    Features of diagnostics of gastroesophageal and duodenogastric reflux in patients with complicated forms of peptic ulcer.

    Purpose – to improve the results of surgical treatment of duodenal ulcer due to preoperative diagnosis of concomitant reflux disease and its correction. Before surgery, 46 patients with complicated forms of peptic  ulcer disease underwent pH monitoring and 23 – impedance-pH monitoring. Pathological gastroesophageal reflux was found in 58 cases. Before surgery, esophageal pH monitoring was performed in 46 patients with complicated forms of peptic ulcer disease. Against the background of hyper – and normacidity of the stomach in 39 patients pathological acidic gastroesophageal reflux disease with a significant increase of De Meester index was confirmed. Impedance-pH monitoring was performed before surgery in 23 patients with complicated forms of peptic ulcer and concomitant gastroesophageal reflux. Erosive form of gastroesophageal reflux disease was confirmed endoscopically in all patients with hyperacidity (17) and normal acidity (6) of the stomach. In 19 cases pathological gastroesophageal reflux disease was found. In 13 cases the cause of disorders in anti-reflux barrier was a diaphragmatic hernia of I-II degree. Gastroesophageal reflux disease was caused by increased intragastric pressure due to ulcer stenosis in 6 patients. pH- monitoring in patients with acidic gastroesophageal reflux allows to establish a pathological or physiological reflux. Conducting impedance-pH monitoring allows to determine the number of reflux episodes and their qualitative characteristics (acidic or alkaline; liquid, gas or mixed), as well as to determine the indications for a differentiated approach in the treatment of gastroesophageal reflux

    Problems of healing chronic wounds

    Aim. The work aimed to analyze and highlight the problems of chronic wound healing and modern methods of stimulation of reparative processes according to literature sources. An analysis of the scientific literature was carried out, in which views on the problems of wound healing are presented, taking into account the factors affecting the formation and healing of chronic wounds, and modern methods that stimulate reparation processes. Identifying the problems of wound healing will allow a comprehensive approach to the assessment of the wound process, taking into account the factors affecting the healing of injuries. Modern scientists are actively discussing the problems of wound healing, researching factors influencing the reparative process, including the high prevalence of chronic wounds, diagnosis and treatment of wound infections, age of patients, and state of immunity. In the case of systemic diseases, such as diabetes, cardiovascular pathology, etc., there is a violation of microcirculation, and as a result, there are violations of the phasing of reparative processes at the cellular level. As a result, the alteration process does not end and the process becomes chronic. In case of a mechanical injury, when adaptation reserves are reduced, primarily of the neurohumoral system, a key role is played by the disruption of regulatory mechanisms at the cellular level. Wound treatment requires surgical treatment, stimulation of angiogenesis, and other reparative processes at the systemic and local levels. In recent years, various methods of stimulating reparative processes have been used in clinical practice to treat wounds: local administration of recombinant growth factors, use of platelet-rich plasma, biotechnological wound dressings, and artificial skin substitutes. One of the most promising areas of wound treatment is the development of methods for targeted activation of stem cells. However, there are still insufficient data for the practical application of this method. Clinically effective and safe methods are still being developed. Physical methods are promising for improving the healing of chronic wounds. However, the selection of optimal parameters for physical action continues to be an urgent task. Despite the positive results obtained using the methods described above, there is a need for additional experimental studies and randomized controlled trials. Conclusions. Chronic wounds develop as a result of severe systemic diseases, and major mechanical injuries against the background of a decrease in physiological reserves due to emotional and psychological stress, obesity, etc. Choosing the right method of influencing the reparative process, taking into account the leading mechanism of damage repair, becomes very important to ensure the best chance for optimal wound healing

    У 10% пацієнтів при гастроезофагеальній рефлексній хворобі (ГЕРХ) відсутні типові прояви захворювання, а основні симптоми виявляють у дихальній, серцево–судинній системах, ЛОР–органах. Мета. Покращення діагностичного алгоритму при виявленні позастравохідних проявів ГЕРХ; встановлення ефективності лапароскопічної фундоплікації у хворих за різних видів гастроезофагеального рефлюксу (ГЕР). Матеріали і методи. З використанням опитувальників якості життя (ЯЖ) SF–36 та GERD–HRQL оцінювали віддалені результати консервативної терапії та хірургічного лікування пацієнтів за наявності респіраторних та ЛОР проявів ГЕРХ. Результати. Більш суттєве покращення фізичного та психологічного компонентів ЯЖ відзначене у хворих, яким виконували лапароскопічну фундоплікацію. За опитувальником GERD–HRQL, ЯЖ хворих пысля хірургічного лікування покращилась через 6 міс – з (16,4 ± 5,3) до (6,2 ± 0,6) бала (р < 0,001), через 12 міс – до (6,4 ± 0,8) бала (р < 0,001); після консервативної терапії через 6 міс – з (15,9 ± 6,6) до (9,2 ± 1,9) бала (р < 0,001), через 12 міс – до (11,4 ± 1,5) бала (р < 0,05). Висновки. Лапароскопічна фундоплікація порівняно з консервативною терапією більш ефективний метод лікування пацієнтів за наявності позастравохідних проявів ГЕРХ
