    'Be active!' Revisiting the South African food-based dietary guideline for activity

    The objective of this paper was to review current evidence on physical activity for health in order to support the food-based dietary guideline (FBDG) “Be active!”. Physical activity, defined as at least 30 minutes of moderate-intensity physical activity per day for adults, and 60 minutes for children and adolescents, is advised in the FBDG because of the role it plays in maintaining energy balance, improving body composition and promoting general health and wellbeing. The reviewed outcome measures are changes in physical activity patterns and the reported prevalence of noncommunicable diseases (NCDs) in South Africa. Despite the previous set of FBDGs, no improvements in physical activity, obesity or NCDs have been reported in South Africa. Recent literature emphasises the beneficial effects of physical activity on the reduction of risk factors associated with the prevalence of NCDs. Physical activity has a positive effect on appetite and weight control, insulin sensitivity, dyslipidaemia, hypertension, stress relief and burnout. Barriers that prevent children and adults from participating in regular physical activity have been identified, and recommendations how to overcome these have been made. It has been concluded that South Africans are not sufficiently physically active for their general health status to be improved. It is recommended that methods to promote physical activity at national, provincial, district and local level need to be developed, implemented and sustained

    Assessing the validity and reliability and determining cut-points of the Actiwatch 2 in measuring physical activity

    Objective: The Actiwatch 2 (AW2) is a wrist-worn accelerometer typically used to measure sleep. Although it can measure physical activity, there is limited evidence supporting its validity. We assessed the validity and reliability of the AW2 to measure sedentary behavior and physical activity (light, moderate, vigorous intensities), and reported their respective count cut-points. Approach: Twenty-eight males and 22 females completed a task battery comprising three sedentary tasks and six randomized physical activity tasks at varying intensities, whilst wearing the AW2, a reference accelerometry device (Actigraph GT3X) and a cardiopulmonary gas analyzer on two separate occasions. Validity was assessed using correlations (AW2 counts versus GT3X counts and metabolic equivalent (MET) values), reliability using Bland–Altman analyses, and cut-points were determined using receiver operating characteristic (ROC) area under the curve (AUC) analyses. Main results: AW2 counts were positively correlated with GT3X counts (rho = 0.902, p < 0.001) and METs (rho = 0.900, p < 0.001). AW2-derived counts were comparable across independent assessment periods. Sedentary (AUC = 0.99, cut-point: 256 cpm) and vigorous activity (AUC = 0.95, cut-point: 720 cpm) were strongly characterized, and moderate activity (AUC = 0.66, cut-point: 418 cpm) was weakly characterized. Significance: The use of the AW2 in physical activity monitoring looks promising for sedentary behavior, moderate and vigorous activity, however, further validation is needed

    A hard day’s night: time use in shift workers

    Background Differences in how shift workers accumulate physical activity (PA) while at work and in leisure time, on days when they are working at night, during the day, or on non-work days, are largely unexplored. The aim of this study was to improve understanding of physical activity patterns in two groups of shift workers, and to measure variations according to their shift schedules. Methods This pragmatic pilot study was conducted in two workplaces. Employees in Workplace 1 (n = 10) were required to drive for most of their shift. Workplace 2 was a manufacturing company where most of the employees’ (n = 30) occupational tasks were completed while standing. Use of time was assessed using the adult version of the Multimedia Activity Recall for Children and Adults (MARCA) administered by telephone interview. Three MARCA interviews were conducted with each participant, in order to capture a typical profile of a day-shift day, a night-shift day and a non-work day, using a two-day recall for each interview. Participants were asked to wear the activPAL3™ activity monitor, for 7 consecutive days. Paired and independent t-tests were used to compute significant differences between day-shift, night-shift and non-work days within and between workplaces. Results The total number of days quantified for the MARCA data was 192 days (64 day-shift, 60 night-shift and 68 non-work days). Workplace 2 participants reported more physical activity and less sedentary behaviour on day-shift and night shift days than on non-work days. Time spent in sedentary behaviour was similar on day-shift, night-shift and non-work days in Workplace 1. Workplace 1 participants were more sedentary (p = 0.003) and engaged in more light intensity PA (p = 0.031) on day-shift and night-shift workdays, than those from Workplace 2. Sleep times were lowest on day-shift days. Conclusion As the occupational tasks for participants in Workplace 2 involved physical activities, the findings do not support the conventional view that shift workers are more sedentary than those who only work during the day. Rather occupational tasks appear to be a more important determinant of physical activity patterns both on work and non-work days than varying shift patterns

    Associations Between Self-Reported Sleep Duration and Mortality in Employed Individuals:Systematic Review and Meta-Analysis

    Objective: Sleeping less or more than the 7-8 h has been associated with mortality in the general population, which encompasses diversity in employment status, age and community settings. Since sleep patterns of employed individuals may differ to those of their unemployed counterparts, the nature of their sleep-mortality relationship may vary. We therefore investigated the association between self-reported sleep duration and all-cause mortality (ACM) or cardiovascular disease mortality (CVDM) in employed individuals. Data sources: Based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses, searches between January 1990 and May 2020 were conducted in PubMed, Web of Science and Scopus. Inclusion/exclusion criteria: Included were prospective cohort studies of 18–64-year-old disease-free employed persons with sleep duration measured at baseline, and cause of death recorded prospectively as the outcome. Gray literature, case-control or intervention design studies were excluded. Data Extraction: Characteristics of the studies, participants, and study outcomes were extracted. The quality and risk of bias were assessed using the Newcastle-Ottawa Scale. Data synthesis: The pooled relative risks (RR) with 95% confidence intervals (CI) were obtained with a random-effects model and results presented as forest plots. Heterogeneity and sensitivity analysis were assessed. Results: Shorter sleep duration (less than or equal to 6 h) was associated with a higher risk for (ACM) (RR: 1.16, 95% CI: 1.11 -1.22) and CVDM (RR: 1.26, 95% CI: 1.12 -1.41) compared to 7-8 h of sleep, with no significant heterogeneity. The association between longer sleep (greater than or equal to 8 h) and ACM (RR: 1.18, 95% CI:1.12 -1.23, P < 0.001) needs to be interpreted cautiously owing to high heterogeneity (I2 ¼ 86.0%, P < 0.001). Conclusion: Interventions and education programs targeting sleep health in the workplace may be warranted, based on our findings that employed individuals who report shorter sleep appear to have a higher risk for ACM and CVDM

    Steps that count: Pedometer-measured physical activity, self-reported physical activity and current physical guidelines ‒ how do they relate?

The association between self-perceived and actual physical activity, with particular reference to physical activity guidelines, may be an important factor in determining the extent of uptake of and compliance with physical activity.Objectives. To examine the association between self-perceived and actual physical activity in relation to physical activity guidelines, with reference to volume, intensity and duration of steps/day, and to establish the level of agreement between pedometer-measured and selfreported ambulatory physical activity, in relation to current guidelines.Methods. A convenience sample of adults (N=312; mean (standard deviation) age 37 (9) years), wore a pedometer (minimum 3 consecutive days) and completed a questionnaire that included information on physical activity patterns. Analyses of covariance, adjusted for age and gender, compared volume- and intensity-based steps according to meeting/not meeting guidelines (self-reported). The extent of agreement between self-reported and pedometer-measured physical activity was also determined.Results. Average (SD) steps/day were 6 574 (3 541). Of a total of 312 participants’ self-reported data, those meeting guidelines (n=63) accumulated significantly more steps/day than those not meeting guidelines (8 753 (4 251) v. 6 022 (3 114) total steps/day and 1 772 (2 020) v. 421 (1 140) aerobic steps/day, respectively; p&lt;0.0001). More than half of the group who self-reported meeting the guidelines did not meet guidelines as per pedometer data.Conclusion. The use of pedometers as an alternative and/or adjunct to self-reported measures is an area for consideration. Steps/day recommendations that consider intensity-based steps may provide significant effects in improving fitness and health

    The association between daily steps and health, and the mediating role of body composition: a pedometer-based, cross-sectional study in an employed South African population

    Background: Walking is recognized as an easily accessible mode of physical activity and is therefore supported as a strategy to promote health and well-being. To complement walking, pedometers have been identified as a useful tool for monitoring ambulatory physical activity, typically measuring total steps/day. There is, however, little information concerning dose-response for health outcomes in relation to intensity or duration of sustained steps. We aimed to examine this relationship, along with factors that mediate it, among employed adults. Methods: A convenience sample, recruited from work-site health risk screening (N = 312, 37 ± 9 yrs), wore a pedometer for at least three consecutive days. Steps were classified as “aerobic” (≥100 steps/minute and ≥10 consecutive minutes) or “non-aerobic” (<100 steps/minute and/or <10 consecutive minutes). The data were sub-grouped according to intensity-based categories i.e. “no aerobic activity”, “low aerobic activity” (1-20 minutes/day of aerobic activity) and “high aerobic activity” (≥21 minutes/day of aerobic activity), with the latter used as a proxy for current PA guidelines (150-minutes of moderate-intensity PA per week). Health outcomes included blood pressure, body mass index, percentage body fat, waist circumference, blood cholesterol and blood glucose. Analysis of covariance, adjusting for age, gender and total steps/day were used to compare groups according to volume and intensity-based steps categories. A further analysis compared the mediation effect of body fat estimates (percentage body fat, body mass index and waist circumference) on the association between steps and health outcomes, independently. Results: Average steps/day were 6,574 ± 3,541; total steps/day were inversely associated with most health outcomes in the expected direction (p < 0.05). The “no aerobic activity” group was significantly different from the “low aerobic activity” and “high aerobic activity” in percentage body fat and diastolic blood pressure only (P < 0.05). Percentage body fat emerged as the strongest mediator of the relationship between steps and outcomes, while body mass index showed the least mediation effect. Conclusion: The study provides a presentation of cross-sectional pedometer data that relate to a combination of intensity and volume-based steps/day and its relationship to current guidelines. The integration of volume, intensity and duration of ambulatory physical activity in pedometer-based messages is of emerging relevance

    Status and Trends of Physical Activity Surveillance, Policy, and Research in 164 Countries: Findings From the Global Observatory for Physical Activity—GoPA! 2015 and 2020 Surveys

    Background: Physical activity (PA) surveillance, policy, and research efforts need to be periodically appraised to gain insight into national and global capacities for PA promotion. The aim of this paper was to assess the status and trends in PA surveillance, policy, and research in 164 countries. Methods: We used data from the Global Observatory for Physical Activity (GoPA!) 2015 and 2020 surveys. Comprehensive searches were performed for each country to determine the level of development of their PA surveillance, policy, and research, and the findings were verified by the GoPA! Country Contacts. Trends were analyzed based on the data available for both survey years. Results: The global 5-year progress in all 3 indicators was modest, with most countries either improving or staying at the same level. PA surveillance, policy, and research improved or remained at a high level in 48.1%, 40.6%, and 42.1% of the countries, respectively. PA surveillance, policy, and research scores decreased or remained at a low level in 8.3%, 15.8%, and 28.6% of the countries, respectively. The highest capacity for PA promotion was found in Europe, the lowest in Africa and low- and lower-middle-income countries. Although a large percentage of the world’s population benefit from at least some PA policy, surveillance, and research efforts in their countries, 49.6 million people are without PA surveillance, 629.4 million people are without PA policy, and 108.7 million live in countries without any PA research output. A total of 6.3 billion people or 88.2% of the world’s population live in countries where PA promotion capacity should be significantly improved. Conclusion: Despite PA is essential for health, there are large inequalities between countries and world regions in their capacity to promote PA. Coordinated efforts are needed to reduce the inequalities and improve the global capacity for PA promotion

    The perceived role and influencers of physical activity among pregnant women from low socioeconomic status communities in South Africa

    Background: Facility-based and context-specific interventions to promote physical activity (PA) among pregnant women from economically underprivileged communities remain sparse and undocumented in South Africa. This study aimed to generate information about pregnant women’s views and experiences of PA during pregnancy, which will later be used to inform the development of a PA-based intervention targeting this group. Methods: Qualitative methods were used and framed on the Theory of Planned Behavior (TPB). Five focus group discussions were conducted at a Community Health Centre in Cape Town, each comprising a stratified random sample of between 8 and 6 pregnant women living in eight low socioeconomic status communities close to the facility. The participants included primi- and multigravida black and mixed racial ancestry women at different stages of pregnancy. Data were analyzed using a Framework approach. Results: PA was considered important for self and the baby for most participants. However, they reported a number of barriers for translating intentions into action including the lack of supportive environment, fear of hurting oneself and the growing baby, lack of time due to work and family responsibilities, and not knowing which and how much PA is safe to do. Some of the incentives to engage in PA included establishing community-based group exercise clubs, initiating antenatal PA education and PA sessions during antenatal visits. Conclusion: Based on our findings the need for an intervention to promote PA in pregnancy is evident. Such an intervention should, however, aim at addressing barriers reported in this study, particularly those related to the behavioral context

    ‘Cost, culture and circumstances’: Barriers and enablers of health behaviours in South Asian immigrants of Australia

    The health behaviours related to chronic diseases experienced by South Asian immigrants are interrelated with their culture and socioeconomic conditions. South Asian immigrants experience a disproportionate burden of chronic disease compared with non-immigrants Australian-born general population. The primary aim of this study was to gain an in-depth understanding of health behaviours and healthcare access in the South Asian immigrant population of Australia. Five focus group discussions (FGDs) were conducted with South Asian immigrants (n = 29; 18 females) aged 27–50 years in Brisbane, Australia. Separate FGDs were conducted for males and females in the English language. Semi-structured guided questions related to the perception, barriers and facilitators of health behaviours. Data were analysed with Nvivo-12 following a thematic analysis. A conceptual model is proposed to provide a summarised understanding of barriers and facilitators of health behaviours in South Asian immigrants. The major reported constraints for participating in physical activity were cultural beliefs, lack of time, work stress and high fees of fitness activities, while parks and peer modelling were mentioned as a strong motivator for walking, cycling and participating in group sports activities. The cultural and religious connections, cost of cigarettes and drink driving penalties were the most mentioned facilitators for a healthy lifestyle. The important factors related to unhealthy eating habits were the traditional cooking methods, social interactions and the high cost of fruits and vegetables. Community perceptions and language barriers were also acknowledged as the main factors for the decrease in accessing health care services. This study illustrates that cultural beliefs, high cost of healthy food and facilities and social circumstances are mainly linked with the health behaviours and healthcare access in South Asian immigrant's lifestyles

    Case study from Africa: physical activity and safety from crime and traffic in Africa

    Although Africa is the most physically active region in the world, about 18% of men and 25% of women are not meeting the recommended levels of physical activity, shown to reduce the risk of non-communicable diseases (NCDs). Further, nearly 30% of the disease burden in Sub-Saharan Africa is attributable to NCDs. Therefore, identifying equitable access to safe and supportive environments for physical activity in African cities is an important research and policy priority for mitigating the worsening burden of NCDs in the region. In this chapter, we explore current knowledge on how safety from crime and traffic on the African continent is associated with habitual levels of physical activity. We note that the prevalence of physical activity and safety issues in Africa varies by country level of income. In addition, while the high prevalence of insecurity from crime and traffic is consistently related to insufficient physical activity among African adults, this relationship is less clear in children and adolescents. Next, we discuss the implications of these findings for practice and policy, and conclude by providing recommendations for future multisectoral interventions, targeting the upstream determinants from a regulatory and built environment perspective as effective practical solutions across the African continent
