; however, the total length and mean velocity in the single-leg standing with eyes open significantly decreased after the exercises. Postural sway velocities in the anteroposterior direction of double-leg standing with eyes closed and in both anteroposterior and mediolateral directions of single-leg standing with eyes open significantly decreased after the exercises. The associations between relative changes in muscle strength after the exercise and relative changes in COP variables after the exercise were not found. These results indicate that postural control while standing could be maintained even though foot muscle strength is decreased after fatiguing foot muscle exercises
